Effect of Nap between Therapeutic Interventions on Motor Learning in Patients with Stroke (치료적 중재 사이 낮잠이 뇌졸중 환자의 운동학습에 미치는 효과)

  • Purpose : The purpose of this research was to determine the effects of nap between therapeutic interventions on motor learning in patients with stroke. Method : Thirty stroke patients with hemiplegia were participated in this study. After the screening by questionnaire about nap time, the patients were assigned to nap or non-nap group. Therapeutic interventions for 30 min were provided two times per day for 5 days per week and the serial reaction time tasks were conducted 2 times before and after a nap per day for 3 days per week. Between the therapeutic interventions, a nap for 60~120 minutes was allowed to the nap group while non-nap group was not. Intervention programs were carried out during the total 15-day. Result : Compared with the non-nap group, the reaction-precision level of nap group was increased (p<.05) for the non-affected upper extremity in the serial reaction time tasks. Conclusion : This study demonstrated that nap between therapeutic interventions has positive effects on motor learning in patients with stroke.

Effect of Phenobarbital on the Nonlinear Pharmacokinetics of Naproxen (나프록센의 비선형 체내동태에 미치는 페노바르비탈의 영향)

  • In order to elucidate the effect of phenobarbital (PB) on the nonlinear pharmacokinetic behavior of naproxen (NAP), we compared the dose dependent hepatic intrinsic clearance, biliary excretion and protein binding of NAP in control rats to those in the PB-pretreated rats which were intraperitoneally pretreated with PB sodium (75 mg/kg) once a day for four days. NAP was injected via femoral (1.5 mg/kg) and portal(0.25, 0.5, 1.5, 15 and 30 mg/kg) vein to the control and PB-pretreated rats, respectively. And also, we measured the plasma free fraction of NAP with the equilibrium dialysis method and the biliary excreted total amounts of NAP in both rats. Plasma free fraction of NAP was decreased in lower concentration than $150\;{\mu}g/ml$ of NAP due to PB pretreatment. In higher concentration, however, plasma free fraction was increased. These in vitro results suggest that the total protein concentration was increased but the total binding capacity of NAP to protein was decreased by PB-pretreatment. The total plasma clearance and the hepatic intrinsic clearance of NAP had similar values in both groups, respectively. And, both clearances of NAP were significantly increased by PB-pretreatment. Even though the plasma free fractions of NAP in both groups were constantly remained within the concentration range according to the increase of administration dose, the hepatic intrinsic clearances of NAP were significantly increased in both groups with the increased dose. And, the biliary excreted total amounts of NAP were significantly increased by PB-pretreatment at the lower dose, but decreased at the higher dose. These in vivo results suggest that NAP represents the uncommon nonlinear pharmacokinetic behavior that the hepatic intrinsic clearance was enhanced with the increased dose, and that PB enhances further the hepatic intrinsic clearance of NAP with the increased dose due to its enzyme induction effect.

Silencing of NbNAP1 Encoding a Plastidic SufB-like Protein Affects Chloroplast Development in Nicotiana benthamiana

  • It was previously shown that AtNAP1 is a plastidic SufB protein involved in Fe-S cluster assembly in Arabidopsis. In this study, we investigated the effects of depleting SufB protein from plant cells using virus-induced gene silencing (VIGS). VIGS of NbNAP1 encoding a Nicotiana benthamiana homolog of AtNAP1 resulted in a leaf yellowing phenotype. NbNAP1 was expressed ubiquitously in plant tissues with the highest level in roots. A GFP fusion protein of the N-terminal region (M1-V103) of NbNAP1 was targeted to chloroplasts. Depletion of NbNAP1 resulted in reduced numbers of chloroplasts of reduced size. Mitochondria also seemed to be affected. Despite the reduced number and size of the chloroplasts in the NbNAP1 VIGS lines, the expression of many nuclear genes encoding chloroplast-targeted proteins and chlorophyll biosynthesis genes remained unchanged.

Assessments of Spatial Neglect : Comparison of KF-NAP (Kessler Foundation - Neglect Assessment Process) and Pencil-Paper test (편측 공간무시 환자의 평가: KF-NAP(Kessler Foundation - Neglect Assessment Process)과 지필검사의 비교)

  • Objective : The aim of this study was to identify correlation between KF-NAP and Pencil-Paper test(Line bisection test, Letter cancellation, Albert's test) and to determine the high sensitive assessment tool for spatial neglect. Method : Thirty-two stroke patients who admitted to an inpatient rehabilitation unit at the medical center in Seoul from September, 2014 to September, 2015 were included in our research. They all have spatial neglect (male=19, female=13, the average age 61.8). KF-NAP and Pencil-Paper test were performed within 7 days. The detection of spatial neglect was analyzed by total scores of each assessments. Results : The highest sensitivity assessments of spatial neglect was KF-NAP. The detection of spatial neglect through KF-NAP was 93.8% (Line bisection test 53.1, Letter cancellation 53.1%, Albert's test 37.5%). Line bisection test (r=.430), Letter cancellation(r=.641), Albert's test(r=.398) and KF-NAP scores had significant correlation(p<0.05). Conclusion : KF-NAP is higher sensitive in comparison with Pencil-Paper test for detection of spatial neglect as the assessment tool of spatial neglect during activities daily livings of patients and neglect severity could be assessed with KF-NAP. Therefore, KF-NAP is recommended for assessment of spatial neglect.

Increase in the Chlorophyll Contents by Over-expression of GmNAP1 Gene in Arabidopsis Plant (애기장대에서 GmNAP1의 과발현으로 인한 엽록소 함량 증가)

  • In the course of a research concerning the molecular mechanism of hypocotyl elongation that occurs during soybean seedling growth in darkness, we have generated a number of ESTs from a cDNA library prepared from the hypocotyls of dark-grown soybean seedlings. Comparison of the ESTs assigned a cDNA clone as a putative plastidic ATP-binding-cassette (ABC) protein homologue. The soybean GmNAP1 protein contains an N-terminal transit peptide which targets it into the chloroplast. The transcription level of the GmNAP1 gene was investigated under continuous red light, continuous far-red light, and complete darkness. The main function of this NAP1 protein is the transport of protoporphyrin IX which is the precursor of chlorophyll from the cytoplasm to the chloroplast. The GmNAP1 gene was transferred into the Arabidopsis under the CaMV 35S promoter. The chlorophyll level of this transgenic Arabidopsis plant was much higher than the chlorophyll level of the wild type Arabidopsis plant.

Anterolateral Thigh Flap for Reconstruction of the Oral Cavity : Anatomic Study and Clinical Application (전외측대퇴피판을 이용한 구강 재건술)

  • The anterolateral thigh flap(ALT flap) was originally described in 1984 as a septocutaneous nap based on the descending branch of the lateral circumflex artery. This nap has some significant advantages for reconstruction of the head and neck. It can be raised as a subcutaneous flap, a fasciocutaneous nap, or a myocutaneous nap and can resurface large defects in the head and neck. In addition, it has a large and long vascular pedicle, and because of the distance of the donor site from the head and neck, it can easily be harvested with a two-team approach. However, the number and locations of cutaneous perforators vary individually, and thus, it is not widely used because nap elevation is often complicated and time-consuming owing to unexpected anatomical variations. The purposes of this study are to clarify the vascular anatomy and to assess the suitability of anterolateral thigh nap for oral cavity reconstruction in Koreans. In addition, we used anterolateral thigh free nap for oral cavity reconstruction in 20 oral cancer patients from 2006 to 2011. Through our clinical experience, we discuss a series of practical "pearls and pitfalls". Our experience has not only given us new flap choice using anterolateral thigh nap in oral cavity reconstruction, but also given us a new possibility on the applicability of chimeric naps.

Noise Assurance Plan in the Project for Design a New Rolling Stock (철도차량 개발과정에서의 소음관리방안)

  • This paper describes an overall Noise Assurance Plan(NAP) in the project for developing a new rolling stock. In this paper, the procedure for implementing noise control activities for each development stage on the basis of the NAP is also described. The NAP was developed by KITECH(Korea Institute of Industrial Technology) and ODS(${\Phi}$degaard & Danneskiold-Samsoe in DENMARK). Generally, the main objective of NAP is that noise assurance plan applies to the establishment of organization and personnel's roles and responsibilities, set-up of overall procedure and internal audit program. Here, a few comments are made to the deviations of the actual procedure(G7 Project) relative to the suggested NAP presented. The major difference between the suggested NAP and the actual procedure was the late involvement of the noise consultant resulting in suggestions for design improvements could not be implemented due to the advanced stage of the design. Similarly, the important task of preparing sub-supplier specifications was performed. The proposed NAP will be an efficient tool for noise management in the R&D project for new rolling stock. Specially, in case that several companies and institutes are involoved in the R&D team

Change of Survival and Reproductive Indicator of the Manila Clam, Ruditapes philippinarum Following Chronic Exposure to Naphthalene (나프탈렌에 노출된 바지락, Ruditapes philippinarum의 생존 및 생식지표 변화)

  • Manila clam, Ruditapes philippinarum (SL; $32.74{\pm}2.18mm$, TW; $8.29{\pm}1.41g$, N; 450) were exposed to various concentrations of naphthalene for 20 weeks. Exposure concentrations of naphthalene established control, solvent control (ethanol), 30, 60, 90 and $120{\mu}g$ Nap $L^{-1}$ following results of 96h acute exposure. After exposure during 20 weeks, survival rate of the manila clam was lowed in 60, 90 and $120{\mu}g$ Nap $L^{-1}$ exposure group compared control group. Also, sex ratio of male was higher in $60{\mu}g$ Nap $L^{-1}$ exposure group (${\chi}^2=5.492$, P < 0.05) but lowed $90{\mu}g$ Nap $L^{-1}$ exposure group (${\chi}^2=4.214$, P < 0.05) and $120{\mu}g$ Nap $L^{-1}$ exposure group (${\chi}^2=30.118$, P < 0.05). Gonad development was delayed in female (> $60{\mu}g$ Nap $L^{-1}$) and male (> $30{\mu}g$ Nap $L^{-1}$). Intersex was 16.28% in female, 1.68% in male. In this result, naphthalene caused survival decrease, imbalance of sex ratio, delay of gonad development of the manila clam. Also, chronic exposure to naphthalene suggested abnormal effects in reproduction of the manila clam.

A Study on Design and Development Process of Narrative Archiving Policy : Focused on S-NAP of Seoul Metropolitan Archives (서사 기반 수집 실행지침의 설계와 개발절차 서울기록원의 S-NAP을 중심으로)

  • It is widely agreed that a documentation strategy that considers the social context of fragmented records is needed for documentation of social memories. This paper examined a new methodology named Seoul Narrative Archiving Policy(S-NAP) which was developed for the documentation of social memories in metropolitan areas. Also, the paper reviewed the applicability of S-NAP through the collection cases of Seoul Metropolitan Archives. The development process of the S-NAP is as follows. First, acquisition topic domains that cover an entire metropolitan region were developed. Next, implementation units(S-NAP) were designed through extensive data analysis that encompasses social, economic, culture, and art areas and reflects outcomes of regional studies. As a result, a total of nine acquisition topic domains, 61 parents S-NAP, and 184 children S-NAP were derived from the aforementioned methodology and methods. Finally, this paper proposed the applicability of S-NAP as archival contents and as a supportive tool for archival activity networks.

Sequencing and Comparative Analysis of napA Genes from Helicobacter pylori Strains Associated with Iron-Deficiency Anemia

  • H. pylori is known to cause severe gastric diseases, including peptic ulcers and gastric cancers, and a link has also been suggested with iron-deficiency anemia (IDA). However, little is known about the pathogenesis of H. pylori-associated IDA. In the present study, to determine whether H. pylori strains are correlated with the prevalence of IDA, we analyzed and compared the sequences of the napA genes encoding a bacterioferritin-like protein in H. pylori strains. A total of 20 H. pylori strains were isolated from antral biopsies of patients with and without IDA, and the napA genes amplified from the genomic DNA were sequenced. A comparison of the deduced amino acid sequences for NapA revealed two sites with major variations. At residue 70, five out of the 12 non-IDA strains ($41.7\%$) contained serine, while only one of the 8 IDA strains ($12.5\%$) contained serine, indicating a significantly higher frequency of serine in the non-IDA strains. In addition, the NapA proteins from all 17 Western strains available on Web sites were found to contain serine residues at this position. Meanwhile, the other major variation was located at residue 73, where all eight IDA strains ($100\%$) contained leucine, while this was only true for eight of the 12 non-IDA strains ($66.7\%$). Therefore, these results indicated that the strains within each group were more genetically related to each other than to strains in the other group. When the expression level of the napA genes in the H. pylori strains was measured using RT-PCR, no significant difference was observed between the two groups, suggesting a similar intensity for the inflammatory responses induced by the NapA protein among the strains. Consequently, when taken together, the present data suggest that the occurrence of H. pylori-associated IDA may be partly determined by the infecting H. pylori strain, and the non-IDA strains are more closely related to Western strains than the IDA strains.
