국내 성인지적장애인 음악치료 중재 현황 (The Current State and Implementation of Music Therapy for Adults With Intellectual Disabilities)

  • 본 연구의 목적은 전문 음악치료사들을 대상으로 국내 성인지적장애인 대상 음악치료 임상중재의 현황과 중재내용을 분석하는 것이다. 이를 위해 성인지적장애인 대상으로 음악치료제공경험이 있고 설문참여에 동의한 음악치료사 39명 가운데 정규 자격증을 취득한 음악치료사 32명의 응답을 분석하였다. 설문지는 음악치료사의 배경정보, 성인지적장애인 음악치료 환경, 형태, 음악치료 진단, 목표설정, 중재전략, 성인지적장애인 대상 음악치료의 어려움에 대해 묻는 문항으로 구성되었다. 분석 결과 성인지적 장애인을 대상으로 한 음악치료는 사회복지시설, 병원, 음악치료센터 등 다양한 시설에서 시행되고 있는 것으로 나타났다. 본 연구에 참여한 음악치료사들이 설정한 주된 음악치료 목표는 상호작용 향상으로 나타났는데, 이는 기관 담당자의 의견과는 일치했지만 음악치료가 소속된 기관의 서비스 항목인 정서 안정과는 일치하지 않았다. 성인지적장애인 대상 음악치료에서 상호작용 향상을 위해 가장 많이 활용된 활동은 악기연주로 나타났고, 음악치료 내에서 음악을 사용할 때 가장 많이 활용한 음악 요소는 형식인 것으로 나타났다. 본 연구를 통해 나타난 성인지적장애인 대상 음악치료 중재에 가장 큰 영향을 미친 기관 관련 요인은 기관의 특성이었고, 대상 관련 요인은 기능수준으로 나타났다. 성인지적장애인 대상 음악치료 중재의 전문성을 강화하기 위해 성인지적장애인 대상 음악치료 진단도구 개발과 기관 담당자와의 원활한 소통이 요구된다. 본 연구의 결과는 음악치료가 성인지적장애인 대상으로 효과적인 중재로 기능할 수 있는 가능성을 시사하고, 향후 전문성 향상을 위한 방안을 제시하는 데 의의가 있다.

The effects of Daegeum Sanjo Rhythm (DSR) compare with Jinyang-jangdan and Jajinmori-jangdan on music therapy

  • The aims of this article is to examine that Daegeum Sanjo Rhythm (DSR) compare with Jinyang-jangdan and Jajinmori-jangdan on music therapy. Daegeum has the largest range of notes in wind instruments through Korean music. Jangdan is the essential element of rhythm in Korean music. Just as human body sound and resonant with their rhyme and meters, jangdan has its own rhythms of physical structures and sequence and repeat. Jinyang-jangdan, which is close to western minor code, expresses heartbreaking grief and great mourning feeling, so it makes one feel the catharsis through that rhythm. Jinyang-jangdan of daegeum music may be slow, but it can be sublimated into grim music for human. So, people overcome the sadness through grim music. On the other hand, jajinmori-jangdan gives charm and gaiety to people and to everything. So, it is exciting that it's often performed in festival and parade. Rhythmical music is a tool to improve the well-being of humanity and increase our life choices. Therefore, music therapy surely needs both influences of daegeum sanjo music regardless of the rhythm. Because, daegeum sanjo music is nature-friendly music of the rhythm.

음악요법이 조기진통 임부의 조기진통 스트레스 및 자궁수축에 미치는 영향 (Effects of Music Therapy on Stress of Preterm Labor and Uterine Contraction in Pregnant Women with Preterm Labor)

  • Purpose: The purpose of this study was to test effects of music therapy on stress due to preterm labor and uterine contraction in pregnant women with preterm labor. Methods: An experimental research design was used. Participants were 35 pregnant women with preterm labor who were between 20 to 37 weeks of pregnancy: control group (n=18) received only tocolytic drugs, while experimental group (n=17) received additional music therapy. In the experimental group, Traumerei was applied before Non-Stress Test (NST) from the second day to fifth day after admission as music therapy. Results: There was a statistically significant difference in stress due to preterm labor (z=-3.368, p<.001) between the two groups. Conclusion: The music therapy is an effective method for reducing the stress of pregnant women with preterm labor.

음악요법이 치매노인의 인지기능, 행동, 정서에 미치는 효과 (The Effect of Music Therapy on Cognitive Function, Behavior and Emotion of Dementia Elderly)

  • Purpose: This study was to observe the effects of music therapy on the cognitive function, behavior, and emotions of elderly dementia patients, and to seek musical mediation for them. This study was conducted with patients in the Dementia Sanitarium in C City from March 13 to April 17. Method: The design of research was a nonequivalent control group non-synchronized design and the subjects were 25 patients-15 of whom were in the experimental group with 10 in the control group. The music therapy consisted of favorite music listening in the morning, favorite music group singing activity after lunch, and relaxing music listening after dinner. The schedule was followed 6 days a week for 2 weeks for a total of thirty-six session. The effect of music therapy was measured by MMSE-K and the behavior and emotion measuring equipment which had been derived by the researcher. The verification of the effects is that the score of cognitive function, behavior, and emotions of the experimental and the control group which were measured after the therapy had been applied was analyzed by descriptive statistics and t - test using SPSS WIN program. Result: 1) The degree of cognitive function of the experimental group which was received the music therapy is $11.53{\pm}5.37$ which is a little higher than the control group which is $11.20{\pm}6.32$, but it is not significant statistically (t= .14, p= .887). The first hypothesis which had assumed the recepients would have had a higher cognitive function level than the other was rejected. 2) Behavior score of the experimental group that received the music therapy is $68.90{\pm}7.86$ which is higher than the control group which is $66.40{\pm}11.13$, but it is not significant statistically(t= .61, p= .548). The second hypothesis which had assumed the recepients would have had a higher behavior level than the other was rejected. 3) Emotions score of the experimental group that received the music therapy is $42.13{\pm}5.04$ which is higher than the control group which is $35.20{\pm}6.12$, and it is significant statistically(t=3..09, p= .009). The third hypothesis which assumed the recepients would have had a higher emotion level was supported. Conclusion: music therapy which is composed of listening to music and group singing activity is an effective strategy for improvement of the emotions of the dementia elderly. But, the effect of music therapy on the cognitive function and behavior of elderly dementia patients is not significant statistically.

음악을 이용한 간호중재가 중환자실 환아의 활력징후와 산소포화도에 미치는 영향 (The effects of music therapy on vital signs and pulsatile oxygen saturation of pediatric intensive care unit children)

  • This study was attempted to prove the effect of emotional stability and vital signs applying music therapy program to the children admitted in the PICU. Data were collected from July to September, 1997. The subjects were 30 patients admitted in the PICU of 'S' University Hospital which were divided into two groups of experimental and control. Each group had 15 subjects. Method was nonequivalent control group pretest-postteset repeated design, observing vital signs and activity of subjects prior, during, and after the music intervention. The study tools were cassette tapes of 'Mother's music whose babies want to listen' and Space-lab patient monitor. Data were analyzed using the $SPSS/PC+;x^2$ test and t-test to analyze of the general characteristics ; paired t-test to prove hypotheses. Result were as follows; 1. Infants lower than seven months showed changing into stable vital signs from applying the music therapy, however infants from eight months to three-year old showed no change in vital signs. 2. Vital signs changed to stabilized condition in infants lower than seven months were heart rate and respiration rate. 3. The stability of vital signs during music therapy turned back to the previous state while terminating music therapy. 4. The effect of music therapy in the state of activity had on both infants group of lower than seven months and from eight months to three-year old, particularly more effective in the later group. I recommend follows on the base of above results ; 1. As above results shows, listening to music is effective on infants and toddler, intervention with music therapy appropriate to chidlren's age is hot recommended. 2. Comparative study with noise blocking effect and music therapy effect within the ICU environment be recommended. 3. The repeated study on when the exact time is and how many repeat the music therapy to show the above mentioned effect be recommended. 4. We recommend this music therapy to be done in the recovery room, isolating room, operating room as well as ICU.

Effect of Music Therapy on Stroke Patients

  • Neurological impairment produces cognitive, communicational, physical, and social deficits. Music has the power to help stroke patients to regain speech and overcome other deficits. Rhythm and melody help to rehabilitate memory, muscles, breathing, etc. This article introduces how music therapy approaches stroke patients and helps them. It focuses particularly on speech; however, music affects not only one part of the body but the whole body. In cases in which music therapy is used, we can see how music helps with stroke patients and how to achieve these goals.

영국 음악치료 사례연구 - 자폐아 치료를 중심으로 - (THE CURRENT TRENDS OF BRITISH MUSIC THERAPY & TWO CASE STUDIES OF AUTISTIC CHILDREN)

  • 영국에서는 현재 음악치료가 자폐아 치료의 주요 수단으로 자리잡고 있고, 그 치료의 효과 또한 의학계와 교육계에서 널리 인정받고 있다. 이 글은 영국 음악치료의 주 동향이라 할 수 있는 즉흥연주를 바탕으로한 정신역동적 음악치료법(Psychodynamically Informed Improvisational Music Therapy)이 자폐증에 효과적 치료법임을 두 개의 사례연구를 통해 소개하고자 한다. 두 개의 사례연구는 매순간 개개인에 알맞게 응용되는 즉흥연주를 바탕으로한 음악치료의 과정이 어떻게 자폐아와 치료자간의 치료 관계를 형성시키고 발전시키는가를 보여준다. 첫번째 사례 Charlie를 통해서는 음악이 비언어적, 언어적 의사 소통과 초기 놀이발달 과정에 미치는 영향을 살펴보겠다. 두번째 사례에서는, 음악치료라는 환경이 어떻게 Mark에게 안전한 자기표현과 의사 소통의 장을 열어 주고, 자신과 다른 사람들에 대한 인식의 발달에까지 영향을 미치는가를 보겠다. 음악치료의 임상 과정에 대한 이해를 돕기 위하여 위의 사례는 발달적, 음악역동적, 정신역동적인 시점에서 고찰되어진다.

음악요법이 산모의 산후우울과 모아애착에 미치는 효과 (The Effects of Music Therapy on Postpartum Blues and Maternal Attachment of Puerperal Women)

  • Purpose: The purpose of this study was to determine the effects of music therapy on postpartum blues and maternal attachment of puerperal women. Methods: The research design was a nonequivalent control group non-synchronized design. The participants were puerperal women who agreed to participate in this study and through a convenience sampling, 60 puerperal women were recruited (30 in the experimental group, 30 in the control group). After measuring postpartum blues and maternal attachment, music therapy was provided to the experimental group over 40 min, once a day, and for 8 days. Then, postpartum blues and maternal attachment for the experimental and control group were measured again on the 8th day. The data were analyzed using the SPSS WIN 12.0 Program. Results: The first hypothesis that "the degree of postpartum blues for the experimental group who participated in music therapy would be lower than that of the control group" was accepted (t=4.350, p<.001). The second hypothesis that "the degree of maternal attachment of the experimental group who participated in music therapy would be higher than that of the control group" was accepted (t=4.828, p<.001). Conclusion: These findings indicate that music therapy has positive influences on decreasing postpartum blues and increasing maternal attachment of puerperal women.

After measuring postpartum stress and maternal attachment, music therapy was provided to the experimental group over 60min, three times a week, and for 2 weeks. Then, postpartum stress and maternal attachment for the experimental and control group were measured again before discharge. The data were analyzed using the SPSS/WIN 14.0 program. Results: The first hypothesis that "the degree of postpartum stress for the experimental group who participated in music therapy would be lower than that of the control group" was accepted. The second hypothesis that "the degree of maternal attachment the experimental group who participated in music therapy would be higher than that of the control group" was accepted. Conclusion: These findings indicate that music therapy has positive influences on decreasing postpartum stress and increasing maternal attachment of puerperal women.

외상후스트레스장애 환자에 대한 음악요법의 적용 일례 (A Case of Clinical Application of Music Therapy in Patient with PTSD)

  • Music therapy had been widely used in the area of psychiatry. However recently, participation of psychiatrist has been decreased, due to recent trends of emphasizing biological psychiatry and rapid effectiveness of pharmacotherapy. A 46 years old male PTSD patient was admitted to psychiatric ward. He has suffered from PTSD symptoms, such as intrusive distressing recollection of his past traffic accident, social avoidance, insomnia and depression. Music therapy, which is conducted by a psychiatrist and a music therapist, was applied. This case suggests the positive role of music therapy in treatment of PTSD and emphasizes the importance of participation of psychiatrist, as an organizer during music therapy.
