  • 저자들은 좌측 흉수액을 주소로 내원한 63세 여자에서 전산화 단층촬영상 복수와 난소의 종양이 발견되어 조직검사상 난소의 양성섬유종으로 확인되었고, 난소종양의 수술적 제거 후 흉수와 복수가 소실되어 Meigs 증후군으로 진단한 1예를 경험하였기에 이를 보고하는 바이다.

Intrathoracic Desmoid Tumor Mimicking Pleural Mass: A Case Report

  • Desmoid tumor (fibromatosis) is a histologically benign fibrous neoplasm showing locally infiltrating growth. This type of tumor commonly occurs in the abdomen, but intrathoracic desmoid tumor is uncommon. To date, 12 cases of intrathoracic desmoid tumor protruding into the pleural cavity, radiologically mimicking pleural masses, have been reported. Here, we report on a case of intrathoracic desmoid tumor protruding into the pleural cavity, and partially covered by parietal pleura. The main preoperative differential diagnoses included pleural solitary fibrous tumor, inflammatory pseudotumor or malignant mesothelioma. A near-total mass excision was performed. Pathologically, the tumor was composed of a paucicellular arrangement of spindle-shaped cells with fibromyxoid stroma. The resection margin was partially involved with spindle cells present. On histochemical staining, the spindle cells were strongly positive for vimentin and negative for CD34, consistent with a desmoid tumor. The patient was stable without further adjuvant treatment during 6-years of follow-up.

  • Objective: This clinical study reports on a case of generalized edema with hypoalbuminemia and pleural effusion improved by combined traditional Korean and Western medical treatments in a patient with chronic myeloid leukemia medicated with dasatinib.Method: Combined traditional Korean and Western medical treatment was carried out, including herbal medicine, acupuncture, moxibustion, and albumin injection. We examined the serum albumin, measured the circumference of both thighs and ankles to estimate the volume of the edema, and evaluated the pleural effusion symptoms.Results: The serum albumin increased, and generalized edema and pleural effusion improved significantly after the administration of Bojungchiseub-tang-gami.Conclusion: These results showed that complications of generalized edema and pleural effusion associated with dasatinib medication have the potential to be improved via combined traditional Korean and Western medical treatments.

  • Primary malignant fibrous histocytoma(MFH) of pleura is very rare. Upon microscopic examination, the tumor was characterized by storiform pattern of growth and intense, diffuse immunostaining for $\alpha$ l-antichymotripsin. We report a case of primary malignant pleural fibers histiocytoma with extension to lung, which was managed by complete excision of pleural mass and lobectomy with thoracoplasty.

Malignant Pleural Effusion: Medical Approaches for Diagnosis and Management

  • Malignant pleural effusions (MPEs) are the second leading cause of exudative pleural effusions after parapneumonic effusions. In the vast majority of cases, a MPE signifies incurable disease associated with high morbidity and mortality. Considerable advances have been made for the diagnosis of MPEs, through the development of improved methods in the specialized cytological and imaging studies. The cytological or histological confirmation of malignant cells is currently important in establishing a diagnosis. Furthermore, despite major advancements in cancer treatment for the past two decades, management of MPE remains palliative. This article presents a comprehensive review of the medical approaches for diagnosis and management of MPE.

다발성 골수종 1예의 체액 세포학적 소견 (Effusion Cytology of Multiple Myeloma - A Case Report -)

  • A case of multiple myeloma with massive pleural effusion is reported. A 53 year-old previous known multiple myeloma patient vistited our hospital complaining of cough with sputum. Radiologic study revealed multiple osteolytic bony lesions and left side pleural effusion. The effusion were bloody exudates containing numerous atypical plasma cells. The tumor cells showed pleomorphism, eccentric nuclei, prominent nucleoli, perinuclear halo, multincleation, and chromatin patterns of occasional cart-wheel appearance. The cytological examination of pleural fluid established the malignant nature of the effusion with multiple myeloma.

Localized pleural mesothelioma -수술치험 1예- (Localized pleural mesothelioma -A case report-)

  • Pleural mesothelioma is the tumor of the cell of mesodermal origin lining the pleura. It is relatively uncommon tumor and its localized form is much rarer than diffuse form. Authors experienced a localized mesothelioma in a patient who was 44 year old male worker at copper plumbing field for 20 years, and admitted due to incidentally found abnormal chest X-ray. Exploratory thoracotomy was done and a 23 x 16 x 8 cm sized solitary mass was resected with adherent right middle lobe. Low grade malignancy of the pleural mesothelioma was confirmed by the pathology. We report the case with a literature review.

흉막액의 임상적 고찰 (Clinical Evaluation of Pleural Effusion -Report of Cases-)

  • A clinical analysis of pleural effusion was done on 100 patients who were between 5 and 67 years old. The etiologic diagnosis of pleural effusion at the time of discharge were pulmonary tuberculosis in 38 cases[38%],pneumonia in 20 cases[20%],lung carcinoma in 16 cases[16%],liver cirrosis in 9 cases[9%],congestive heart failure in 4 cases[4%],nephrosis in 3 cases[3%],Meig,s syndrom in 3 cases[3%].liver abscess 2 cases[2%],pancreatitis in 1 cases[1%] and other metastatic malignancy in 4 cases[4%]. Their chief complaints was chest pain in 89 cases[89%],dyspnea in 74 cases[74%],coughing in 46 cases[46%], fever in 13 cases[13%],weakness in 12 cases[12%] and dizziness in 11 cases[11%].

흉막에 발생한 다발성 지방종의 치험 1례 (A Case Report of Lipomatosis in the Pleura)

  • Lipomatosis is a condition containing multiple lipomatous masses.Lipoma is a benign neoplasm composed of adult adipose tissue, and occur most often in the fifth or sixth decade and rarely in the pleura. Pleural lipomas are usually asymptomatic and revealed as an incidental roentgenographic findings. The patient was 59 year-old male and admitted because of dyspnea-on-exertion for 30 years. Chest CT revealed right pleural mass abutting on the chest wall, measuring minus 80 hounsfield units. The mass was resected with calcified pleural plaque and confirmed to be lipomatosis with collagenous fibrosis arising from viserai pleura.
