Comparison of Asymmetries on Masseter Muscle Thickness and Range of Motion in Subject With and Without Temporomandibular Disorders (턱관절 장애 유무에 따른 깨물근의 두께와 턱관절 가동범위의 비대칭성 비교)

  • Background: Temporomandibular disorder (TMD) is characterized by pain and limited range of motion in the jaw. TMD patients generally prefer to chew on the unaffected or less-affected side, and this tendency often results in asymmetries in masseter muscle thickness and range of mandibular motion. Objects: The purpose of this study was to compare the asymmetries in masseter muscle thickness and range of mandibular motion in subjects with and without temporomandibular disorders. Methods: Thirty-nine subjects were divided into two groups: A TMD group ($n_1=19$) and a control group ($n_2=20$). The jaw opening range and laterotrusion were measured using a digital vernier caliper. The masseter muscle thickness was examined in both the resting state and the maximal clenching state using ultrasonography. The absolute asymmetry indices calculated based on the laterotrusion and masseter muscle thickness of the respective right and left sides. A two-way ANOVA and a Mann-Whitney U test were used for statistical analysis. Results: No significant different was found in the masseter muscle thickness between the TMD and control group. A significant difference was found in the absolute asymmetry indices of mandibular laterotrusion between the TMD and control groups (p<.05). Furthermore, the ranges of jaw opening were significantly different between males and females (p<.05). The absolute asymmetry index values of masseter muscle thickness at rest and during maximal clenching were also significantly different between males and females (p<.05). Conclusion: These results demonstrated that the subjects with TMD had a larger degree of asymmetry in laterotrusion than those without TMD. Therefore, a physiotherapy program needs to be designed to restore normal laterotrusion capacities for TMD subjects. These results also showed that female subjects had greater absolute asymmetry indices in masseter muscle thickness than male subjects. Therefore, more training is needed to promote bilaterally balanced chewing among women.

  • Background: In this study, the importance of constipation and back pain was assessed by regression analysis of the effects of stress, dietary habits, and water intake on constipation in women in their twenties and the influence of constipation and body mass index (BMI) To provide basic data. Methods: This study selected 109 having constipation of 120 students attending G University in Gwangju and eating habits, water intake level, stress and BMI. Trigger point at the tip of erector spina was palated with tenderness set in order to examine whether muscle tenderness and actual low back muscle tenderness level were same and left and right parts were measured three times and average of Max values was used. We examined the effect of constipation on low back pain and examined constipation and BMI to determine whether they affected low back pain. Results: There was no significant difference in eating habit although there was a significant difference in the effect of stress and water intake on constipation. Constipation had significant difference in back pain. However, there was no significant difference in BMI, normal weight, overweight, and obesity except for low body weight. Conclusions: This study found that constipation was associated with stress levels, water intake, and back pain. In conclusion, this study suggests basic data to prevent and treat constipation-related back pain, and recommends plenty of water intake, proper exercise and stress management to prevent constipation.

Feasibility Assessment of Physical Factors of Rectal Cancer Short-Course Chemoradiotherapy with Delayed Surgery

  • To verify the correlations between the clinical outcomes and physical factors of short-course chemoradiotherapy (SCRT) and long-course chemoradiotherapy (LCRT) with delayed surgery in patients with rectal cancer. Seventy-two patients with rectal cancer were enrolled in this study. Nineteen patients were treated with SCRT (25 Gy, 5 fractions) by intensity-modulated radiation therapy (IMRT), and 53 patients were treated with LCRT (50.4 Gy, 28 fractions) by three-dimensional conformal radiation therapy (3DCRT). Various physical factors for the target and organs at risk (OARs) were calculated to compare the clinical outcomes. The organ equivalent dose (OED) and lifetime attributable risk (LAR) of bowels and bladders were similar between the SCRT and LCRT groups, whereas the values of femurs were higher in the LCRT group. The equivalent uniform dose and normal tissue complication probability were higher in the LCRT than the SCRT group for most organs. Treatment complications, including anastomotic leakage, bowel adhesion, and hematologic toxicity, were not significantly different between SCRT and LCRT groups. CIs were $0.84{\pm}0.2$ and $0.61{\pm}0.1$ for SCRT and LCRT, respectively. The CVIs were $1.07{\pm}0.0$ and $1.10{\pm}0.1$, and the HIs were $0.09{\pm}0.0$ and $0.11{\pm}0.1$ for SCRT and LCRT, respectively. The sphincter-saving rates were 89.5% and 94.3% for SCRT and LCRT, respectively. The complete pathologic remission rates were 21.1% and 13.2%, and the down-staging rates were 47.4% and 26.4% for SCRT and LCRT, respectively. SCRT with IMRT is comparable to conventional LCRT in both physical indexes and clinical outcome. The preoperative SCRT, compensated by IMRT, is an effective and safe modality.

  • Long term water quality change was analyzed to evaluate the effect of the Total Maximum Daily Load (TMDL) policy. A trend analysis was performed for biochemical oxygen demand (BOD) and total phosphorus (TP) concentrations data monitored at the outlets of the total 41 TMDL unit watersheds of the Nakdong River in the Republic of Korea. Because water quality data do not usually follow a normal distribution, a nonparametric statistical trend analysis method was used. The monthly mean values of BOD and TP for the period between 2004 and 2015 were analyzed by the seasonal Mann-Kendall test and the locally weighted scatterplot smoother (LOWESS). The TMDL policy effect on the water quality change of each unit watershed was analyzed together with the results of the trend analysis. From the seasonal Mann-Kendall test results, it was found that for BOD, 7.8 % of the 41 points showed downward trends, 26.8 % and the rest 65.9% showed upward and no trends. For TP, 51.2% showed no trends and the rest 48.8% showed downward trends. From the LOWESS analysis results, TP began to decrease in most of the unit watersheds from mid-2010s when intensive chemical treatment processes were introduced to existing wastewater treatment plants. Overall, for BOD, relatively more points were improved in the main stream compared to the points of the tributaries although overall trends were mostly no trend or upward. For TP, about half of the points were improved and the rest showed no trends.

Dental students' ability to detect maxillary sinus abnormalities: A comparison between panoramic radiography and cone-beam computed tomography

  • Purpose: To compare the diagnostic ability of undergraduate dental students to detect maxillary sinus abnormalities in panoramic radiographs(PR) and cone-beam computed tomography (CBCT). Materials and Methods: This was a retrospective study based on the evaluation of PR and CBCT images. A pilot study was conducted to determine the number of students eligible to participate in the study. The images were evaluated by 2 students, and 280 maxillary sinuses were assessed using the following categories: normal, mucosal thickening, sinus polyp, antral pseudocyst, nonspecific opacification, periostitis, antrolith, and antrolith associated with mucosal thickening. The reference standard was established by the consensus of 2 oral radiologists based on the CBCT images. The kappa test, receiver operating characteristic curves, and 1-way analysis of variance with the Tukey-Kramer post-hoc test were employed. Results: Intraobserver and interobserver reliability showed agreement ranging from substantial (0.809) to almost perfect (0.922). The agreement between the students' evaluations and the reference standard was reasonable (0.258) for PR and substantial(0.692) for CBCT. Comparisons of values of sensitivity, specificity, and accuracy showed that CBCT was significantly better(P<0.05). Conclusion: CBCT was better than PR for the detection of maxillary sinus abnormalities by dental students. However, CBCT should only be requested after a careful analysis of PR by students and more experienced professionals.

A Preliminary Study on Effects of Abdominal Bracing Exercise on Respiratory Function of Normal Adults (복부 브레이싱 운동이 20대 성인의 호흡기능에 미치는 영향에 관한 예비연구)

  • Lumbar stabilization exercise is a popular exercise method in recent clinicians, as it strengthens muscles around the spine and abdominal muscles, but there is a lack of research on the effects of pulmonary function and respiratory pressure. So, this study examined whether abdominal bracing exercise, a typical lumbar stabilization exercise, was effective in pulmonary function and respiratory pressure. In this study, a total of 20's 40 subjects were recruited and randomly divided into an abdominal bracing exercise group(n=20) and control group(n=20). Abdominal exercise group underwent 25 minutes of exercise. Pulmonary function and respiratory pressure values were measured to analyze respiratory function. As a results of the experimental group, there were significant improvements in FVC, FEV1, PEF and MEP(p<.05) and there was a significant difference in the comparison between groups, except the MIP. These results suggest that abdominal bracing exercise can be presented as effective exercises to improve respiratory function.

Comparison of mean platelet volume levels in coronary artery ectasia and healthy people: systematic review and meta-analysis

  • Coronary artery ectasia (CAE) is defined as the dilation of a segment of a coronary vessel to at least 1.5 times the diameter of its normal adjacent segment. Mean platelet volume (MPV) plays a role in acute coronary syndromes, with high MPV correlating to poor prognosis for acute thrombotic events and CAE. Several studies investigated the relationship between MPV and CAE, resulting in conflicting results. These results led us to systematically review all studies investigating the relationship between MPV and ectatic heart diseases by performing a meta-analysis study in order to report a unified result. This meta-analysis study investigated all case-control articles examining the relationship between MPV and CAE. All studies in the following databases published until January 31, 2018, were investigated: Science Direct, Scopus, PubMed, Google Scholar, and Web of Science. Following a quality control evaluation, 14 articles, all of which were published following studies performed in Turkey from 2007 to 2016, met the criteria for study inclusion. After pooling the results from all of the articles, a total standardized mean difference (SMD) value of 0.584 (95% CI, 0.219, 0.95) was obtained using the D+L pooled SMD, indicating a significant difference (P=0.002) between the two groups, with higher MPV values in ectatic patients when comparing to healthy individuals. Therefore, increased MPV levels were significantly related to CAE, suggesting that platelets, with their inflammatory and thrombotic activities, play a role in this disease. Therefore, anti-platelet and anti-inflammatory therapies may be effective in treating CAE.

A Review of the Study Trends on the Relationship between Primary Dysmenorrhea and Doppler Indicies of Uterine Artery (일차성 월경통과 자궁동맥의 혈류역학적 측정값의 관련성에 대한 최신 연구 동향)

  • Objectives: This study was performed to review the research trends in the relationship between primary dysmenorrhea and doppler indicies of uterine artery. Methods: The search for related papers used 'Pubmed', a reserch engine in the America National Library of Medicine and Korean studies Information Service System (KISS). Used searching terms were 'primary dysmenorrhea', 'menstrual pain' in all cases. And among these studies, we searched by using key word 'uterine artery', 'doppler indices', 'doppler parameters', 'pulsatile index', 'resistance index'. Results: Overall 49 studies, 8 studies were finally selected to this study. There were 6 controlled studies and 2 randomised-controlled studies. In all 8 studies, transvaginal ultrasound was used to measure the resistance of uterine blood vessels. All of these studies reported that in patients with primary dysmenorrhea, hemodynamic values of uterine arteries measured by Doppler ultrasound were significantly higher than in normal subjects. Conclusions: According to the results, it was found that there was a positive correlation between the pain level of primary dysmenorrhea and the pulsation index and resistance index of the uterine artery. Based on these results, it can be seen that the doppler indicies of uterine artery have the potential to be used as an evaluation scale for Korean traditional medicine for primary dysmenorrhea.

Effects of Black rice (Oryza sativa L.) Aleurone layer on Cyclophosphamide-induced Immunosuppression in Mice (Cyclophosphamide에 의해 유도된 면역 저하 마우스에서 흑미 (Oryza sativa L.) 호분층의 면역 활성 효과)

  • Anthocyanins are considered as one of the major functional components including cyanidin-3-glucoside and peonidin-3-glucoside anthocyanins of black rice (Oryza sativa L., BR) aleurone layer (OAL) extract. The immunomodulatory biological effects of OAL has rarely been evaluated. This study has extensively studied the immunomodulatory effects of BR in immunosuppressed mice by CPA. The immunostimulatory effects of BR extract in in vivo at 150 (OAL-1) and 300 mg/kg body weight (OAL-2) for OAL groups were measured and compared to the normal group (CON) ingested with distilled water only or other groups. After the 14 days of oral administration, immune related organ weights, serum immunoglobulin (Ig) A and IgG, and cytokines (IL-12, TNF-α) levels, splenocytes proliferation rate, and NK cell activity were evaluated as immune related biomarkers. The serum IgA and IgG values in the OAL-treated groups increased to the level of the CON. OAL significantly and effectively controlled the IL-12 and TNF-α levels and NK activity compared to the negative control (NC) group. The results suggest that BR aleurone layer with immunomodulatory activities has a great potential as a functional food by itself or as a supplement.

Comparative Evaluation of North American Symptomatic Carotid Endarterectomy Trial and Warfarin-Aspirin Symptomatic Intracranial Disease Methods for Measurement of Middle Cerebral Artery Stenosis in Digital Subtraction Angiography and Magnetic Resonance Angiography (디지털 감산 혈관조영술과 자기 공명 혈관조영술에서 중대뇌동맥의 협착 측정을 위한 North American Symptomatic Carotid Endarterectomy Trial 및 Warfarin-Aspirin Symptomatic Intracranial Disease 방법의 비교 평가)

  • This study aimed to determine whether there was a difference in measurements between North American Symptomatic Carotid Endarterectomy Trial(NASCET) and Warfarin-Aspirin Symptomatic Intracranial Disease(WASID) methods that measure the middle cerebral artery stenosis in Digital Subtraction Angiography (DSA) and Magnetic Resonance Angiography (MRA). It involved 24 patients who had completed both DSA and MRA among patients with Middle Cerebral Artery (MCA) stenosis. The Middle Cerebral Artery (MCA) stenosis was measured using the NASCET and WASID methods through a retrospective analysis. For the NASCET and WASID methods, they performed measurements on normal blood vessels located far from and close to the stenosis, respectively. The mean value and standard deviation of the Digital Subtraction Angiography (DSA) measured by the NASCET method were 59.23% and 13.27%. On the other hand, those of the Digital Subtraction Angiography (DSA) measured by the WASID method were 66.64% and 12.47%. And, the mean value and standard deviation of the Magnetic Resonance Angiography (MRA) measured by the NASCET method were 49.82% and 12.06%. By contrast, those of the Magnetic Resonance Angiography (MRA) measured by the WASID method were 56.63% and 10.67%. All the p-values obtained by the Pearson and Spearman correlation tests in the Digital Subtraction Angiography (DSA) and the Magnetic Resonance Angiography (MRA) were <0.01. In conclusion, this study suggests that both the NASCET and WASID methods to measure the middle cerebral artery stenosis in the Digital Subtraction Angiography (DSA) and the Magnetic Resonance Angiography (MRA) can be used if they are not used interchangeably.
