Clinical Nurses' Interpretation of the Experiences of Breast Cancer Women after Breast Conservative Operation (유방암 여성의 유방보존술 후 경험에 대한 임상 간호사의 인지)

  • Purpose: Even though the breast is not removed, women with a breast conservative operation(BCO) are suffering from the psychosocial distress comparable with mastectomies. The purpose of this study was to identify how clinical nurses view essential nursing problems and nursing approaches to post operative breast cancer experiences. Method: To identify the clinical nurses's interpretation, they were asked to respond to a case involving a woman struggling with breast cancer. The nurses' responses were analyzed using a manifest content analysis strategy to determine themes in problem identification and nursing care strategies. Result: The clinical nurses recognized that most of the patient's experiences originated from a psycho-social problem. Fifteen nurses(83%) interpreted the breast cancer woman's experience derived from her psychosocial problem. They identified the most effective nursing approaches for breast cancer women following surgery as counseling and providing information. Conclusion: In conclusion, the results revealed that a psychosocial skill including an empathic communication skill is essential for excellent nursing in post operative patient care.

The Effects of Club Activities on Communication Skills and Psychological well-being of Nursing Students (간호대학생의 동아리 활동이 대인관계능력 및 심리적 안녕감에 미치는 영향)

  • This study was therefore undertaken to investigate the effects of club activities on the communication skills and psychological well-being of nursing students. Convenience sampling method was applied for 101 students enrolled in the nursing department of university in K city, of which 78 subjects participated in club activities. Data was collected from October 31 to November 30, 2022. Data were analyzed by t-test, and hierarchical multiple regression.The average score for Communication skill was 3.97±0.47, psychological well-being values was 3.62±0.44. Statistically communication skill showed a significantly environmental control(β=.27,p=.010) and positive interpersonal relationships(β=.23, p=.026)accounting for 46.8% of the total change and psychological well-being were significantly influenced by communication(β=.29, p=.009) accounting for 47% of the total change. Based on the results of this study, educational strategies and programs are needed to improve the communication skill and psychological well-being of nursing students.

An Identification Study on Core Nursing Competency (간호역량 규명을 위한 문헌 분석)

  • Purpose: This study was to scrutinize not merely the nomenclature of clinical competency suggested in nursing literature but also what core clinical competency nursing students should be focused on for improving competency-based curriculum. Methods: A comprehensive review on 69 domestic and 89 foreign related literature was conducted. After reviewing the full text of a total of 158 articles, only 23 articles with measurement tools were selected for scrutinizing while 135 articles with obscure definitions of clinical competency were excluded. Results: Clinical competencies including 120 concepts were identified. Those concepts were categorized as 30 clinical competencies according to their similarities. Seven core clinical competencies including 1)nursing knowledge, 2)nursing skill, 3)interpersonal skill/cooperation, 4)problem-solving, 5)professionalism, 6)nursing management/leadership and 7)research ability were derived from the 30 clinical competencies through the categorizing process. Conclusion: Teaching & learning strategies should focus on the integration of nursing theories and clinical practices based on competency-based curriculum considering the 7 core clinical competencies. Nonetheless, they include somewhat abstract concepts and some were not concrete enough to be applied to the nursing curriculum. Thus, further research is needed in order to develop consensus-driven clinical competencies and competency modeling which can suggest the interrelation between the core competencies.

Development of an Integrated Clinical Nursing Practice Course for Improvement of Nursing Competency among Nursing Students (간호학생의 간호수행능력 향상을 위한 통합실습평가 프로그램 개발)

  • Purpose: The purpose of this study was to develop and evaluate an integrated clinical evaluation program to increase the competency of senior nursing students. Method: A cross-sectional one group test design was utilized. Based on a framework derived from the prior studies, an integrated clinical evaluation program was developed. In order to assess the effect of the developed program, the senior students' experience with the program was measured. The participants were senior nursing students and the program was performed on November, 2005 after their semester was all finished. Result: The evaluation score for ability in application of nursing process was 83.87 and nursing skill was 85.69 by evaluators. The students reported that the program provided a chance to relearn and practice basic nursing skills and it was helpful in increasing decision making competency and their ability to apply the nursing process. Conclusion: This study suggests that the integrated clinical evaluation program contributes to cultivating a nurse with comprehensive nursing competencies and to improve the nursing students' knowledge, attitude, and skill.

A Comparison of Life Skill's Levels of Adolescents by Health Behavior (청소년들의 건강행태에 따른 라이프스킬(Life Skills) 수준 비교)

  • Purpose: The purpose of this study was to compare life skill's levels of adolescents. Methods: This was descriptive comparative survey using as convenience sample of 460, fifth and sixth grades in elementary school in Seoul Korea. The life skill was measured by life skill scale that 78 items regarding self esteem, social skill, decision making skill, and goal setting skill. Collected data were analyzed by frequency, mean, t-test and Pearson correlation using the SPSS/WIN 20.0. Results: The family-related self-esteem was higher ($26.02{\pm}3.73$) than other self-esteem score. Low family-related self-esteem associated with health behavior such as current drinking, accessing to pornography sites on internet in adolescents. There was a significant positive correlation between self-esteems (cognitive, peer, family, body, general), decision making skill and goal-setting skill. Conclusion: The results of this study suggest that it is important to develop a program that focuses on enhancing family-related self-esteem to effectively reduce the risk behaviors among adolescents.

Simulation Nursing Education Research Topics Trends Using Text Network Analysis (텍스트네트워크분석을 적용하여 탐색한 국내 시뮬레이션간호교육 연구주제 동향)

  • Purpose: The purpose of this study was to analyze the topic trend of domestic simulation nursing education research using text network analysis(TNA). Methods: This study was conducted in four steps. TNA was performed using the NetMiner (version 4.4.1) program. Firstly, 245 articles from 4 databases (RISS, KCI, KISS, DBpia) published from 2008 to 2018, were collected. Secondly, keyword-forms were unified and representative words were selected. Thirdly, co-occurrence matrices of keywords with a frequency of 2 or higher were generated. Finally, social network-related measures-indices of degree centrality and betweenness centrality-were obtained. The topic trend over time was visualized as a sociogram and presented. Results: 178 author keywords were extracted. Keywords with high degree centrality were "Nursing student", "Clinical competency", "Knowledge", "Critical thinking", "Communication", and "Problem-solving ability." Keywords with high betweenness centrality were "CPR", "Knowledge", "Attitude", "Self-efficacy", "Performance ability", and "Nurse." Over time, the topic trends on simulation nursing education have diversified. For example, topics such as "Neonatal nursing", "Obstetric nursing", "Pediatric nursing", "Blood transfusion", "Community visit nursing", and "Core basic nursing skill" appeared. The core-topics that emerged only recently (2017-2018) were "High-fidelity", "Heart arrest", "Clinical judgment", "Reflection", "Core basic nursing skill." Conclusion: Although simulation nursing education research has been increasing, it is necessary to continue studies on integrated simulation learning designs based on various nursing settings. Additionally, in simulation nursing education, research is required not only on learner-centered educational outcomes, but also factors that influence educational outcomes from the perspective of the instructors.

Comparison of Factors affecting Clinical Competence between Associate and Bachelor Nursing Students Completed Nursing Courses (학제별 간호학생의 임상수행능력 영향요인 비교)

  • Purpose: This study was to compare the influencing factors of clinical competence between associate and bachelor nursing students completing nursing courses. Method: The subjects were 429 students completing the nursing courses (172 bachelor course students, and 257 associate course students). The study design was cross sectional. Results: Bachelor students showed a significant higher score in clinical competence than associate students. There were significant influencing factors on clinical competence including critical thinking disposition, professional conduct, communication skill, theoretical education satisfaction, and practice education satisfaction. The strongest factor influencing clinical competence was critical thinking disposition. All the factors mentioned above explained 49% of clinical competence in associate courses, and 40.0% in bachelor courses excluding the factor of practice education satisfaction. The level of theoretical and practice education satisfaction of bachelor courses was significantly lower than associate. Conclusion: It is recommended that associate course nursing students should be encouraged in critical thinking disposition, professional conduct, and communication skill. Bachelor course nursing students should be encouraged in practical education including practical environments, instruction methods, and content for improving clinical competence.

Effects of Open Laboratory Self-directed Practice on Knowledge, Self-confidence, Skill Competency, and Satisfaction of Intravenous Infusion (개방 자율실습이 간호학생의 정맥주입술기에 대한 지식, 자신감, 수행능력 및 실습만족도에 미치는 효과)

  • Purpose: This study was done in order to identify the effectiveness of open laboratory self-directed practice on knowledge, self-confidence, skill competency, and satisfaction of intravenous infusion in nursing students. Methods: This is a quasi-experimental study using a non-equivalent control pre-posttest design. The participants were 32 students comprising an experimental group and 30 students comprising a control group. The experimental group was allowed to perform open laboratory self-directed practice only. Pretest and posttest was performed through questionnaires of knowledge and self-confidence, and competence and satisfaction were assessed using checklists at posttest. Results: The score of Knowledge in the posttest showed no significant difference between both groups (F=1.28, p=.290), and those of self-confidence (F=3.87, p=.014) and competency (F=9.55, p<.001) of the experimental group were significantly higher than those of the control group. Conclusion: Open laboratory self-directed practice was effective in improving nursing students' self-confidence and competency of intravenous infusion. Therefore, students should be encouraged to actively participate in open laboratory self-directed practice and the effective strategies should be developed.

Effectiveness of Student Learning with a Simulation Program focusing on Cardiac Arrest in Knowledge, Self-confidence, Critical Thinking, and Clinical Performance Ability (심정지 시뮬레이션 교육이 간호학생의 지식, 자신감, 비판적 사고성향 및 임상수행능력에 미치는 효과)

  • Purpose: This study was designed to describe the effects of a nursing simulation focused on patients with cardiac arrest. The study was designed to measure knowledge, self-confidence, critical thinking, and clinical performance ability of nursing students. Methods: A non-equivalent control group quasi-experimental design was used. Thirty students in the experimental group received two hours of pre-learning and three hours of skill and simulation practice. Another thirty students in the experimental group received a two hour conventional lecture and three hours for skill practice. The post survey was completed by both groups. Results: Students in the experiemetal group scored significantly higher than students in control group. The critical thinking disposition was not significantly different between the two groups of students. Conclusion: The results indicate that a simulation education program is more effective in a number of areas including knowledge, self-confidence, and clinical performance. It is recommended that simulation education should be expanded to various clinical situations.

Effects of a Self-Management Program for Community Dwelling Patients with Chronic Mental Illness (간호학생을 활용한 재가 만성정신질환자의 자기관리 프로그램 개발 및 효과)

  • Purpose: The purpose of this study was to develop and evaluate a self-management program for community dwelling patients with chronic mental illness so that they could improve their daily living skills and interpersonal relationships. Methods: A nonequivalent control group pre-post test design was used. The participants with chronic mental illness were recruited from a community mental health center. Twenty-two participants were assigned to the intervention group and 19 to the control group. The self-management was provided by 8 trained nursing students, and consisted of 8 sessions including education and practice. Each session was composed of strategies to improve self-care of community dwelling patients with chronic mental illness. Baseline and post-intervention measurements were done using a daily living skill performance scale and interpersonal relationship instrument. Results: Scores for daily living skill performance were not statistically significant, but scores for interpersonal relationship were statistically significant. Conclusion: The results of this study show that the self-management program had a positive impact on patients with chronic mental disorders. Therefore, it is suggested that the program could be used as an effective nursing intervention for patients with chronic mental disorders being cared for in community mental health centers.
