Objective: To determine the unique contribution of geometrical design characteristics of orthodontic mini-implants on maximum insertion torque while controlling for the influence of cortical bone thickness. Methods: Total number of 100 cylindrical orthodontic mini-implants was used. Geometrical design characteristics of ten specimens of ten types of cylindrical self-drilling orthodontic mini-implants (Ortho Easy$^{(R)}$, Aarhus, and Dual Top$^{TM}$) with diameters ranging from 1.4 to 2.0 mm and lengths of 6 and 8 mm were measured. Maximum insertion torque was recorded during manual insertion of mini-implants into bone samples. Cortical bone thickness was measured. Retrieved data were analyzed in a multiple regression model. Results: Significant predictors for higher maximum insertion torque included larger outer diameter of implant, higher lead angle of thread, and thicker cortical bone, and their unique contribution to maximum insertion torque was 12.3%, 10.7%, and 24.7%, respectively. Conclusions: The maximum insertion torque values are best controlled by choosing an implant diameter and lead angle according to the assessed thickness of cortical bone.
Objective: The purpose of this study was to evaluate the effect of length and shape of cutting flute on mechanical properties of orthodontic mini-implants. Methods: Three types of mini-implants with different flute patterns (Type A with 2.6 mm long flute, Type B with 3.9 mm long and straight flute, Type C with 3.9 mm long and helical flute) were inserted into the biomechanical test blocks (Sawbones Inc., USA) with 2 mm and 4 mm cortical bone thicknesses to test insertion and removal torque. Results: In 4 mm cortical bone thickness, Type C mini-implants showed highest maximum insertion torque, then Type A and Type B in order. Type C also showed shortest total insertion time and highest maximum removal torque, but Type A and B didn't showed statistically significant difference in insertion time and removal torque. In 2 mm cortical bone thickness, there were no significant difference in total insertion time and maximum removal torque in three types of mini-implants, but maximum insertion torque of Type A was higher than two other Types of mini-implants. Conclusions: Consideration about length and shape of cutting flute of mini-implant is also required when the placement site has thick cortical bone.

STATEMENT OF PROBLEM: The application of a simple, clinically applicable noninvasive test to assess implant stability are considered highly desirable. So far there is still a controversy about correlation of various tests and implant stability. PURPOSE: In order to assess implant stability, the development of a new method is critical. It's possible to assess implant stability by calculating energy and angular momentum during implant installation. The purpose of this study is to evaluate the correlation of energy and implant stability. MATERIAL AND METHODS: Twenty three implants were installed in two different types of pig bone. Type I bone was retrieved from the distal aspect of the rib, with more cortical bone. Type II bone came from a more proximal region with less cortical components and a higher content of bone marrow and spongeous trabeculae. Insertion torque, removal torque, ISQ values and angular momentum and energy were measured. Pearson Correlation test was done to analyze the relation between RFA, maximum insertion torque, mean insertion torque, bone type, energy and removal torque. RESULTS: Type I bone showed higher removal torque than type II bone. Energy value was significantly correlated with maximum insertion torque and mean insertion torque. RFA values were related with insertion torques but the significance was lower than Energy value. CONCLUSION: Within the limitation of this study energy values were considered clinically predictable method to measure the implant stability.

Purpose: To evaluate the effect of different-sized drill tips and laser irradiation times on the initial stability of orthodontic miniscrews placed in Er,Cr:YSGG-laser pre-drilled holes in an animal model. Materials and Methods: Laser pre-drilled holes were made in dog mandibular bone with an Er,Cr:YSGG laser using irradiation times of 5, 7, 9, 11, and 13 seconds, and tip diameters of 0.4 and 0.6 mm. The maximum diameter and depth of the pre-drilled holes was measured with micro computed tomography. The maximum insertion torque was measured during placement the miniscrew. Result: Laser pre-drilled holes were conical shaped. The maximum diameter of pre-drilled holes increased with longer laser irradiation times (P>0.05) and larger tip diameters (P<0.05). The depth of pre-drilled holes increased with longer laser irradiation times and larger tip diameters (P<0.05). When the 0.4 mm tip, but not the 0.6 mm tip, was used, the insertion torque decreased significantly with longer laser irradiation times (P<0.05). Conclusion: Tip diameter impacted insertion torque more than irradiation time. It takes at least 9 seconds using a 0.6 mm tip to create a 0.8 mm diameter and 1.0 mm depth hole in thick cortical bone.
PURPOSE. The aim of this study was to investigate a comparison of implant bone bed preparation with Er,Cr:YSGG laser and conventional drills on the relationship between implant stability quotient (ISQ) values and implant insertion variables. MATERIALS AND METHODS. Forty implants were inserted into two different types of pig rib bone. One group was prepared with conventional drills and a total of 20 implants were inserted into type I and type II bone. The other group was prepared with a Er,Cr:YSGG laser and a total of 20 implants were inserted into type I and type II bone. ISQ, maximum insertion torque, angular momentum, and insertion torque energy values were measured. RESULTS. The mean values for variables were significantly higher in type I bone than in type II bone (P < .01). In type I bone, the ISQ values in the drill group were significantly higher than in the laser group (P < .05). In type II bone, the ISQ values in the laser group were significantly higher than in the drill group (P < .01). In both type I and type II bone, the maximum insertion torque, total energy, and total angular momentum values between the drill and laser groups did not differ significantly (P ${\geq}$ .05). The ISQ values were correlated with maximum insertion torque (P < .01, r = .731), total energy (P < .01, r = .696), and angular momentum (P < .01, r = .696). CONCLUSION. Within the limitations of this study, the effects of bone bed preparation with Er,Cr:YSGG laser on the relationship between implant stability quotient (ISQ) values and implant insertion variables were comparable to those of drilling.
Objective: The purpose of this study was to evaluate the mechanical performance of mini-screws during insertion into artificial bone with use of the driving torque tester (Biomaterials Korea, Seoul, Korea), as well as testing of Pull-out Strength (POS). Methods: Experimental bone blocks with different cortical bone thickness were used as specimens. Three modules of commercially available drill-free type mini-screws (Type A; pure cylindrical type, Biomaterials Korea, Seoul, Korea, Type B; partially cylindrical type, Jeil Medical, Seoul, Korea, Type C; combination type of cylindrical and tapered portions, Ortholution, Seoul, Korea), were used. Results: Difference in the cortical bone thickness had little effect on the maximum insertion torque (MIT) in Type A mini-screws. But in Type B and C, MIT increased as the cortical bone thickness Increased. MIT of Type C was highest in all situations, then Type B and Type A in order. Type C showed lower POS than Type A or B in all situations. There were statistically significant correlations between cortical bone thickness and MIT, and POS for each type of the mini-screws. Conclusion: Since different screw designs showed different insertion torques with increases in cortical bone thickness, the best suitable screw design should be selected according to the different cortical thicknesses at the implant sites.

The successful outcome of dental implants is mainly the result of intial implant stability following placement. The aim of this study was to investigate the effect of a self-tapping blades and implant design on initial stability of two tapered implant systems in poor bone quality. The two different implant systems included one with self-tapping blades and one without self-tapping blades. D4 bone model using Solid Rigid Polyurethane Form was used to simulate poor bone densities. The insertion torque during implant placement was recorded. Resonance frequency Analysis (RFA), measured as the implant stability quotient (ISQ), was assessed immediately after insertion. Finally, the implant-bone specimen was transferred to an Universal Testing Machine to measure the axial pull-out force. Insertion torque values and maximum pull-out torque value of the non self-tapping implants were significantly higher than those in the self-tapping group (P = 0.008). No statistically differences were noted between the two implant designs in RFA. Within the each implant system, no correlation among insertion torque, maximum pull-out torque and RFA value could be determined. Higher insertion torque of the non-self-tapping implants appeared to confirm higher clinical initial stability. In conclusion, implants without self-tapping blades have higher initial stability than implants with self-tapping blades in poor bone quality.
Objective: The purpose of this study was to measure the insertion torque of orthodontic miniscrews regarding changes in their shape, diameter, and length. Methods: Torque values were measured during continuous insertion of the miniscrews into solid rigid polyurethane foam, using a torque tester of driving motor type with a regular speed of 3 rpm. Orthodontic miniscrews (Biomaterials Korea, Seoul, Korea) of cylindrical type and taper type were used. Results: Increasing the length and diameter of the miniscrews increased the maximum insertion torque value in both cylindrical and taper type screws. Insertion torque was increased at the incomplete head of the cylindrical type screw, and at the tapered part of the taper type screw. The insertion torque value of miniscrews was influenced most by diameter, then shape and length. As a result, it was shown that the diameter of the screw had the most influence on insertion torque, and the taper type screw had a higher torque value than the cylindrical type screw. Conclusion: Therefore, a large diameter or taper type screw are adequate for areas of thin cortical bone with a large interdental space, and a small diameter or cylindrical type screw are adequate in the mandibular molar area or the midpalatal area having thick cortical bone.
PURPOSE. This study aimed to evaluate the effect of implant thread depth on primary stability in low density bone. MATERIALS AND METHODS. The insertion torque was measured by inserting Ti implants with different thread depths into solid rigid polyurethane blocks (Sawbones) with three different bone densities ($0.16g/cm^3$, $0.24g/cm^3$, and $0.32g/cm^3$). The insertion torque value was evaluated with a surgical engine. The static compressive strength was measured with a universal testing machine (UTM) and the Ti implants were aligned at $30^{\circ}$ against the loading direction of the UTM. After the static compressive strength test, the Ti implants were analyzed with a Measurescope. RESULTS. The Ti implants with deeper thread depth showed statistically higher mean insertion torque values (P<.001). Groups A and group B had similar maximum static compressive strengths, as did groups C and D (P>.05). After the static compressive strength, the thread shape of the Ti implants with deeper thread depth did not show any breakage but did show deformation of the implant body and abutment. CONCLUSION. The implants with deeper thread depth had higher mean insertion torque values but not lower compressive strength. The deep threads had a mechanical stability. Implants with deeper thread depth may increase the primary stability in areas of poor quality bone without decreasing mechanical strength.

Objective: Small orthodontic mini-implants are useful as anchorage. However they have some weaknesses such as loosening. This study was carried out to analyze the mechanical effects of the dual pitch and diameter on the insertion and removal torque of mini-implants. Methods: The threads of mini-implants were mono and dual pitch. The diameters of mini-implants were 1.4 mm and 1.6 mm. Four groups were tested (mono 1.4 mm, mono 1.6 mm, dual 1.4 mm and dual 1.6 mm). All were inserted and removed on polyurethane foam with the torques being measured. Results: The maximum torque of the dual pitch groups was higher than the mono pitch groups during removal but lower during insertion. The maximum torque of the 1.6 mm diameter groups was higher than the 1.4 mm diameter groups during insertion and removal. The dual pitch 1.4 mm group showed the lowest insertion torque but had similar or superior levels of removal torque to that of the mono pitch 1.6 mm group. Conclusions: The dual pitch especially showed a continuous high removal torque after the peak. Despite the small diameter, the dual pitch might improve the initial mechanical stability.
