Survey on Current Situations of Geriatric Patients' Medication and Drug Interactions Based on Prescriptions (노인환자들의 복약실태와 처방전상의 약물상호작용 검토)

  • It is common that geriatric patients are on several medications at the same time. With this situation on hand, this study has collected prescriptions of individual geriatric patient and investigated possible drug interactions. In order to minimize the drug interactions and protect those patients from adverse reactions of medication, the gradual implementation and management of the medication history of each individual patient, the establishment of medication counseling system on medicines particularly in need of obligatory advice, the use of an inspection system on drug interactions at the time of prescription, and the implementation of a patient monitoring system on the drugs with narrow margin of safety at hospitals.

  • Current trend of aging society suggests that many patients are at risk for various preoperative preparations and postoperative complications during and following invasive dental procedures, due to an acquired medically compromised conditions from systemic disease and/or from medications. The medical history is critical for the identification of patients potentially at risk for medically compromised and old aged patients' factors from dental treatment. The proper dental management requires an understanding of certain principles of pathophysiology for these medical conditions and some standard laboratory tests. Polypharmacy in old age, besides representing a risk in and of itself, points to the potential risk the underlying diseases that necessitated the drugs can present in the dental office. These diseases and medications can also present a risk to oral health. A sequence for categorizing drugs in a medication list is presented here to aid in the identification of potential risks in the dental treatment and management of patients with complex medical histories and drug regimens. Specific patient populations, such as pediatric, may have specific drugs or additional criteria that need to be considered. Practitioners must use the health history and the medication list in concert, using one to make sense of the other and utilizing all the information available from reviewing each one carefully in order to manage their increasingly complex patients safely and effectively.

Analgesic Effect of Electroacupuncture after Femoral Head Osteotomy in Dogs (개에서 대퇴골두 제거술 후 전침 자극이 통증완화에 미치는 영향)

  • This experiment was designed to investigate the analgesic effect of non-invasive electroacupuncture treatment on femoral head osteotomy in dogs. Twelve dogs were divided into 3 groups in this experiment. Electroacupuncture group was non-invasively electrostimulated in Huan Tiao (GB-30) acupoint for 10 minutes by 4.5V, 5Hz for 10 days. Medication group was treated with carprofen (4mg/kg, P.O.) for 10 days. Control group was not treated. Each group was pre-operatively and post-operatively scored by using VAS (Visual Analogue Scale), SDS (Simple Descriptive Scale), NRS (Numerical Rating Scale), and measured serum cortisol levels. The results of VAS, SDS and NRS in electroacupuncture and medication groups were better than those of control group. And, the results of electroacupuncture group were better than those of medication group since 6th day after osteotomy in pain management(p<0.05). But, there were no significances in serum cortisol level among 3 groups. In conclusion, non-invasive electroacupuncture treatment showed sufficient analgesic effect on postoperative pain management after femoral head osteotomy, and VAS, SDS, and NRS scoring system would be adapted in evaluation of pain management.

That medication management effective and gives the enhanced way to provide information about seamen's medical condition check and the medicine habit. Transmission of detail information about medicine inventory through the wireless communication network is able to reduce the time required and workload. Does the Use of Asthma-Controller Medication in Accordance with Guidelines Reduce the Incidence of Acute Exacerbations and Healthcare Costs?

  • Background: In asthma, consistent control of chronic airway inflammation is crucial, and the use of asthma-controller medication has been emphasized. Our purpose in this study is to compare the incidence of acute exacerbation and healthcare costs related to the use of asthma-controller medication. Methods: By using data collected by the National Health Insurance Review and Assessment Service, we compared one-year clinical outcomes and medical costs from July 2014 to June 2015 (follow-up period) between two groups of patients with asthma who received different prescriptions for recommended asthma-controller medication (inhaled corticosteroids or leukotriene receptor antagonists) at least once from July 2013 to June 2014 (assessment period). Results: There were 51,757 patients who satisfied our inclusion criteria. Among them, 13,702 patients (26.5%) were prescribed a recommended asthma-controller medication during the assessment period. In patients using a recommended asthma-controller medication, the frequency of acute exacerbations decreased in the follow-up period, from 2.7% to 1.1%. The total medical costs of the controller group decreased during the follow-up period compared to the assessment period, from $3,772,692 to $1,985,475. Only 50.9% of patients in the controller group used healthcare services in the follow-up period, and the use of asthma-controller medication decreased in the follow-up period. Conclusion: Overall, patients using a recommended asthma-controller medication showed decreased acute exacerbation and reduced total healthcare cost by half.

In the present study, we describe design and implementation of a personal digital assistant(PDA) based asthma management system for personal application including symptom and medication to prevent from the potential exacerbation of the disease. The software program was written by the Visual C++ tool in the mobile computing environment and Object Store was applied for data management. User friendly GUI environment was provided for the patient to input his/her daily condition and self treatment such as medication for successful management. The input screen design substituted for keyboard input to a mouse in order to easy to select an item and minimize the keyboard input. The implementation results of this system., Real-time data collection and process were possible and be able to have been carried effectively out a continuous symptom, a medication of asthma patients, risk management.

Sufficient explanation of management affects patient satisfaction and the practice of post-treatment management in spinal pain, a multicenter study of 1007 patients

  • Background: Spinal pain is most common symptom in pain clinic. In most cases, before the treatment of spinal pain, physician explains the patient's disease and treatment. We investigated patient's satisfaction and physician's explanation related to treatments in spinal pain patients by questionnaires. Methods: Anonymous questionnaires about physician's explanation and patient's satisfaction in each treatment and post-treatment management were asked to individuals suffering from spinal pain. Patients who have spinal pain were participated in our survey of nationwide university hospitals in Korea. The relationships between patient's satisfaction and other factors were analyzed. Results: Between June 2016 and August 2016, 1007 patients in 37 university hospitals completed the questionnaire. In the statistical analysis, patient's satisfaction of treatment increased when pain severity was low or received sufficient preceding explanation about nerve block and medication (P < 0.01). Sufficient explanation increased patient's necessity of a post-treatment management and patients' performance rate of post-treatment management (P < 0.01). Conclusions: These results show that sufficient explanation increased patients' satisfaction after nerve block and medication. Sufficient explanation also increased the practice of patients' post-treatment management.

Effects of perceptions of the importance of patient safety management and patient safety competency on patient safety management activities among nurses at unaccredited general hospitals (미인증 종합병원간호사의 환자안전관리 중요성 인식, 환자안전역량이 환자안전간호활동에 미치는 영향)

  • Purpose: This descriptive research study attempted to determine how general hospital nurses' awareness of the importance of patient safety management and patient safety competency affected patient safety management activities. Methods: From September 13 to 26, 2022, a survey was administered to 230 ward nurses who provided direct care to patients at five non-accredited general hospitals being evaluated for accreditation located in metropolitan cities. The collected data were analyzed with descriptive statistics, the t-test, one-way analysis, the Scheffé test, Pearson correlation coefficients, and hierarchical regression using SPSS for Windows version 26.0. Results: In total, 221 (96.1%) respondents were female. The average age was 32.2 years, and the average clinical experience was 3.5 years; 196 participants (85.2%) were general nurses. Patient safety competency (β = .44, p < .001), awareness of the importance of safety management (β = .31, p < .001), and medication error experience (β = -.15, p = .002) all had statistically significant associations with patient safety management activities. The explanatory power of these variables for patient safety management activities was 50.7%. Conclusion: This study confirmed that patient safety competency, awareness of the importance of patient safety management, and experience with medication errors significantly influenced patient safety management activities.

Related Factors with Medication Task Ability in Rural Elderly (일부 농촌 노인에서의 약물복용 수행능력과 관련된 요인)

  • Medication non-compliance among the elderly results in medical problems and substantial cost to the health care system. This study investigate predicted variable related to the medication task ability among elderly. This study was done in the selected 4 villages in Kimchun County of Kyungbuk Province from July to August, 1996. The subject was the resident that 202 adults above 60 years of age. The questionnaire of interview included medication task ability, socio-demographic data, COOP/WONCA chart, family ABGAR score. BDI(Beck depression inventory), ADL(activities of daily living), IADL(instrumental activities of daily living), and MMSE-K(minimental state examination-Korean version). The results were as followed : 1. Approximately 49% of study population was taking drug medication currently. We found that 93% of study population was successful at the medication task all alone, 6% was failure at the medication task all alone, so need help partly or completely. 2. Significant variables between group of medication task ability were age, educational attainment, IADL, and MMSE-K in univariate analysis. And significant correlated variables with medication task ability were ADL, IADL, MMSE-K, and BDI in correlation analysis. 3. Major predictors to medication task ability on multiple logistic regression were IADL and sex finally. Findings suggest that IADL is related to medication task ability than other test battery of health status, so IADL could be used to necessary for medication management and add information to conventional methods of assessing mental status.
