A study on a hospital services evaluation method by physician survey (임상전문분야별 의사 설문조사를 통한 병원서비스 평가 방법 연구)

  • A physician survey was done by mailing for the purpose of performing hospital services evaluation and ranking. A slightly over one thousand samples were drawn from the list of professional societies, and 324 physicians(about 32 percent) replied. This study has focused on developing easy and simple method to evaluate hospital services, and providing patients with useful information. Hospital service structure and process were evaluated without outcome evaluation, because it is difficult to obtain reliable data regarding health services outcome indicators. Clinical specialty was targeted to evaluate, and three specialties were chosen, that is obstetrics & gynecology, cardiology, and proctology. Among 16 structural indicators, four indicators were finally chosen in each specialty by respondent specialists. And then using these indicators, structural score was calculated for study hospitals. For process evaluation, physicians were requested to nominate five most famous hospitals. The nomination score and structural score were summed up to produce final score and hospital ranking. This method is very easy to conduct rather than other hospital services evaluation methods prevailing in Korea. And it is more useful for patients to choose hospitals, according to his/her own purpose, because it gives high ranking hospitals with specific clinical specialty.

The Development and Evaluation of Web-based Flash Content for Fighting Spirit Promotion in Patients with Cancer (암 환자 투병의지 증진을 위한 플래시 콘텐츠 개발 및 평가)

  • Purpose: The purpose of this study was to develop a Web-based flash content for fighting spirit promotion and to test its effect on cancer patients' fighting spirit. Methods: The 15-minute long Web-based flash content was developed using the following 5 process: analysis, planning, development, program operation and evaluation stages and utilized the multiple edition and revision processes from December 2005 to August 2006. The evaluation was done by one group pretest-posttest design. Study subjects were 17 cancer patients undergoing chemotherapy. The group were received a 2-week intervention, a day weekly, and measured the variables at baseline, 4 weeks later. The study was performed from August 2006 to February 2007 at a cancer center in Korea. Fighting spirit was measured by Mini-MAC(Mental Adjustment to Cancer). Data was analysed by descriptive statistics and paired t-test using SAS 9.13 program. Results: On the Web-based flash content, there are 4 menu bars that consisted of cancer diagnosis, symptom management, stress management, and cancer survivorship. The study group revealed significantly more fighting spirit than pre-test(t=-3.04, p=.008). Conclusion: This Web-based flash content can be utilized in psychosocial interventions for promoting fighting spirit in patients with cancer.

Neurocognitive Functions in Posttraumatic Stress Disorder (외상후 스트레스 장애 환자의 신경인지기능)

  • Objective:The differences of various neurocognitive functions, including attention, memory, motor function, and higher cognitive function were compared between PTSD patients and normal control subjects. Also, correlation with PTSD symptom severity and neurocognitive functions were evaluated between PTSD patients and normal control subjects. Method:We assessed the neurocognitive functions by computerized neurocognitive test(CNT) batteries. The visual continuous performance test(CPT) and digit span test, finger tapping test and Wisconsin card sorting test(WCST) were executed. The Impact of Event Scale-Revised(IES-R) was used in the evaluation of the severity of PTSD. Result:The PTSD patients showed significantly impaired neurocognitive performance in all of the items, compared with normal control subjects. The relation between impairment in neurocognitive functions and symptom severity showed significant correlations. Conclusion:These results imply that PTSD patients have impaired neurocognitive functions concerning with specific brain areas, especially the frontal area. For the thorough evaluation of further neurocognitive functions, more detailed evaluation items of neurocognitive functions and brain imaging studies are necessary in the future study.

Retrospective Study of Success Rate of the $XiVE^{(R)}$ Implant;Early evaluation of clinical performance (($XiVE^{(R)}$ 임프란트의 성공률에 관한 후향적 연구;임상성적에 관한 조기평가)

  • This retrospective study evaluates the clinical performance of the recently introduced $XiVE^{(R)}$ implant(Dentsply-Friadent) with a new macro-design to improve primary stability. A total of 208 $XiVE^{(R)}$ implants (101 in the maxilla and 107 in the mandible) were placed in 71 patients. The average age of the patients was 49 years. Of the 208 implants, 190 (91.3%) were posterior implants and 82 (39.4%) were placed in compromised sites (grafted sites). Clinical and radiographic evaluation were made at second stage surgery for exposure and after functional loading. 192 implants in 64 patients were evaluated at exposure and 146 implants in 50 patients were loaded (average 170 days-loading) and evaluated after functional loading. Of 192 implants available for evaluation before loading, 3 implants failed (early failure) ; 1 before exposure, 1 at exposure and 1 during prosthetic procedure. 2 implants were in the maxilla and 1 was in the mandible. The success rate before loading was 98.4%. After functional loading, no implant failure was occurred in 146 implants evaluated during this period (100% interval success rate). This preliminary data with a new implant showed excellent success rate although the majority of implants evaluated in this study were placed in the posterior region of the jaw and compromised sites.

  • Since the late 1990s, public nursing hospitals have been established, and as of 2018, 79 hospitals account for 5.1% of all nursing hospitals. Announcing the public Moon Jae-in dementia in the state government responsibility for the convalescent hospital with dementia to patients for specified to the hospital with severe dementia specialist care institution. A different policy options and development. In this regard, the Dementia Management Act was revised in 2018 to provide a legal basis for the operation evaluation of public nursing hospitals, and this evaluation will begin in 2020. As part of the study to conduct the operation evaluation of public nursing hospitals to be implemented from 2020, the research established a function to foster public nursing hospitals as specialized institutions for dementia patients and competent institutions for treating rehabilitation patients during the period of recovery, and developed an evaluation system to evaluate their behavior as medical services, public nature and public institutions.

Retrospective Study on 469 Halitosis Patients (구취환자 469례에 대한 후향적 연구)

  • There is a growing interest in halitosis and diverse Korean medical studies are being conducted about it. But there are few study about teatment effect of halitosis after Korean medical care, treatment duration, and factor affecting recovery rate. Thus, the purpose of this study is to research clinical characteristic of halitosis patients, factor affecting halitosis, recovery rate of halitosis, treatment duration, factor affecting recovery rate by analyzing halitosis patients retrospectively. People who were over 19 and visited Korean medical clinic in Seoul to want to be treated halitosis in 2014 were analyzed retrospectively. We analyzed general and living characteristics of halitosis patients, halitosis-related symptoms and diseases, level of halitosis and halitosis-related symptoms both before and after treatment, treatment duration of halitosis patients, prescription of halitosis patients, recovery rate of level of halitosis and halitosis-related symptoms by general characteristic and duration of symptoms. Female, thirties, and Nonsmoker had the highest proportion in general characteristic. The average duration of halitosis is 41.6months and treatment duration is 2.55months. The average of sensory evaluation score is 3.40±2.53, subjective evaluation score is 5.02±1.53, lump sensation on throat is 3.52±2.50, postnasal drip is 1.11±1.88, dry mouth is 4.13±2.17, quality of life is 6.07±1.13. Gamichiuitang is used the most among prescriptions. Sensory evaluation, subjective evaluation, lump sensation on throat, postnasal drip, dry mouth, and quality of life had better score compared with pre-treatment. Duration of symptoms is in weak inverse proportion to sensory evaluation, subjective evaluation, dry mouth, quality of life. This study is meaningful in the way to find recovery rate of halitosis after korean medical care, treatment duration, and factor affecting recovery rate not progressed until now. But this study also has limits such as not analyzing objective data using halitosis measuring instrument and lack of methodical scale about quality of life. Therefore, succeeding study such as clinical trials is needed to verify reliability of Korean medical treatment.

A Study on Medical Costs for Patients with Vertigo Based on 2014 Health Insurance Review & Assessment Service-National Patients Sample Data (2014년 건강보험심사평가원 환자표본데이터 분석을 이용한 현훈환자의 의료비용에 관한 연구)

  • Objectives: The purpose of this study was to analyze the medical cost for patients with vertigo and to examine associations between chronic vertigo and mental disorder using 2014 Health Insurance Review & Assessment Service-National Patients Sample (HIRA-NPS). Methods: We analyzed sociodemographic characteristics, medical cost and medical care use pattern for vertigo patients. We used hierarchical multiple logistic regression analysis to examine odds ratio between chronic vertigo and mental disorder. Results: A total of 46,502 people and 118,504 claims data were identified for vertigo cases. Characteristics of vertigo patients have significant differences on proportion of female patients (68.36%), patients' average age (54.98) and proportion of medical assistance (5.76%) compared with non-vertigo patients. Results revealed that Korean medicine are one of frequent methods among total treatments for vertigo patients. Total days of medical care and total costs are 2.78 days and 111,362 won, respectively, and days for outpatients in Korean medical care (mean: 2.26 days) are more than those (mean: 5.05 days) in Western medical care. There is significant difference relative to sex between acute vertigo and chronic vertigo. The odds ratio between chronic vertigo and mental disorder is estimated as 1.34, that means risk of becoming chronic is 34% higher for vertigo patients with mental disorder. Conclusions: This study assessed socio-demographic characteristics, medical care use and expenses related to vertigo, and estimated associations between chronic vertigo and mental disorder. Findings provide a basis for economic evaluation studies on vertigo patients and development of clinical practice guidelines for vertigo patients with mental disorder.

Change of Symptoms after Total Hip Arthroplasty in Patients with Hip-Spine Syndrome

  • Purpose: Elderly patients with degenerative diseases undergo treatment for the hip and spine; these patients present with various symptoms. This study focused on patients with residual symptoms, predominantly pain, even after receiving treatment for their spinal lesions. Materials and Methods: Patients who underwent total hip arthroplasty (THA) between 2016 and 2022 at a single tertiary hospital were included in the study. Of the 417 patients who underwent primary THA, a retrospective review of 40 patients with previous lesions of the spine was conducted. Patients were stratified to two cohorts: Patients with symptoms related to the spine (Group A), and those with hip-related symptoms (Group B). Preand postoperative comparisons of groups A and B were performed. Results: Improvements in patients' symptoms were observed in groups A and B after THA. In Group A, the mean preoperative visual analog scale (VAS) score was 5.10±0.876, which showed a postoperative decrease to 2.70±1.767. In Group B, the mean preoperative VAS score was 5.10±1.539, which showed a postoperative decrease to 2.67±1.493. Conclusion: According to the findings, promising results were achieved with THA in treatment of debilitating diseases of the hip for both the prognosis of the disease, as well as the patients' symptoms. In addition, in some cases elderly patients with dual pathologies underwent treatment for spinal lesions without performance of any evaluation related to the hip. Thus, evaluation of a patient's hip must be performed and performance of THA in patients with symptoms even after treatment of spinal lesions is recommended.

Development of Dementia Care Standards (치매 간호 표준 개발)

  • Purpose: This study was done to develop standards for dementia care as a baseline for professional nurses to provide a framework for dementia care evaluation Methods: The dementia care standards were developed through a literature review and focus group discussions. According to the Delphi method, the data analysis was conducted using the Content Validity Index (CVI). Results: The final set of 18 standards on dementia care was developed through one round of CVI. The standards included four structural standards: 'Organization of nursing system', 'Operating system', 'Management of human resources', 'Management of material resources', 13 procedural standards: 'Advanced assessment', 'Nursing diagnosis', 'Nursing plam', 'Advanced nursing implementation', 'Evaluation', 'Education', 'Research', 'Consultation', 'Counseling and cooperation', 'Development of specialty', 'Utilizing resources', 'Nursing quality assurance', 'Ethics', and one standard concerning outcome ('Evaluation of nursing tasks in care of patients with dementia'). The final set of 55 criteria on care of patients with dementia was confirmed through two rounds of CVI. The final 171 indicators were confirmed through four rounds of CVI. Conclusion: These dementia care standards provides a framework that allows registered nurses to clarify their roles and tasks in the care of patients with dementia and provides evaluation criteria.

A Study of Prescription Analysis on Patients with Ischemic Heart Disease and Evaluation of Antihypertensive Drug Use in General Hospital (종합병원 허혈성 심장질환 환자의 항고혈압약제 사용 현황 및 평가)

  • Treatment of hypertension is important in reducing death and attack rates by Ischemic heart disease (IHD). The purposes of this study are to investigate recent prescriptions for patients with IHD and to evaluate antihypertensive drug use. On the basis of the guidelines proposed by Seventh Report of the Joint National Committee on prevention, detection, evaluation, and treatment of high blood pressure, we analyzed prescriptions of antihypertensive drugs for patients with IHD. It is necessary to set up new guideline for appropriate treatment to lower death and attack rates for patients with IHD.
