Effect of Dialysis Nurses' Perception of Patient Safety Risk Factors and Patient Safety Competency on Safety Nursing Activities (혈액투석실 간호사의 환자안전에 대한 위험요인 인식과 역량이 안전간호활동에 미치는 영향)

  • Purpose: The study was conducted to investigate the correlation between perception of patient safety risk factors, patient safety competency, and safety nursing activities of nurses in hemodialysis units and identify factors affecting patient safety activities. Methods: The participants were 146 nurses from 16 hemodialysis units located in Gyeongsangnam-do. Data were analyzed using descriptive statistics, independent t-test, one-way ANOVA, Pearson correlation coefficient, and multiple regression analysis using the SPSS, version 24.0. Results: The mean safety nursing activity score was 3.47±0.38. safety nursing activities of the participants were significantly correlated with patient safety competency. The characteristics showing significant differences in safety nursing activities were educational level, hospital type, hospital work experience, number of hemodialysis treatment per day, number of hemodialysis treatment per nurse, educational experience of patient safety, presence of a patient safety incident report registration system, and direct registration of patient safety incident report. The multiple regression analysis revealed that the factors influencing safety nursing activities were patient safety incident report, patient safety competency, and number of daily hemodialysis treatment (<5~7 times/day) per nurse (R2=.34). Conclusion: The results of this study suggest that the safety nursing activities of hemodialysis unit nurses should be intensified. In addition, the registration system of patient safety incident report and nurses' competency on patient safety should be improved, and the number of hemodialysis per nurse should be fewer than 7 times per day.

Analysis of Traffic Characteristics of General National Roads by Snowfall in Gangwon-do (강원도에서 적설에 의한 일반국도 교통 특성 분석)

  • To investigate the effect of snowfall on the traffic of general roads in Gangwon-do, case analysis was performed in Gangneung, Pyeongchang, and Chuncheon using ASOS (Automated Synoptic Observing System) snowfall data and VDS (Vehicle Detector System) traffic data. First, we analyzed how much the traffic volume and speed decrease in snowfall cases on regional roads compared to non-snow cases, and the characteristics of monthly reduction due to snowfall were investigated. In addition, Pearson correlation analysis and regression analysis were performed to quantitatively grasp the effect of snowfall on traffic volume and speed, and sensitivity tests for snowfall intensity and cumulative snowfall were performed. The results showed that the amount of snowfall caused decrease both in the traffic volume and speed from usual (non-snowfall) condition. However, the trend was different by region: The decrease rate in traffic volume was in the order of Gangneung (17~22%), Chuncheon (14~17%), and Pyeongchang (11~14%). The decrease rate in traffic speed was in the order of Chuncheon (9~10%), Gangneung (8~9%), Pyeongchang (5~6%). No significant results were found in the monthly decrease rate analysis. In all regions, traffic volume and speed showed a negative correlation with snowfall. It was confirmed that the greater the amount of traffic entering the road, the greater the slope of the trend line indicating the change in snowfall due to the traffic volume. As a result of the sensitivity test for snowfall intensity and cumulative snowfall, the snowfall information at intervals of 6-hours was the most significant.

Comparison between Korean Triage and Acuity Scale and Injury Severity Scoring System in Emergency Trauma Patients (외상환자의 한국형 중증도 분류와 손상중증도 점수체계의 비교)

  • Purpose: We compared the Korean Triage and Acuity Scale (KTAS), Injury Severity Score (ISS), and Revised Trauma Score (RTS) determined the validity of KTAS for classifying trauma patients. Methods: A retrospective chart review of 10,865 trauma patients (aged ≥15 years) who visited a single regional trauma and emergency medical center from January 1, 2016, to December 31, 2020, was conducted. Data were collected from the Korean Trauma Data Bank. Based on KTAS classification, the rates of intensive care unit admission, surgery and intervention, transfusion, emergency room (ER) and hospital mortality, and ER stay time were investigated. Data were analyzed using Chi-square test, Pearson's correlation coefficient, receiver operating characteristic curve, and area under the ROC curve. Results: In the KTAS, severe trauma patients (ISS ≥16) were classified as Level 1 (79.6%), 2 (44.8%), 3 (15.5%), 4 (4.0%) and 5 (7.6%). The following were the predictive powers of KTAS, ISS, and RTS for different parameters: surgery and intervention rate, KTAS (.71), ISS (.70), and RTS (.63); transfusion rate within 4h, KTAS (.82), ISS (.82), and RTS (.74); ER stay time within 90 min, KTAS (.72), ISS (.62), and RTS (.56); and ER mortality, KTAS (.84), ISS (.72), and RTS (.88). These findings were statistically significant (p<.001). The sensitivity and specificity of KTAS for trauma patients were .88 (.87~.90), and .38 (.37~.39), respectively. Conclusion: KTAS is a useful classification system that can predict the clinical outcomes of patients with trauma, and effectively triage acutely ill trauma patients, thus provide appropriate treatment.

Hybrid adaptive neuro fuzzy inference system for optimization mechanical behaviors of nanocomposite reinforced concrete

  • The application of fibers in concrete obviously enhances the properties of concrete, also the application of natural fibers in concrete is raising due to the availability, low cost and environmentally friendly. Besides, predicting the mechanical properties of concrete in general and shear strength in particular is highly significant in concrete mixture with fiber nanocomposite reinforced concrete (FRC) in construction projects. Despite numerous studies in shear strength, determining this strength still needs more investigations. In this research, Adaptive Neuro-Fuzzy Inference System (ANFIS) have been employed to determine the strength of reinforced concrete with fiber. 180 empirical data were gathered from reliable literature to develop the methods. Models were developed, validated and their statistical results were compared through the root mean squared error (RMSE), determination coefficient (R2), mean absolute error (MAE) and Pearson correlation coefficient (r). Comparing the RMSE of PSO (0.8859) and ANFIS (0.6047) have emphasized the significant role of structural parameters on the shear strength of concrete, also effective depth, web width, and a clear depth rate are essential parameters in modeling the shear capacity of FRC. Considering the accuracy of our models in determining the shear strength of FRC, the outcomes have shown that the R2 values of PSO (0.7487) was better than ANFIS (2.4048). Thus, in this research, PSO has demonstrated better performance than ANFIS in predicting the shear strength of FRC in case of accuracy and the least error ratio. Thus, PSO could be applied as a proper tool to maximum accuracy predict the shear strength of FRC.

Factors Affecting Fall-Prevention Behavior of Long-Term Care Nurses (요양병원 간호사의 낙상예방행위에 미치는 영향요인)

  • Purpose: The purpose of this study was to identify factors influencing fall-prevention behaviors of nurses working in long-term care hospitals. Methods: Participants included 147 nurses working in 10 long-term care hospitals in B city. Data were collected from September 20-October 12, 2016. SPSS/WIN 21.0 was used for analysis with t-test, ANOVA, Scheffé test, Pearson correlation coefficients, and multiple regression. Results: It was found that attitude toward fall (r=.29, p<.001) and patient safety culture (r=.25, p=.002) had a significant positive correlation with fall-prevention behaviors of nurses working in long-term care hospitals. The factors influencing fall-prevention behaviors in participants were clinical career and patient safety culture (β=.21, p=.012), contributing to 19% of the total variance in fall- prevention behaviors. Conclusion: The findings showed that systematic delivery of differentiated fall prevention education is preferred to nurse's clinical career as a private factor to improve fall-prevention behaviors of nurses in long term care hospital. Particularly, it is imperative to conduct periodical and practical fall-prevention education for nurses to prevent career discontinuity. An independent report system and open communication system as well as a scheme that can disseminate patient safety culture in individual departments to implement patient direct nursing are required to encourage patient safety culture in organizations.

Correlation Analysis between Rating Time and Values for Time-aware Collaborative Filtering Systems

  • In collaborative filtering systems, the item rating prediction values calculated by the systems are very important for customer satisfaction with the recommendation list. In the time-aware system, predictions are calculated by reflecting the rating time of users, and in general, exponentially lower weights are assigned to past rating values. In this study, to find out whether the influence of rating time on the rating value varies according to various factors, the correlation between user rating value and rating time is investigated by the degree of user rating activity, the popularity of items, and item genres. As a result, using two types of public datasets, especially in the sparse dataset, significantly different correlation index values were obtained for each factor. Therefore, it is confirmed that the influence weight of the rating time on the rating prediction value should be set differently in consideration of the above-mentioned various factors as well as the density of the dataset.

An experimental study on the size selectivity of fusiform fish by the netting characteristics (망지 특성에 따른 방추형 어류의 크기 선택성에 대한 실험적 연구)

  • This study quantitatively evaluated size selectivity for three netting shapes (T0; regular, T45, T90) and hanging ratio (35%, 70%) of T0 netting used for trawl codend. The size selectivity experiment was performed in a tank using a cube experimental model with a length of 50 cm on one side and 389 experimental individuals, jack mackerel (Trachurus japonicus). In the selectivity analysis, a selectivity curve was created based on the selection ratio using a logistic function, and the 25%, 50%, and 70% selection length and selection range (SR) were obtained. The T0 netting was 19.54 cm when the 50% selective length, which is a selectivity evaluation index, had a hanging ratio of 35%, a selection range of 0.51 cm, and 22.70 cm and 3.08 cm for the hanging ratio of 70%. The T45 netting was 24.34 cm and 2.13 cm, and the T90 netting was 23.51 cm and 2.84 cm. The results of the T45 netting and the T90 netting are similar, and the 50% selection length and selection range were relatively larger than the T0 netting. There was a significant difference in the correlation between the circumference of the inner circle of the mesh by the shape of the netting and the body girth of the experimental individual (Pearson test, r = 0.86, p < 0.05). There was no significant difference in the correlation between the selection ratio by the T0 netting, T45 netting, and T90 netting with a 70% hanging ratio (one-way ANOVA, p > 0.05). The results of this study showed that selectivity such as T45 netting and T90 netting appeared when the hanging ratio, which maximizes the area of T0 netting, was maintained at 70%.

Comparison of Arterial Oxygen Saturation Measured by Pulse Oximetry at Different Sensor Sites in Neurocritical Patients (신경계 중환자의 측정부위별 맥박 산소포화도의 비교)

  • Purpose : This study aimed to compare peripheral pulse oxygen saturation (SpO2) values, measured at different monitoring sites, and arterial oxygen saturation (SaO2) of neurocritical patients. Methods : The study included 110 patients admitted to the neurosurgical intensive care unit of a university hospital. The patients' SpO2 values were measured in their index fingers, both second toes, both earlobes, and foreheads, using the patient monitoring system. These values were compared with the standard value of SaO2 measured using a blood gas analyzer. Data were analyzed using descriptive values, Pearson's correlation coefficients, Lin's concordance correlation coefficients (CCC), and Bland-Altman plots. Result : Regardless of the measuring site, SpO2 was correlated with the paired measurements of SaO2 (r=.40~.60, p<.001, CCC range=.40~.58). No significant bias in paired measurements of SpO2 and SaO2 was observed at all sites (-0.06~0.19%, p>.05). SpO2 values at the left finger and right earlobe had the narrowest range, with a 95% limits of agreement (LOA) (left finger -3.04~2.93% and right earlobe -3.18~2.79%). SpO2 at the index finger, on the side without an arterial catheter, had a narrower range of 95% LOA than that of the opposing finger (-3.00~2.97% vs. -3.73~3.26%). Conclusion : SpO2 at the finger without an arterial catheter had the highest level of precision. This study suggests using the index finger, on the side without an arterial catheter, for pulse oximetry in neurocritical patients.

Time Perception and Memory in Mild Cognitive Impairment and Alzheimer's Disease: A Preliminary Study

  • Background and Purpose: Episodic memory is a system that receives and stores information about temporally dated episodes and their interrelations. Our study aimed to investigate the relevance of episodic memory to time perception, with a specific focus on simultaneity/order judgment. Methods: Experiment 1 employed the simultaneity judgment task to discern differences in time perception between patients with mild cognitive impairment or dementia, and age-matched normals. A mathematical analysis capable of estimating subjects' time processing was utilized to identify the sensory and decisional components of temporal order and simultaneity judgment. Experiment 2 examined how differences in temporal perception relate to performance in temporal order memory, in which time delays play a critical role. Results: The temporal decision windows for both temporal order and simultaneity judgments exhibited marginal differences between patients with episodic memory impairment, and their healthy counterparts (p = 0.15, t(22) = 1.34). These temporal decision windows may be linked to the temporal separation of events in episodic memory (Pearson's ρ = -0.53, p = 0.05). Conclusions: Based on our findings, the frequency of visual events accumulated and encoded in the working memory system in the patients' and normal group appears to be approximately (5.7 and 11.2) Hz, respectively. According to the internal clock model, a lower frequency of event pulses tends to result in underestimation of event duration, which phenomenon might be linked to the observed time distortions in patients with dementia.

THE INFLUENCE OF CAVITY CONFIGURATION ON THE MICROTENSILE BOND STRENGTH BETWEEN COMPOSITE RESIN AND DENTIN (와동의 형태가 상아질과 복합레진 사이의 미세인장결합강도에 미치는 영향)

  • This study was conducted to evaluate the influence of the C-factor on the bond strength of a 6th generation self-etching system by measuring the microtensile bond strength of four types of restorations classified by different C-factors with an identical depth of dentin. Eighty human molars were divided into four experimental groups, each of which had a C-factor of 0.25, 2, 3 or 4. Each group was then further divided into four subgroups based on the adhesive and composite resin used. The adhesives used for this study were AQ Bond Plus (Sun Medical, Japan) and XenoIII (DENTSPLY, Germany). And composite resins used were fantasists (Sun Medical, Japan) and Ceram-X mono (DENTSPLY, Germany). The results were then analyzed using one-way ANOVA, a Tukey's test, and a Pearson's correlation test and were as follows. 1. There was no significant difference among C-factor groups with the exception of groups of Xeno III and Ceram-X mono (p<0.05). 2. There was no significant difference between any of the adhesives and composite resins in groups with C-factor 0.25, 2 and 4. 3. There was no correlation between the change in C-factor and microtensile bond strength in the Fantasista groups. It was concluded that the C-factor of cavities does not have a significant effect on the microtensile bond strength of the restorations when cavities of the same depth of dentin are restored using composite resin in conjunction with the 6th generation self-etching system.
