A Model for Nursing Workforce in Hemodialysis Room Using System Dynamics Approach (시스템다이내믹스를 이용한 혈액투석실 간호인력 산정 연구)

  • Purpose: The objective of this study was to determine the cause-effect and dynamic change mechanism among factors that affect calculation of the nursing workforce in the hemodialysis room and to establish a system dynamics model and create a strategic simulation to be used to increase efficiency in the clinical situation. Method: The system dynamics approach was adapted to build a simulation model for calculating the nursing workforce. The model was built using Vensim 5.0b DSS. Results: The results were as follows for Scenario A: The highest level of nursing service quality, patient satisfaction, and nurse satisfaction, and the lowest level of turnover intention is maintained when the ratio of patients to nurse is 3.5:1 compared to all other situations. Scenario B: At the ratio of patients to nurse 4.0:1 nursing service quality, patient satisfaction, and nurse satisfaction dropped slightly, and turnover intention also rose slightly. However with time the results showed improvement. Conclusion: In terms of economic efficiency in the hemodialysis room, the ratio of patients to nurse for the best nursing workforce should not exceed 4.0:1.

The Effects of an Advanced Cardiac Life Support Training via Smartphone's Simulation Application on Nurses' Knowledge and Learning Satisfaction (스마트폰 어플리케이션을 활용한 전문심폐소생술 시뮬레이션 재학습이 간호사의 지식 및 교육 만족도에 미치는 효과)

  • Purpose: The purpose of this study was to examine how an Advanced Cardiac Life Support (ACLS) simulation application via smartphone effects nurses' ACLS knowledge and learning satisfaction. Methods: The participants were selected from nurses in medical Intensive Care Unit (ICU), surgical ICU and emergency room. The experimental group consists of fifty nurses who were self-learned with ACLS simulation application via smartphone and the control group of seventy-one nurses who used traditional learning materials. Outcome variables included nurses' knowledge and learning satisfaction which were collected before and after the intervention. Results: The scores of ACLS knowledge were higher in the control group compared to the experimental group (p=.001) while learning satisfaction showed no statistical difference (p=.444). In learning satisfaction, the experimental group showed higher interest than the control group (p=.019) while the control group rated higher on the item, 'the contents of education was reliable' (p=.007). Conclusion: ACLS knowledge score was graded higher in control group that used traditional learning method than the experimental group that used the smartphone application. This study showed that training with the new material significantly increased nurses' interest in ACLS education. Hence, more applications for smartphones should be developed to provide self-learning environment for nurses and improve care quality.

Effects of Simulation Education on the Communication Competence, Academic Self-efficacy, and Attitude About the Elderly for Nursing Students: A learning approach based on an elderly-with-cognition-disorder scenario (인지장애 노인 시뮬레이션 교육이 간호대학생의 의사소통능력, 학업적 자기효능감, 노인에 대한 태도에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effects of simulation in nursing education based on caring for elderly cognition disorder patients. The education consisted of a caring program for patients that included a process of assessment of a patient's mental status, diagnosis of the patient's health condition, and intervention to address the problems by using therapeutic communication. Methods: A nonequivalent control group pretest-posttest design was used. A total of 69 subjects (undergraduate students) participated in the education and they were assigned to two groups: the experimental group (n=32) and the control group (n=37). Data-gathering structured questionnaires that included communication competence, academic self-efficacy, and attitudes about the elderly. The data were collected from October 2013 to December 2013, and statistical analyses were conducted with-test and t-test using the SPSS 21.0 program. Results: With respect to education, there was significant improvement in communication competence in the experiment group (t=2.41, p=.022) compared with in the control group (t=.69, p=.494). However, there was no statistically significant difference in academic self-efficacy and attitude about the elderly. Conclusion: Simulation-based education should continue to be developed further for better elderly-patient care. Integrated education in particular using a high-fidelity simulator will contribute to improvements in nursing competence in this area.

Effects of Simulation Based Education Using Standardized Patient for Contact Precaution Infection Control for Nursing Students (표준화 환자를 활용한 접촉주의 환자 간호 시뮬레이션 교육의 효과)

  • The purpose of this study was to investigate effect of simulation based education using standardized patient for contact precaution infection control for nursing student. This study was conducted by including 67 nursing student A university from October to December 2019. This study was mixed method research design. Knowledge and performance confidence related to multidrug resistant organism(MDRO) infection control were measured using questionnaires pre and post test, analyzed using paired t-test and reflection sheet was analyzed using content analysis method. After intervention, two variables were increased significantly. Results of the content analysis showed there were 39 significant statements, which were classified into 13 categories. These results suggest that education on simulation program using standardized patient for contact precaution infection control is effective strategy to enhance knowledge and performance confidence related to MDRO infection control and practical nursing infection control skill, patient centered care, interprofessional collaboration.

The Effects of Simulation-based Education on Nursing Students' Presence in Education, Systems Thinking and Proactivity in Problem Solving (시뮬레이션 교육이 간호대학생의 교육실재감, 시스템사고능력 및 문제해결 적극성에 미치는 효과)

  • Purpose: This study aimed to develop and apply simulation-based education, and to verify the effects of this type of education on nursing students' presence in education, systems thinking, and proactivity in problem solving. Method: Subjects were 69 senior college students recruited through convenient sampling. This study used a one-group pre-posttest quasi-experimental design. A structured survey was administered a week before and after provision of the simulation-based education once a week for 4 weeks. Data were analyzed using descriptive statistics and t-tests. Results: Teaching presence increased after the educational intervention; instructional design and organization, and aspect of direct facilitation subscales improved. Learning presence increased after the educational intervention; although cognitive presence did not change, emotional presence increased. With regard to emotional presence, perception and expression of one's emotional status increased, although emotional management did not change. Moreover, systems thinking increased. Among the subscales, team learning showed an increase after the educational intervention. However, there was no significant difference in proactivity in problem solving, although it showed an increase after the educational intervention. Conclusion: Based on the aforementioned study results, there is need to establish educational environments for qualitative teaching and learning presence, and devise strategies to increase learning effects with various teaching methods and type of content.

Content analysis of real-time simulation video observation records about cases of patients with chronic obstructive pulmonary disease-focusing on nursing skills performance (만성폐쇄성폐질환 환자 사례에 대한 실시간 시뮬레이션 동영상 관찰기록 내용분석-간호술 수행을 중심으로)

  • This study is a qualitative study in which nursing students analyzed the contents of nursing skills recorded on a structured video observation record sheet while observing a colleague team's real-time video of chronic obstructive pulmonary disease scenario implementation during simulation practice. As a result of the analysis using the content analysis method, categories and topics of effective and ineffective aspects were derived in the areas of observation record: accuracy of procedures, adherence to aseptic technique, consideration of safety and safety, explanation and education, and purpose explanation and method education. This study is meaningful in that it presents factors that can increase the efficiency of nursing education through simulation-based practice.

  • This study examined the effect of convergence action learning based virtual simulation program for nursing college students. The study was carried out according a nonequivalent control group design. The study subjects were 54 nursing college students. The data collection period was from April 12, 2021 to June 18, 2021. Collected data were analyzed using SPSS PC+ 23.0. The experimental group had significantly different to communication skills, and clinical competency in comparison to the control group. This suggests that the convergence action learning based virtual simulation program can be applied as a way to increase nursing students' communication skills, and clinical competency.

The Effect of Senior Simulation on Nurses′ Attitude Toward the Elderly (노인유사체험이 간호사의 노인에 대한 태도에 미치는 영향)

  • Purpose: This study was performed to explore the effect of senior simulation on nurses' attitudes toward the elderly. Method: Twenty-seven nurses working in various settings such as acute hospitals, community health centers, geriatric hospitals, and clinics were recruited. Among them, 25 subjects completed the whole experimental protocol. Aging Semantic Differential Scaling was utilized to evaluate attitudes toward the elderly and 'Suit for Experiencing Being Aged' from the Sakamoto Model was provided for the experiment. Before and after the experiment subjects filled out questionnaires. Result: Attitude score before experiment was 4.36, which indicates neutral attitude. Objective attitude scores were not different significantly after experiment. However subjective statements indicated attitude changes in a positive way. Conclusion: Senior simulation can affect nurses' attitude toward elderly in subjective way. That is, nurses became more empathetic and understanding to elderly's physical limitations and felt more initiative nursing approach were needed in caring elderly.

Development and Evaluation of a Scenario for Simulation Learning of Care for Children with Respiratory Distress Syndrome in Neonatal Intensive Care Units (시뮬레이션 학습을 위한 호흡곤란증후군 환아 시나리오 개발 및 학습 수행 평가)

  • Purpose: This study was done to develop a scenario and evaluate student performance in simulation learning of care for children with respiratory distress syndrome in neonatal intensive care units. Methods: To test the application effect, a one group pre-test design was applied. The scenario based on actual patients and textbook material was developed through several meetings of experts. The scenario was used with 17 groups of 55 senior nursing students who participated voluntarily. Results: Contents were organized focusing on the nursing process for simulation learning. In the application of knowledge and skills, nursing students had high scores in the contents of observation of oxygen saturation, and care to relieve dyspnea. Participants' ability, especially in suction and oxygen supply in the evaluation of objective structured clinical examination was not adequate. There was a significant positive correlation between problem-solving ability and satisfaction in learning. Conclusion: The respiratory distress syndrome simulation scenario developed in this study was an effective tool to give students experience in problem solving and critical thinking ability under conditions similar to reality. The development of various scenarios for child nursing care is needed.
