죽상경화성 뇌혈관질환 환자에서 성공적인 EC/IC 우회술 후 혈류역학적 변화: 기저/아세타졸아미드 SPECT를 이용한 연구 (Hemodynamic Outcome of Successful Bypass Surgery in Patients with Atherosclerotic Cerebrovascular Disease: A study with Acetazolamide and $^{99m}Tc-ECD$ SPECT)

  • 목적: 폐색성 뇌혈관질환에서 EC/IC 뇌혈관 우회술은 해당 뇌조직의 혈류 증가를 기대하게 된다. 본 연구에서는 $^{99m}Tc-ECD$ 뇌혈류 스펙트를 이용하여 폐색성뇌질환 환자에서 성공적인 우회로 시술 후 단기, 장기간의 혈류역학적 변화 경과를 알아보고자 하였다. 대상 및 방법: 뇌경색 또는 뇌허혈 증상을 가진 죽상경화성 뇌폐색성 혈관질환으로 성공적 STA-MCA 문합술을 시행한 14명의 환자(남:여=8:6, 평균연령: $60{\pm}9$세)를 대상으로 하였다. 술전과 술후 10일, 6개월에 시행한 $^{99m}Tc-ECD$ 기저/아세타졸아미드 부하 뇌 혈류 SPECT에서 기저 뇌혈류 및 뇌혈류예비능의 변화를 관찰하였다. 결과: 우회술 전 원인 내경동맥 측의 비경색뇌부위의 기저 혈류 및 뇌혈류예비능은$52.4{\pm}3.5$$-7.9{\pm}4.7%$이었다. 수술 후 초기 및 후기의 내경동맥 공급 뇌부위의 혈류는 각각 $53.7{\pm}2.7,\;53.3{\pm}2.5$로 술전과 비교하였을 때 유의한 증가를 보였다 (p<0.05). 술후 10일과 6개월 사이의 기저뇌혈류에는 유의한 차이가 없었다. 한편, 뇌혈류예비능은 술전과 비교하여 초기 및 후기에 유의한 호전(초기, $-3.7{\pm}2.6%$, 후기: $-1.6{\pm}2.3%$)을 보였으며, 수술 후기에서 수술 초기와 비교하여서도 유의미한 추가의 뇌혈류예 비능 증가가 관찰되었다 (p<0.05). 반면, 수술을 시행하지 않은 쪽의 내경동맥 공급 뇌부위에서는 수술 후 추적검사에서 기저혈류 및 뇌혈류예비능에 유의한 변화를 보이지 않았다. 결론: 성공적 뇌혈관 우회술 후의 초기 뇌혈류 SPECT에서의 단기적 혈류변화는 뇌혈류의 재개통에 의한 즉각적인 변화를 반영하고, 반면에 수술 후기 SPECT에 관찰되는 장기적인 혈류변화는 우회술 후에 신생혈관에 의한 측부순환에 기인하는 것으로 생각되며, 이러한 변화가 EC/IC 우회술 후 뇌졸중의 발병빈도의 감소와 상관되리라 여겨진다.

활동성 및 비활동성골수염에서의 삼상골신티그라피 (Three Phase Bone Scintigraphy in Active and Inactive Osteomyelitis)

  • To Appreciate the value of bone scintigraphy in determination of the bony infection, we performed three phase bone scintigraphy in 34 cases of osteomyelitis of extremities prospectively. They were clinically inactive in 11 and active in 23 cases. We confirmed the active osteomyelitis by operation or aspiration within one week after scintigraphy. Perfusion, blood pool and delayed images were analyzed respectively and compared with the plain roentgenograms. All 23 active lesions showed diffusely increased perfusion in affected limbs. The areas of the increased activities on blood pool images were larger than or similar to those on delayed images in 17 cases (73.9%) with active osteomyelitis and smaller in 6 cases (26.1%). 5 of the latter 6 cases showed definite soft tissue activities on blood pool images. In inactive cases bone scintigrams were completely normal in 4 cases. Two of those were normal on plain films and remaining two showed mild focal bony sclerosis. Among 7 inactive lesions, perfusion was normal in 2 cases, diffusely increased in 4 cases and diffusely decreased in 1 case. 6 of these 7 cases showed increased activities both on blood pool and delayed images and the areas of increased activities on blood pool images didn't exceed those on delayed images. Bony sclerosis was noted on plain films in those 7 inactive lesions and the extent of the sclerosis correlated well to delayed images. Large blood pool activity was characteristics of active osteomyelitis. Normal three phase bone scintigram may indicate the time to terminate the treatment, but increased activity on perfusion and blood pool scans is not absolute indication of active lesion if the extent of the lesion on the blood pool image is smaller than that on delayed image and if no difinite soft tissue activity is noted on perfusion and blood pool images in clinically inactive patient.

3상 골신티그램을 이용한 급성 일과성 활막염의 진단 (Three-Phase Bone Scintigraphic Diagnosis of Acute Transient Synovitis)

  • Acute transient synovitis of the hip presents clinically pain and limping. But in the majority of the cases, definite positive findings are not manifest in roentgenogram in its early phase. However radionuclide bone imaging combines with the assessment of vascularization and bone tracer uptake is of great value in solving this diagnostic problem. The materials for this study consisted of 29 children with acute transient synovitis of the hip, characterized by symptoms and physical signs of an arthritis, negative X-ray findings and disappearance of all symptoms and signs within a short period of time. They were twenty males and 9 females and age ranged from 1 to 12 years. We took pelvic reontgenogram in AP and frog-leg views. After intravenous bolus injection of 10 to 15 mCi of $^{99m}Tc-methylene$ diphosphonate, 24 sequential image of the pelvis was taken at 2-second interval for blood flow study. The scintigrams were made using a gamma camera with high resolution parallel hole collimator. Blood pool imaging was obtained at 2 minutes after tracer administration. After 3 hours, static images were taken and then closeup image of the hip using pin-hole collimator was followed. The results were as follows: 1) Bone scintigram was much more sensitive than conventional roentgenogram in diagnosis of acute transient synovitis of the hip. 2) Three-phase imagings showed increased vascular activities in blood pool scintigrams in 96%. 3) Pin-hole imaging showed increased tracer uptake in the regional bones of the hip, par ticularly in the medial aspect of femoral head and acetabulum. 4) We confirmed that three-phase imaging reinforced with pin-hole technique were very useful in diagnose of acute transient synovitis of the hip.

반사성 교감신경계 기능장애 증후군(RSDS)의 손 3상 골스캔 소견 (Three-Phase Bone Scintigraphy in Reflex Sympathetic Dystrophy Syndrome of the Hand)

  • Reflex sympathetic dystrophy syndrome (RSDS), known also as Sudeck's atrophy, is an uncommon disorder recognized by its distinctive symptom complex consisting of pain and tenderness, vasomotor instability, swelling, and dystrophic skin changes and radiologic changes. The present study has been carried out to prospectively establish scintigraphic diagnostic criteria for RSDS using three-phase radionuclide bone scintigraphy (TPBS). In addition, the usefulness in the evaluation of treatment of RSDS was assessed. Patients included were 6 men and 7 women with the age ranging from 25 to 63 years (average 47 years). Diagnosis was based on typical clinical symptoms and signs as described above. Associated clinical conditions in these patients were cerebral infarction (4 patients), lung cancer (2 patients), trauma (1 patient), lymphoma (1 patient), and unknown cause (5 patients). All patients showed diffuse radionuclide accumulation in juxtaarticular region on the delayed static image and 11 patients showed diffusely increased activities also on scintiangiogram and blood-pool image. Fillow-up TPBS after corticosteroid therapy in 4 patients revealed near normal return of abnormal radionuclide accumulations in the affected hand. TPBS is an useful test for the diagnosis of as well as the evaluation of the therapeutic effects of RSDS.

Supine Position에서 Modified Y-View의 제안 (Suggestion of Modified Y-View in Supine Position)

  • 본 연구는 어깨 통증을 호소하는 환자들 중 어깨충돌 증후군(SIS : Shoulder Impingement Syndrome)이 의심되어 진단을 위해 Y-view 검사가 필요하지만 몸이 불편하여 자세를 취하기 힘든 환자들을 위하여 편안하게 누운 자세에서 검사할 수 있는 modified Y-View를 고안하기 위해 연구를 실시하였다. 환자는 바로 누운 자세를 취하고 X선관(tube)의 각도를 내측으로 35도, 40도, 45도로 변화를 주어 검사한 영상을 비교 분석해 본 결과 X선관(tube)의 각도를 내측방향으로 40도를 주는 것이 견봉(acromion)과 쇄골(clavicle)의 공간(space), 오훼견봉궁(coracoacromial arch)의 모양을 잘 묘출 할 수 있어 진단에 가장 유용한 영상을 얻을 수 있었다. 따라서 환자가 Y-View 검사를 할 수 없을 경우, 이를 대체하기 위한 유용한 수단으로 modified Y-View를 활용할 수 있음을 알 수 있었다. 이 연구를 통하여 어깨충돌 증후군뿐만 아니라 견관절 질환에서의 다양한 적용을 기대해 본다.

핵의학 융합영상의 표준섭취계수 차이에 관한 연구 (Study on the Difference of Standardized Uptake Value in Fusion Image of Nuclear Medicine)

  • PET-CT and PET-MRI which integrates CT using ionized radiation and MRI using phenomena of magnetic resonance are determined to have the limitation to apply the semi-quantitative index, standardized uptake value (SUV), with the same level due to the fundamental differences of image capturing principle and reorganization, hence, their correlations were analyzed to provide their clinical information. To 30 study subjects maintaining pre-treatment, $^{18}F-FDG$ (5.18 MBq/㎏) was injected and they were scanned continuously without delaying time using $Biograph^{TM}$ mMR 3T (Siemens, Munich) and Biograph mCT 64 (Siemens, Germany), which is an integral type, under the optimized condition except the structural differences of both scanners. Upon the measurement results of $SUV_{max}$ setting volume region of interest with evenly distributed radioactive pharmaceuticals by captured images, $SUV_{max}$ mean values of PET-CT and PET-MRI were $2.94{\pm}0.55$ and $2.45{\pm}0.52$, respectively, and the value of PET-MRI was measured lower by $-20.85{\pm}7.26%$ than that of PET-CT. Also, there was a statistically significant difference in SUVs between two scanners (P<0.001), hence, SUV of PET-CT and PET-MRI cannot express the clinical meanings in the same level. Therefore, in case of the patients who undergo cross follow-up tests with PET-CT and PET-MRI, diagnostic information should be analyzed considering the conditions of SUV differences in both scanners.

종양 모델 연구를 위한 소동물 $[^{18}F]$FDG PET 영상화 (Small Animal Small Animal $[^{18}F]$FDG PET Imaging for Tumor Model Study)

  • PET allows non-invasive, quantitative and repetitive imaging of biological function in living animals. Small animal PET imaging with $[^{18}F]$FDG has been successfully applied to investigation of metabolism, receptor-ligand interactions, gene expression, adoptive cell therapy and somatic gene therapy. Experimental condition of animal handling impacts on the biodistribution of $[^{18}F]$FDG in small animal study. The small animal PET and CT images were registered using the hardware fiducial markers and small animal contour point. Tumor imaging in small animal with small animal $[^{18}F]$FDG PET should be considered fasting, warming, and isoflurane anesthesia level. Registered imaging with small animal PET and CT image could be useful for the detection of tumor. Small animal experimental condition of animal handling and registration method will be of most importance for small lesion detection of metastases tumor model.

Comparison of Ictal-Interictal Subtraction and Statistical Parametric Mapping in Patients with Temporal Lobe Epilepsy

  • The aim of this study was investigate the epileptogenic zone in temporal lobe epilepsy (TLE). We evaluated the subtraction image of interictal SPECT from ictal SPECT coregistered to 3-dimensional (3D) MRI, and compared with the normal healthy SPECT using a SPM99. Forty-nine patients with TLE (M:F=28:21, mean age: 33${\pm}$2.1 years) underwent a pairs of ictal and interictal SPECT. We performed subtraction interictal SPECT from ictal SPECT in TLE patients. In addition, using SPM methods and t-statistics, SPECT images of the TLE patients were compared with normal healthy SPECT on a voxel by voxel basis. The voxels with a p-value of less than 0.05, 0.005, 0.001 were considered to be significantly different. The subtraction results by ictal and interictal SPECT coincided with the significant rCBF changes when compare of the normal healthy SPECT using a SPM99. The results suggested that analysis of difference of the two methods using healthy normal SPECT with SPM99 is useful tool in evaluation of seizure focus in epilepsy.

PET/CT 팬텀의 3D 프린팅 소재 적용 가능성 평가 (Evaluation of the Applicability of PET/CT Phantom as a 3D Printing Material)

  • The purpose of this study is to present objective information in applying 3D printing technology for PET/CT (Positron Emission Tomography/Computed Tomography) performance evaluation and use it as a basic research that can be applied to various purposes in the future. Phantoms were manufactured with step wedge of ABS(Acrylonitrile Butadiene Styrene) and ACR(Acrylic acid) material. The counts for each ROI(Region of Interest) were analyzed through image acquisition in PET/CT. And the variation rate of counts and CNR(Contrast Noise Ratio) was evaluated. In the counts analysis, the effect of thickness occurred. In addition, in the variation rate analysis, the thickness setting of steps wedge 4 to 5 levels should be considered first. These results minimize quantitative and qualitative changes in the phantom manufactured based on 3D printing, and enable more stable PET/CT performance evaluation. Based on 3D printing in PET/CT, various phantoms are expected to be produced in the future. If the characteristics of each material are considered and applied through the basic research such as this research, the result of the phantom manufactured through 3D printing can be more meaningful and will be used in a wide range.

Deep-Learning-Based Molecular Imaging Biomarkers: Toward Data-Driven Theranostics

  • Deep learning has been applied to various medical data. In particular, current deep learning models exhibit remarkable performance at specific tasks, sometimes offering higher accuracy than that of experts for discriminating specific diseases from medical images. The current status of deep learning applications to molecular imaging can be divided into a few subtypes in terms of their purposes: differential diagnostic classification, enhancement of image acquisition, and image-based quantification. As functional and pathophysiologic information is key to molecular imaging, this review will emphasize the need for accurate biomarker acquisition by deep learning in molecular imaging. Furthermore, this review addresses practical issues that include clinical validation, data distribution, labeling issues, and harmonization to achieve clinically feasible deep learning models. Eventually, deep learning will enhance the role of theranostics, which aims at precision targeting of pathophysiology by maximizing molecular imaging functional information.
