A Clinical Report on One Case of Nuclear Facial Nerve Paralysis (핵성(核性) 안면신경마비(顔面神經麻痺) 환자 증례 1례(例))

  • Facial nerver paralysis is classified as Supranuclear, Peripheral and Nuclear. It is mostly Spontaneous Peripheral Facial Paralysis(Bell's palsy) or Supranuclear Paralysis by C.V.A, but Nuclear Facial Nerve Paralysis is rarely reported. We treated a 64-years-old female patient who had 7-years history of C.V.A with hypertension and heart disease, and complained of these symptoms; left facial palsy, ocular dysmetria, diplopia, and right extremity weakness. We diagnosed as direct attack from the wind pathogen(風邪入中) with deficiency of both Gi(Qi, vital energy) and blood(氣血雨虛), and employed Oriental medical treatments; herb-medication, acupuncture and moxa therapy. The result was relatively acceptable. So We report this case with a brief review of related literatures.

Clinical Research of Korean Medical Treatment for the Peripheral Facial Paralysis (안면신경마비의 치료와 관련된 한의학 보고들에 대한 고찰)

  • Objectives : The aim of this study is to propose the treatment of Peripheral Facial paralysis using Korean medical treatment methods as acupuncture, Phmacopuncture, medicine and etc.Methods : Papers were searched in internet sites such as Nation Digital Science Library, Koreanstudies Information Service System, Research Information Sharing Service and The National Assembly Library with title and abstract ʻBellʼs palsyʼ or ʻFacial paralysisʼ and ʻphamacopunctureʼ or ʻbee venomʼ orʻhominis placentaʼetc.Results : 18 papers using phamacopuncture were selected. Most of all is used Bee Vemon Phamcopuncure and all of them used Acupuncture and Medicine therapies. And Groups used Phamacopuncture such as ʻBee Venomʼ,ʻHominis Placentaʼ, ʻSoyeomʼ, ʻBUMʼ,ʻJungsongoulhyulʼetc are statistically improved compared to unused Groups.Conclusions : Acupuncture and Phamacopuncture are useful methods to improve the Peripheral Facial Paralysis, but further researchs are needed.

Comparative Study of BUM Pharmacopuncture and Soyeom Pharmacopunture on Peripheral Facial Paralysis with Postauricular Pain (이후통을 동반한 말초성 안면신경마비환자에 대한 BUM약침과 소염약침의 비교연구)

  • Objectives : The purpose of this study is to evaluate the effect of BUM pharmacopuncture and Soyeom pharmacopuncture on postauricular pain accompanied peripheral facial paralysis. Methods : We observed 31 peripheral facial paralysis patients with postauricular pain at Oriental Hospital of Semyung University from Jan. 1st, 2010 to Feb. 29th, 2012. One group(group A) was treated BUM Pharmacopuncture in the posterior ear with general oriental medical treatment and the other(group B) was treated Soyeom pharmacopuncture in the posterior ear with general oriental medical treatment. Results : 1. Postauricular pain was significantly decreased in each group but no significant difference between two groups. 2. Facial paralysis was significantly improved in each group and significant difference in group B. Conclusions : In peripheral facial paralysis patients with postauricular pain, both BUM pharmacopuncture and Soyeom pharmacopuncture were effective on postauricular pain.

Effect of Scratching Method Complex Therapy on the Patients with Peripheral Facial Paralysis (말초성 안면신경마비에 대한 괄법(括法) 복합치료 효과)

  • Objectives : This study was done for reporting the effect of scratching method on patients with peripheral facial palsy. Methods : We investigated 34 cases of patients with peripheral facial paralysis, and devided patients into two groups : We treated one group by complex oriental medical treatment with scratching method therapy, and did the other group by complex oriental medical treatment without scratching method therapy. gross grading system of House-Brackmann(HB score) and Yanagihara's unweighed grading system(Y score) at baseline and final were used for evaluating the effect of the treatment. Results : 1. In scratching method group and non-scratching method group, compared with baseline, at final, HB score was significantly decreased and Y score was significantly increased. 2. At final, scratching method group showed significant decrease on HB score and significant increase on Y score compared with non-scratching method group. Conclusions : scratching method showed significant improvement in peripheral facial palsy.

Effect of Bloodletting Therapeutics Complex Therapy on Peripheral Facial Paralysis Patients with Back of the Affected Ear Pain (이후통(耳後痛)을 동반한 말초성 안면신경마비에 대한 자락요법(刺絡療法) 복합치료 효과)

  • Objectives : This study was done for reporting the effect of bloodletting therapeutics on peripheral facial paralysis patients with back of the affected ear pain. Methods : We investigated 37 cases of peripheral facial paralysis patients with postauricular pain, and devided patients into two groups : We treated one group by complex oriental medical treatment with bloodletting therapeutics, and did the other group by complex oriental medical treatment without bloodletting therapeutics. Visual analogue scale(VAS) and gross grading system of House-Brackmann(H-B grade) and at baseline and final were used for evaluating the effect of the treatment. Results : 1. In bloodletting therapeutics group and non-bloodletting therapeutics group, compared with baseline, at final, VAS and H-B grade was significantly decreased. 2. At final, bloodletting therapeutics group showed significant decrease on VAS and H-B grade compared with non-bloodletting therapeutics group. Conclusions : Bloodletting therapeutics showed significant improvement in peripheral facial palsy with postauricular pain.

The Clinical Research of the Effectiveness of "Danmuji Anchu Traction technique" on Acute Peripheral Facial Paralysis (초기 말초성 안면신경마비에 대한 단무지 안추 신전법 병행 치료 효능의 임상적 고찰)

  • Objectives : The aim of this study is to investigate the effectiveness of combining "Danmuji Anchu Traction technique" on acute peripheral facial paralysis. Methods : Clinical studies were done 36 patients who were treated acute peripheral facial paralysis to Dept. of Acupuncture and Moxibustion, of Oriental Medicine Hospital Dongeui University from July 10, 2009 to September 15, 2010. subjects were randomly divided into 2 groups. : Complex oriental medical treatment without "Danmuji Anchu Traction technique" treated group (Group A, n=18), Complex oriental medical treatment with "Danmuji Anchu Traction technique" treated group (Group B, n=18). All process of treatment were performed by double blinding method. To compare the effectiveness of treatment applied for two groups, we used Yanagihara's unweighed grading system at before treatment, after 1week and 2weeks of treatment. Results: The Yanagihara's Scores of Group B and Improvement Indexes of Group B were higher than those of Group A, and showed significant difference statistically. "Danmuji Anchu Traction technique" can be available for relieving symptoms related with Acute Peripheral Facial Paralysis. And there were significant differences statistically between Complex oriental medical treatment without "Danmuji Anchu Traction technique" and Complex oriental medical treatment with "Danmuji Anchu Traction technique" on Acute Peripheral Facial Paralysis. Conclusions: These results suggested that "Danmuji Anchu Traction technique" effected for Acute Peripheral Facial Paralysis.

The influence of complex traditional Korean medical treatment on the Peripheral Facial Paralysis induced stress (한방복합치료(韓方複合治療)가 STRESS로 유발된 말초성안면신경마비(末梢性顔面神經麻痺)에 미치는 영향)

  • Objective: This study was to evaluate the influence of stress on Peripheral Facial Paralysis during the complex traditional korean medical treatment. Methods: We investigated 41 cases of patients with Peripheral Facial Paralysis who were given the complex traditional korean medical treatment. one group had stress as main factor, the other group didn't it. we evaluated the treatment effect of each group by using Gross Grading System of House-Brackmann, Yanagihara's Unweighed Grading System. Results : 1. In age, sex, lesion, duration of disease, frequency of treatment, duration of treatment, we found that two groups have no significant differences. 2. Pain back of the ear showed the highest frequency in symptoms at onset. 3. Boyangwhanotang showed the highest frequency in oriental herb medicine treatment. 4. As a result of evaluation by using Gross Grading System of House-Brackmann, Yanagihara's Unweighed Grading System, treatment score after 2weeks was marked more higher than score before treatment and treatment score after final treatment was more higher than treatment score after 2weeks on each groups. 5. After final treatment, Non-stress group had signficant result on Gross Grading System of House-Brackmann, Yanagihara's Unweighed Grading System compared with stress group. Conclusion : These results suggested that patient with Non-stress group should be get better than stress group.

The clinical research of the efficacy of bee venom aqua-acupuncture on peripheral facial paralysis (봉독(蜂毒) 약침(藥鍼)이 구안와사에 미치는 영향에 대한 임상적 고찰)

  • Objective: Ths research was to investigate the efficacy of bee venom theraphy on peripheral facial paralysis. Methods: Thirty patients were deyided into two groups with fifteen patients each. Group A and B were both treated with basic oriental medicine treatment, and also group A was treated with bee venom aqua-acupunture additionaly. The groups were evaluated by Yanagihara's unweighted grading system before treatment, and at every one week till four weeks after treatment. Results: The Yanagihara's scores of group B were higher than those of group A before treatment, after one week and two weeks, but not statistically significant. After three and four weeks, the Yanagihara's scores of group A were higher than those of group B, but not statistically significant. The improvement indexes of group A were higher than those of group B from the first week and statistically significant after three and four weeks. Conclusion: Combinding bee venom aqua-acupunture theraphy on peripheral facial paralysis was more efficacious than the only use of basic oriental medicine treatment.

  • Objectives : This study was to compare the effect of acupuncture on peripheral facial nerve paralysis between affected part, and unaffected part in the incipient stage. Methods : We investigated 32 cases of patient with peripheral facial nerve paralysis, and divided into two groups. One group was treated by acupuncture on the affected part, and the other was treated by acupuncture on the unaffected part. Both groups were evaluated by Yanagihara's unweighted grading system and House-Backmann grade before treatment, and in every one week and after treatment. Results & Conclusion : 1. The number of female was more than that of male and the number of left side was more than that of right side in distribution of lesion. Post ear pain was the highest related symptom at onset, Unknown was a major cause of peripheral facial nerve paralysis in distribution of cause. 2. Yanagihara's unweighted grading system and House-Backmann grade score between two groups were not statistically significant in every one week. 3. Yanagihara's unweighted grading system and House-Backmann grade score between two groups were not statistically significant in before treatment and final.

  • Objectives : This study was designed to evaluate the effect of treatment Acute peripheral facial paralysis patient with postauricular pain by Bee Venom Pharmacopuncture Methods : This report have been observed among thirty patients with peripheral facial paralysis patient with postauricular pain who admitted to oriental medicine hospital with Daejeon university During 11-01-2008 to 08-31-2009. These patients were diveided into two groups; One was control group that was treated general oriental medical treatment(Group I) and The other was treated Bee Venom Pharmacopuncture in the posterior ear with general oriental medical treatment. (Group II) Results : 1. Group II was more effective than Group I in the VAS score after 1st, 2nd, 3rd Bee Venom Pharmacopuncture treatmenton posterior ear about postauricular pain. 2. In Group II compared with Group I, postauricular pain duration was reduced. 3. As a result of evaluation by using Yanagihara score, they were not significant score within two groups after final treament. Conclusions : Bee Venom Phannacopuncture on peripheral facial paralysis patient with postauricular pain in the posterior ear was more effective in reducing the pain.
