Evaluation of MR Safety of Breast Expander on 1.5T and 3.0T MRI (유방 조직 확장기의 1.5T와 3.0T MRI 환경 내 안정성 평가)

  • The purpose of this study is to evaluate the safety of the breast tissue expander implanted patients who require MRI examination. Torques were 0ml, 150 ml, 300 ml, 450 ml at 1.5 Tesla forward direction, 4, 3, 3, and 2 respectively, and 1.5 Tesla reverse direction at 4, 4, 4, 3 respectively. In the 3.0 T environment, 4 was shown in all conditions. In the overturning experiment, no overturning occurred in more than 300 ml in the 1.5Tesla environment, and most of the overturning occurred in the 3.0 Tesla environment. In terms of safety, MRI scans of patients with breast tissue expanders should be avoided at 3.0 Tesla and conditionally at 1.5 Tesla.

  • The magnetic valve of the breast tissue expander generates imaging artifacts during MRI examination, so MRI examination is limited. To evaluate the effect of imaging artifacts on the diagnosis area for patients with breast tissue expander who need MRI examination. Imaging artifacts were measured using self-made phantoms and actual clinical conditions. Imaging artifacts were measured differently depending on the environment of 1.5 Tesla and 3.0 Tesla, and the effects of imaging artifacts were less in the C-spine and L-spine tests. If MRI due to breast cancer metastasis is absolutely necessary, head & neck examination and L-spine can be examined mainly at 1.5 Tesla, but some sequences may cause distortion due to image artifacts. In terms of safety, MRI scans of patients with breast tissue expanders can be performed conditionally at 1.5T, avoiding 3.0T.

  • 1895년 독일 과학자 Wilhelm Roentgen 박사가 X-ray를 발견하고 미국의 Herman Carr가 MRI(Magnetic Resonance Imaging)의 기본 원리를 개발하여 발표한 이후 전자기장을 이용한 비침습적 의료영상 이미지 시스템은 놀라운 발전을 계속하여 왔다. 하지만, X-ray, CT(Computed Tomography), PET(Positron Emission Tomography)의 경우는 방사능의 사용과 조영제의 독성이라는 문제로 인하여 안전성에 대한 계속적인 문제를 야기해왔고 상대적으로 안전성이 입증된 MRI의 경우는 장비 자체 및 운용비가 일반화되기에는 고가여서 우리나라를 포함한 선진국에서도 아주 제한적으로 운영되고 있는 실정이다. 따라서, 세계적으로도 이러한 문제를 해결하기 위하여 안전하면서도 상대적으로 저렴한 비용으로 운용이 가능한 의료영상장비를 개발하기 위하여 많은 노력들이 경주되고 있어 관련 연구동향 및 산업화 동향을 소개하고자 한다.

  • Although ultrasonography is the most commonly used first-line imaging modality of the female pelvis because of diagnostic accuracy, low cost and safety, MRI is the best imaging modality of choice for the evaluation of the female pelvis. The indication of female pelvis MRI is diverse and includes the evaluation of M$\ddot{u}$llerian duct anomaly, differential diagnosis and characterization of uterine and ovarian tumors, and staging of malignant uterine and ovarian tumors. Understanding of MR protocols according to the specific gynecologic pathology allows accurate diagnosis and proper patient management.

The Feasibility for Whole-Night Sleep Brain Network Research Using Synchronous EEG-fMRI (수면 뇌파-기능자기공명영상 동기화 측정과 신호처리 기법을 통한 수면 단계별 뇌연결망 연구)

  • Objectives: Synchronous electroencephalogram (EEG) and functional magnetic resonance imaging (fMRI) has been used to explore sleep stage dependent functional brain networks. Despite a growing number of sleep studies using EEG-fMRI, few studies have conducted network analysis on whole night sleep due to difficulty in data acquisition, artifacts, and sleep management within the MRI scanner. Methods: In order to perform network analysis for whole night sleep, we proposed experimental procedures and data processing techniques for EEG-fMRI. We acquired 6-7 hours of EEG-fMRI data per participant and conducted signal processing to reduce artifacts in both EEG and fMRI. We then generated a functional brain atlas with 68 brain regions using independent component analysis of sleep fMRI data. Using this functional atlas, we constructed sleep level dependent functional brain networks. Results: When we evaluated functional connectivity distribution, sleep showed significantly reduced functional connectivity for the whole brain compared to that during wakefulness. REM sleep showed statistically different connectivity patterns compared to non-REM sleep in sleep-related subcortical brain circuits. Conclusion: This study suggests the feasibility of exploring functional brain networks using sleep EEG-fMRI for whole night sleep via appropriate experimental procedures and signal processing techniques for fMRI and EEG.

In Vitro Assessment of MRI Safety at 1.5 T and 3.0 T for Bone-Anchored Hearing Aid Implant (Bone-Anchored Hearing Aid Implant에 대한 1.5 T와 3.0 T에서 MRI 안전성의 생체외 평가)

  • The aim of this study was to evaluate Magnetic Resonance Imaging safety by measuring the translational attraction, torque and susceptibility artifact for Bone-Anchored Hearing Aid (BAHA) implant at 1.5 T and 3.0 T MRI by standard criteria. In vitro assessment tools were made of acrylic-resin by American Society for Testing and Materials (ASTM) F2052-06 and F2119-07 standard. Translational attraction of BAHA implant was measured by the maximum deflection angle at 96 cm position, where the magnetically induced deflection was the greatest. The torque was assessed by the qualitative criteria of evaluating the alignment and rotation pattern, when the BAHA implant was positioned on a line with $45^{\circ}$ intervals inside the circular container in the center of the bore. The susceptibility artifact images were obtained using the hanged test tool, which was filled with $CuSO_4$ solution. And then the artifact size was measured using Susceptibility A rtifact Measurement (SA M) software. In results, the translational attraction was 0 mm at both 1.5 T and 3.0 T and the torque was 0(no torque) at 1.5 T, and +1(mild torque) at 3.0 T. The size of susceptibility artifacts was between 13.20 mm and 38.91 mm. Therefore, The BAHA implant was safe for the patient in clinical MR environment.

Implicit Distinction of the Race Underlying the Perception of Faces by Event-Related fMRI (Event-related 기능적 MRI 영상을 통한 얼굴인식과정에서 수반되는 무의식적인 인종구별)

  • A few studies have shown that the function of fusiform face area is selectively involved in the perception of faces including a race difference. We investigated the neural substrates of the face-selective region called fusiform face area in the ventral occipital-temporal cortex and same-race memory superiority in the fusiform face area by the event-related fMRI. In our fMRI study, subjects (Oriental-Korean) performed the implicit distinction of the race while they consciously made familiar-judgments, regardless of whether they considered a face as Oriental-Korean or European-American. For race distinction as an implicit task, the fusiform face areas (FFA) and the right parahippocampal gyrus had a greater response to the presentation of Oriental-Korean faces than for the European-American faces, but in the conscious race distinction between Oriental-Korean and European-American faces, there was no significant difference observed in the FFA. These results suggest that different activation in the fusiform regions and right parahippocampal gyrus resulting from superiority of same-race memory could have implicitly taken place by the physiological processes of face recognition.

Speckle Reduction based on Neuro-Fuzzy Technique (뉴로-퍼지를 이용한 스펙클 제거)

  • Medical ultrasound has benefits in mobility and safety than any other medical techniques such as X-ray, CT and MRI but has speckle noise which decrease the ability of an observer to distinguish the fine details in diagnostic examination. But simple removing of speckle often causes losing boundary information. Then, in this paper, we presented a novel neuro-fuzzy method which could remove speckle efficiently without loss of boundary information. Proposed method consists of image clustering by fuzzy algorithm and image processingby neural networks which was learned by back propagation. From the experiments for simulation image and real ultrasound image, we could verify the proposed method.

Recent Perspectives on Oncoplastic Breast Surgery in Korea (우리나라의 종양성형학적 유방암 수술에 대한 최신 동향)

  • Oncoplastic surgery (OPS) combines oncologically safe tumor resection with aesthetically satisfying reconstruction of defects using established plastic surgery techniques. OPS is characterized by initial excision as extensive as is beneficial for oncological safety, and, once sufficient resection is complete, displacement or replacement techniques are selected based on remnant volume. The size of the lesion and the individual patient are important factors when considering the appropriate approach, and when pre-operative imaging, including MRI, is used to determine the technique, the complete removal of cancer cells by permanent pathology is essential. A frozen section is used during the operation to reduce the reoperation rate, but it is difficult to cover the entire margin surface theoretically and even harder in practice. A recent report about adequate margins has empowered OPS in its oncological safety. Considering the patients to whom each modality could be applied, basic breast volume is an important factor, and this is influenced by ethnic differences. In Europe or the US, for example, the average breast size is 36D (600 ㎤) and reduction mammoplasty is predominantly used. However, the average size of patients in our institution is 33A (300 ㎤), and so quite different approaches are selected in most cases. New techniques involving radiofrequency and fluorescence have been proposed as safe and easily accessible ways of reducing complications.

Morphometric Analysis of Distances between Sacral Hiatus and Conus Medullaris Using Magnetic Resonance Image in Korean Adult (자기공명영상을 이용한 한국 성인의 엉치뼈틈새와 척수원뿔 사이 거리 연구)

  • The purpose of this study was to provide basic biometric data on Korean adults through magnetic resonance imaging (MRI)-based measurements of the distances between the apex of sacral hiatus (SH) and the termination of dural sac (DS), and between SH and conus medullaris (CM) because they are critical to the performance of epidural neuroplasty. A total of 200 patients(88 males and 112 females) with back pain, who had no spine fracture, significant spinal deformity, and spondyloisthesis were selected for this study. The subjects were of mean age 54.3 (20~84) years and mean height 161.3 cm (135~187). T2-weighted MRI images were used for correlation analysis to evaluate the relationships between the distances, and variables such as sex and height. In all patients, the mean distance between SH and DS was $62.8{\pm}9.4mm$ and the mean distance between SH and CM was $232.2{\pm}21.8mm$. The minimum distance and the maximum distance between SH and DS were 34.8 mm and 93.9 mm respectively, and the minimum distance and the maximum distance between SH and CM were 155.0 mm and 284.0 mm respectively. In female patients, both the distances between the SH and DS, and between SH and CM were shorter when compared to those of the male patients(p<0.05). Both the distances between SH and DS and between SH and CM showed a significant correlation with height(p<0.01). The results of this study will provide a useful biometric data on the distances between SH and DS and between SH and CM in Korean in ensuring clinical safety and in the development of more effective catheterization techniques for epidural neuroplasty in Korean.
