Fluoroscopic the equipment study in accordance with the entrance surface dose study of patients and practitioners (투시 검사 시 장비에 따른 환자와 시술자의 입사표면선량 연구)

  • Purpose : Fluoroscopy equipment, depending on the type of changes that occur in the patient's position ESD and study the patient's scatter ray of ESD Practitioners considered a comparative analysis was to evaluate the correct dose. Materials and Methods : HITACHI four overtube type TU-8000 Flat Detector and Under tube C-Arm Philips' Multi Diagnost Eleva with Flat Detector type were measured by. Results : 10 seconds and the patient perspective of the c-arm ESD 1.2 times smaller on the AP and oblique measurements were measured in the 6-13 times smaller. spot positions to changes in the measured three times on the AP of the abdomen, ESD is 18 times smaller c-arm measurements and the oblique measurement was 19-30 times smaller.   • According to recent trends in technological advances and globalization, medical device industry may improve the constitution to meet worldwide medical equipment management system. Also it is holding a strand of mitigation to reach the level of international regulation. In addition, recent legislation tends are to stay limited ranges of industry regulations at least and take as open attitude for integration of new technologies combined medical devices. A greater environmental risk is not likely to work in medical technology, Combinded medical device is used as close to zero risk in most of the human body, which is classified as Class 1. Even medical device such as little or no risk in handling, it is possible to minimize the unnecessary administrative power and a waste of time to occur. For the medical device may be improving people's choices and access, medical equipment operator is expanding to include dealers, because this will be exalted to particular area of the business of the company. In this paper, we investigate the legal prerequisites for the establishment of a medical device. And propose improved regulations in topics in order to facilitate the repair and distribution markets for fair trade.

The Income and Cost Estimate for the Medical Clinic Services Based on Available Secondary Data (이차자료원을 활용한 의원 의료서비스 수입 및 비용 산출)

  • Purpose: The purpose of this study is to estimate incomes and costs of the medical clinics by using secondary data. Methodology: The medical incomes and costs were estimated from 405 clinics operated by sole practitioner providing out-patient services among all clinics subject to the Medical Cost Survey on National Health Insurance Patients in 2017, excluding dental clinics and oriental medical clinics. The incomes and costs of the medical clinics were reflected with incomes and costs of health insurance benefits and were calculated by types of medical services (i.e., basic care, surgery, general treatment, functional test, specimen test and imaging test). The costs were classified as follows: labor costs, equipment costs, material costs and overhead costs. Secondary data was used to estimate the incomes and costs of the medical clinics. For allocation bases for costs for each type of the medical service, the ratio of revenue from health insurance benefits by types of medical services was applied. However, labor costs were calculated with the activity ratio by types of medical services and occupations, using clinical expert panel data. Finding: The percentage of health insurance income for all medical income was 73.1%. The health insurance cost per clinic was 401,864 thousand won. Labor cost accounted for the largest portion of the health insurance income was 191,229 thousand won (47.6%), followed by management cost was 170,018 thousand won (42.3%), materials cost was 35,434 thousand won (8.8%), and equipment costs was 5,183 thousand won (1.3%). Practical Implications: This study suggests a method of estimating incomes and costs of medical clinic services by using secondary data. It could efficiently provide incomes and costs to assess an appropriate level of the health insurance fee to the clinics.

  • Detection of small defects by medical ultrasonic testing when the thermal sprayed coating by $MoSi_2$ on the metal is done. The defects may occur at the bonded surface. So, the detecting method of the defects by non-destructive in spection is desired. Here, in order to examine the possibility of the detection of the small defects by the ultrasonic. The electronic scanning ultrasonic equipment using an array probe developed as the medical ultrasonic diagnostic equipment is applied for the detection of the metal material defects. It's validity is investigated.

  • In this study, we have established the technique to estimate the gradient waveforms distorted by the eddy current in MRI. After obtaining the eddy current effect transfer function using the finite element method, we have used the transfer function to estimate the output gradient waveforms at any points inside the imaging region. We also present experimental results to be compared with estimated ones.

  • The demand of cyclotron for PET (positron emission tomography) has rapidly grown as the more use of PET or PET-CT equipment requires the increased amount of radioactive isotopes for clinical positron emission. While research on failure statistics of medical equipment used in medical centers has continued to be done, the analysis and study on failure statistics of cyclotron have rarely been conducted. However, the growing demand shows the urgency of systematical management guideline and countermeasures for device failure to minimize any supply delay of radiopharmaceuticals occurred by such failure and complains from waiting patients for PET-CT diagnosis. Therefore, this study aims to analyze the failure report from Minitrace equipped in cyclotron which is owned by the department of nuclear medicine at Yeungnam University Medical Center and draws on the data to build effective management system for cyclotron.

  • Though the planar gradient coils, designed by the magnetic energy minimization procedure, have smaller inductance than conventional gradient coils, the planar gradient oils often suffer from their poor magnetic field linearity. Scaling the spatial frequencies of the current density function designed by the magnetic energy minimization procedure, magnetic field linearity of the planar gradient coils can be featly improved with small sacrifice of gradient coil inductance.

Survey of Staphylococcus epidermidis Contamination on the Hands of Dental Hygienists and Equipment Surface of Dental Clinics (치과의료기관 의료장비 표면 및 치과위생사 손의 Staphylococcus epidermidis 오염도 조사)

  • The purpose of this study was to investigate Staphylococcus epidermidis contamination on hands of 20 dental hygienists and 140 equipment surface of 20 dental clinics in a local area, from July to August 2017. The degree of S. epidermidis contamination was measured using a hand plate and a rodac plate and then cultured at $35^{\circ}C$ for 24 hours. Based on hand plate criteria, hand contamination was classified into low, middle, and high groups. Analysis of the variance (ANOVA) of the contamination level of the hand parts of the group surface contamination level of the dental clinic equipment was descriptive statistics after clustering lock count. S. epidermidis contamination was moderate in 55% of the hands of dental hygienists. The area of contamination was 29.45 colony-forming units (CFU) on the palm, followed by the middle finger 7.8 CFU, ring finger 6.4 CFU, and thumb 6 CFU. Medical equipment surface contamination was showed that 3-way handle 4.45 CFU, computer mouse 3.37 CFU, mirror handle 1.60 CFU were higher than other areas. The group with high hand contamination had a high positive correlation with the S. epidermidis contamination of the hand. S. epidermidis contamination level was higher on hands than on the medical equipment surface contamination. Therefore, medical staff should recognize the importance of hand hygiene which should be practiced in the manner suggested by World Health Organization. In addition, the medical team needs to be responsible for performing infection control tasks, implementing infection management guidelines and providing systematic education on infectious disease management.

  • Purpose: As medicare services have gotten spreaded, clinical laboratory has been dominant position. So, it has been acted for quality control and clinical pathology accreditation. But there has been quite deficient information to evaluate working space and technical standards of medical laboratory for accreditation. This study goals to figure out accreditation standard and design guideline for clinical laboratory, and to give safe and efficient design information. Methods: This study has been searched by literature for accreditation standards and design guidelines of clinical pathology in USA, UK, and Germany. Results: Three countries have accredited based on working lab space, staff space, storage space, patient space and health and safety equipment. Design guidelines of three countries commonly have focused on worktable layout, worktable distance and module, and specific laboratory biosafety level. And USA guidelines stress on the architectural design such as design process and passage distance for escape, UK stress on the efficiency as functional work flow and construction cost, lastly Germany design guidelines stress on the operator's safety distance and workstation. Three countries have not only accreditation standards but also design guidelines for more specific quality management, separating from accrediting institute. Implications: In korea, it has been needed to make clinical laboratory design guideline for the safe and efficient environment and reliable and competitive medical service.

Fundamental research for the development of personal protective equipment for emergency medical technicians (구급대원용 개인보호복 개발을 위한 기초연구)

  • Purpose: The purpose of this study was to provide basic data for the development of personal protective equipment (PPE) for emergency medical technicians (EMTs) during chemical, biological, radiological, nuclear, and explosive situations. Methods: Body measurements were obtained for adults aged from 20 to 59 years from the data in the 6 th Size Korea national sizing survey. These data were compared to the sizes of protective clothing currently available in the market. In-depth interviews with active paramedics with experiences of wearing PPE were conducted. Results: Most of the imported protective clothing turned out to be unfit for Korean adults. This showed the urgent need for developing appropriately sized protective clothing for Korean emergency technicians. In total, 55.0% of the respondents indicated that the current protective clothing is unsafe, and 71.0% requested the clothing to have level C protective performance. Regarding the design, many people wanted hooded all-in-one type of clothing. Conclusion: Considering these requirements, most of the wearers wanted their protective clothing to be fundamentally protective of their body, be available in various sizes with adjustable parts, and easy to wear and take off. They also wanted the clothing to be secure in clear sight, while not revealing any parts of their body and not interfringe with their ability to communicate with others.
