아버지의 영아 돌봄에 관한 문헌연구 (A Study of father's care giving in infancy)

  • These days social and economical changes have influence on the structure of family and the role of family members. Working mothers and widowers with children are increasing because of economical difficulties. Support from relatives are decreasing because of the conspicuous trend toward nuclear families. According to these reasons androgynous fathers are required. Today's fathers in Korea socially and culturally have learned about traditional parenting, but they are changing their fathering styles to meet the demands of the times. However they don't have their own fathering models. Therefore nurses who hold an advantageous position to teach and support from clinic have to encourage them to care their infants. The purposes of this study were to define father's care giving in infancy, understand influencing factors on fathering, and the differences between fathering and mothering, then contribute to nursing implementation for supporting fathers. This study was designed to review references about father's care giving. The results were follows: Six aspects of parent participation were direct care. indirect care, play, decision-making concerning the child, amount of time of sole responsibility for the child and overall availability to the child. Direct care involved feeding, bathing, going to child if child awakens. dressing, putting child to bed, taking child to doctor, nurse, or dentist, transporting child to and from sitter, day care, or school, washing child's hair. Indirect care involved cleaning up after child, preparing child's food, fixing child's broken playthings, washing child's clothes, arranging baby-sitting, shopping for child's toys and clothes, transporting baby-sitter to and from your home. Young fathers were gradually participating in direct care like feeding, taking child to doctor. Father's care giving stimulated mothering and promoted parent-infant relationship. Influencing factors of fathering would be divided into father characteristics, surrounding factors, infant attributes. Father characteristics were age, role perception, relationship with parent. Surrounding factors were the opportunity of early contact, support system, spouse's expectation, marital adjustment, feeding type, past experience of care giving. Infant attributes were temperament, behavior, age, sex. The differences between fathering and mothering were reviewed. Fathers were poor at care giving. but their caring was similar to mother's. This subtle difference positively worked upon infant's growth and development. On the basis of these theoretical data, nurses can empower fathers to cooperate with mothers in caring infants.

요양병원의 입원급여 적정성 평가 결과에 미치는 영향 요인 분석 (Factors influencing the composite quality score from the quality assessment program for long-term care hospitals)

  • Purposes: This study purposed to identify factors influencing the composite quality score from the quality assessment program for long-term care hospitals Methodology: The study variables was obtained from HIRA(Health Insurance Review and Assessment Service): the composite quality scores and hospital variables such as number of doctors, nurses, beds, medical technicians, medical equipments, administrative region, ownerships from 3rd (2010) to 7th (2018) quality assessment program. National Statistical Portal(www.kosis.go.kr) provided the number of senior citizens aged over 65 in city·county·district area. SAS 9.4 was used for the data processing and used to analyze the data. Findings: The results showed that composite quality score increased past 9 years. Hospital variables such as number of doctor, nurse, medical technicians, bed and public hospitals had significant positive relationship with the composite quality score. Administrative region(district) showed higher scores compare to that of city. Compare to the score of quality assessment year(2010), as the assessment year move to 2012, 2013, 2015, 2018, results showed higher significant positive coefficients. Practical Implication: Continuously improve the performance of long-term care hospitals, current quality assessment program are needed to update their system such as adopting indicators measuring the service process, or compensating the cost for quality assessment program. It will enable to provide more reasonable and accurate performance assessment scores.

의료기관에서 조직역량과 개인역량이 직무만족 및 업무수행에 미치는 영향 (The Effect of Organizational and Individual Capabilities on Job Satisfaction and Work Performance in Healthcare Institutions)

  • 본 연구는 의료서비스 산업에서 역량을 의료기관 조직과 개인의 차원으로 분류하여 조직역량과 개인역량이 조직구성원들의 직무만족 및 업무수행에 미치는 영향 정도를 살펴보고자 하였다. 이를 위해 개인역량과 조직역량의 구성요인과 함께 직무만족, 업무수행에 대한 개념을 정립하고 이들간의 관계를 검증하여 의료기관에 필요한 역량이 무엇인지를 파악하였다. 본 연구는 국내의 한 의료기관에 근무하는 의사와 간호사 243명을 대상으로 하였으며, 설문 분석결과, 첫째, 조직역량은 직무만족 및 업무수행에 유의한 영향이 있는 것으로 나타났다. 둘째, 개인역량은 직무만족에 유의한 영향이 있는 것으로 나타났지만 업무수행에는 유의한 결과를 얻지 못했다. 셋째, 직무만족은 업무수행에 유의한 영향이 있는 것으로 나타났다. 이러한 결과는 의료기관 경영진 및 정책입안자에게 조직역량과 개인역량 개발의 중요성에 관한 기초적인 정보를 제공하였다.

응급구조과 학생이 임상실습시 경험하는 스트레스요인에 관한 분석 (An Analysis Study on Stressor Experienced by Emergency Medical Technology Students during Clinical Practice)

  • This study is attempted for the purpose of clearly understanding the stressors which emergency medical technology students experience during clinical practice, giving guidance in desirable direction and improvement of clinical practice, and giving the basic materials so that emergency medical technology students may practice effectively. The design of this study was a descriptive survey. The subjects in this study were 59 emergency medical technology students who were 2nd grade in D college. The collection of questionnaires took 3 days from September 1, 1999 to September 3, 1999. This researcher explained the purpose of this study to students, after they agreed to this survey, the researcher gave the questionnaire to them, had them answer the questions and collected the questionnaire immediately. Data analysis included percentage, mean and standard deviation, t-test and ANOVA trough SPSS computer program. The results of this study are, briefly, as follows; 1. In 'satisfaction-degree to specializing in the science of emergency medical technology' 27.1% of the subjects is revealed as 'satisfied' and in 'satisfaction-degree of clinical practice', 8.5% of the subjects is revealed as 'satisfied'. 2. Viewed from each of the categories of stressors which emergency medical technology students experience during clinical practice, 'ideals and values' is the category in which emergency medical technology students experience most severe stress at a mean 3.25 and 'personal relations' is the category in which emergency medical technology students experience mildest stress at a mean 2.82. The total stress degree mean is 3.06, which emergency medical technology students experience during clinical practice is revealed as 'moderate' stress. 3. Considering the stressor involved in each category, the results are as follows; (1) In 'environment', 'orientation to the hospital before clinical practice' is revealed as the most severe stressor at a menu 3.44. (2) In 'personal relations', 'relations with doctor and nurse' is revealed as the most severe stressor at a mean 3.59. (3) In 'role', 'unilaterally when their role is seems to be obscure' is revealed as the most severe stressor at a mean 4.10. (4) In 'ideals and values', 'difference between college practice and hospital practice' is revealed as the most severe stressor at a mean 3.88. (5) In 'activity', 'repetition of simple and functional work during clinical practice' is revealed as the most severe stressor at a mean 3.98. (6) In 'total stress factors', 'unilaterally when their role is seems to be obscure' is revealed as the most severe stress factor at a mean 4.10 and 'relations with comrade students' is revealed as the stress factor causing mildest stress at a mean 2.20. 4. Considering the relationship between the categories of stressors and the general characteristics, the results are as follows; (1) 'Environment' is revealed as being positively correlated with 'satisfaction degree of clinical practice'(F=3.53, p= .0359). (2) 'Ideals and values' is revealed as being correlated with 'sex'(t=-2.31, p= .0240) and 'satisfaction degree of clinical practice'(F=3.31, p= .0438).

Identification of Hepatitis B (HBV) and C (HCV) Virus Infection among Doctors and Nurses in Tertiary Hospitals in Mongolia

  • The studies of M. Colombo (1989) and W. Lange (1992) showed that 30~40% of people became chronic after suffering from hepatitis B virus (HBV) and C virus (HCV) infection, and about 50% of the chronic cases transformed into primary liver cancer. There have been few studies done in Mongolia on hepatitis infection among health professionals, particularly in nurses. In a study done by Chimedsuren (8), the study showed that 19.4% of people with identified surface hepatitis B antigen (HBsAg) and antibodies to hepatitis C virus and 8% of people with the identified nucleotide of RNA for the hepatitis C virus (polymerase chain reaction) had an acute form of hepatitis C. Studies on the hepatitis virus genome damaging effect on liver cells showed that genotype 8 (A, B, C, D, E, F, G, TTV) had the most damaging effect on liver cells (Hahn and Faeka, 2007). Several studies have shown a relationship between hepatitis B virus infection and a lack of compliance regarding safety regulations and rules by medical personnel. Results of a study from the Maternal and Child Health Research Center showed that tests done to detect hepatitis B virus antigen and antibodies to C virus did not reveal anything. Both antigen and antibodies in 69% cases did not show, and separately, B virus and antibodies to hepatitis C virus were identified in 13% and 9%, respectively. Results of the tests taken from health personnel in Shastin Central Hospital showed that in 76% of the cases, the B virus antigen with C virus antibodies was not identified. In 8% of the cases, the B virus antigen was present on its own. The combination of B the virus antigen and C virus antibodies were present in 8% of nurses and doctors, respectively. 82% of the cases had negative results for the detection of a combination of B virus antigen and C virus antibodies taken from health personnel from the State Central Clinical Hospital whereas the B virus antigen and C virus antibodies by themselves were present in 7% and 14% of the cases, respectively. Combined cases of the B virus antigen and C virus antibodies were identified in 4% of the personnel. Results of the tests taken from the health personnel in the Hospital of the Ministry of Justice and Internal Affairs showed that in 79% of the cases, the B virus antigen with C virus antibodies were not identified. Separately, the B virus and antibodies to hepatitis C virus were identified in 8% and 13% of the cases, respectively.

서울소재 일개 종합병원의 CRM에 대한 기대만족도와 실제만족도 비교와 VOC활용 (A Comparison Research on the Expected Satisfaction and Actual Satisfaction on VOC of one General Hospital)

  • 본 한국 보건산업은 병원들의 경쟁과 함께 급격히 변화하고 있다. 과거에 병원은 경쟁력있는 전략방법없이 이윤을 창출하였다. 게다가 병원은 고객들의 요구와 원하는 것을 중요하게 여기지 않았다. 그러나, 요즈음 점점 심해지는 병원들의 경쟁은 경쟁적인 관리 전략과 고객관계관리를 매우 중요시 다루고 있다. 본 연구는 의료기관을 방문하는 외래환자와 입원환자에 대해서 의료서비스를 받기 전의 기대만족도와 의료서비스를 받은 후의 만족도를 비교하고 VOC에 대한 환자들의 반응을 분석한 후 환자만족도를 높이기 위한 방안을 찾고자 한다. 연구의 방법은 환자의 만족도를 좀 더 자세하게 파악하기 위해 본 연구의 목적에 맞게 개발된 설문지를 외래, 입원환자 각각 100명을 조사하였으며 분석은 의료서비스의 전, 후의 만족도 비교이므로 Paired - Test와 기술통계등을 이용하였다. 연구결과 외래환자는 간호사의 친절도에서만 만족도의차이가 통계적으로 유의한 것으로 나왔으나 입원환자의 경우 의료진과 직원의 친절도 그리고 회진서비스에서 만족도의 차이가 통계적으로 유의한 것으로 나왔다. 따라서, 조사된 입원, 외래환자 만족도 항목 중 의료서비스관련 만족도에 차이를 보이고 있으므로 그에 대한 좀 더 상세한 분석과 대책 방안 강구를 위해 VOC 를 지속적으로 발전 활용하는 것이 환자만족도를 높이고 그에 따른 의료기관의 경영효율화를 높이는데 매우 도움이 클 것으로 사료된다.

처방전달체계에 대한 사용자 만족도에 영향을 미치는 요인 (Factors Affecting Users' Satisfaction with Order Communicating System)

  • Objectives : To identify the factors affecting users' satisfaction with the Order Communicating System(OCS) and to highlight the factors important for the successful establishment of OCS. Methods : A Users Satisfaction survey was sent to 4,513 people, consisting of 1,503 doctors, 2,379 nurses, 255 pharmacists and 370 administrative workers in 16 hospitals which had introduced OCS. The response rate was 63.9%. Measurement of users' satisfaction was peformed with the instrument which was used in Doll's study. Some aspects of Doll's instrument were adjusted according to the aims of this study. The classifying sections of this survey included age, job and status classification, computer experience, OCS education, duration of daily OCS use, type of order entering, number of personnel in the Hospital Information System's department, cost of OCS, problem frequency, proportion of work managed by hand, OCS type, and Hospital establishment type. Results : There was a positive correlation between satisfaction level and managerial status throughout all job classifications. Irrespective of the importance of OCS education as a factor relating to users' satisfaction, the additional work load caused by OCS lowered users' satisfaction. Different factors affected users' satisfaction according to job and status classification. The composition of factors affecting the pharmacist and administrative worker satisfaction levels was simpler than that of the doctor and nurse levels. There were no statistically significant differences between the actual computer experience duration of daily OCS use and users' satisfaction with OCS. Conclusions : There was an understandable relationship between users' attitude to OCS and factors affecting users' satisfaction. The results of this study could be used as a basis for the successful expansion of the operation of OCS. But more detailed studies on users' satisfaction and further improvements of methodologies are required for the successful establishment of OCS.

말기 암환자와 가족을 위한 집단상담 프로그램 - 서울대학교병원 경험의 분석- (The Group Counseling Program for Terminal Cancer Patients and their Family Members in the Seoul National University Hospital)

  • 목적 : 본 연구는 서울대학교병원의 말기암환자와 가족을 위한 집단상담 프로그램을 소개하고, 현황 및 문제점을 파악하고자 하였다. 방법 : 1996년 한 해동안 상담에 참석한 말기암환자 및 가족들의 상담기록지를 중심으로 연구자들이 상담한 내용을 분석하였다. 결과 : 참석자 312명은 가족(84%)이 환자(16%)의 4배 이상 참석했고, 대부분 1회만 참석하고 있음을 보여주었다. 참석한 환자 또는 가족이 돌보는 환자의 현황은 나이별로 60대, 50대, 40대가 많았고, 암의 종류는 폐암, 위암, 간암 순으로 많았다. 가족의 특성은 261명으로 배우자, 자녀, 며느리, 형제자매, 부모순으로 많았다. 프로그램에 오게된 경로는 의사의 권유(69%), 병원 포스터(26%), 기타 순이었다. 이것은 의사가 환자와 가족을 집단에 참여시키는데 중요한 역할을 담당하고 있음을 보여준다. 질문은 우선적으로 의료적인 정보에 대한 욕구가 많았다. 이것은 환자나 가족이 의료진으로부터 정보를 제대로 전달받지 못하고 있음을 보여준다. 또한 가족은 환자를 돌보는데 있어서 정보의 제공만으로 해결될 수 없는 여러가지 실제적인 어려움을 주고 있었다. 그 결과 계속적인 24시간 전화상담 서비스 호스피스 시설 가정간호 서비스의 확대, 3차 의료기관과 1,2차 의료기관과의 의뢰 체계 등을 필요로하고 있었다. 따라서 병원에서 제공될 수 있는 프로그램과 지역사회에서 제공될 수 있는 자원, 호스피스 시설과의 연계가 필요하며 이를 관리해줄 수 있는 환자 관리 전담 인력이 필요하다. 결론 : 본 프로그램은 1회(single-session)적인 성격이 강하지만 환자와 가족이 궁금해하는 점들을 만족시켜주므로서 암에 대한 인식이 증가하고 대처능력이 향상되고 있음을 볼 수 있다. 이것은 이 프로그램이 위기에 처한 말기암환자와 가족을 지지하는 프로그램으로서 활용가치가 높다고 볼 수 있다. 추후 연구는 프로그램의 효용성에 대한 평가가 검토되어야 할 것이며, 다른 병원에서도 각 병원의 특성과 참석자들의 특성을 고려하여 보다 발전된 프로그램이 나오기를 바란다.

응급실 간호사의 폭력경험, 자아탄력성, 직무스트레스와의 관계연구 (Relationships among Violence Experience, Resilience and Job Stress of Nurses Working in Emergency Department)

  • 본 연구는 응급실 간호사의 폭력경험, 자아탄력성과 직무스트레스정도를 확인하고 관계를 확인하기 위한 서술적 조사연구이다. 본 연구대상은 D시 소재의 4곳과 C시 소재 2곳의 500병상 이상의 응급실에서 근무 중인 경력 1년 이상의 간호사 143명을 대상으로 하였으며, 자료수집기간은 2018년 11월 6일부터 15일까지이었다. 대상자의 폭력경험은 4점 만점에 평균평점 1.26±1.31점이었고, 자아탄력성은 4점 만점에 2.50±0.55점, 직무스트레스 정도는 5점 만점에 3.62±0.49점이었다. 폭력경험과 직무스트레스 하위영역별 상관관계에서 언어적 폭력 경험은 간호업무(r=.194, p=.010), 역할갈등(r=.158, p=.030) 스트레스와 유의한 양의 상관관계가 있었고, 신체적 위협 경험은 간호업무(r=.200, p=.008), 역할갈등(r=.162, p=.027), 의사와의 갈등(r=.145, p=.042) 스트레스와 양의 상관관계가 있었다. 자아탄력성과 직무스트레스 하위영역별 상관관계에서 간호업무 스트레스는 자아탄력성의 강인함(r=-.189, p=.012), 인내력(r=-.165, p=.025), 낙관성(r=-.186, p=.013) 영역과 음의 상관관계가 있었으며 전문지식 스트레스는 강인함(r=-.230, p=.003), 인내력(r=-.195, p=.010), 낙관성(r=-.194, p=.010) 영역과 음의 상관관계가 있었고, 간호사 대우 스트레스는 영성(r=.154, p=.033) 영역과 양의 상관관계가 있었다. 폭력경험과 자아탄력성과의 하위영역별 상관관계에서 강인함은 신체적 위협(r=-.150, p=.037) 경험과 음의 상관관계가 있었으며, 인내력은 신체적 위협(r=-.138, p=.050) 경험과 음의 상관관계가 있었고, 낙관성은 신체적 폭력(r=-.151, p=.036) 경험과 음의 상관관계가 있었다. 이상의 결과로 볼 때 응급실 간호사의 직무스트레스 감소를 위해 폭력이 발생하지 않는 안전한 근무 환경을 조성하고, 폭력상황 발생 시의 대처방법에 관한 지속적인 교육 및 스트레스 대처방법, 자아탄력성을 높이기 위한 프로그램개발 및 중재가 필요하다.
