Health Locus of Control in Indonesian Women with Breast Cancer: a Comparison with Healthy Women

  • The aims of this study were to assess whether Indonesian women with breast cancer havea higher external health locus of control (HLC) than healthy women, and to explore the association between HLC and symptoms of anxiety and depression. In this study, 120 consecutive women with breast cancer were recruited at the outpatient surgical oncology clinic at the Hasan Sadikin Hospital in Bandung. One hundred and twenty two healthy women were recruited from the Bandung area as controls. A standard demographic form, Form C of the Multidimensional Health Locus of Control, as well as the Hospital Anxiety and Depression Scale and patients' medical records were used. Data were analyzed using descriptive statistics, t-test, Pearson's correlation, MANOVA and multiple linear regressions. Women with breast cancer had higher scores on all external HLC subscales, i.e. chance, doctor, powerful others and God, and lower internal HLC compared to healthy women. High God LHC scores were associated with a high level of anxiety (${\beta}=0.21$, p<0.05), whereas none of the HLC subscales were associated with depression. Our results suggest that women with breast cancer tend to have high external HLC, while healthy women tend to have high internal HLC. A strong belief in an external source of control, i.e. God, might be negatively associated with patient emotional adjustment. Further research is needed to give an insight into the direction of this association.

Health Status and Self-management Barriers in People with Diabetes -A Comparison by Medicaid Beneficiary Status- (성인당뇨병환자의 건강수준 및 질병관리장애요인 -의료급여환자와 건강보험환자의 비교-)

  • Academic interest has been drastically increased for the health disparities due to socioeconomic factors. For those who have a chronic illness such as diabetes, various psychosocial barriers related to illness management might affect and aggravate this disparity. This study focused on illness management barriers experienced by people with diabetes, and examined the differences by their Medicaid beneficiary status. The between group differences in illness management barriers, family support and health status were examined as well as the association between illness management barriers and health status. The sample of this study consists of 144 community dwelling adults who have diabetes, recruited from an outpatient diabetes clinic and a community welfare center. Medicaid beneficiaries reported poorer health status, and experienced more illness management barriers compared to their counterparts. Statistically significant differences were observed in barriers due to mental health, financial status, and lack of support. The between group difference in health status remained significant after controlling for the effect of demographic characteristics and illness related factors. Social work practitioners working with this population should address these illness management barriers to reduce socioeconomic health disparity.

A Study of the Relationship among Self-Efficacy, Sick-Role Behavior and Metabolic Control in Diabetic Patients (당뇨병 환자의 자기효능, 환자역할행위 이행 및 대사조절간의 관계)

  • The purpose of this study was to identify the degree of self-efficacy, sick-role behavior and metabolic control in patients with diabetes and to identify the relationships among those variables. The subject of the study were composed of 204 NIDDM patients from 2 hospitals, who were visiting the outpatient clinic in Taegu. Data were collected from Sep. 18th to Oct. 5th, 1996. In data analysis, an SPSS/PC+ program was utilized for descriptives correlation, T-test, and ANOVA. The results are as follows. 1) The mean scores for self-efficacy were 65.74 (range 30.83-90.68), on a 100 point scale. 2) The mean scores for sick-role behavior were 3.36(range 1.80-4.67), on a 5 point scale. 3) The mean score for metabolic control was 7.63 (range 5.0-13.2). 4) Self-efficacy was significantly correlated with sick-role behavior(r=.3614, p<0.01) but there were no significant correlations with self-efficacy and metabolic control (r=-.1045, p>0.01), sick-role behavior and metabolic control (r= - .1288, p>0.01). 5) There were significant differences in self-efficacy according to sex(t=4.47, p=0.000) and education(F=8.27, p=0.000). 6) There were no significant differences in sick -role behavior according to demographic characteristics. 7) There were significant differences in metabolic control according to the duration of diagnosed diabetess(F=4.13, p=0.007) and complication (t=2.47, p=0.014). These results suggest that for improvement in sick-role behavior nursing intervention needs to be directed at promoting self-efficacy.

Depression and Quality of Life among Family Caregivers of Patients with Parkinson's Disease in South Korea (파킨슨병 환자 가족의 우울과 삶의 질)

  • ression and quality of life of family caregivers of patients with Parkinson's disease(PD). Methods: A cross-sectional descriptive study was conducted in one neurology outpatient clinic in Seoul, Korea from March to June, 2006. Sixty eight family caregivers of PD patients were participated to the study, using CES-D and SF-36. Results: Mean scores of depression were $16.18{\pm}8.39$ (range: 0-48) and it was a little lower than caregiver's who took care of Dementia patients and were higher than primary caregivers of the patients with Stroke. Time for caregiving, perceived severity, duration of PD were significantly related with depression respectively. Higher ADL scores which mean greater motor disabilities were related to higher caregiver depression. Lower income and greater medical expenditure were closely related with the depression of family caregivers respectively. The mean scores of total QOL were $435.5{\pm}96.5$ and the mean scores of PF, SF, RE and MH were lower than general population. Time for caregiving, depression, patients' ADL scores were significantly associated with QOL respectively. People who were older and had lower educational background showed lower QOL scores respectively. Conclusion: Healthcare professionals should pay more attention to emotional aspects of caregivers who take care of PD patients, and develop comprehensive management strategies both for patients and their caregivers.

Endoscopic Findings of Children with Familial Mediterranean Fever

  • Purpose: Familial Mediterranean fever (FMF) is an auto inflammatory disease characterized by periodic fever, synovitis and serositis. Patients may be admitted to gastroenterology units due to gastrointestinal symptoms. In this study; we aimed to analyze endoscopic findings and diagnostic utility of endoscopic procedure in children with FMF. Methods: Patient with FMF that was performed endoscopy for the gastrointestinal symptoms were included to the study (39 of 164 patients, 53 procedure). A control group was randomly designed as age and gender matched four endoscopic procedures per one endoscopic procedure of patients with FMF (n=212). Results: No different was found between the patients and control group in esophagogastroscopy findings. However, the diagnosis of gastrointestinal pathology was made by esophagogastroscopy in 46.2% patients. Colonoscopic examination revealed that the frequency of inflammatory bowel disease (IBD) was higher in undiagnosed patients compared to both the control group (50.0% vs. 6.9%, p<0.05, odds ratio [OR]:13.4 and 95% confidence inteval [95% CI]: 2.1-84.3) and the patients under colchicine treatment (50.0% vs. 8.3%, p<0.05, OR: 11 and 95% CI: 0.8-147.8). Colonoscopic procedure that was made after the diagnosis was found to provide contribution by 16.7% in determining the etiology of the additional symptoms. Conclusion: Patients with FMF may be admitted to pediatric gastroenterology outpatient clinic prior to diagnosis or during the follow-up period. The frequency of IBD is high in undiagnosed patients with FMF. Endoscopic procedures may be helpful in these patients for the diagnosis accompanying mucosal lesions.

Effect of 12-Week Tai Chi Exercise on Glucose Control, Peripheral Nerve Modulation, and Perceived Health for Type 2 Diabetic Patients with Neuropathy (12주간의 타이치 운동이 신경병증을 가진 당뇨환자의 혈당, 말초감각신경전달도 및 건강상태에 미치는 효과)

  • Purpose: The purpose of this study was to test the effect of 12-week Tai Chi exercise on glucose control, peripheral nerve modulation, and perceived health for Type 2 diabetic patients with neuropathy. Methods: A pretest posttest design with a nonequivalent control group, 44 diabetic patients with neuropathy were recruited from an outpatient clinic of a university hospital and assigned into Tai Chi or Control groups. The Tai Chi exercise was based on Tai Chi for Diabetes program developed by Lam (2006) and performed one hour for each session twice a week for 12 weeks. Outcome variables were HbA1c, Michigan Neuropathy Screening Instrument scores and perceived health. A total of 25 patients completed both measures of pretest and posttest. Results: The study participants were 67 years old in average, diagnosed by DM for more than 15 years. Those who participated in 12-week Tai Chi exercise (n=13) significantly improved in HbA1c (t=2.23, p=.035) and perceived health (t=-2.28, p=.032) than the control group (n=12). Conclusion: Tai Chi exercise may improve glucose control and health status in patients with Type II diabetes. Further study with larger sample size would be necessary to confirm the effect of Tai Chi on peripheral nerve modulation.

Prevalence and Genotype Distribution of Human Papillomavirus among Women from Henan, China

  • Human papillomavirus (HPV) infection has been implicated as a causative of cervical cancer. In the present study, a total of 578 samples from females attending the gynecological outpatient clinic in Henan province, China, were collected and the HPV genotypes were detected by gene chip and flow-through hybridization. Overall, 44.5% (257/578) females were found to be HPV DNA positive, and the high risk HPV (HR-HPV) rate was 35.1% (203/578). The first peak of HR-HPV infection appeared in the >60 year-old group (55.0%), and the second was within the 51-55 year-old group (50.0%) (${\chi}^2$=19.497, p<0.05). HPV 16 was the most prevalent genotype (9.2%), followed by HPV 52 (7.8%), HPV 6 (6.9%), HPV 11 (5.9%) and HPV 42 (5.0%). The single type HPV infection was 30.4%, with the five majority prevalent genotype HPV 16 (16.5%), HPV 52 (14.3%), HPV 6 (12.6%), HPV 42 (8.6%), HPV 31 (5.1%). The multiple-type HPV infections were 14.0%, and HPV 16 was the most prevalent type (29.6%), followed by HPV 52 (24.7%), HPV 6 (22.2%), HPV 11 (22.2%), HPV 42 (17.3%) and HPV 39 (17.3%).

Perinatal and Developmental Risk Factors of ADHD Children Diagnosed with a Structured Interview (구조적 면담으로 ADHD로 진단받은 아동의 주산기 요인 및 발달력상 위험인자)

  • Objectives:The objective of this study was to examine the perinatal and developmental risk factors of attention-deficit hyperactivity disorder (ADHD) diagnosed with a structured interview among Korean children. Methods:The current study included 924 children (6-15 years) recruited from schools in five Korean cities or a child psychiatry outpatient clinic of Seoul National University Children's Hospital. The parents of the children completed the structured diagnostic interview for attention-deficit hyperactivity disorder, as well as questionnaires on perinatal and developmental risk factors. Results:Preterm delivery, severe maternal stress during pregnancy, change in primary care taker during the first three years, postpartum depression, and delayed first sentence showed a significant association with ADHD diagnosis. Conclusion:These findings suggest that perinatal and developmental factors contribute to development of ADHD in Korean children. Conduct of future research using a prospective design is needed in order to identify the causal relationship between observed risk factors and development of ADHD.

Popliteal Artery Entrapment Syndrome - A case report - (슬와동맥 포착증후군 - 1예 보고 -)

  • The entrapment of the popliteal artery is a rare cause of ischemia of the lower extremities among young males. The development of local occlusive or aneurysmal changes of the popliteal artery is caused by abnormal anatomic relationships between vascular and musculo-tendinous structures in the popliteal fossa. An 18-year-old male visited our outpatient clinic with the chief complaint of claudication in his right calf. Three dimensional CT angiography showed an occlusion of the popliteal artery and less opacified arteries of the right leg. Intraoperatively, the popliteal artery was compressed by an accessory muscle band arising from the medial head of the gastrocnemius. After release of the muscle band, thrombectomy with endarterectomy was done. Three years after surgery, he is doing well without any problems.

  • Objectives: The purpose of this study was to examine the performance and electrophysiological characteristics of drug-naive children with attention-deficit/hyperactivity disorder(ADHD) during the Go/NoGo task. Methods: Twenty-three boys with ADHD and 18 age-matched normal boys were recruited at a child psychiatric outpatient clinic in Seoul. All subjects were assessed by the Kiddie Schedules for Affective Disorders and Schizophrenia Present and Lifetime version. The investigator also assessed all subjects using the ADHD Rating Scale-IV(ADHDRS). Event-related potentials were recorded from 8 scalp electrodes during the visual Go/NoGo task. Results: Children with ADHD showed a larger mean of standard deviation of response time during the Go/NoGo task than normal children. The temporal N200 and P300 amplitudes were larger in children with ADHD relative to controls. The parietal N200 and P300 latencies were more prolonged in children with ADHD compared to normal controls. Conclusion: These results suggest that psychotropic-naive children with ADHD may have more variable performance ability, more difficulty in discriminating visual stimuli, and slower information processing speed than their normal age-matched counterparts.
