암환자의 대응양식과 불안과의 관계 (Correlation between Mental Adjustment to Cancer and Anxiety)

  • Purpose: This study was to identify the relationship between mental adjustment to cancer and anxiety. Method: This study used a cross-sectional descriptive design. A total of 124 ambulatory cancer patients completed the Mental Adjustment to Cancer (MAC) scale and Hospital Anxiety and Depression Scale (HADS). Data were analyzed using descriptive statistics, t-test, ANOVA and Pearson correlation. Results: There was a significantly negative correlation between anxiety and fighting spirit which was adaptive adjustment to cancer (r=-0.29, p<0.001). However, anxiety had significantly positive correlation with helplessness/hopelessness (r=0.38, p<0.001), anxious preoccupation (r=0.55, p< 0.001), and fatalism (r=0.22, p<0.05) to cancer. Conclusion: Cancer patients' mental adjustment is correlated with the degree of the psychosocial distress. Anxious preoccupation and helplessness/hopelessness are the most maladaptive adjustments whereas fighting spirit is one of the most adaptive adjustment to cancer.

Associations between Psychological Distress and Body Mass Index among Law Enforcement Officers: The National Health Interview Survey 2004-2010

  • Objectives: To investigate the association between psychological distress and obesity among law enforcement officers (LEOs) in the United States. Methods: Self-reported data on psychological distress based on six key questions were obtained from LEOs who participated in the National Health Interview Survey (2004-2010). We used Prochaska's cut-point of a Kessler 6 score ${\geq}5$ for moderate/high mental distress in our analysis. Mean levels of body mass index (BMI) were compared across three levels of psychological distress. Results: The average age of LEOs (n = 929) was 39.3 years; 25% were female. Overall, 8.1% of LEOs had moderate or high psychological distress; 37.5% were obese (BMI ${\geq}30$). Mean BMI increased with increasing psychological distress (no distress, BMI = $27.2kg/m^2$; mild distress, $27.6kg/m^2$; and moderate/high distress, $33.1kg/m^2$; p = 0.016) after adjustment for age, race, income, and education level among female officers only. Physical activity modified the association between psychological distress and BMI but only among male LEOs (interaction p = 0.002). Among male LEOs reporting low physical activity, psychological distress was positively associated with BMI ($30.3kg/m^2$ for no distress, 30.7 for mild distress, 31.8 for moderate/high distress; p = 0.179) after adjustment, but not significantly. This association was not significant among males reporting high physical activity. Conclusion: Mean BMI significantly increased as psychological distress increased among female LEOs. A longitudinal study design may reveal the directionality of this association as well as the potential role that physical activity might play in this association.

대장암 환자의 항암화학요법 관련 인지손상과 삶의 질: 심리적 디스트레스(우울·불안)의 매개효과 (Chemotherapy-related Cognitive Impairment and Quality of Life in People with Colon Cancer: The Mediating Effect of Psychological Distress)

  • Purpose: The purpose of this study was to test the mediating effect of psychological distress in the relationship between chemotherapy related cognitive impairment (CRCI) and quality of life (QOL) in people with cancer. Methods: A purposive sample of 130 patients undergoing chemotherapy was recruited for the cross-sectional survey design. Data were collected from November 2014 to June 2015. The instruments were K-MMSE (Korean Mini-Mental State Examination), Everyday Cognition (ECog), Hospital Anxiety Depression Scale (HADS), and Functional Assessment of Cancer Therapy-General (FACT-G). Data were analyzed using descriptive statistics, correlation, and multiple regression using Baron and Kenny steps for mediation. Results: The mean score for objective cognitive function was 27.95 and 69.32 for perceived cognitive decline. Overall quality of life was 91.74. The mean score was 17.52 for psychological distress. The prevalence was 56.2% for anxiety and 63.1% for depression, and 20.0% for CRCI. There were significant correlations among the variables, objective cognitive function and self-reported cognitive decline, psychological distress, and quality of life. Psychological distress was directly affected by CRCI. ($R^2=29%$). QOL was directly affected by CRCI. Psychological distress and CRCI effected QOL ($R^2=43%$). Psychological distress had a partial mediating effect (${\beta}=-.56$, p <.001) in the relationship between self-reported cognitive decline and quality of life (Sobel test: Z= -5.08, p <.001). Conclusion: Based on the findings of this study, nursing intervention programs focusing on managing cognitive decline, and decreasing psychological distress are highly recommended to improve quality of life in cancer patients.

Predictors of Obesity and Physical Health Complaints Among 911 Telecommunicators

  • Background: This study aims to: (1) examine rates of obesity and physical health complaints among 911 telecommunicators; and (2) document the role of emotion dysregulation, psychological inflexibility, duty-related distress and dissociation, and psychopathology in predicting obesity and physical health complaints in this population. Methods: The sample consisted of 911 telecommunicators from across the country (N = 758). Participants completed an online survey assessing their mental and physical health functioning. Results: A total of 82.5% of the sample reported a body mass index that fell within the overweight or obese category and an average of 17 physical health complaints within the past month. Peritraumatic reactions (distress and dissociation), emotion dysregulation, and psychological inflexibility had effects on physical health largely through psychopathology (alcohol abuse, post-traumatic stress disorder, and depression). Conclusion: Development of adapted prevention and intervention efforts with this population is needed.

A comparative study of the psychological impacts of tasks related and unrelated to COVID-19 on nurses: a cross-sectional study

  • Background: This study assessed the psychological impact of the outbreak of coronavirus disease 2019 (COVID-19) on university hospital nurses. It provides an assessment of mental health issues, including depression, anxiety, distress, and burnout of nurses dealing directly and indirectly with COVID-19. Methods: In a web-based, cross-sectional study, 111 nurses from Daegu Catholic University Hospital in Korea were enrolled from August 4 to August 9, 2020. Patient Health Questionnaire-9, Generalized Anxiety Disorder-7, Impact of Event Scale-Revised, and Maslach Burnout Inventory were used to assess the psychological symptoms of depression, anxiety, psychological distress, and burnout among the study participants. Results: Of 111 nurses, 35 (31.5%), nine (8.1%), 26 (23.4%), and 49 (44.1%) experienced depression, anxiety, distress, and burnout, respectively. Nurses who performed COVID-19-related tasks were more likely to have moderate depression (related vs. unrelated, 52.0% vs. 25.6%; p=0.037). There were no differences in anxiety, distress, and burnout between nurses with and without COVID-19-related tasks. More than 50% of the participants showed receptive and positive attitudes toward caring for COVID-19 patients. Conclusion: Nurses who performed COVID-19-related tasks had a higher risk of depression. There were no significant differences in anxiety, distress, and burnout between the two groups. Since nurses who perform COVID-19-related tasks are more prone to psychological distress, continued psychiatric interventions are required for infectious disease outbreaks with a high mortality rate for healthcare workers who are emotionally vulnerable.

Formulations of Job Strain and Psychological Distress: A Four-year Longitudinal Study in Japan

  • Background: Different job strain formulations based on the Job Demand-Control model have been developed. This study evaluated longitudinal associations between job strain and psychological distress and whether associations were influenced by six formulations of job strain, including quadrant (original and simplified), subtraction, quotient, logarithm quotient, and quartile based on quotient, in randomly selected Japanese workers. Methods: Data were from waves I and II of the Survey of Midlife in Japan (MIDJA), with a 4-year followup period. The study sample consisted of 412 participants working at baseline and had complete data on variables of interest. Associations between job strain at baseline and psychological distress at follow-up were assessed via multivariable linear regression, and results were expressed as β coefficients and 95% confidence intervals including R2 and Akaike information criterion (AIC) evaluation. Results: Crude models revealed that job strain formulations explained 6.93-10.30% of variance. The AIC ranged from 1475.87 to 1489.12. After accounting for sociodemographic and behavioral factors and psychological distress at baseline, fully-adjusted models indicated significant associations between all job strain formulations at baseline and psychological distress at follow-up: original quadrant (β: 1.16, 95% CI: 0.12, 2.21), simplified quadrant (β: 1.01, 95% CI: 0.18, 1.85), subtraction (β: 0.39, 95% CI: 0.09, 0.70), quotient (β: 0.37, 95% CI: 0.08, 0.67), logarithm quotient (β: 0.42, 95% CI: 0.12, 0.72), and quartile based on quotient (β: 1.22, 95% CI: 0.36, 2.08). Conclusion: Six job strain formulations showed robust predictive power regarding psychological distress over 4 years among Japanese workers.

일 지역에 거주하는 암환자의 삶의 질, 정신건강 및 주관적 건강상태 조사 (Survey on Quality of Life, Mental Health and Subjective Health Status of Community Dwelling Cancer Patients)

  • Purpose: This study investigated the quality of life, mental health and subjective health status of community dwelling cancer patients. Methods: Subjects were 212 out-patients with cancer at Kangwon University Hospital. The study was conducted from July 10 to August 14, 2008. Quality of life as determined by the Euro Quality of life-5 Dimensions (EQ-5D) was measured using Korea Centers for Disease Control and Prevention(KCDC). Mental health and subjective health status were measured using KCDC. Results: EQ-5D determined degrees of difficulty were 25.5%(mobility), 25.2%(anxiety or depression), 23.6%(pain or discomfort), 20.3%(activities of daily living), and 13.7%(self care). An analysis of the association between several factors and mental health showed that the statistically significant factors were age, gender, medical coverage, and private insurance. An analysis of the association between several factors and subjective health status showed that the statistically significant factor was occupation. Conclusion: Cancer patients suffer from significant psychological distress. Programs that address mental and physical health would be beneficial.

국내 수의과대학생의 스트레스 경험 연구 (A study on stress experiences of Korean veterinary students)

  • The quality of a veterinary service depends on the veterinarian's physical and mental health. However, a veterinarian's mental health is generally considered to be more vulnerable than that of other health care professionals. As many veterinary students will enter the veterinary profession in near future, it is necessary to investigate their psychological status. This study examined stressors affecting student life in one veterinary school. Anxiety, depression, and stress levels were assessed using questionnaires and qualitative research techniques. The results obtained from questionnaires showed that the proportion of veterinary students with psychological distress was higher than expected. The major stressors experienced by the veterinary students were academic, economic, and environmental. Qualitative study using a photovoice method revealed that the students in this study felt relatively deprived compared with those in other veterinary schools. In addition, they were distressed by the school environment and limited human relationship. Our results suggest that veterinary school administrators should be aware of students' psychological distress and should undertake systematic improvement in their educational system by altering counseling programs, curricula, and school culture principles.

새로운 작업조직 시스템이 직업성 긴장수준 및 직무스트레스에 미치는 영향 (The Impact of New Work Organizational System on Job Strain, and Psychosocial Distress)

  • Objectives : New organizational work systems, and their impact on the mental health of employees, are considered to beone of the most important topics in the area of industrial health. This study was conducted to compare job characteristics (job demand and decision latitude) levels, and psychosocial distress of workers in acompany introducing to new organizational work systems, to those of workers managed by traditional work systems. Methods ; A study sample of 627 shipbuilding workers (446the new work organizational system and 181 the traditional system) were recruited for this study. A structured-questionnaire was used to assess general characteristics, job characteristics(work demand, decision latitude), and psychosocial distress. Results : The decision latitude was not significantly higher in the new work system compared to the traditional system. However, the job demand was significantly higher in the new work system than in the traditional system. The psychosocial distress was higher within the new work system than the traditional system, but no significant relationships were found. The proportion of increased strain was significantly greater with the new system than the traditional system. Conclusion : These results suggest that increases in the decision latitude did not sufficiently compensate for higher job strain or increased work intensity. If the increase in the decision latitude was temporary, with the typical job demand remaining high, such work can be still be considered to have a job strain Futureresearch should consider psychosocial distress and fatigue as importantproblems caused by new work organizational systems, and should be performed to assess their impact through out industry.

모바일 승선신고시스템 개선을 통한 해양수색구조 강화방안 연구 (A Study on Strengthening of Maritime Search and Rescue by Improving the Mobile-phone Boarding Reporting System)

  • 해양 조난사고가 발생하면 해양경찰은 수색계획을 수립하기 위하여 조난자의 수와 신원을 파악하는 등 사고 현황을 조사한다. 선장은 출항 시 선원과 승객의 신분을 포함한 명부를 첨부하여 관할기관에 신고서를 제출하도록 명시되어 있지만 승선자들이 수기로 승선자명부를 작성하면서 대기 시간이 길어지고 조난사고 발생 시 이러한 내용을 즉시 파악하기 어렵다. 승선 신청을 편리하게 할 수 있도록 해양수산부에서 앱을 개발하였지만, 활성화되고 있지 않아 본 연구에서 이를 개선하는 방안을 제안하였다. 모바일 승선신고 제도를 활성화하여 조난사고 시 조난자의 초기 위치와 조난시각을 신속히 확보하고 조난신고 접수 사실을 문자로 통보하여 조난자에게 심적 안도감을 제공할 수 있다. 더불어 이 정보 이용을 통해 해양경찰을 포함하여 주변에 있는 민간 선박과 함께하는 민관협력 형태로 신속히 구조할 수 있는 효과를 기대할 수 있다.
