Novalis Radiosurgery of Primary Central Nervous System Lymphoma in Elderly Patients : Preliminary Results

  • Objective : Primary central nervous system lymphoma[PCNSL] is a non-Hodgkin's lymphoma arising in the central nervous system. Combined chemotherapy and radiation therapy is the standard treatment for PCNSL. However, treatment induced neurotoxicity is a major problem especially in elderly patients. Methods : From May 2001 to April 2004, elderly five patients with PCNSL confirmed via pathological examination were underwent Novalis radiosurgery in authors' institution, who were investigated retrospectively. Of these patients, 2 were male and 3 were female, with a mean age 68 years old [range $65{\sim}73$]. The number of lesions was 1 in 2 patients, 2 in 2, and 3 in 1. The mean follow up period was 12.6 months [range $8{\sim}16$]. Results : Clinical symptoms and signs in all patients markedly improved within 1 weeks after Novalis radiosurgery. The Karnofsky performance status score was also improved from a pre-radiosurgery average of 68 to a post-radiosurgery one of 82. All of treated lesions showed a partial or complete regression of the original mass. There have been no complication following Novalis radiosurgery. Conclusion : The preliminary results of our experience indicate that radiosurgery with Novalis provides a safe and effective therapeutic alternative treatment of PCNSL in elderly patients.

Content Analysis of On-Line Consumer Information for Elderly - Comparison between the US and Korea Apparel E-tailing Sites - (노년층 의류 소비자를 위한 온라인 소비자 정보에 대한 분석 - 한국과 미국의 웹사이트의 비교연구 -)

  • Internet apparel businesses are required to provide various kinds of consumer information which includes both product information and customer service information. "New Age Elderly" who become an aged man in 21th century are familiar with internet and other high technological tools from their life time experience with technological development and ready to enjoy the convenience the technology offers. As a preliminary step to develop internet consumer information model for elderly consumers, this study examines the US and Korea apparel e-tailing sites targeting elderly consumers. Ten Korean sites and nine US sites were selected and compared for their provision of consumer information in terms of contents, ease to access, and the completeness of the information. Results of data analysis indicate that elderly market in Korea is not yet clearly defined compared to the US. In Korea, elderly consumer market is considered as a part of middle-aged market, and there was no unique target marketing efforts observed. Korean sites are better than the US's sites in terms of shopping information, while the US sites offer detailed information about the company, and customer service information. In both countries, fashion related information is lacking and failing to satisfy fashion conscious elderly consumers. Managerial implications were discussed.

  • Purpose: This study evaluates the effectiveness of providing frailty prevention services by living support workers through a case of community resource connection centered on living support workers to revitalize frailty prevention programs for vulnerable elderly people. Methods: This is a research study using secondary data from a neighborhood health-sharing project among the integrated health promotion projects of one public health center in Daegu Metropolitan City. To assess frailty effects pre-assessments were conducted in August, and post-assessments were conducted in November. Frailty was measured using a 20-item frailty instrument used in home healthcare projects. Data were analyzed using the chi-square, independent t-test, and paired t-test. Results: Preliminary measurements showed that older elderly had higher frailty scores than younger elderly. However, among the elderly aged 75 or older the total frailty score decreased statistically significantly from 5.97 points to 5.30 points (t=3.03, p=.003). Conclusion: The older elderly showed greater effect of frailty prevention than the younger elderly.

The Development of Wholeness Program for Effects Dementia-Buffering Testing of the Demented Elderly (노인의 치매행동 관련 홀니스 프로그램 개발과 완충효과 검증)

  • This study of the Wholeness Program relevant to dementia behavior and verification of buffering effects. For the purpose of this, among those who were attending Day Care Centers belonging to Seniors Welfare Center of M1 and M2 located in Seoul. Among those who met all these requirements, 40 senior citizens were finally selected. After carrying out a preliminary test to the selected subjects, 20 elderly of Seniors Welfare Center of M1 were randomly placed as an experimental group, while other 20 people of Seniors Welfare Center of M2, as a control group. The Wholeness Program relevant to dementia behavior was carried out from March 14 to May 30 of 2016, for 12 weeks, twice a week. As for the measurement tool, this study depended on the Korean version of Mini Mental State Examination (MMSE-K), the Korean version of Modified Barthel Index (K-MBI) and the Korean version of Geriatric Depression Scale (GDS-K). While the elderly were participating in the Wholeness Program relevant to dementia behavior, they were measured two times in total, before and after the participation in dementia behavior. Based on SPSS-PC (version 21.0), firstly, independent t-test was carried out to understand the homogeneity between the experimental group and the control group. Secondly, matched-pair t-test was carried out between preliminary test and post-test to inquire into the effects of MMSE-K, K-MBI and GDS-K. Thirdly, ANOVA was conducted to confirm the variation (change amount) between preliminary test and post-test. According to the survey results, the Wholeness Program relevant to dementia behavior of the elderly shows overall effects in all spheres of changes of cognitive ability (MMSE-K), changes of physical capacity (K-MBI) and changes of depression level (GDS-K).

  • This study analyzed the determinant factors of elderly housing by housing type according to the Financial Retirement plan of preliminary elders and it aimed to draw a future development scheme of elderly housing. This study used parameters of existing research as control variables and it has a meaningful point that the variables of Financial Retirement plan of elders were verified through the research model which this study used. In addition, there was a difference between the detailed models. As a comprehensive analysis result, the Choice Model of elderly housing type has a difference between determinants, the single-family housing and the multi-family housing, based on the Welfare Facility for the Aged from Financial Retirement plan of elders.

Development of an Instrument to Measure Risk Factor-related Deviant Behavior for the Elderly in South Korea (노인의 일탈행동과 관련된 위험요인 측정도구 개발 - 노인 범죄 증가와 관련하여 -)

  • Purpose: The study was designed to develop an instrument to measure risk factor-related deviant behavior (RFRDB) for the elderly. Methods: The preliminary instrument including 52 items to measure the risk factor-related deviant behavior for the elderly was developed through conceptual framework based literature review. The items were reviewed by experts to reveal the Content Validity Index (CVI). Then, construct validity and reliability were tested using factor analysis, comparative groups, and Cronbach's alpha with data from 233 elderly. Results: Among 52 items, 27 questions in the RFRDB instrument were selected from content validity and 21 questions in the final RFRDB were developed from testing validity and reliability. Results of empirical analysis(retest) of RFRDB were supported by 70% congruity with conceptual framework through literature review. The RFRDB has been completed by validity testing by known-group technique. The final developed RFRDB of the elderly has 21 questions each with a 4-point Likert Scale. Conclusion: The RFRDB may be utilized as a measurement to assess the risk of elderly deviant behavior.

Development of Food Behavior Evaluation Items for the Elderly Based on Korean Dietary Guidelines (한국인 식생활지침에 기반한 노인의 식행동 평가항목 개발)

  • A checklist of dietary behavior items for the elderly was developed based on Korean dietary guidelines. First, a literature review was conducted, and 63 preliminary items, including 30 items on food intake, 12 on eating habits, and 21 on dietary culture, were obtained to evaluate the dietary guidelines. The preliminary items were evaluated by experts using Lawshe's method to verify the content validity. They were then revised, resulting in 52 items: 25, 11, and 16 in the food intake, eating habits, and dietary culture domains, respectively. A face-to-face survey was conducted on 331 people over 65 years of age in the metropolitan area, and their one-day food intake was surveyed using a 24-hour recall method in August 2022. The 48 items, including 24, 11, and 13 items on food intake, eating habits, and dietary culture, respectively, correlated significantly with the evaluation indicators of nutrient intake, such as mean adequacy ratio, dietary diversity score, and nutrient intake density. These items were considered suitable for evaluating the elderly's compliance with dietary guidelines.

  • The active living of the elderly leads to improve their lives and enhance social networks. In the view of the active living, the walkability is an essential factor for the elderly's daily life. To support the active living, making age-friendly environment is important. Considering that the elderly mainly carry out activities through walking, making the age-friendly walking environment is a preliminary action. The existing studies applied various methods such as surveys by experts. In spite of the benefits in theirs, there is still a limitation that current walkability measurement methods did not incorporate the actual elderly's walking activity. Thus, the purposes of this study is to measure the elderly's walking quantitatively using a wearable sensor, and to investigate the feasibility of comparing several walking environments based on the data collected from the actual elderly's walking. To do this, experiment was conducted in four types environments with 22 senior subjects. The walkability was measured by walking stability represented quantitatively as Maximum Lyapunov Exponent (MaxLE). Through the experiment results, it was confirmed that the stability of the elderly walking was different according to the walking environment, which also meant that bodily responses (walking stability) is highly related to walkability. The results will provide an opportunity for the continuous diagnosis of walking environments, thereby enhancing the active living of the elderly.

Scale Development on Health Conservation of the Institutionalized Elderly (시설 노인의 건강보존에 관한 도구 개발)

  • Purpose: The purpose of this study was to develop a health conservation scale with high validity and reliability for institutionalized elderly. Method: The process of development of this scale was as follows. A conceptual framework composed of 4 phases of health conservation of institutionalized elderly was identified based on the literature review with elderlies and discussions with experts in health conservation. A total of 75 items, on a 4-point scale were developed. Through reliability testing and factor analysis, 57 preliminary items were selected. By means of internal consistency of the 57 items, 18 items whose inner-items correlation coefficient was below .40 were deleted. Through factor analysis, 2 items whose factor loading was below .40 were deleted. Finally 37 items remained. To verity the 37 items, factor analysis, reliability testing, and correlation was done. Data were collected from 207 institutionalized elderly subjects in Daegu, Kyungpook, Busan, and KyungNam Province from August. 2003 to February. 2004. Result: In the result of factor analysis of the 37 items, 4 factors were extracted. These factors were labeled as ‘personal integrity’, ‘conservation of energy', ‘structural integrity’, and ‘social integrity'. These factors included 4 phases of health conservation. Cronbach's Alpha of 37 the items was .9424 and the correlation coefficient of HPLP was .723. Conclusion: The researchers recommend the following: An explorative study on the variables related to health conservation is needed for criterion validity of this scale. Studies on health conservation of different age groups, and subjects are needed for verification.

Development of a Stress Scale for Elderly Patients with Coronary Artery Disease (노인 관상동맥질환자의 스트레스 측정도구 개발)

  • Purpose: The purpose of this study was to develop a scale to evaluate stress in elderly patients with coronary artery diseases (CAD) and to examine validity and reliability of the scale. Methods: The development process for the preliminary scale included construction of a conceptual framework and initial items, verification of content analysis, sentence correction, and pilot study. This study was conducted using a questionnaire survey with one-to-one interviews during January and February, 2012. Participants were 240 elderly patients with CAD. Data were analyzed using item analysis, factor analysis, criterion related validity, and internal consistency. Results: The developed scale consisted of 32 items and 6 factors - aging and disease (7 items), family relations (5 items), anxiety and withdrawal (9 items), management of daily living (3 items), compliance of medical regimen (4 items), poverty and finance (4 items), and explained 68.5% of total variance. The scale had significantly positive correlation with the Korean Perceived Stress Scale (KPSS). Cronbach's alpha was .96, and Guttman split half coefficient was .91. Conclusion: Results indicate that the Stress Scale for Elderly Patients with CAD has validity and reliability, and is a suitable scale in health care settings to assess stress in elderly patients with CAD.
