Purpose: This is a mixed method study to understand the experience regarding the job satisfaction of Physician Assistant (PA) male nurses. Methods: First, factors influencing job satisfaction were analyzed using quantitative approach and job adaptation processes by qualitative research was done through interviews. Finally both of the collected data were comprehensively compared. Results: The factors which had the greatest influence on job satisfaction in the regression analysis were job stress (${\beta}=-.49$, p<.001) and job identity (${\beta}=.46$, p<.001) and the explanatory power explaining these variables was 58% (Adj. $R^2=.58$). The results of qualitative research were categorized using the same four variables with qualitative research variables. Five themes in job satisfaction, three themes in job identity, five themes in job stress and six themes in job value were identified by content analysis. Conclusion: This finding identified that the job stress and job identity were important variables related to job satisfaction. Therefore, the factors influencing job identity have to be strengthened, and a systematic management and interest to develop them are necessary.
Purpose: This study was a qualitative study done to investigate the experience of role conflict by physician assistant nurses. Methods: The basis for this study was qualitative research using the phenomenological method. Research participants were 10 physician assistant nurses. Data collection methodology was in-depth interviews. The interviews were conducted 1 to 2 times and each interview lasted within the range of 45 minutes to 2 hours. The data collection and analysis were carried out simultaneously and the Colaizzi (1978) methodology was adopted for data analysis. Results: The results showed 46 significant statements, 13 meaningful statements, 7 themes, and 3 clusters of themes. The 3 clusters of themes were: 'Identity conflict', 'Relationship role conflict' and 'Institutional role conflict'. Conclusion: The findings from this study suggest that legal status guarantees have to be prepared in order to resolve the role conflict of physician assistant nurses.
본 연구는 전담간호사의 운영현황 및 업무 실태를 전수조사 하여 전담간호사 관련 정책 수립의 기초 자료를 제공하기 위해 시도되었다. 총 318개의 종합병원 이상의 의료기관 부서장에게 설문조사를 시행했고, 141개 병원에서 근무하는 704명의 전담간호사를 대상으로 2011년 4월부터 5월에 자료수집이 시행되었으며, 수집된 자료는 SPSS 12.0 program을 이용하여 분석하였다. 연구결과 전담간호사의 평균연령은 32.5세이며, 교육수준은 대졸이 가장 많았고, 전문간호사 자격증 보유자는 13.4%이었고, 전담간호사의 전반적인 업무 만족도는 5점 만점에 평균 3.13점이었으며, 불명확한 업무 구분이 전담간호사를 가장 힘들게 하는 요인으로 나타났다. 전담간호사 역할정립을 위해 업무에 대한 법적 보호 장치 마련(5점 만점에 평균 4.71)이 가장 시급한 것으로 나타났다. 현재 전담간호사 실태는 열악한 근무환경으로 인한 낮은 만족도, 부적합한 자격과 업무에 대한 법적 보호장치가 마련되어 있지 않은 속에서 어려움을 겪고 있다. 향후 전담간호사의 활용도를 향상시키기 위해 전담간호사를 법적으로 보호할 수 있는 장치를 마련하고, 교육 프로그램을 개설하며, 한국 실정에 맞는 전담간호사 제도 확립이 절실하다고 할 수 있겠다.
In this paper, we propose a privacy-preserving emergency call scheme, called PEC, enabling patients in life-threatening emergencies to fast and accurately transmit emergency data to the nearby helpers via mobile healthcare social networks (MHSNs). Once an emergency happens, the personal digital assistant (PDA) of the patient runs the PEC to collect the emergency data including emergency location, patient health record, as well as patient physiological condition. The PEC then generates an emergency call with the emergency data inside and epidemically disseminates it to every user in the patient's neighborhood. If a physician happens to be nearby, the PEC ensures the time used to notify the physician of the emergency is the shortest. We show via theoretical analysis that the PEC is able to provide fine-grained access control on the emergency data, where the access policy is set by patients themselves. Moreover, the PEC can withstandmultiple types of attacks, such as identity theft attack, forgery attack, and collusion attack. We also devise an effective revocation mechanism to make the revocable PEC (rPEC) resistant to inside attacks. In addition, we demonstrate via simulation that the PEC can significantly reduce the response time of emergency care in MHSNs.
Professional identity formation (PIF) is an essential concept in professional education. Many scholars have explored conceptual frameworks of PIF and conducted empirical studies to advance an understanding of the construct in medical education. Despite its importance, it is unclear what educational approaches and assessment practices are actually implemented in medical education settings. Therefore, we conducted a literature review of empirical studies reporting educational practices for medical learners' PIF. We searched the Web of Science database using keywords and chose 37 papers for analysis based on inclusion and exclusion criteria. Thematic analysis was conducted. Most empirical papers (92%) were from North America and Western Europe and used qualitative research methods, including mixed methods (99%). The papers reported the use of reflection activities and elective courses for specific purposes, such as art as an educational activity. Patient and healthcare experiences were also found to be a central theme in medical learners' PIF. Through an iterative analysis of the key themes that emerged from the PIF studies, we derived the following key concepts and implications: (1) the importance of creating informal and incidental learning environments, (2) ordinary yet authentic patient experiences, (3) a climate of psychosocial safety in a learning environment embracing individual learners' background and emotional development, and (4) the reconceptualization of PIF education and assessment. In conclusion, research on PIF should be diversified to include various cultural and social contexts. Theoretical frameworks should also be diversified and developed beyond Kegan's developmental framework to accommodate the nonlinear and dynamic nature of PIF.
Medical errors such as adverse drug event, improper transfusion, wrong-site surgery, mistaken patient identity and so on commonly occur at health care practice. Information technology, like Drug Utilization Review(DUR) system which reviews, analyzes, and interprets medication data when prescribing, can play a key role in reducing such medical errors and improving patient safety. Korean Government has guided all hospitals to implement concurrent DUR(cDUR) system, which is the first case worldwide in that all healthcare providers have to use cDUR system when prescribing. This paper introduced a case study that a tertiary hospital has integrated the cDUR system into its comprehensive Hospital Information System(HIS) and analyzed the whole prescription data during a week right after adoption of cDUR system. Considering technical strength and weakness, the cDUR system was integrated into the HIS, using Broker Servers for minimizing doctors' anxiety. As the quantitative analysis of the whole prescription data, DUR conflict events, which mainly included duplicate medications and contra-indicated drug interactions for outpatients, were 2.77%. Although only 0.7% is for the contra-indicated drug interactions, it will be greatly devoted to achieve the purpose of DUR such as improving patient safety.
The treatment of Patients with bacteraemia and septicemia requires accurate and rapid identification of the pathogen so that the physician can be guided regarding the selection of the proper antimicrobial therapy. The usual procedure is to withdraw an aliquot of the positive blood culture sample for gram staining and subculturing on the media for the growth and subsequent identification, and susceptibility determinations. It was noticed that during the process some microbiologists would sniff the effluent gases that are products of metabolism and in some cases guess the identity of the bacterium. That Prompted us to engage in systematic investigation of two gram positive and two gram negative bacteria using an electronic nose that had been proven successful in distinguishing the aroma of coffee beans from different sources. The investigation was successful in illustrating the efficacy of such a device in this clinical setting to distinguish Escherichia coli, Pseudomonas aeruginosa, Staphylococcus aureus and Enterococcus faecalis. 본 연구는 하인즈 코헛의 자기(Self)이론을 기반으로, 자기대상(Selfobject) 기능을 상실한 내담자의 치료적 활성화를 위해 상담자의 전이대상 역할을 기술하였다. 본 연구에서 인간은 평생을 통해서 자기대상이 필요함을 확인하는 계기가 되었다고 할 수 있다. 다음으로는 변형적 내재화 과정을 언급하면 유아는 자기대상을 통한 최적의 좌절로 인하여 부모의 한계를 체험하면서 환상의 세계에서 현실로 돌아오게 된다. 전이대상으로 상담자 역할은 내담자가 응집적 자기를 확립하여, 적절한 자기대상을 선용하도록 하여야 한다. 그리고 내담자의 과대주의와 과시주의를 공감하고 상담자는 이상화 대상이 되어서 내담자가 인정받고 동일시할 수 있는 기회를 주어야 한다. 상담자는 상담 과정에서 내담자에게 최적의 좌절을 제공할 수 있다. 이때 상담자는 자신의 실수를 인정할 때 내담자는 상담자를 현실적으로 바라보게 되고 건강한 자기를 구축하여 변형적 내면화를 이룩하게 된다. 상담을 통해서 내담자에게 응집적 자기가 형성되면 타인을 공감하고 건강한 인간관계를 형성할 수 있다. 그러면 자신의 감정을 통제할 수 있고 비전을 가지게 된다. 그리고 내담자는 삶을 살아가면서 일생동안 적절한 대상과 관계를 맺으면서 살아갈 수밖에 없다고 인식하게 된다.
본 연구는 사회복지현장실습이 전문직업인으로서 성장하는데 있어 진로의식에 미치는 영향력을 검증하기 위하여 진행되었다. 연구대상은 G지역 4년제 대학교 사회복지전공학생 198명을 목적표집에 의하여 조사하였으며, 자기기입식 설문을 통하여 진행하였다. 분석방법은 대상 및 변수의 특성을 파악하기 위해 빈도 및 기술분석, T-test, F-test, ANOVA, 상관관계(correlation), 회귀분석(regression)을 위해 SPSS 14.0을 사용하였다. 연구결과는 다음과 같다. 첫째, 개인요인으로 실습생이 개인의 의사가 반영(${\beta}=.197$)되는 것과 사회복지학 전공을 선택한 것에 대해 만족감(${\beta}=.205$)이 사회복지전공학생의 진로정체감에 유의미한 영향을 미쳤다. 둘째, 개인요인으로 자신이 사회복지학을 전공으로 선택한 것에 만족감(${\beta}=-.291$)을 느끼는 경우와 부모가 사회복지학을 전공으로 선택한 것에 만족감을 느끼는 경우 사회복지전공학생의 진로결정수준에 유의미한 영향을 미쳤다. 셋째, 개인요인으로 개인의사 반영(${\beta}=.156$)이 진로준비행동에 유의미한 영향을 미쳤다.
The objective of the present study was to determine the prevalence and type of nutritional ergogenic aids use, and to determine the frequency, reasons for use of nutritional ergogenic aids. Thirty-four male bodybuilders (mean age = 27.0 years), twenty-four male weight lifters (mean age = 20.9 years) participated in the study. Participants completed a comprehensive survey detailing their usage patterns. In this study, 78.1% of bodybuilders and 79.2% of weight lifters reported using nutritional ergogenic aids. The most frequently taken nutritional ergogenic aids, in ranking order, were protein/amino acid powders (79.4%), multivitamin/minerals (67.7%) and creatine (67.6%) for bodybuilders, in contrast to sports drinks (100.0%), protein/amino acid powders (50.5%) and creatine (50.5%) for weight lifters. Over the half of the respondents, 79.4% of bodybuilders and 50.6% of weight lifters, used protein/amino acid powders to gain muscle mass and to stay healthy. Bodybuilders, 67.6% and weight lifters, 41.7%, used multivitamin/minerals to stay healthy and for energy. The intakes of most vitamin and minerals through diet and nutritional ergogenic aids were much greater than RDA. Vitamin $B_1$, vitamin $B_2$, niacin, vitamin $B_6$ and folate intakes were ranged at 400-900%. Vitamin C intake was 1285.4% (for bodybuilders) and 1322.6% (for weight lifters). The correct answer rate of nutritional ergogenic aids was 46.0% for bodybuilders and 52.0% for weight lifters. Both bodybuilders and weight lifters took highly nutritional ergogenic aids and it tended to be taken irrespective of scientific background. Specific sport nutrition education applicable to athletes, especially strength athletes, is recommended. The findings of this investigation could be used to enable the professionals (sports dietician and physician) to identity common misconceptions regarding nutritional ergogenic aids and to implement educational programs.
