방문건강관리사업 대상 재가노인의 허약정도와 예측요인 (Predictors and Frailty Level in the Frail Elderly Receiving Home Visiting Health Care Services)

  • 본 연구는 재가 허약노인의 허약정도에 영향을 미치는 예측요인을 파악하기 위해 대상자의 허약정도를 확인하고 일반적인 특성, 건강증진행위, 역량강화, 사회참여, 주관적 건강상태 간의 관계를 분석한 후 다중회귀분석을 실시하였다. 연구결과로는 재가노인의 허약정도 점수는 31점 만점에 10.05점으로 고위험 허약노인군에 포함되었고, 건강증진행위는 4점 만점에 2.56점, 역량강화는 5점 만점에 2.74점, 사회참여는 5점 만점에 2.58점, 주관적 건강상태는 14점 만점에 7.17점으로 중간 정도였다. 재가노인의 허약정도는 연령이 높을수록, 생활만족도가 불만족일수록 심했으며, 허약정도와 건강증진행위, 역량강화, 사회참여, 주관적 건강 상태와는 부적인 상관관계를 나타내었다. 대상자의 허약정도를 설명하는 예측요인으로는 주관적 건강상태, 건강증진행위, 연령으로 나타났고, 이 세 변수가 허약정도의 37.4%를 설명하였으며, 이중 주관적 건강상태가 가장 주요한 예측인자로 나타났다. 따라서 재가노인의 허약을 예방하고 관리하기 위해서는 허약에 영향을 미치는 주요 예측 요인들을 반영한 중재전략을 개발, 적용해야 할 것으로 사료된다. 본 연구의 결과를 토대로 다음과 같은 제언을 하고자 한다. 첫째, 재가노인의 허약정도를 사정할 수 있는 표준화된 도구의 마련이 필요하다. 둘째, 재가노인의 허약정도를 가장 잘 예측할 수 있는 변수로는 주관적 건강상태로 나타났으므로 재가노인의 허약을 예방, 관리하는 프로그램에는 주관적 건강상태를 강화하는 내용을 포함할 것을 제언한다.

중환자실 근무환경 특성에 대한 간호사의 인지도와 선호도 도구 검증 (Confirmatory Analysis of Perception and Preference Scales for Work Characteristics among Korean Nurses)

  • The study was conducted to centum the construct of individual perception and preference for work characteristics as personal factors influencing Korean nurses' job satisfaction. The subjects of the study were 231 nurses who are currently working in intensive care units and have been for a minimum of 6 months. The study used the Staff Perception and Preference Scale(Song et al., 1997) to measure the individual's perception and preference on the technical. practice. and management components of the ideal work environment. The Korean version of the Staff Perception and Preference Scale consists of 16 items on perception and 13 on preference with each item related on a scale from 1(not at all) to 4(a great deal). Psychometric testing revealed that the preference and perception scale is internally consistent with Chronbach's alphas of .83 for perception scale arid .80 for preference scale. The subscales of the perception and preference scale also showed acceptable reliability for the early stage of the development of the instruments with Chronbach alphas of .62-.76 and .69-.83 respectively. Criterion-related validity of the scale was tested by examining correlations with individual growth need that is conceptually close to individual preference. but not to individual perception. Individual growth need was significantly related to individual preference(r=.63, p<.05), but the correlation with the perception scale was not significant. A separate factor analysis for the each of perception and preference scales was performed with a three-factor loading solution based on a previous study. The results on the staff perception scale confirmed with varimax rotation that the items were cleanly and strongly loaded on technique. practice and management components, which together explained 50.7% of the variance. The factor analysis on the staff preference scale also yielded a three factor solution that explained 56.7% of the variance. but items on technique and management components were loaded together. This phenomena may due to the current nursing delivery system in Korea where nurses never experience either shared governance nor case management, and as a results they may not be able to consider management roles as their potential extended roles. Therefore, more efforts should be given to enhance nurses' autonomy and decision making in the technique, practice and management components of their work environment. Meanwhile, there is a need for continuously confirming and developing tools for individual perception and preferences to effectively enhance job satisfaction among Korean nurses through innovative work environments.

지역사회 재가 암환자 관리 체계 구축 - 일 시 지역을 중심으로 - (Development of a Community-Based Management System of Home-Stay Cancer Patients)

  • Purpose : This study, as the first year project of setting up a community based management system, was attempted for the cancer patients and their family to improve their quality of life; investigating and managing the cancer patients, educating volunteers and connecting the patients with the volunteers were performed. Method and result : The education of managing cancer patients for the volunteers was done once in lune for 2 days to the 80 volunteers. Questions about education effect, volunteer motivation and so on were made up. The survey showed, generally, education satisfaction level was high and a longer education and an intensive course were needed and was suggested that organizing a volunteer community be needed for the continuous further education and systematic management. As the result, after the public health center and volunteers deliberated, a volunteer community consisting of 4 teams, 28 members was organized, launched in Oct. and operated for the cancer patients and their family. For investigating and enrolling the patients, advertising on a local information paper, recommending of local doctors, publicizing by educating the heads of a subdivision of the city, the heads of a neighborhood association and the people in charge of the related local communities such as women's society, and surveying the community by volunteers were performed and the total, 41 patients were registered. Management of cancer patients was carried out by volunteers in a community and in a nursing school. A regional volunteers' community is composed of 23 members and they have worked 87 times, that is 3.8 times per capita on an average. The content of duties is attending the education (41.1%) the most. A volunteers' community of nursing students composed of 12 members have worked 135 times, that is 11.3 times per capita on an average. The content of duties - consulting with patients and home visiting (37.8 %) were the most and survey for investigating the cancer patients was the second. Conclusion : This study has the meaning that this is the guiding attempt in building a community based management system, and especially the achievement of this study is that a regional society organized a volunteer community for the cancer patients by itself and went into action for the cancer patients and their family. Furthermore, to activate this volunteering, it is necessary to keep managing volunteers and running continuing education or the intensive course of the volunteers. Indeed we should let the patients have good impression on this program through publicity and education for the residents to keep track of more cancer patients. For that, systematic and powerful cooperation of a self-administrative organization is required.

노인의 가족지지와 삶의 질 (A Study on Perceived Family Support and the Quality of Life in the Elderly)

  • In modern society, the human average life span has been prolonged due to medical benefits and changes in society, which results in the rapid and world -wide increase in the population of elderly. Consequently, the field of nursing science, as well as the field of many other discipline, has shown increasing interests in issues on the elderly. In addition, to improve the quality of life for elderiy people a great deal of effort has been made. The purpose of our study is to analyze the correlation between family support and quality of life in order to develop basic data for nursing interventions to maintain life satisfaction of the elderly. The sample consists of 108 subjects residing at home whose ages are over 65 years old. The data has been collected, from November 11, 1996 to November 23, 1996, through interviewing the elderly using a questionnarie. For the family supoort we used two: 1) the 5 - point Likert scale questionnarie developed by Gallo and Warren (Family support I) and 2) the 5-point Likert scale developed by H. S. Kang(Family support II), and for the quality of life we used the 3 - point likert scale questionnarie developed by Choi, Young Hee. For data analysis we used percentages, means, Pearson Correlation Coefficient and ANOVA. The results of our study are as follows: 1. For perceived family support I & II, the minimum score is 12 & 19, the maximum score is 32 & 46, the mean score is 24.49 & 34.90, respectively. 2. For Quality of life, the minimum score is 13, the maximum score is 39, the mean score is 28.61. 3. there is a very strong correlation between the perceived family support of the subjects and Quality of life (for I r=0.35047, p<.001 and for II r= 0.60558, p<.001). 4. The relationship between the general characteristics of the elderly and our two variables. family support and Quality of life, is as follows: 1) According to age(for II F=5.32, p<.01), the amount of monthly pocket money(for II F= 3.52, p<.05), inmate(for I F=2.93, p<.05, for II F=2.84, p<.05), economics(for I F=8.99, p<.01. for II F=7.51. p<.01), supporter(for I F=4.01. p<.01. for II F=3.43, p<.01), there is a statistically significant difference in family support. 2) According to the amount of monthly pocket money(F=6.69, p<.01), inmate(F=2.24, p<.05), economics(F=15.38, p<.01), there is a statistically significant difference in Quality of life. In conclusion, it can be said that the family support is an important variable to the Quality of the elderly life.

보건진료원의 지역사회 몰입과정 (Community Health Practitioner's Commitment into Community : on the Aspect of Primary Health Care)

  • Primary health care(PHC) has been established since A Health Law for rural residents has been legislated in 1980 following the WHO declaration, 'Health for All 2000'. in 1978. PHC services are presently assumed to be provided by 2038 Community Health Practitioners(CHP) to about 28% out of rural population in Korea. Most CHPs have confronted the adaptation process to the community being practiced although a CHP's role is to evoke community participation for the improvement of their health by themselves. So the purpose of this study is to describe and explain of the commitment of CHPs into the community. Data were collected by direct interview and tape-recording under subjects' permission till theoretical saturation were occured from 6 CHPs. The subjects were 41 years old and have served in the community for 9 years in average. Main questions and concepts were explored from data according to the procedure of the grounded theory methodology. The results are as follows. 1) The number of the main concepts were twenty four that identified Motive, Desire, Personal characteristics, Unfamiliarity, Denial, Feeling of isolation, Self-sacrifice, Kindness, Patience, Assimilation, Respect for the residents, Support by the family, Support by the residents, Achievement, Acceptance of realities, Use of resources, Inducement of cooperation from the residents, Changes of the difference from time orientation between CHP and residents, Attitude as a official, Technical support, Cost management, Satisfaction level, Acknowledgement by the residents and discrepancy. 2) The twenty four concepts were categorized to seven groups such as Motivation, Feeling of Heterogeneity, Self-discipline, Social support, Induced changes in the attitudes of residents, Familarity and Persistent discrepancy. 3) The categorized groups were analyzed on the base of the Causal Conditions, Central Phenomena, Contexts, Intervening Conditions, Action / Interaction Strategies, and Consequences. Central phenomenon in this study was identified to be the feeling of heterogeneity. Community health practitioners experienced unfamiliarity and denial from the community and felt themselves isolated in the first. In time, they won the trust of residents by their efforts including self-sacrifice, kindness, patience, and assimilation. Afterward, practitioners got self-confidence and familiarity with lesser feeling of heterogeneity. Nevertheless, practitioners could not commit themselves completely because of the persistent discrepancy between CHP and residents. 4) On the commitment process, the CHPs' feeling of heterogeneity were decresed and social support increesed and newly evolved induced change of residents through the continuous interaction between CHP and them The contribution of this study would be concluded as follows. 1) It is expected that effective strategies for more rapid committment into the community can be developed based on this study. 2) More easy committment would be possible for the newly appointed CHP through understanding of the committment process identified on this study.

우리나라 어머니의 자녀 양육의 의미 - 거제지역을 대상으로 - (An Ethnography of Child-Rearing Experiences of Korean Mothers Living on Koje Island)

  • Nursing practices should be based on the understanding of human beings. In order to understand human beings, it is important to study the lifestyles and thoughts of people in their natural environment. In this sense, the cultural aspects of a society need to be studied for a culture-bound nursing service. Child care, which is an important element of nursing, is also strongly influenced by the culture of a society. Therefore, a cultural study is necessary to understand the child-rearing practices of any society. The major purpose of this dissertation is to provide basic foundations for developing a culture-based theory for nursing intervention through studying traditional cultural elements of child care in Korean society. The study examined child-rearing practices in a small village on Koje Island in the southern part of Korea. It utilized ethnographic methodologies including participatory observations and in-depth interviews. The study participants were 9 Korean mothers living on Koje Island. The average age was 52. The data were collected between July in 1998 and December in 1999. The average number of interviews per person was 7-8, and the duration of each interview was approximately 2 hours. The data were analyzed using the Spradley Analytical Method. The following 9 major child-rearing aspects of mothers on Koje Island were discovered as a result of the study: 1. Firstly, mothers on Koje Island were mostly concerned about the "Old Birth Goddess' Curse", especially during their child's early years. This concern was evidenced by their careful behavior when their child was very young and by their praying to the Old Birth Goddess not to be jealous of their babies. 2. Secondly, they wished their children to live a different and better life than themselves. It was represented by their strong motivation toward their children's education as well as their expectation for their children's success. In traditional Korean culture, Korean people think that the rise and fall of the household depend on their offsprings. Therefore, Korean mothers wish their children attain to a higher level of social status through education. 3. Third, mothers are concerned about their children's righteousness. Mothers on Koje island expect their children to live with discretion, justice, strength, respect, harmony, and to do their best in life. 4. Next was an 'anticipation of their children's happy marriage'. The attributes of this category were an 'anxiety about their children's married life', and 'an expectation of a good spouse for their children'. Because Korean people believe that only a son can continue the bloodline of a family, especially Korean mothers have a great concern of the possibility of their daughters not having a son after marriage. Also they have different expectations toward their daughter-in-laws than son-in-laws. 5. Korean mothers also derived their satisfaction from their son. It was characterized by 'excessive affection toward their son', 'dependency on their son', and 'being afraid of their married daughter having a girl like themselves'. Korean society has been a patriarchy. Therefore, a son is beloved as someone who will take care of his old parents, be in charge of ancestral rites, and provide a daughter-in-law who can conceive a son. 6. The sixth category concerned 'the differences in their expectations for their children'. The attributes in this category were 'different expectations depending on their children's gender', 'different expectations depending on their children's ability', and a 'great sympathy toward children with low abilities'. Korean mothers expect their son to become better than their daughter. 7. The seventh category was related to their 'roles in child-caring practices'. Traditionally a child was raised in an extended family system in Korea So it was not the sole duty of a mother to bring up the child. Korean mothers used to receive much help rasing children from their in-laws, and family members. On the other hand, many children grew up by themselves, because their mothers were very busy taking care of housework. Furthermore, many children also grew up in poverty. 8. Mothers also had issues related to 'conflicts in child rearing'. They were characterized by 'lack of understanding', 'rudeness of children', and 'giving vent to one's anger'. 9. Finally, mothers regretted not doing their best in child-rearing practices. It was characterized by a 'bitter feeling of repentance', 'feeling irritated', and 'feeling of unsatisfaction'.

제주지역 암환자의 의료서비스 요구도 및 만족도 분석 (Needs and Satisfaction of Cancer Patients on the Medical Services in Jeju Special Self-Governing Province)

  • 목적: 본 연구는 제주지역에 거주하는 암환자의 요구도와 만족도를 파악하기 위한 조사연구이다. 방법: 2009년 7월 13일부터 7월 30일까지 제주대학교 병원에 내원한 암환자 174명의 의료서비스에 대한 요구도와 만족도를 분석하였다. 보건복지부에서 재가암환자 관리사업 시행 시 제시한 서비스 요구도 및 충족도 조사 항목을 이용하여 조사하였으며, 수집된 자료는 SPSS 12.0 for Windows program을 이용하여 분석하였다. 결과: 대상자는 위암과 대장직장암 등 위장관계 암환자가 33.3%로 가장 많았으며, 52.3%가 진단 후 경과 기간이 1년 이하였으며, 진단 후 5년 이상 경과한 대상자는 5.2%로, 최장기간은 12년이었다. 전체의 60.9%가 암으로 인한 수술 경험이 있었으며, 28.2%가 항암화학요법 경험이 있었다. 요구도가 가장 높았던 항목은 경제적지원(3.38점)이었으며, 치료 계획에 대한 상담(3.22점), 질병에 대한 정보(3.07점), 암 이외의 다른 질병 관리(2.97점) 순이었다. 만족도가 가장 높았던 항목은 종교상담(3.41점)이었으며, 간병보조(3.39점), 직업상담(3.26점), 가족 및 대인관계 상담(3.26점), 물리치료 및 재활치료(3.20점), 보험 상담(3.18점) 순이었으며, 만족도가 가장 낮은 항목은 경제적 지원(1.98점)이었다. 남성의 서비스 만족도는 3.1점으로 여성(2.8점)보다 높았으며, 자녀와 사는 경우 서비스 요구도가 가장 높았으며, 홀로 살거나 자녀와 사는 경우 서비스 만족도가 가장 낮았다. 의료보호 환자 및 암보험에 가입하지 않은 경우 서비스 요구도가 더 높은 것으로 나타났다. 대상자의 연령, 직업, 학력수준, 결혼여부, 종교, 수입에 따른 차이는 없었으나, 60대 및 수입이 높은 그룹에서 서비스 만족도는 가장 높고, 요구도는 가장 낮은 것으로 나타났다. 의학적 진단 및 치료 방법에 따른 서비스 요구도 및 만족도의 차이는 없었으나, 갑상샘암 환자에서 서비스 만족도가 다소 낮았으며, 방사선 치료 및 색전술을 받은 경우 만족도가 다소 낮은 것으로 나타났다. 결론: 암환자들은 경제적 지원 및 치료계획이나 질병에 대한 정보를 얻고자 하는 요구도가 높으며, 실제 경험하는 증상에 대한 관리는 잘 이루어지지 않는 것으로 보인다. 또한, 환자의 지지체계 및 경제적 지원 체계의 확보 여부에 따라 의료서비스에 대한 요구도도 다양하고, 주요 5대암 이외의 암환자에 대한 지원이 다소 부족한 것으로 보인다. 따라서, 의료기관에 내원 중인 암환자 및 재가암환자 모두를 대상으로, 대상자 특성 및 요구도를 고려한 지속적인 정책적 노력이 필요할 것이다.

말기 암 환자 가족의 전화상담을 통한 정보요구와 만족도 조사 (Information Needs and Satisfaction among Family Members of Terminal Cancer Patients through Phone Cancer Information Services)

  • 목적: 본 연구는 국가암정보센터의 전화상담을 통해 말기 암 환자가족의 정보요구와 만족도 조사를 통해 말기 암 환자 간호의 질을 향상하고자 시도되었다. 방법: 2007년 6월부터 2008년 3월까지 국가암정보센터를 통해 문의한 말기 암 환자 가족 중 만족도 조사에 동의하고 실제 통화된 113명을 대상하였다. 결과: 1. 정보를 요구한 말기 암 환자 가족은 113명으로 가족 중 자녀(82명)와 배우자(8명)가 주로 이용하였고, 연령은 40대(40명)와 30대(36명)의 이용이 많았다. 가장 많은 질문은 치료방법 117건, 말기 암 환자의 관리 46건, 말기 암 환자의 생활은 27건, 임종과 수명예측 18건, 입원기관에 관한 정보 16건, 경제적 지원은 15건 으로 나타났다. 전화상담 서비스에 대해 전반적으로 높은 만족을 보였다. 전화상담 서비스를 알게 된 경위는 인터넷이 69%로 가장 많았고 상담 후 개선사항을 묻는 질문에 홍보가 부족하다는 대답이 10.6%로 가장 많았다. 결론: 이 연구의 결과를 통해 말기 암 환자 가족은 치료방법에 매여 있어 남은 삶의 질을 높일 수 있는 호스피스에 대한 인식이 적은 만큼 국가적 차원에서 매스미디어를 활용한 호스피스 홍보방안을 모색해야 한다.

호흡기환자 시뮬레이션 교육에서의 동영상 제작 과제 활용 효과 (The effects of a simulation-based learning method utilizing the task of making video in raspiratory patients care)

  • 본 연구는 보건계열 대학생을 대상으로 시뮬레이션 기반 호흡기환자 교육에서 동영상 제작과제를 활용한 후 그 효과를 평가하기 위해 시행되었다. 연구는 J시에 소재한 D대학교 보건계열 학생 56명을 실험군 28명과 대조군 28명으로 나누어 실시하였다. 실험군에게는 2회기의 동영상과제를 활용한 사전교육이 시행되었으며, 각 회기당 소요되는 시간은 120분이었다. 대조군에게는 전통적인 강의식 사전교육이 1회기당 120분씩 2회 실시되었다. 본 연구의 자료수집은 2014년 11월에 시행되었고, 자료분석은 SPSS 18.0을 사용하였다. 일반적 특성과 각 변수들은 실수와 백분율로, 효과분석은 paired t-test와 unpaired t-test를 이용하여 분석하였다. 연구결과 동영상 제작과제를 활용한 시뮬레이션 교육군에서 학습만족도의 모든 하부영역과(p=.008~p<.001) 자기효능감 하부영역 중 자신감(p=.010)이 전통적인 강의를 활용한 대조군보다 통계적으로 유의하게 향상 되었다. 본 연구결과에서 동영상 과제를 활용한 교육이 학습자들의 흥미를 강화하고 자기주도학습과 의사소통 능력을 향상시키는 효과가 있음을 알 수 있었다. 따라서 보건대학생들의 전문직 역량을 강화하기 위한 프로그램으로 확대 시행할 필요가 있음을 제언하는 바이다.

지역 종합병원 간호사의 감정노동과 직무만족도 및 이직의도 (Emotional Labor, Job Satisfaction and Turnover Intention of Nurses in the Regional General Hospital)

  • 본 연구는 지역 종합병원 간호사의 감정 노동과 직무만족, 그에 따른 이직의도를 파악하여 감정노동 수준을 낮추고, 직무만족을 높여 이직의도를 감소시키면서도 효율적인 인사관리를 하여 질적 간호를 제공하기 위해 시행되었다. D시에 소재하는 종합병원에 간호사 중 참여에 동의한 대상자 100명을 편의 추출하여 2015년 4월 6일부터 4월 12일까지 설문지를 이용하였으며, 분석 방법으로 t-test, ANOVA, Pearson's correlation 을 사용하였다. 본 연구의 감정노동 평균 점수는 $3.37{\pm}.85$, 직무만족 평균 $2.86{\pm}.66$, 이직의도 평균 $3.47{\pm}.87$로 측정되었고, 감정노동은 직무만족과는 음의 상관관계(r=-.56, p<.001), 이직의도와는 양의 상관관계(r=.67, p<.001)를 보였으며, 직무만족은 이직의도와 음의 상관관계(r=-.64, p<.001)를 보였다. 본 연구 결과에 따라 병원 조직에서 중요한 위치인 간호사의 감정노동 수준을 낮추고, 직무만족을 높일 수 있는 제도적 방안 검토가 요구되며, 또한 간호사 사기를 충족시켜 질 높은 간호 서비스를 제공할 수 있도록 근본적 문제해결을 위한 적극적인 노력이 필요하다.
