Acceleration of Wound Healing Using Adipose-derived Stem Cell Therapy with Platelet Concentrates: Plateletrich Plasma (PRP) vs. Platelet-rich Fibrin (PRF) (혈소판 농축재제를 이용한 창상치유의 촉진)

  • Purpose: Although platelet-rich plasma (PRP) potentiate the wound healing activity of adipose-derived stem cells (ADSCs), its effect cannot be sustained for a prolonged period of time due to short duration of action. This led us to design and produce platelet-rich fibrin (PRF), in an effort to develop a tool which lasts longer, and apply it on wound healing. Methods: Two symmetrical skin defects were made on the back of seven nude mice. ADSCs were applied to each wound, combined with either PRP or PRF. The wound area was measured over 14 days. By day 16, the wound was harvested and histologic analysis was performed including counting of the blood vessel. Results: The healing rate was more accelerated in PRP group in the first 5 days (p<0.05). However, PRF group surpassed PRP group after 6 days (p<0.05). The average number of blood vessels observed in the PRF group was $6.53{\pm}0.51$, compared with $5.68{\pm}0.71$ for the PRP group. Conclusion: PRF exerts a slow yet pervasive influence over the two-week course of the wound healing process. Thus, PRF is probably more beneficial for promoting the activity of ADSCs for a sustained period of time.

  • Platelets serve many biological functions, including a major role in the haemostatic process. But platelets also play a crucial role in the formation of arterial thrombosis, arteriosclerosis and other pathologic processes. Thus, there have been many studies to develop new antiplatelet agents from foods and plants for decades. Inthis study, inhibitory effects of the oriental onion (Allium fistulosum) on platelet aggregation were investigated using platelet rich plasma (PRP). Water extracts of oriental onion was separated into two fractions (Fraction I and Fraction II) by Sephadex G-150 column. Platelet aggregations were inhibited by total water extracts as well as Fraction I and II. IC50 value of Fraction I was much lower than that of Fraction II. Inhibitory effects of total water extracts of oriental onion on ATP release by PRP were also observed.

  • New techniques for regenerating the destructed periodontal tissue have been studied for many years. Current acceptable methods of promoting periodontal regeneration are basis of removal of diseased soft tissue, root treatment, guided tissue regeneration, graft materials, and biological mediators. Platelet Rich Plasma has been reported as a biological mediator which regulates activities of wound healing progress including cell proliferation, migration, and metabolism. The purpose of this study is to evaluate the effects of using the Platelet Rich Plasma as a regeneration promoting agent for furcation involvement defect. Five adult beagle dogs were used in this experiment. The dogs were anesthetized with Ketamin HCl(0.1 ml/kg, IV)and Xylazine hydrochloride($Rompun^{(R)}$, Bayer, 0.1 ml/kg, IM) and conventional periodontal prophylaxis were performed with ultrasonic scaler and hand instruments. With intrasulcular and crestal incision, mucoperiosteal flap was elevated. Following decortication with 1/2 high speed round bur, degree II furcation defect was made on mandibular third(P3), forth(P4) and fifth(P5) premolar, and stopping was inserted. After 4 weeks, stopping was removed, and bone graft was performed. Ca-P was grafted in P3(experimental group I), Combination of Ca-P and plasma rich platelet were grafted in P4(experimental group II), and P5 was remained at control group.Systemic antibiotics(gentamicin sulfate)and anlgesics(phenyl butazone) were administrated intramuscular for 2 weeks after surgery. Irrigation with 0.1% Chlorhexidine Gluconate around operate sites was performed during the whole experimental period except one day immediate after surgery. Soft diets were fed through the whole experiment period. After 4, 8 weeks, the animals were sacrificed by perfusion technique. Tissue block was excised including the tooth and prepared for light microscope with Gomori's trichrome staining. At 4 weeks after surgery, there were rapid osteogenesis phenomenon on the defected area of the Platelet Rich Plasma plus Ca-P BBP group and early trabeculation pattern was made with new osteoid tissue produced by activated osteoblast. Bone formation was almost completed to the fornix of furcation by 8 weeks after surgery. In conclusion, Platelet Rich Plasma can promote rapid osteogenesis during healing of periodontalregeneration.

  • The successful implantation necessitate tissue regeneration m site of future implant placement, there being severe bone defect. Therapeutic approaches to tissue regeneration in the site have used bone grafts, root surface treatments, barrier membranes, and growth factors, the same way being applied to periodontal tissue regeneration. Great interest in periodontal tissue regeneration has lead to research in bone graft, guided-tissue regeneration, and the administration of growth factors as possible means of regenerating lost periodontal tissue. The blood component separated by centrifuging the blood is the platelet-rich plasma. There are growth factors, PDGF, $TGF{beta}1$, $TGF{beta}2$ and IGF in the platelet-rich plasma. The purpose of this study was to study the histopathological correlation between the use of platelet-rich plasma and the healing of bone defect around implant fixture site. Implant fixtures were inserted and graft materials were placed into the left femur of in the experimental group, while the control group received only implant fixtures. In the first experimental group, platelet-rich plasma and BBP xenograft were placed at the implant fixture site, and the second experimental group had platelet-rich plasma, BBP xenograft, and the e-PTFE membrane placed at the fixture site. The degree of bone regeneration adjacent to the implant fixture was observed and compared histopathologically at 2, 4, and 8 weeks after implant fixture insertion. The results of the experiment were as follows: 1. Bone remodeling in acid etched surface near the implant fixture of all experimental groups was found to be greater than new bone formation. 2. Bone remodeling in acid etched surface distant to the implant fixture of all experimental groups was decreased and new bone formation was not changed. 3. Significant new bone formation in machined surface near the implant fixture of bothl experimental groups was observed in 2 weeks. 4. New bone formation in machined surface distant to the implant fixture of both experimental groups was observed. Bone remodeling was significant in near the implant fixture and not in distant to the implant fixture. The results of the experiment suggested that the change of bone formation around implant. Remodeling in machined surface distant to the implant fixture of both experimental groups, and new bone formation and remodeling near the implant fixture were significant.

  • During the past decade, many studies using platelet-rich plasma (PRP) or adipose-derived stem cells (ASCs) have been conducted in various medical fields, from cardiovascular research to applications for corneal diseases. Nonetheless, there are several limitations of practical applications of PRP and ASCs. Most reports of PRP are anecdotal and few include controls to determine the specific role of PRP. There is little consensus regarding PRP production and characterization. Some have reported the development of an antibody to bovine thrombin, which was the initiator of platelet activation. In the case of ASCs, good manufacturing practices are needed for the production of clinical-grade human stem cells, and in vitro expansion of ASCs requires approval of the Korea Food and Drug Administration, such that considerable expense and time are required. Additionally, some have reported that ASCs could have a potential risk of transformation to malignant cells. Therefore, the authors tried to investigate the latest research on the efficacy and safety of PRP and ASCs and report on the current state and regulation of these stem cell-based therapies.

Effect of Dietary Fatty Acids on Fatty Acid Composition of Platelet Phospholipids, Thromboxane B2 Formation, and Platelet Aggregation in Men (식이 지방산이 혈소판 인지질의 지방산 조성, 혈장 Thromboxane B2의 농도 및 혈소판 응집에 미치는 영향)

  • The degree of platelet aggregation, thromboxane B2(TXB2)formation and fatty acid composition of platelet phospholipids(PL) were investigated in 24 healthy male subjects who for five weeks consumed either corn oil(CO) rich in linoleic acid(LA), perilla oil (PO) rich in $\alpha$-linoleic acid($\alpha$-LAN), or canola oil(CNO) rich in oleic acid(OA) as a major fat source. Total fat intake was 30% of total calories and prescribed oil intake of each dietary group was 50% of the total fat intake. In the CO group, significantly decreased contents of polyunsaturated fatty acids(PUFA), n-6 PUFA, n-3 PUFA and eicosapentanoic acid(EPA) were observed, and significantly increased contents of OA and saturated fatty acids(SFA) were observed in platelet PL after 3 weeks and 5 weeks of dietary treatment. In the PO group, contents of OA and docosahexanoic acid(DHA) were increased, and the ratio of n-6/n-3 was decreased significantly in platelet PL after dietary treatment. The CNO group showed significatnlty decreased contents of PUFA, P/S ratio, n-6 PUFA, LA,(EPA+DHA)/arachidonic acid(AA), and significantly increased SFA contents after 3 weeks of the oil-based diet. The dietary-induced effects on fatty acid composition of platelet PL were observed mostly after 3 weeks of the oil-based diet. The dietary-induced effects on fatty acid composition of platelet PL were observed mostly after 3 weeks. Plasma TXB2 levels were increased after 3 and 5 weeks of dietary treatment. However, only the CO and CNO groups showed significantly increased plasma TXB2 levles after 3 and 5 weeks of dietary treatment. However, only the CO and CNO groups showed significantly increased plasma TXB2 levels after 5 weeks of experimental diets, when compared with initial values. Degree of platelet aggregation increased only in the CO group after dietary treatment. As a result, at week 5 the degree of platelet aggregation of the CO group was significantly higher than those of the PO and CNO groups. Among the three oil-based diets, the PO-based diet seems to have beneficial effects on atherosclerosis by influencing plasma TXB2 levels and the degree of platelet aggregation, while the CO-based diet showed the most adverse effects. Our results imply that plasma TXB2 levels might be affected by dietary fatty acid composition.

The Effect of Platelet-rich Plasma on Wounds of OLETF Rats Using Expression of Matrix Metalloproteinase-2 and -9 mRNA

  • Background : Complicated diabetic patients show impaired, delayed wound healing caused by multiple factors. A study on wound healing showed that platelet-rich plasma (PRP) was effective in normal tissue regeneration. Nonetheless, there is no evidence that when platelet-rich plasma is applied to diabetic wounds, it normalizes the diabetic wound healing process. In this study, we have analyzed matrix metalloproteinase (MMP)-2, MMP-9 expression to investigate the effect of PRP on diabetic wounds. Methods : Twenty-four-week-old male Otsuka Long-Evans Tokushima Fatty rats were provided by the Tokushima Research Institute. At 50 weeks, wounds were arranged in two sites on the lateral paraspinal areas. Each wound was treated with PRP gel and physiologic saline gauze. To determine the expression of MMP-2, MMP-9, which was chosen as a marker of wound healing, reverse transcription polymerase chain reaction (RT-PCR) was performed and local distribution and expression of MMP-2, MMP-9 was also observed throughout the immunohistochemical staining. Results : RT-PCR and the immunohistochemical study showed that the levels of MMP-2, MMP-9 mRNA expression in PRP applied tissues were higher than MMP-2, MMP-9 mRNA expression in saline-applied tissues. MMP-9 mRNA expression in wounds of diabetic rats decreased after healing began to occur. But no statistical differences were detected on the basis of body weight or fasting blood glucose levels. Conclusions : This study could indicate the extracellular matrix-regulating effect observed with PRP. Our results of the acceleration of wound healing events by PRP under hyperglycemic conditions might be a useful clue for future clinical treatment for diabetic wounds.

An Assessment on effect of Bioabsorbable membrane, allogenic bone and Platelet Rich Plasma in Class II furcation involvement by digital subtraction radiography (2급 치근이개부 치료 시 흡수성 차폐막, 동종골 이식 및 혈소판 농축 혈장의 골 재생 효과에 대한 디지털 공제술의 정량적 분석)

  • The purpose of this study was to evaluate effect of platelet rich plasma on the treatment of Grade II furcation involvement, with Demineralized Freeze-Dried Bone(Dembone(R)) and bioabsorable membrane(BioMesh(R)) in humans by digital subtraction radiography. 12 teeth(control group) were treated with Demineralized Freeze-Dried Bone(Dembone(R)) and bioabsorable membrane(BioMesh(R)), and 12 teeth(test group) were treated with Demineralized Freeze-Dried Bone(Dembone(R)), bioabsorable membrane(BioMesh(R)) and Platelet Rich Plasma. The change of bone density was assessed by digital subtraction radiography in this study. The change of mineral content by as much as 5%(vol) could be perceived in the subtracted images. The change of mineral content was assessed in the method that two radiographs are put into computer program to be overlapped and the previous image is subtracted by the later one. Both groups were statistically analyzed by Wilcoxon signed Ranks Test and Mann-whitney Test using SPSS program (5% significance level). The results were as follows: 1. In test group, the radiopacity in 3 months after surgery were significantly increased than 1 month after surgery(p<0.05). However. there were no significant difference between 1 month after surgery and 3 months after surgery in control group(p>0.05). 2. In test and control group, the radiopacity in 6 months after surgery were significantly increased than 1 month after surgery(p<0.05) 3. In test and control group, the radiopacity in 6 months after surgery were significantly increased than 3 months after surgery(p<0.05). 4. There were no significant difference between test group and control group at 1 month, 3 months after surgery, but radiopacity in test group were significantly increased than control group at 6 months after surgery(p<0.05). In conclusion, Platelet Rich Plasma can enhance bone density.

  • The present study evaluated of regeneration effect of platelet rich plasma on the treatment of grade II furcation involvement, with coralline calcium phosphate bone in humans. 30 teeth with grade II furcation defect were selected and 15 teeth(control) were treated with coralline calcium phosphate bone, the others(test) were treated with coralline calcium phosphate bone and platelet rich plasma. Pocket depth, clinical attachment level, width of keratinized gingiva width were measured at baseline, postoperative 3, 6months. from cementoenamel junction to alveolar crest and fundus were measured at baseline, 6months(re-entry). Both groups were statistically analyzed by Wilcoxon signed Ranks Test & Mann-whitney Test using SPSS program(5% significance level). The results were as follows: 1. Pocket depth, clinical attachment level, keratinized gingva width, cementoenamel junction - alveolar crest, cementoenamel junction - fundus were not differ significantly in both groups at baseline 2. The change of pocket depth, clinical attachment level, keratinized gingva width, cementoenamel junction - alveolar crest, cementoenamel junction - fundus in both groups were decreased significantly at 3, 6months(p<0.05). 3. The change of pocket depth, clinical attachment level in test group decreased significantly than control at 3, 6months(p<0.05). 4. The change of keratinized gingiva width, cementoenamel junctional - alveolar crest, cementoenamel junction - fundus were not differ significantly in both groups at 3, 6months. 5. The pocket depth, clinical attachment level, keratinized gingiva width exhibited marked changes at 3 months in both groups. In conclusion, the results of this study suggest that platelet rich plasma have adjunctive clinical treatment effect to periodontal regeneration in grade II furcation defects.
