Failure to Support Associations of Neurotrophin-3(NT-3) Gene Polymorphism in Korean Schizophrenic Patients

  • Though initial report from Japan showed positive association of schizophrenia with dinucleotide repeat polymorphism in the NT-3 gene, subsequent studies showed mixed results. Therefore we conducted a replication study with Korean schizophrenics and matched controls who share similar ethnic background with Japanese population. The frequency of allele of dinucleotide repeat at 147 base pairs in the NT-3 gene was slightly increased, however, failed to reach statistical significance(${\chi}^2$=1.884, df=1, p<0.170) between the two groups. These findings do not support an association of NT-3 gene polymorphism with schizophrenia in Korean sample.

  • 연구목적 정신건강 지식에 영향을 미치는 요인을 알아보고자 이 연구를 수행하였다. 방 법 2021년 만 15세 이상 69세 이하 일반인구 2016명으로 대상으로 연구를 수행하였다. 연구는 보건복지부에서 시행한 2021년 '대국민 정신건강 지식 및 태도 조사'의 데이터를 이용하였으며, 인구사회학적 특성, 거주지별 특성, 정신건강 상태, 정신건강 문제 수준, 자원 인지를 조사하였으며, 32문항의 정신건강 지식 설문을 이용하여 정신건강 지식을 측정하였다. 결 과 다중회귀분석 결과 여성(p<0.001), 높은 연령(p=0.005), 높은 소득 수준(p<0.001)이 통계학적으로 유의하게 정신건강 지식과 상관관계가 있었으며, 지역사회정신보건기관 인지(p=0.002), 정신건강상담전화 인지(p<0.001)도 통계학적으로 유의하게 정신건강 지식과 상관관계가 있었다. 결 론 지역사회 진료나 정신건강 사업에 있어 정신건강 지식에 영향을 미치는 요인들을 고려한 접근이 필요하다.

학교 기반 정신건강증진 및 학교폭력 예방 프로그램 단기적 실시의 효과에 대한 예비연구 (School-Based Short Term Mental Health Awareness and School Bullying Prevention Programs : Preliminary Report)

  • Objectives : The purpose of this study is to evaluate the preliminary effects of school-based short term mental health awareness and school bullying prevention programs. Methods : From April to December 2013, the Child and Adolescent Mental Health Promotion team of Seoul National Hospital conducted school-based short term social skill training (N=56, 4 sessions), mental health awareness (N=84, 4 sessions) and school bullying prevention programs (N=171, 1 session) in elementary and junior high schools located in Seoul. The changes in the subjects before and after the program were assessed. Results : The social skills training program improved the 'helping' behaviors in boys (t=-2.355, p<.05) and 'sharing' (t=-3.223, p<.01), 'cooperation' (t=-2.235, p<.05), and 'comforting' (t=-2.830, p<.01) behaviors in girls, assessed using the Prosocial Behavior Questionnaire. The mental health awareness program improved 'general health awareness' (t=2.620, p<.05), measured using the Korean General Health Questionnaire. The school bullying prevention program resulted in decreased 'self esteem' (t=3.769, p<.01), measured using the Self Esteem Scale and decreased 'anger' (t=4.198, p<.01), assessed using the Novaco Anger Scale. Conclusion : The results of our preliminary analysis suggest that school-based mental health awareness and school bullying prevention programs may be effective even when conducted for a short term. Future investigation is necessary in order to validate the long term effects of these programs.

Significant Domains of Life Satisfaction That Affect Suicidal Behavior in Adolescents

  • Objectives: Adolescent suicide is a serious national issue in Korea. Recently, life satisfaction has been recognized as a major factor related to this issue. The main purpose of this study was to identify the domains of life satisfaction that affect suicidal behavior in adolescence. Methods: Data were collected from eight middle schools in Incheon, Korea. A total of 1297 students answered questions regarding their demographic characteristics, happiness, self-related life satisfaction domains (appearance, leisure time, physical health, and mental health), depressive symptoms, and suicidal behavior. Results: In the Spearman correlation analysis, female sex, perceived socioeconomic status (SES), happiness, and all four self-related satisfaction scores showed significant correlations with depression and suicidality. Multivariate regression analysis suggested that suicidality was significantly affected by perceived SES, satisfaction with appearance, mental health satisfaction, and depression. Finally, depression was identified as a partial mediator of the association between mental health satisfaction and suicidality, and a complete mediator of the association between female sex and suicidality. Conclusion: Perceived SES, satisfaction with appearance, and mental health satisfaction significantly affected students' suicidality, with or without the effect of depression. Health authorities, educators, and family members must be aware of this to identify adolescents at suicide risk earlier.

  • An association study with Korean schizophrenic patients(N=84) and normal controls(N=87) was performed to find the relationship between catechol-O-methyltransferase(COMT) gene polymorphism and schizophrenia using polymerase chain reaction-restriction fragment length polymorphism. When we compared the allele and genotype frequencies of Bgl I COMT gene polymorphism in schizophrenics and normal controls, there was no significant difference between two groups. Our results do not support an association between the Bgl I polymorphism of COMT gene and schizophrenia.

The Impacts of Childhood Trauma on Psychosocial Features in a Chinese Sample of Young Adults

  • Objective The aims of the present study were to explore the occurrence of childhood trauma and importantly to determine the impacts of childhood trauma on psychosocial features in a Chinese sample of young adults. Methods A survey was carried out in a group of 555 university students by using Childhood Trauma Questionnaire (CTQ), Self-rating Depression Scale (SDS), Self-rating Anxiety Scale (SAS), Dysfunctional Attitudes Questionnaire (DAS), Eysenck Personality Questionnaire (EPQ), and Social Support Rating Scale (SSRS). The moderate-severe cut-off scores for CTQ were used to calculate the prevalence of childhood trauma, and then psychosocial features were compared between individuals with and without childhood trauma. Results A proportion of 18.6% of university students had self-reported childhood trauma exposures. Subjects with childhood trauma reported higher scores of SDS, SAS, DAS, and psychoticism and neuroticism dimensions of EPQ (t=4.311-5.551, p<0.001); while lower scores of SSRS and extraversion dimension of EPQ (t=-4.061- -3.039, p<0.01). Regression analyses further revealed that scores of SAS and DAS were positively (Adjusted B=0.211-0.230, p<0.05), while scores of SSRS were negatively (Adjusted B=-0.273- -0.240, p<0.05) associated with specific CTQ scores. Conclusion Childhood trauma is still a common social and psychological problem. Individuals with childhood trauma show much more depression, anxiety, distorted cognition, personality deficits, and lower levels of social support, which may represent the social and psychological vulnerability for developing psychiatric disorders after childhood trauma experiences.

종합병원 정신건강의학과에 대한 공간적 접근성과 외래 의료이용 분석 (A Study on the Spatial Accessibility to the Psychiatry Department in General Hospital and Its Relationship with the Visit of Mental Patients)

  • Background: This study was purposed to analyze the effect of spatial accessibility to the psychiatry department in general hospital on the outpatient visit of mental patients. Methods: Data was provided from the Statistics Korea and Statistical Geographic Information Service, National Health Insurance Service, Health Insurance Review and Assessment Service, and Korea Transport Institute in 2015. The study regions were 103 administrative regions such as Si and Gu. The 103 regions had at least one general hospitals with a psychiatry department. The number of outpatient visit of mental patients in regions was used as the dependent variable. Spatial accessibility to mental general hospital was used as the independent variable. Control variables included such as demographic, economic, and health medical factors. This study used network analysis and multi-variate regression analysis. Network analysis by ArcGIS ver. 10.0 (ESRI, Redlands, CA, USA) was used to evaluate the average travel time and travel distance in Korea. Multi-variate regression analysis was conducted by SAS ver. 9.4 (SAS Institute Inc., Cary, NC, USA). Results: Travel distance and time had significant effects on the number of outpatient visits in mental patients in general hospital. Average travel time and travel distance had negative effects on the number of visits. Variables such as (number of total population, percentage of aged population over 65, and number of mental general hospital) had significant effects on the number of visit in mental patients. Conclusion: Health policy makers will need to consider the spatial accessibility to the mental healthcare organization in conducting regional health planning.

지역사회 정신보건관련 센터 근무자와 병원 근무자들의 자살에 대한 인식 및 태도 (Awareness and Attitude Toward Suicide in Community Mental Health Professionals and Hospital Workers)

  • Objectives : The purpose of this study was to evaluate community mental health professionals and hospital workers attitude and awareness towards suicide. Methods : This study investigated 264 community mental health professionals and 228 hospital workers. SOQs (Suicidal Opinion Questionnaires) were used from July 2007 to September 2007. After a factor analysis for the attitude towards suicide, the items on ethics, mental illness, religion, risk, and motivation were included in the subsequent analysis. Results : There were significant differences in the attitude towards suicide according to religion, age, educational background, the marriage status, the economic position, and different professional licenses. Hospital workers' view was different from the community workers'. The hospital workers judged that suicide was due to mental illness, and suicide was high for the people in a special environment and who lacked motivation, which caused them to fall in a dangerous situation. For the lower educational group, they thought that suicide was attributable to mental illness. The awareness for suicide was significantly higher in the group with a postgraduate education, unmarried people, mental health professionals and the persons who had concern and experience with suicide. The factors that had an influence on the awareness of suicide were the items of mental illness, religion, risk and motivational factors. Conclusions : This study suggested that the factors to increase the awareness and attitude for suicide were the experience of increased education and case management of suicide. Therefore, education dealing with suicide and reinforcement of crisis management programs should be developed.

정신과 근무 간호원의 정신질환 및 정신질환자에 대한 태도 연구 (A Study of the Attitudes of Psychiatric Registred Nurses towards Mental illness and Mental Patients)

  • The main purpose of this study was to investigate attitudes toward mental illness and the mentally ill of psychiatric nurses in Korea and to identify the relationships between the nurses' attitudes and demographic variables. Subjects in this study were 122 psychiatricnurses and 111non-psychiatric nurses in active service at 12 hospitals in Seoul. The insrtument used for measuring attitudes was a questionaire developed by Cohen and Struening (1962) referred to as the“Opinions about Mental illness(OMI) Scale”made up to 51 Likert-type items. The findings of this study indicated hatnurses showed negative attitudetoward mental illness and the mentally ill: Very high on social restrictiveness (factor D), low on Mental Health Ideology (factor C), and Interpersonal Etiology (factor E). Since the high score on Factor A, B and the low score on Factor B, C, E reflect a negative altitudes toward mental illness, this study population related extremely negative attitudes compared to any other surveys. And of the demographic variables that related to their attitudes the education degree and the kind of the hospital in active service showed very significant differences. On the basis of the investigation the findings indicated the followings; 1) Althohg attitudes toward mental illness and the mentally ill of psychiatric nurses were not significant differences from non-psychatric nurses, there was a trend that attitudes of psychaitric nurses did show more negative responses rather than those of non-psychiatric nurses. 2) Demographic variables that relate to their attitudes on the OMI scale were the education degree of the respondent and the kind of their hospital in active service. 3) About attitudes toward mental illness and the mentally ill baccalaureate nursesshowed more positive attitudes in Factor A (Authoritarianism), Factor C (Mental Health Ideology) and Factor D (Social Restrictiveness) than diploma program murses. 4) Nurses in active Service in the private hospital revealed more positive attitudes(A,B,C,D) except Factor (E) than those in the national or public hospital. 5) The ages, duration of work, wanted or unwantedG roup of psychiatric ward and satisfactory level of psychiatric nursing service were non-significant.

한방병원에 근무하는 간호사의 정신건강에 대한 연구동향: 국내연구를 중심으로 (Scoping Review of Research on Mental Health of Nurses Working in Korean Medicine Hospitals)

  • Objectives: The aim of this scoping review was to analyze research trends about mental health of nurses working in Korean medicine (KM) hospitals in Republic of Korea. Methods: Searches were conducted using four electronic databases including Oriental Medicine Advanced Searching Integrated System, Korean Studies Information Service System, Research Information Sharing Service, Korea Citation Index to collect relevant studies. The search date was March 4, 2021. All studies published up to the search date were considered. Observational studies reporting mental health outcomes of nurses working at KM Hospital were included. Results: A total of 11 cross-sectional observation studies were included. Four of them compared mental health of nurses working on KM and Western medicine (WM). Commonly reported outcomes related to mental health were job satisfaction, role conflict, and work stress. As a result of meta-analysis based on a 5-point Likert scale, the following factors were related to mental health of KM nurses: job satisfaction (2.844±0.067 points), role conflict (3.678±0.058 points), work stress (3.142±0.021 points), turnover intention (3.483±0.028 points), and burnout (3.180±0.033). Compared to WM nurses, KM nurses had significantly less work stress (p=0.000), role conflict (p=0.039), and job satisfaction (p=0.000). Conclusions: Mental health problems of nurses are known to be very common. Although improving them is an important social task, studies on mental health of KM nurses remain insufficient. Based on findings of this study, more cooperation between nurses and KM doctors should be made to improve the mental health of KM nurses, especially their job satisfaction in the future. The Korean Society of Oriental Neuropsychiatry needs to pay more attention to this topic.
