Attitude and Participation Status on District Health Planning in Officials of Health Centers (보건소 공무원의 지역보건의료계획 수립 참여실태 및 태도)

  • Objectives: This study was performed to investigate attitude and participation status on district health planning of officials in Health Centers. Methods: The data collected by self-administered questionnaire survey of 674 officials of Health Centers and the data on district health planning of 28 Health Centers in Gyeongsangbuk-do and Daegu metropolitan city were analyzed. Results: Only 13.6% of officials of Health Centers responded that they had read 'the second district health planning paper' thoroughly and 12.5% of officials of Health Centers replied that they had known contents of 'the second district health planning paper' fully. 56.9% of officials of Health Centers didn't have 'the second district health planning paper'. Thirty five point four percent(35.4%) of health center officials replied that the mayor's or county executive's concern about the district health planning was high, 22.4% in councilors, 77.8% in Health Center chiefs, 44.9% in Health Center officials, 43.9% in him or her. Among respondents, 58.6% of Health Center officials replied that district health planning was necessary and 38.0% of subjects replied that the degree of utilization of district health planning was high. About thirty seven percent(36.9%) of respondents participated in making out 'the second district health planning paper' and 49.6% in 'the third district health planning paper'. Health Center officials replied that the most serious problem of district health planning was 'lack of budget and personnels'(39.8%), 'lack of concern for district health planning'(21.4%) and the most important thing to improve district health planning was 'establishing department for health planning', 'adjustment of establishment time for district health planning'. Conclusions: In order to establish effective district health planning, it would be necessary to secure budget and personnels, to promote the county executive's concern and Health Center officials' concern about the district health planning, to establish department for health planning, and to adjust establishment time for district health planning.

Experiences of Public Officials for the COVID-19 Response in the Community Health Center (보건소 공무원의 코로나19 대응 경험)

  • Purpose: The purpose of this study was to examine the experiences of public officials working for the Coronavirus Disease-19 (COVID-19) response in community health centers in South Korea. Methods: A qualitative thematic analysis was conducted using data collected from three focus groups and two individual interviews. Results: The participants performed quarantine tasks in a poor working environment with several problems, such as significant workload, lack of manpower, and inappropriate compensation system. Participants experienced obstacles in performing quarantine works, which had the lack of the detailed quarantine guidelines, work support and cooperation system. Participants suffered from civil complaints. Participants endured sacrifices in their personal lives while partaking in COVID-19 response work without holidays, and subsequently experienced health problems. And also participants said that it was necessary to secure expertise and effective communication for infectious disease management. Conclusion: The study results suggest that policies are required that are aimed to improve the working environment and the recruitment of experts in infectious diseases. In addition, the job stress related to the COVID-19 response by public officials in community health centers must be evaluated, and the relation of their job stress to physical and mental problems, as well as psychosocial stress, must be examined.

Influencing Factors of COVID-19 Stress, Impulsiveness, and Family Strength on Suicidal Ideation of Public Nursing Officials in Community Health Center (간호직 공무원의 코로나19 스트레스, 충동성, 가족건강성이 자살생각에 미치는 영향요인)

  • Purpose: This study was conducted to identify factors of COVID-19 stress, impulsiveness, and family strength on suicidal ideation of public nursing officials in community health centers. Methods: This study was a descriptive analysis study the subjects of this study were a total of 145 public nursing officials from community health centers in Busan. The data were analyzed using IBM SPSS/WIN 23.0 version. Result: The result of the predictor analysis showed that the motor impulsiveness (β=.383, p<.001), workload (β=.222, p=.003), cognitive impulsiveness (β=-.205, p=.012), bond (β=-.169, p=.033). The regression model showed an explanatory power of 24.1%. Conciusion: it is necessary to increase impulse and stress control ability, and to adjust the workload. In addition, it is necessary to systematically guarantee a working environment where practical vacation can be used, but it may not be a realistic alternative in a disaster crisis such as COVID-19, so indirect alternatives such as reinforcing infectious disease experts, improving the work environment, and psychological support to prevent suicide in advance are required.

Mental health literacy of local public officials and the mental health business attitude survey (일 지역사회 공무원의 정신건강지식 및 정신건강사업에 대한 태도조사)

  • The purpose of the present study was to investigate perceptions of mental health literacy and mental health services of local public officials in Korea. The participants were 273 civil servants working in three Gangwondo provinces. Data were collected from July 1, 2018 to August 15, 2018 using structured questionnaires, and SPSS Ver. 22.0 was used for data analysis. Results showed mental health literacy of public officials was 101.45 points (range 35-160). Mental health literacy level was found to be significantly dependent on occupation and levels of mental health education, mental health treatment experience, and family mental health treatment experience. Only 39.9% of respondents stated they were aware of mental health welfare centers. The most common condition that caused respondents to seek mental health services was depression (35.9%). In addition, 44.7% of the respondents opined that mental health care was improving due to community changes. An analysis of the relationship between perceptions of mental health welfare centers and mental health literacy scores showed a positive relationship between participant prior knowledge about mental health welfare centers and investment in mental health management (p=.012), suggesting a need for further development of educational programs using mental health nurses to improve mental health literacy among community officials.

Task Status of Dental Hygienists of Health Centers and Subcenters (일개 도지역 보건기관 근무 치과위생사의 직무실태와 개선방안)

  • This study was performed to investigate the task status and performance improvement plan of dental hygienists of Health Centers and Subcenters. The Data collected by self-administered Questionnaire survey of 203 dental hygienists of Health Centers and Subcenters located in Gyeongsangbuk-do were analyzed. The major results are as follows: For the degree of satisfaction in tasks, 73.9% of dental hygienists of Health Centers and Subcenters felt proud, 52.2% felt overwork, 32.0% hoped transferring to other worksite and the major reason of transferring was lack of promotion opportunity in officials of dental hygiene department. Dental hygienists received job education in addition continuous job education in last 3 years were 47.3%, 19.2% of dental hygienists performed the special dental health program for residents in last 3years. Almost half(53.7%) of dental hygienists replied that dental hygienists who were not engaged in dental health tasks should be engaged in dental health tasks. The major dental hygienic tasks performed by dental hygienic officials were support for dental treatment(41.6%). Dental hygienic officials answered that dental health tasks, school dental health program, vertical dental health program should be conducted as important tasks in order. And they replied that the most serious problem of dental hygienic tasks was not conducting dental health affairs due to 'lack of dental doctor'(40.9%), 'lack of concern for dental health tasks'(26.4%), and 'lack of budget and personnels for dental tasks'(19.0%), and the most important thing to improve dental hygienic tasks was 'posting dental hygienists in ministry of health and welfare(MOHW) and province', 'securing of budget and personnels for dental health tasks'.

A survey of public officials' perception on PHPP-TKM (한의약건강증진사업 담당자 인식도 조사)

  • Objectives : This study was aimed to survey the perception of public official on public health promotion programs in Traditional Korean Medicine (PHPP-TKM). Methods : Data were collected from 129 public officials who were in charge of PHPP-TKM business in the public health centers using the structured questionnaire. Collected data were analyzed through frequency analysis and T-test using SPSS 13.0. Results : Even though stroke prevention programs are implemented most often at present, respondents hope to practice dementia prevention programs in the future. The obstructive factor was measured by 5-point Likert scale, and the most obstructive factor was that there were neither standard manuals nor methodological guidelines for the programs($3.73{\pm}0.97$). The second most obstructive factor was that TKM-PHPP duplicated other public health programs($3.67{\pm}1.07$). The plan to activate TKM-PHPP was also measured by 5-point Likert scale. The most suggested plan was to develop competitive programs ($4.20{\pm}0.88$). The second most suggested plan was that of various integration of public health programs ($4.14{\pm}0.80$). Conclusions : The result of survey questionnaire suggests to develop various integrated programs, and to draw up a standard manual to activate the TKM-PHPP in public health centers.

Accreditation Standards and Procedures for Institutions of Early Childhood Education and Care (유아교육·보육기관 평가인정제 개발 연구)

  • A large sample (1090) of randomly selected early childhood education professionals and government officials rated each of the 133 standards of "A Model for Institutional Accreditation for Early Childhood Education and Care"(Yang, 1999) on a scale of 1 (least important) to 5 (most important). Findings were that all kindergartens and child care centers should be evaluated for accreditation every 3 years with 3-6 months for self-study and on-site validation visits by representatives of the appropritates agencies for 1-2 days. Evaluation results are should be used by institution personnel as a guide to self-supervision, by government officials as a funding standard and by parents as criteria of program quality. Essential accreditation standards included: facilities and equipment; curriculum; nutrition, health and safety; administration and management; and support systems. Safety and teacher-child interactions were most highly rated while parent involvement was not highly rated.

A Study on the Necessity of Psychological First Aid for Firefighters (소방공무원의 심리적 응급처치 필요성에 관한 연구)

  • The purpose of this study is to study the need for psychological first aid for stress of fire officials. a 1:1 in-depth interview with 10 paramedics to solve the research problem. Through interviews, the policy on the need for psychological first aid to improve the mental health of firefighters was proposed as follows by analyzing the working environment of paramedics, post-traumatic stress experience, and psychological first aid experience. First, fire officials need to be assigned to the site to educate in advance about post-traumatic stress and the resulting physical symptoms before being exposed to traumatic events. Second, fire officials should provide psychological first aid at an appropriate time after exposure to trauma cases. Third, it is necessary to supplement the manpower and institutional devices that will specialize in mental health work. Fourth, for customized professional medical services and psychological support suitable for fire officials, it is necessary to link fire-fighting complex healing centers and national firefighting hospitals.

Multilevel Analysis of Factors Associated with Perceived Good Health and Multimorbidity among Older Adults: Using the 2017 Community Health Survey

  • Purpose: This study aimed to identify individual- and regional-level factors associated with perceived good health and multimorbidity among older adults. Methods: Secondary analysis of the 2017 Korea Community Health Survey was conducted on a sample of 67,532 older adults. The individual level data set was combined with regional-level factors from the administrative data released on the Korea National Statistical Office website. Distribution of perceived good health and multimorbidity in 254 public health centers were calculated using sampling weights and presented as percentages. Multilevel logistic regression analyses were used to identify individual- and regional-level factors associated with perceived good health and multimorbidity. Results: Overall, 21.1% of subjects perceived their health to be good, ranging from 9.3% to 39.4% by region. The prevalence of multimorbidity was 15.9%, and varied between 6.6% and 22.6% by region. At the individual level, perceived good health was associated with men, younger age, higher educational levels, higher income levels, and those married and living with a partner and maintaining a healthy lifestyle. At the regional level, higher rates of health center personnel among public officials and higher levels of financial independence were associated with perceived good health. Multimorbidity was associated with marital status and healthy lifestyle, and higher rates of health center personnel among public officials. Conclusion: Regional factors such as health care personnel and local economy could affect population health. Our findings suggest the need to consider individual- and regional-level factors to promote good health among older adults and reduce the health gap by region.

Effects of Emotional Labor and Perceived Organizational Support on the Job Attitude of Public Health Workers (보건소 공무원들의 감정노동과 조직지원인식이 직무태도에 미치는 영향)

  • Objectives: This study was designed to determine the relationship among emotional labor, perceived organizational support, and job attitude in public health centers. Results: First, emotional dissonance had no effect on perceived organizational support. However, emotional endeavor had a positive effect on perceived organizational support. Second, perceived organizational support had a significant effect on job attitude. Third, emotional labor had no effect on job satisfaction, but emotional labor had a positive effect on job commitment. Fourth, perceived organizational support had a full mediating effect on the relationship between emotional labor and job attitude. The implications of this study are as follows. Conclusions: Higher emotional labor leads to lower job satisfaction and job commitment in public officials, thereby affecting job attitude. Accordingly, various systematic methods, such as flexible working hours and personnel policy, need to be implemented to alleviate the negative effects of emotional labor.
