Comparison of Medical Care Patterns of Hypertensive Patients between Rural and Urban Areas (도시와 농촌지역 고혈압 환자의 의료기관 이용 형태 비교)

  • Objectives: This study was conducted to compare the medical care patterns of hypertensive patients between rural and urban areas. Methods: We selected one rural county(Region A where there were 19 public health centers; one health center, 8 health sub-centers and 10 community health posts) and two urban districts(Region B and C where there was no health sub-center and community health post) in Daegu city. Region B had similar socioeconomic characteristics with rural county A while region C had different characteristics. The medical insurance records of 14,422 incident patients (2,501 in region A, 4,873 in region B and 7,048 in region C) with diagnostic code of hypertension from September 1998 to August 1999 were reviewed. Incident patient was defined as a patient who had no record of medical fee claim for hypertension to the national health insurance corporation in past 6 months and visited a medical facility for hypertension for the first time. The data for annual visit days, annual prescription days and annual total medical expenses were abstracted. The medical care pattern was categorized by the number of annual visit days and prescription days. The most proper care group was defined as the patient who visited 6-15 days with 240 prescription days or more in a year. Results: The type of medical facilities for the most visit was clinics, 373.% and it was followed by general hospitals, 28.2%; public health centers, 24.7%; and hospitals, 9.8% in region A(p<0.05). In region B, it was clinics, 63.1% and followed by general hospitals, 27.6%; health center, 5.2%; and hospitals, 4.1%(p<0.05). In region C, it was clinics, 53.8% and followed by general hospitals, 35.0%; health center, 6.3%; and hospitals, 4.9%(p<0.05). Annual mean total medical expenses per patient was highest in region C(won195,993) and followed by region A(won191,683) and region B(won178,713). The proportion of the most proper care group was 7.7% in region A, 5.2% in region B and 6.7% in region C(p<0.05). According to the type of medical facilities for the first visit, the proportion of the most proper care group was highest(14.7%) in the patients of public health centers, and it was followed by general hospitals, 8.8%; clinics, 3.6%; and hospitals, 2.0% in region A(p<0.05). In region B, it was highest in general hospitals, 9.7% and followed by hospitals, 4.0%; health center, 3.6%; and clinics, 3.4%(p<0.05). In region C, it was highest in general hospitals, 10.1% and followed by clinics, 5.2%; hospitals, 4.1%; and health center, 3.1%(p<0.05). Conclusions: The proportion of proper care for hypertension was higher in rural area and it was attributed to the care of health center, sub-centers and community health posts which appeared to follow patients better than hospitals and clinics.

The Influence of K-Content Experience on National Image, Tourism Attitude and Visit to Intention: Targeting Chinese (K-콘텐츠 경험이 국가이미지와 관광태도 및 방문의도에 미치는 영향 : 중국인을 대상으로)

  • This study attempted to empirically verify the possibility of K-content for Chinese people who have recently slowed down due to restriction of movement and political and diplomatic conflicts, although it is a very meaningful market for Korea's content industry and tourism industry. As a result of the study, each item of K-content experience, national image, tourism attitude, and visit intention was derived as one factor, and only the national image factor was derived as two factors: 'functional image' and 'cultural image'. As a result of examining the influence relationship between them established based on previous studies focusing on the derived factors, all five research hypotheses were adopted. K-content experience was found to have a significant influence on both factors of the national image. It was found that it had a greater influence on cultural image factors than functional image factors, cultural image factors were found to have a greater influence on tourism attitudes, K-content experiences had a significant effect on tourism attitudes, and tourism attitudes had a significant effect on visit intentions. Based on the results of this study, it was once again confirmed that the national image even comtually bees an important factor for linking to practical tourism behavior, and in this respect, "culture" is an important key factor that can lead to practical tourism and visits. Previous national images indicate that if the functional aspect of the country was more emphasized, it is now necessary to focus more on the importance of culture than on the functional aspect. As the K-content experience has a significant effect on tourism attitude, it can have a positive effect on the formation of a positive tourism attitude that can lead to actual tourism behavior, so various efforts will be needed to form an active tourism attitude using K-content in the future. As the content and target scope of K-content are expanded and diversified, specific strategies for each sub-market using cultural contents in various fields should be established and implemented.

Acupuncture and Moxiburstion Alleviates Chronic Postoperative Pain as a Comlication of Gastrointestinal Surgery: Apropos 8 Cases (침구치료를 통해 소화기계 수술 후 발생한 만성적 수술 후 통증이 호전된 환자 8례)

  • Here we described 8 patients treated with acupuncture and moxiburstion due to chronic postoperative pain as a comlication of gastrointestinal surgery. The patients were suffering from pain that continued for more than 6 months after laparotomy for gastric cancer (n=1), rectal cancer (n=2), cholangiocarcinoma (n=2), hepatocellular carcinoma (n=2) and ischemic colitis (n=1), respectively. Mean time elased from surgery were 31.25(${\pm}21.72$)months, pain grade on first visit by VAS(visual analogue scale, 0~10mm) was 3.75(${\pm}0.97$)point, and acupuncture and moxiburstion were performed 12.63(${\pm}5.74$)times during 5.5(${\pm}2.4$)weeks, on average. At the end of treatment, pain was decreased 2.75(${\pm}0.97$)point compared to first visit. Based on the that results, it is reasonable to assume that acupuncture and moxiburstion can be effective to persistent post-surgical pain after laparotomic gastrointestinal surgery, at least to some laparotomized patients.

The Relation between Deqi and Anti-itching Effect of Acupuncture Stimulation (득기와 침의 항소양 효과 간의 상관관계 분석)

  • Objectives : 'Obtaining qi(deqi)' is a sensation experienced by a patient or an acupuncturist during acupuncture treatment, and it is considered to be an important factor in acupuncture treatment. However, previous studies reported conflicting results about the relationship between 'obtaining qi(deqi)' sensation and the effect of acupuncture treatment. Also, most of the previous studies investigating 'obtaining qi(deqi)' and the acupuncture effect, dealt with the analgesic effect of acupuncture. The objective of this study was to analyze the relation between the 'obtaining qi(deqi)' and the anti-itching effect of acupuncture treatment. Methods : Twenty one healthy subjects participated in this study. At the first visit, itch was induced to all subjects using intradermal injection of histamine on the lower arm. At the second visit, acupuncture treatment was applied using LI11 and 'obtaining qi(deqi)' was assessed. After acupuncture treatment, itch was induced again, and itch VAS and skin flare were assessed. Results: At the 6,8,10 minutes after histamine injection, the change of itch VAS between before and after acupuncture treatment was significant. Among 'obtaining qi(deqi)', subjects reported that 'dull ache', 'pricking', 'heavy' were most strong. Total score of 'obtaining qi(deqi)' sensation was not related to the change of itch VAS or skin flare. The 'electric shock' sensation is related to the change of itch VAS, and 'bruised' and 'heavy' sensations were related to the change of skin flare. Conclusions : Total 'obtaining qi(deqi)' is not related to the anti-itching effect of acupuncture, but several sensations showed relationship with the effect of acupuncture on itch severity and skin flare. Further researches based on various symptoms or acupuncture points, are needed.

Data Mapping of the Terms for Developing an Integrated Information System in Home and Visiting Healthcare Documents (재가.방문 건강관리 통합정보시스템 구축을 위한 관련 서식지 항목 매핑 연구)

  • Purpose: This study sought to determine the possibility of developing the data-sharing infrastructure of an integrated information system to improve the quality of home and visit-based healthcare services. Methods: The articles of study here were the forms used by a visiting healthcare agency, a home healthcare system of a home healthcare agency, and those used in long-term care insurance for elderly. We visited a visit-based healthcare agency and a home healthcare agency to survey their forms and interviewed relevant practitioners, and we searched for forms associated with long-term care insurance for the elderly on the Internet. We then organized the terms in each form and mapped them among the form after analyzing the concepts as a whole to inquiry into the possibility of integration. Results: The mapping procedure divided the terms into those related to personal information, problems and interventions. Mapping between the standard system (Omaha system) and the type of form was also done. Conclusion: In this study, we found that programs were configured differently depending on the objectives of the service. It is necessary to develop the program with an integrated information system by comparing the three services in terms of their distinct advantages, after which such a service should be utilized. The results of this study can serve as a database for the creation of a new integrated system.

Effects of Face-to-face and Non-face-to-face Exercise Programs on Exercise Satisfaction for the Elderly in the Community (지역사회 노인을 위한 대면과 비대면 운동프로그램이 운동 만족도에 미치는 영향)

  • PURPOSE: The study compared the satisfaction of the exercise program in a face-to-face and non-face-to-face manner with the same exercise program. METHODS: This study was conducted through convenience sampling for 57 seniors over 65 years of age. After conducting 30 minutes of the same exercise program face-to-face on the first visit and non-face-to-face on the second visit, a satisfaction survey was conducted. The survey data were processed statistically using SPSS 26.0 (SPSS Inc, IBM, USA). An independent t-test was used to compare the type of exercise program. The Spearman's Rank Correlation was conducted to correlate the satisfaction between cognitive function and education level. RESULTS: The overall satisfaction with the face-to-face program was higher than the non-face-to-face exercise programs. All three satisfaction items in face-to-face exercise programs were higher than in non-face-to-face exercise programs (p < .05). The main source of dissatisfaction in the case of face-to-face exercise programs was having a good relationship with people for face-to-face and a 'lack of communication' in the case of non-face-to-face. CONCLUSION: The elderly showed higher satisfaction in face-to-face exercise than in non-face-to-face exercise. This study is significant in that accurate satisfaction comparison was possible by conducting the same exercise program on the same subject for a relatively short time, and through this, the development of an exercise program was suggested.

Satisfaction Factors and Determinants of Visitors in Byeonsanbando National Park, Korea (변산반도국립공원의 탐방객 만족요인 및 예측모형 분석)

  • The purpose of this study was to provide basic data for developing the practical park management. For this, satisfaction degree, satisfaction factors and estimated model by satisfaction variables of visitors analyzed through a questionnaire survey in Byeonsanbando National Park, Korea. In the evaluation of the satisfaction, variables of 'careless cooking'. 'collection of natural plants & animals', 'Noise and urination on the road' and 'Littering problem' at Naesosa and 'crowding', 'touting', 'Indiscreet use' and 'Illegal merchant' at Gyeokpo was most unsatisfied than any other variables. Satisfaction factors by Factor Analysis were loaded with 'user management' at Naesosa(65.1%) and 'business behavior management at Gyeokpo(68.4%) by the highest contribution degree. In estimated model of satisfaction by Multiple Regression Analysis showed in order of 'Injury of landscape resource', 'Lack of information facility', 'crowding' and 'Lack of employee's guidance or kindness' at Naesosa and 'Injury of landscape resource', 'Lack of information facility', 'Illegal merchant', Lack of facility to stay' and 'Lack of employee's guidance or kindness' at Gyeokpo. So, Mountain and marine National Parks needed different management policy and are required to institutional apparatus for user education.

Visitors' Behavioral Characteristics and Attitudes to the Use and Managerial Attributes in Hallasan National Park, Korea (한라산국립공원에서의 탐방객 이용행태와 이용.관리속성에 대한 태도)

  • This study was designed to provide basic visiter information for establishing effective park management strategies. A questionnaire survey was at major entry points in Hallasan National Park(Eorimok and Youngsil). 237 random samples among the visitors on the way back home from their visit were chosen to represent general visitor population. As for the results, visitors' recreational satisfaction level with the area was relatively high. The majority of the visitors were satisfied with 20 uses and management related attributes provided by Hallasan National Park system. However, considering lack of sense of hygienic sanitary facilities, consequential deterioration of natural and cultural resources, and entrance fees as well as other costs, there needs to be a managerial priority on visiter service.

Surveying Visitors′ Behavior in Chuwangsan National Park (주왕산국립공원의 이용자 행태조사)

  • Visitors to Chuwangsan National Park were survayed from August 3 to 5 n 1994. During this time, 346 visitors were contacted. Of those individuals, 65% were males. 63% of respondents reported that they had gone as far as college. 48% were 20 years of age. 97% of the survey respondents had experience to visit other national parks. The largest percentage of respondents were reported that they visited Chuwangsan Nat'1 Park for enjoying natural landscape. In group type, 50% were traveling with their family and 36% with their friends. In activity characteristics, 51% were day-time visitors, and 18% mentioned carrying in their on food. Generally most respondents were very interested in the environmental problem in national parks. The majority of visitors perceived that the environment of Chuwangsan Nat'1 Park were good enough. In six types of normative violations, the major reasons of littering were unintentional violation and releaser-cue violation. Most respondents were not likely to intervene to stop other visitors' depreciative behavior (Bystander intervention behavior). In two dilemmas, the more likely the intention to obey a regulation the less likely the intention to disobey a regulation, and vice-versa.

The Influence of Park Users' Cognition about the Park Design Concept and Strategy to Satisfaction - A Case Study of the Neighborhood Parks of Gwanggyo New Town - (공원이용자의 설계개념과 설계전략에 대한 인식이 공원 만족도에 미치는 영향 - 광교신도시의 근린공원을 대상으로 -)

  • The purpose of this study is analyze the impact on users' cognition of design concept, satisfaction of design concept, adapted park space, and effect of design concepts on park use satisfaction. The study sites are Central Mountain Park and Lake Park in Gwanggyo New Town, Suwon. We analyzed importance-satisfaction of park facilities in the park and preference-satisfaction of park. The results of this study are as follows. First, generally users visit the park as a family unit. Second, trails got high scores in importance-satisfaction of park facilities. Third, park users' satisfaction received a high score generally, but it doesn't show statistic significance with users' cognition of design concept. Fourth, cognition of the two parks' design concept and design strategy got a high score - over three points on average - based on the 5 - point Likert scale. However, upon analysis of users' cognition of design concept and satisfaction of design concept adapted park space, there is no statistic significance. Therefore, we noticed that the park's design concept had an effect on park users' satisfaction; rather, personal preference and satisfaction of users had more impact on satisfaction. On the other hand, this study was limited in that we didn't survey across all four seasons and had few target areas. But it has some significance in that we used a visual documentation, attaching existing photos including design concept, to increase users' understanding. Also, we directly asked about the parks' user design concept and strategy.
