A Clinical Study of Lumbago patients on the Effect of Oriental Medicine treatment with $Dokhwalgisaeng-tang$($Duhuojisheng-tang$) $Gamibang$ (독활기생탕가미방(獨活寄生湯加味方)을 활용한 요통 및 요각통 환자 13례에 대한 임상적 고찰)

  • Objectives : To evaluate the effectiveness of oriental complex therapy with $Dokhwalgisaeng-tang$($Duhuojisheng-tang$) $Gamibang$ on the lumbago patient. Methods : 13 lumbago patients were treated with $Dokhwalgisaeng-tang$($Duhuojisheng-tang$) $Gamibang$, acupunture, moxibustion, pharmacopuncture, cupping, oriental physical therapy and exercise for lumbago. ODI(Oswestry diability index), ROM(range of motion), pain self assessment were checked to evaluate the effectiveness of the treatment. Results : ODI, pain self assessment and ROM were all improved but there were no statistical significance except pain self assessment($p$<0.001). Coclusions : Oriental complex therapy with $Dokhwalgisaeng-tang$($Duhuojisheng-tang$) $Gamibang$ might improve low back pain.

  • Objectives: The study aims to look into how the application of EA(Electro-acupuncture) in the initial step of ischemic brain injury affects the changes in Bax and Caspase-3 expression among forebrain apoptosis. Methods: It caused GI by using common carotid artery occlusion and conducted the test by dividing into the no applied EA group after 6 hours and 12 hours, the LI 4 EA group that applied EA to LI4, and GV 20 EA group that was applied to GV 20. After that, the following results were obtained by comparing Bax and Caspase-3 expression, which were apoptosis-related factors, with its application time through immunohistochemical staining after extracting brain of each group. Results: According to each group's Bax expression by the application time, the order of expression increase after 6 hours is shown to be the no applied EA group, LI 4 group, and GV 20 group. The stimulation after 12 hours was most lowly expressed in the GV 20 group, and the no applied EA group and LI 4 group showed similar level. Conclusions: EA stimulation in the initial step after ischemia seems to affect positively Bax and Caspase-3 expression that are index of forebrain apoptosis.

  • The effects of scald bum Injury ($40-45\%$ of total body surface area), there were not inhalation and secondary infection, on the histological changes and the levels of oxygen free radical generating and scavenging enzymes have been determined in liver tissue of rat models. It was found that dermal epithelium was left out with edema of dermis layer and hydorpic swelling of hepatocytes, Burn injury increased liver weight (L.W./B.W.) and serum aspartate aminotransferase content (pThe data of this study suggest that liver damage Induced by scald burn injury leads to dysbalance of oxygen free radical generating and scavenging enzymes.

  • There were a few case reports on the treatment of Takayasu's arteritis. We had a good effect on one patient with Takayasu's arteritis by oriental medicine therapy. We use acupuncture, herbal acupuncture, electro-acupuncture, moxa, negative thrapy, kinesio taping therapy, physical therapy and exercise for 82 days. In this case, Left hemiparesis, dysarthria, pulseless, disturbances of urination, headache, dizziness improved after treatment. We experienced improvement in this symptoms of Takayasu's arteritis by oriental medicine treatment.

Necessity and Features of Service Required for Home Visiting Physical Therapy (가정방문 물리치료의 필요성 및 요구하는 서비스의 특성)

  • This study was performed to investigate the degree of demand and general features of services required of home visiting physical therapy for chronic ill patients. The study subjects were ambulatory and admitted patients treated with physical therapy at six general and one oriental hospitals, one welfare center, four health centers located in Taejon from March 2, 1999 to March 16. Authors developed structured questionnaire, and distributed it to each physical therapist of study organizations. Total number of distributed questionnaire was 500, and 405 questionnaire were collected and analysed finally. 1. $82.4\%$ of patients and $90.0\%$ of caregivers are showed that home visiting physical therapy was needed(p<0.05) 2. The rate of necessity for home visiting physical therapy by kinds of disease was $949\%$ in cerebral palsy, $95.0\%$ in upper spinal cord injury, $83.3\%$ in lower spinal cord injury, $84.5\%$ in cerebral vascular accident, $89.6\%$ in traumatic brain injury, $83.5\%$ in other diseases. 3. In the general features of required service for home visiting physical therapy, $33.7\%$ of patients and $34.4\%$ of caregivers want special isolated physical therapy center, $33.1\%$ of patients and $43.3\%$ of caregivers want 3 times per week in frequency, $46.7\%$ of patients and $45.0\%$ of caregivers want 30-60minutes in treatment duration, and $48.0\%$ of patients and $46.7\%$ of caregivers want more intensive care than general hospitals. 4. In the working place of home visiting physical therapy, $36.1\%$ of patients and $36.2\%$ of caregivers wants physical therapist worked in general hospital. Also, $53.3\%$ of patients and $52.2\%$ of caregivers answered no interested in physical therapist's gender. The most preferential age of home visiting physical therapist is thirties in $43.2\%$ of patients and $63.4\%$ in caregivers

  • Objectives: The purpose of this study is to report the clinical effects of Korean medical treatment on the patient with cervical radiculopathy. Methods: A 16-year-old female patient, who was suffering from the cervical radiculopathy on her Lt. shoulder: ROM limit, swelling and arthralgia, was treated by herbal medicine, physical therapy, acupuncture and pharmacupuncture. Results: As a result, the patient's left upper limb disability was improved by Korean herbal medicine (ssanghwatang hap gamiseokyungtang), hominis placenta (Ja-ha-geou) pharmacupuncture and physical therapy. Conclusions This study showed that Korean medical treatments can be an effective choice for cervical radiculopathy.

  • ${\ulcorner}$Euibangyoochui${\lrcorner}$ is a great medical encyclopedia composed of 91 symptoms and signs of diseases which classified 150 kinds of reference books. It consisted mainly of fifty thousands of herbal prescriptions and therapeutics but it deals with other treatments like diet therapy, precautions, acupuncture and moxibustion, physical and breathing exercises as well. Unlike other medical references, this book made up these treatments as independent chapters. We tried to catch the meanings and characteristics of each chapters.

  • The purpose of this case study is to show a case of acute posterior cruciate ligament(PCL) complete injury improved by conservative acupuncture therapy. The patient was treated for 100 days. The patient received deep interarticular electroacupuncture(EA) stimulation at S35, LE200, LE211 and treated by other acupoint stimulation, herb medication, cupping, physical treatments and exercise. We evaluated through MRI and Lysholm knee score. We suggest to treat acute posterior cruciate ligament(PCL) injury with acupuncture therapy meaning conservative therapy and would like to research acupuncture treatment plan for the further treatments.

  • This study was performed to report the effectiveness of oriental medical treatment and muscle energy techniques on a patient with piriformis syndrome. The patient was diagnosed as piriformis syndrome and treated with acupuncture, herb medicine, physical therapy and muscle energy techniques. We have evaluated curative efficacy with measuring changes of visual analogue scale and physical examination according to period of hospitalization. After treatment, pain and visual analogue scale decreased. Physical examinations were recuperated nearly to normal findings and ambulation was improved. These results suggest that oriental medical treatment and muscle energy techniques were effective on patient with piriformis syndrome.

Effect of Scapular Stabilization Exercise on Patients With Neck Pain Classified According to Passive Scapular Elevation Test (수동 견갑골 거상 검사에 따라 분류된 경부통 환자의 견갑골 안정화운동 효과 비교)

  • This study aimed to determine the usefulness of classifying patients with neck pain on the basis of the results of passive scapular elevation test. We classified 21 patients with neck pain into positive (n=12) and negative (n=9) groups on the basis of passive scapular elevation test; the 2 groups then equally performed scapular stabilization exercise program for 30 min, 3 times a week, for 4 weeks. Visual analogue scale (VAS), neck disability index (NDI), and range of motion (ROM) were recorded both before and after the intervention for both groups. Paired t-test was used to determine that there were significant changes between before and after the intervention, and independent t-test was used for analyzing changes between two groups of dependent variables. After 4 weeks of training, we observed significant decrease in pain and disability (p<.05) and a significant increase in rotation, flexion, extension, and side-bending ROM (p<.05) in both groups. Further, between pre- and post-intervention evaluations, we observed a significant decrease in pain and disability and a significant increase in rotation and flexion ROM in the positive group than in the negative group (p<.05). These results indicate that passive scapular elevation test may be used to identify mechanical disorders of the cervicoscapular muscle in patients with neck pain. Therefore, we recommend the use of passive scapular elevation test to determine appropriate treatment intervention when treating patients with neck pain.
