Direct-to-consumer genetic testing

  • Direct-to-consumer (DTC) genetic testing is a controversial issue although Korean Government is considering to expand DTC genetic testing. Preventing the exaggeration and abusing of DTC genetic testing is an important task considering the early history of DTC genetic testing in Korea. And the DTC genetic testing performance or method has been rarely reported to the scientific and/or medical community and reliability of DTC genetic testing needs to be assessed. Law enforcement needs to improve these issues. Also principle of transparency needs to be applied.

  • In "Huangdineijing", detailed descriptions of the human physiology, pathology, causes of diseases, and symptoms can be found. The doctors who studied "Huangdineijing" interpreted "Huangdineijing" from many different points of view and such contents are being put to practical use even today. Some parts, however, are not translated clearly enough. Contents of the book of Fuqibing falls into this category. It cannot be explained why the description of Fuqibing is mentioned after Fuqibing. This study is an organization of the writer's opinion on Fuqibing.

구급대원의 임파워링 리더십과 성장욕구 및 이직의도와의 상관관계에 관한 연구 (A study on correlation between empowering leadership, growth needs and turnover intentions in 119 emergency medical technicians)

  • Purpose: The purpose of this study is to investigate how empowering leadership effects on growth needs and turnover intentions to emergency medical technicians. Methods: The study was based on seventy six paramedics and seventy EMTs-basic who were working at the fire stations. A survey questionnaire was used to prove connection between empowering leadership, growth needs, and turnover intentions. Results: There was a positive correlation between empowering leadership and growth needs, and negative correlation between empowering leadership and turnover intentions. There was a positive correlation between emergency medical technicians growth needs and turnover intentions. Conclusion: This analysis is empirically applied to study the empowering leadership focused on the empowerment act of leader to a leading role to empowerment in fire service organization.

Beta-Meta: a meta-analysis application considering heterogeneity among genome-wide association studies

  • Many packages for a meta-analysis of genome-wide association studies (GWAS) have been developed to discover genetic variants. Although variations across studies must be considered, there are not many currently-accessible packages that estimate between-study heterogeneity. Thus, we propose a python based application called Beta-Meta which can easily process a meta-analysis by automatically selecting between a fixed effects and a random effects model based on heterogeneity. Beta-Meta implements flexible input data manipulation to allow multiple meta-analyses of different genotype-phenotype associations in a single process. It provides a step-by-step meta-analysis of GWAS for each association in the following order: heterogeneity test, two different calculations of an effect size and a p-value based on heterogeneity, and the Benjamini-Hochberg p-value adjustment. These methods enable users to validate the results of individual studies with greater statistical power and better estimation precision. We elaborate on these and illustrate them with examples from several studies of infertility-related disorders.

영남대학교 의과대학 의예과 교육과정 개발과 편성 사례 (Curriculum Development for Preclinical Medical Education at Yeungnam University)

  • After Yeungnam University's College of Medicine was established in 1979, the curriculum for a preclinical medical education course was developed and implemented. Several modifications have since been made to the curriculum which was driven by changes in national policies and in the medical education environment. In recent years, it has become necessary to complement the weaknesses or shortcomings in the curriculum that were discovered during the basic medical education assessment process of the medical college. Since 2009, Yeungnam University has run two medical courses: a 6-year college of medicine course and a 4-year medical school course. However, as a result of changes in national policy, Yeungnam University decided to offer only the 6-year college of medicine course with an entirely new curriculum which will be implemented in 2017. The new curriculum for the preclinical medical education course consists of 36 credits of cultural essentials courses, 44 credits of major required courses, and 2 credits of major elective courses. The curriculum development requires the support of the university and/or college, the ensured independence of the curriculum development organization, and the cooperation and attention of fellow professors. Continuous efforts are needed to check, evaluate, and improve the curriculum.

119구급대원의 업무스트레스와 피로 및 직무만족도에 관한 연구 (A Study on Job Stress, Fatigue and Job Satisfaction of 119 Emergency Medical Technician)

  • This study selected emergency medical technicians at 24 fire police boxes in Jeonnam in order to examine the relationship between job stress, fatigue and job satisfaction of 119 emergency medical technicians. Data were collected from Apr. 10 to May 9, 2004, questionnaires were mailed and returned by mail. Total 109 questionnaires were distributed and 89 questionnaires were collected and 80 except 9 omitting answers and showing inappropriate answers were used for final analysis. The results of this study are as follows. A. The total mean of areas composing job stress in the degree of job stress of 119 emergency medical technicians was over the average as 2.54 (SD .44) of 4 full marks. According to the degree of job stress by each area, the highest area of job stress was crisis situation (2.98), followed by role conflict as professional job (2.58), social factors (2.57), involvement in organization (2.55). To the contrary, the areas of the lowest job stress were professional knowledge and technique (2.35), psychological burden of medical limitation (2.38), conflicts in personal relationship and services (2.45) and improper treatment (2.53). B. The degree of fatigue of 119 emergency medical technicians by areas was over mean as 2.09(SD .49) of 4 full scores and physical fatigue was highest (2.18), followed by mental fatigue (2.11) and neurosensory fatigue (1.99). C. The degree of job satisfaction of 119 emergency medical technicians was 2.71 of 5 full scores and working condition area (3.70) showed the highest job satisfaction, followed by requirement of organization (2.85) and the lowest job satisfaction area was job itself (2.18), followed by desire for job (2.49). D. According to differences in demographical characteristics of 119 emergency medical technicians, there was no variable of statistical difference at job stress and job satisfaction in significance level .05, but fatigue showed statistical difference at final education (F=2.393, p=.046). E. According to differences related to job characteristics of 119 emergency medical technicians, job stress showed statistical differences at motif of current services (F=4.935, p=.003), fatigue showed those at health conditions (F=2.732, p=.008), job satisfaction at overtime (F=3.038, p.034) and the greatest reason of having job (F=3.217, p=.017), F. As a result of analyzing correlations between job stress. fatigue and job satisfaction of the subject, job stress showed positive correlations (r=.411, p=.000) with fatigue and negative correlation (r=-.267, p=.008) with job satisfaction. Fatigue showed negative correlation (r=-.287, p=.005) with job satisfaction. Therefore, the hypothesis of this study. "There will be relationship between degree of stress, fatigue and job satisfaction of subjects." was supported.

병원 원가관리자의 원가인식 및 원가체계 구축 방향 (Hospital Cost Analysts' Perception on Prime Cost of Medical Services and Future Direction to Establish a Cost Accounting system)

  • It is necessary to calculate prime cost of medical services accurately in order to evaluate the adequacy of medical fee. This paper aims to identify cost analysts' perception on prime cost of medical services and needs in establishing a cost accounting system in hospitals, proposing future directions and guidelines for the calculation of medical fee. A self-administered questionnaire and telephone survey on operation of a hospital cost-accounting system was conducted in November, 2012, among cost analysts currently working in the hospitals and hospital administrators planning to implement the hospital cost-accounting system. Our study shows that most of the cost analysts were aware of the importance of calculating prime cost and responded that collection of the prime cost data from government is necessary although they are less likely to provide the data in the future concerning the risk of data misuse and data security. They also responded that lack of budget allocation and excessive workload were the main reasons for not estimating the prime cost and operating cost management information system. Results show that hospital cost analysts considered the data accuracy is the most critical factor in calculating prime costs of medical services. However, there was no investment budget allocated in some hospitals or limited to less than 100 million, indicating that hospitals are reluctant to invest on implementing the cost accounting system. Respondents stated the organization that collects the prime cost of medical services among hospitals should display strong analytical capabilities, ensure data security, and maintain independence, which is most demanded. There are 57 hospitals that calculated the prime cost of medical services for 2012 by each medical department and 20 hospitals that calculated the prime cost by fee-for-services, aiming to establish a cost accounting system. Our results indicate that hospitals should voluntarily provide the accurate prime cost for medical services in order to properly evaluate the adequacy of medical fee. Consequently, it is critical to establish an independent organization to collect and appraise the data. It is also recommended that government should implement various policies to encourage hospitals to participate in the data collection to achieve the data accuracy and representativeness.

소자본계통(小字本系統) 『금궤요략(金匱要略)』 오천본(吳遷本)의 특징(特徵)에 대한 고찰(考察) - 등진본(鄧珍本)과의 비교(比較)를 중심(中心)으로 - (A Study on the Characteristics of the Wuqian Edition(吳遷本) of Jinguiyaolue(金匱要略) written in Small Letters)

  • Objective : The recently reported Wuqian edition(吳遷本) Jinguiyaolue(金匱要略) is known to be written in small letters unlike other large lettered versions of the book. The Wuqian edition(吳遷本) takes after the name of Wuqian who copied an ancient version that was found at the time(Ming dyanasty, 1395). It is known to differ in organization and contents to other versions. In this study, I would like to examine how the small-lettered version of the Jinguiyaolue had been created, through a thorough examination of the Wuqian edition, and evaluate its value as a new version of the Jinguiyaolue. Method : First, the systems of both large-lettered and small-lettered versions of the "Jinguiyaolue" were briefly examined, followed by comparison from chapter 1 to 22, of organization, contents, and formulas between the oldest version of the large-lettered versions, the Deng-zhen edition, and the newly discovered Wuqian edition of which examples of each item are listed. The original source was identified for parts that were different. Result & Conclusion : 1. The Deng-zhen edition and Wuqian edition show vast differences in the organization, table of contents, name of chapter, location of each verse or line, existence of certain verses or lines, name of formula, quantity of medicinal ingredients, processing methods and other aspects. 2. The small-lettered edition was published based on the large-lettered edition of the Jinguiyaolue published by the Jiaozheng-yishuju(校正醫書局) after a general cross-examination process followed by selection, modification and supplementation of the large-lettered edition. Reference texts used in cross-examination were Qianjinyaofang(千金要方), Waitaimiyao(外臺秘要), Maijing(脈經), and Shanghanlun(傷寒論). 3. Considering Wuqian's own annotations and preface, it is clear that he wanted to create the best version of the "Jinguiyaolue". Wuqian himself seems to have had professional knowledge of bibliography and medicine.

국회 청원제도를 통한 보건의료 입법사례 연구: 13-20대 보건복지위원회 청원을 중심으로 (Healthcare Legislation Cases in the National Assembly Petition System: Focused on Petitions to the Health and Welfare Committee of the 13th National Assembly through the 20th Assembly)

  • The right to petition is a classical right of the people in constitutional states, and in Korea, it is a statutory right in the Constitution, the National Assembly Law, the Petition Law, and the Local Autonomy Act. The healthcare community first made a successful petition to the National Assembly when it achieved the amendment of the Government Organization Act through a petition to the National Assembly for the independence of the Ministry of Health, and this achievement served as the basis for further petitions. Since then, the healthcare community has successfully achieved the enactment and amendment of related occupational laws through National Assembly petitions, such as the amendment of Article 41, Paragraph 7 of the former Medical Insurance Act (Korean Medical Association, 14th Assembly), enactment of the Dental Health Act (Korean Dental Association, 15th Assembly), and amendment of the Health Functional Foods Act (Korea Pharmaceutical Association, 16th Assembly). Its petition accomplishment rate is higher than the total petition accomplishment rate of the Health and Welfare Committee of the National Assembly. However, along with the overall decrease in the number of National Assembly petitions, the Korean Medical Association and Korea Pharmaceutical Association have not achieved any results through petitioning since the 16th Assembly (June 2000), and the Korean Hospital Association and Korean Nurses Association have not achieved any results through petitioning since the 17th Assembly (April 2004). Furthermore, no National Assembly petitions have been made at all for 5 years (2014-2018). The Korean Medical Association and Korea Pharmaceutical Association previously showed a high petition accomplishment rate through their accumulated experience with National Assembly petitions and vigorous policy assistance from doctors/pharmacists/nurses turned lawmakers. More specifically, healthcare organizations have achieved results by actively conducting organized activities with the National Assembly, as implemented by a national assembly director and employees, and in case of petitions for legislation, each group has established infrastructure for reviewing the relevant laws by appointing a legislative director, as well as a legal advisor and advisory counsel. Although the organization that has submitted the most petitions to the National Assembly is the Korean Hospital Association, the group with the highest petition success rate is the Korean Medical Association, which may be linked to the relatively high proportion of doctors who have become lawmakers. Furthermore, the fact that other healthcare organizations were highly interested in petitioning the National Assembly has had major implications for the petition activities of healthcare organizations.

주요 상병 별 환자의 의료기관 선택성향 분석 (An Analysis of the Diseases Specific Medical Service Organization Selection Factors of Patients)

  • The relaxation of the regulation in selection of medical institution allows patients to use their own judgement in choosing proper institution for their diseases. Since the change of the regulation, there should have been many changes in medical institution selection behavior. The analysis of the change in disease specific selection pattern is critical because there be an optimal selection criteria that ensure the efficient and effective utilization of medical resources. This study analysis the institution selection factors by comparing the choice among the cases of acute diseases, the cases of chronic diseases, inpatient services, outpatient services, and emergency medical service. The comparisons performed in terms of size, class and other characteristics of medical institutions. For the study the nationally surveyed database was used and the data were analyzed using logistic regression procedure. The results indicates that the primary care facilities were not properly utilized. This study speculates that the reason for the undesirable pattern of utilization is that the roles of primary care facilities in the healthcare delivery system was not clearly defined. Based on the results, the medical policy implications are discussed.
