고콜레스테롤혈증 치료 약물들에 대한 비용-효과 분석 (Cost-effectiveness Analysis of Pharmacologic Treatment in Hypercholesterolemia)

  • This paper was performed for a cost-effectiveness analysis of pharmacologic treatment of hypercholesterolemia. Agents modeled were cholestyramine, gemfibrozil. bezafibrate, lovastatin, pravastatin, simvastatin. Pharmacologic effectiveness was estimated by regression from reported clinical trials. Pharmacologic effects were expressed as the percent change of blood cholesterol level. Cost estimates included patients' travel expenses and time loss as well as resource consumption in the health care sector. Bezafibrate was the most efficient agent for reducing total cholesterol levels, having an cost over 1 year of ₩31.400 per percent reduction in total cholesterol. Simvastatin (10mg/d) was also efficient(₩33,100 per percent reduction). Chole styramine(8g/d) was least efficient at ₩90,200. For low-density lipoprotein cholesterol. simvastatin(10mg/d) was most efficient, at ₩23,200 per percent reduction, followed by lovastatin(20mg/d) at ₩28,000. Gemfibrozil was least efficient at ₩77,800 per percent reduction. For high-density lipoprotein cholesterol. bezafibrate(400mg/d) was most efficient at ₩39,300 per percent increase of high-density lipoprotein cholesterol. Cholestyramine was least efficient at ₩514,700. Analyses combining low-density lipoprotein cholesterol and high-density cholesterol effects suggest that bezafibrate(600mg/d) and simvastatin (10mg/d) were most efficient for reducing cardiovascular risk. The cost-effectiveness analysis results show that both simvastatin and bezafibrate could be efficient treatment. Simvastatin provide more effective treatment at higher cost, whereas bezafibrate is more cost-effective, as it may be less effective, at lower cost. Therefore, clinicians should choose reasonable treatment according to the patient's needs This pharmacoeconimc analysis will provide a guideline for efficient pharmacologic treatment and also be reference data for pricing new drugs.

지방의 종류가 다른 식이에 L-카르니틴을 첨가했을 때 흰쥐의 혈청과 간의 지질성분과 항산화계에 미치는 영향 (Effect of Dietary L-carnitine Supplementation on Serum and Liver Lipid Composition and Antioxidant Defense System in Rats fed with Different Types of Fat)

  • The aim of this study was to investigate the effects of L-carnitine on the components of serum and liver and the effects on the anti-oxidant system. For this purpose, five experimental groups were setup. For fat source, perilla oil enough with unsaturated fatty acid and beeftallow enough with saturated fatty acid were supplemented together with L-carnitine to the rats. Five experimental groups kept eight Sprague-Dawley rats respectively, They were co group supplemented with basic diet or AIN-93, PO group supplemented with perilla oil, POC group supplemented with perilla oil and L-carnitine, BT group supplemented with beeftallow, and BTC group supplemented with beeftallow and L- carnitine. The results are. 1) Weight gain, food intake and FER were not different significantly among the experimental groups. 2) Significant difference was observed in serum total lipid(P<0.05), serum triglyceride(P<0.05), serum total cholesterol (P<0.05)and serum LDL cholesterol(P<0.05). Serum total lipid and serum triglyceride were significantly low in the groups supplemented with L-carnitine. Serum total cholesterol showed difference with the supplementation of L-carnitine in BTC only. LDL cholesterol showed no significant difference with the supplementation of L-carnatine, but total values of LDL-cholesterol were high in groups supplemented with beeftallow. 3) Total cholesterol in liver was low in POC group with the supplementation of L-carnitine however, there was no difference in BTC group with the supplementation of L-carnitine. In summary, dietary L-carnitine did not influence the weight gain, food intake and food efficiency ratio among the experimental groups, but had an effect of lowering the serum total lipid and triglyceride significantly in both groups which were supplemented with L-carnitine. The effect of lowering of sew total cholesterol with the supplementation of L-carnitine in beeftallow group(BTC) only. The effect of lowering of liver total cholesterol with the supplementation of L-carnitine in perilla oil group(POC) only.

청각의 지질강하 및 항산화효과 (Effects of Cheunggak (Codium fragile) on lowering Lipid and Antioxidant)

  • 청각 추출액이 비만을 유도한 흰쥐의 지질강하 및 항산화효과에 미치는 영향을 검토했다. 각 처리군 별 유리지방산의 농도는 청각 처리군 모두가 대조군보다 낮은 값을 보였다. Triglyceride 량은 대조군과 비교하여 청각 처리군이 유의하게 낮은 값을 보였으며, 청각 첨가량이 증가함에 따라 하락했다. 혈액 내 total cholesterol 및 LDL-cholesterol농도는 청각 처리군 모두에서 감소하는 경향을 보였다. 그러나 HDL-cholesterol량은 대조군을 비롯하여 전 처리군에서 유의한 차이를 나타내지 않았다. 간장 내 total cholesterol과 triglyceride량은 청각 처리군 모두에서 하락하는 경향을 보여 정상군의 수준을 유지하였다. 혈장 및 간장의 TBARS 농도는 청각처리군 모두가 하락하는 경향을 보였으며, 정상군의 수준과 유사한 수치를 나타내었다. GSH-Px의 활성치는 청각 처리량이 증가함에 따라 증가하는 경향을 보여, 200mg/kg 처리군에서는 정상군의 활성치와 유의한 차이를 나타내지 않았다. 그러나 SOD 활성치는 청각처리에 관계없이 정상군을 비롯한 전 처리군에서 유의한 차이를 나타내지 않았다. 또한 CAT의 활성치는 청각 처리군에서 다소 증가하는 경향을 보였으나, 대조군과 유의한 차이를 나타내지는 않았다.

Efficacy of nobiletin in improving hypercholesterolemia and nonalcoholic fatty liver disease in high-cholesterol diet-fed mice

  • BACKGROUND/OBJECTIVES: Nobiletin (NOB), a citrus flavonoid, is reported to have beneficial effects on cardiovascular and metabolic health. However, there is limited research investigating the effect of long-term supplementation with low-dose NOB on high-cholesterol diet (HCD)-induced hypercholesterolemia and non-obese nonalcoholic fatty liver disease (NAFLD). Therefore, we investigated the influence of NOB on hypercholesterolemia and NAFLD in HCD-fed mice. SUBJECTS/METHODS: C57BL/6J mice were fed a normal diet (ND) or HCD (35 kcal% fat, 1.25% cholesterol, 0.5% cholic acid) with or without NOB (0.02%) for 20 weeks. RESULTS: HCD feeding markedly reduced the final body weight compared to ND feeding, with no apparent energy intake differences. NOB supplementation suppressed HCD-induced weight loss without altering energy intake. Moreover, NOB significantly decreased the total cholesterol (TC) levels and the low-density lipoprotein (LDL)/very-LDL-cholesterol to TC ratio, and increased the high-density lipoprotein-cholesterol/TC ratio in plasma, compared to those for HCD feeding alone. The plasma levels of inflammatory and atherosclerosis markers (C-reactive protein, oxidized LDL, interleukin [IL]-1β, IL-6, and plasminogen activator inhibitor-1) were significantly lower, whereas those of anti-atherogenic adiponectin and paraoxonase were higher in the NOB-supplemented group than in the HCD control group. Furthermore, NOB significantly decreased liver weight, hepatic cholesterol and triglyceride contents, and lipid droplet accumulation by inhibiting messenger RNA expression of hepatic genes and activity levels of cholesterol synthesis-, esterification-, and fatty acid synthesis-associated enzymes, concomitantly enhancing fatty acid oxidation-related gene expression and enzyme activities. Dietary NOB supplementation may protect against hypercholesterolemia and NAFLD via regulation of hepatic lipid metabolism in HCD-fed mice; these effects are associated with the amelioration of inflammation and reductions in the levels of atherosclerosis-associated cardiovascular markers. CONCLUSIONS: The present study suggests that NOB may serve as a potential therapeutic agent for the treatment of HCD-induced hypercholesterolemia and NAFLD.

복부 초음파 검사 기반 신장결석과 비만 및 대사증후군 관련성 분석 (Analysis of the Relationship between Abdominal Ultrasound Based Kidney Stones, Obesity and Metabolic Syndrome)

  • 신장결석은 국내에서 연간 3만 명 정도 유병률이 증가하고 있는 흔한 질환으로 만성콩팥병, 고혈압, 관상동맥질환, 대사증후군, 제2형 당뇨병, 고지혈증, 복부 중심 비만 증가와 관련이 깊다. 이에 본 연구에서는 2019년 5월부터 2020년 6월까지 달성군에 소재한 종합병원에서 건강검진을 위해 내원한 수검자 135명을 대상으로 복부초음파 검사에서 발견된 신장결석과 비만 및 대사증후군과 관련된 인자와의 관련성을 알아보고자 하였다. 연구결과 총 콜레스테롤 인자에서는 비정상 군이 정상 군에 비해 신장결석 발생 위험도가 4.255배, 저밀도 지단백 콜레스테롤 인자에서는 비정상 군이 정상 군에 비해 신장결석 발생 위험도가 2.072배 증가하였고, 총 콜레스테롤 인자와 저밀도 지단백 콜레스테롤 인자가 신장결석과 대사증후군 유병률에 영향을 주는 인자임을 알 수 있었다. 신장결석 발생 위험도는 총 콜레스테롤, 저밀도 지단백 콜레스테롤 인자와 관계가 있기에 건강검진을 통해 예방 목적에 있어 적극적인 관심을 가져야 할 것이다.

  • There have been growing interests in olive oil, which is attributed to the beneficial health effects of the Mediterranean diet. Numerous studies suggest that oleic acid, the major fatty acid in olive oil, may contribute to less incidence of coronary heart disease(CHD) in the Mediterranean area. Although the major effects of high monounsaturated fatty acid(MUFA) consumption on lowered plasma cholesterol are generally attributed to the consequential replacement of saturated fatty acid in diets, some studies have shown a modest cholesterol lowering effect of MUFA alone when it replaces dietary carbohydrate at an equal amount of energy. Furthermore, MUFA increases the level of the protective high-density lipoprotein cholesterol more than polyunsaturated fatty acid(PUFA) does when these two classes of fatty acids replace carbohydrates in the diet. It has been generally recommended that optimal dietary fat should be rich in MUFA, such as oleic acid, low in saturated fatty acids and provide adequate amounts of PUFA. However, no conclusive observations have been made yet mainly due to a difficulty in controlling the proportion of oleic acid without adjusting the levels of the other fatty acids to be constant, which could also affect plasma cholesterol levels. Recent findings also suggest that dietary PUFA results in higher tendency toward lipid peroxidation whereas with MUFA the susceptibility of low density lipoprotein to oxidative stress is reduced. More studies are needed to clarify the effects of MUFA on reducing the incidence of CHD.

감귤박, 다시마, 손바닥 선이장 분말을 함유한 식이의 급여가 고콜레스테롤혈증 흰쥐의 체내 지질수준과 장내 콜레스테롤 흡수, 혈소판 응집성 및 간 조직에 미치는 영향 (Hypocholesterolemic Effect of Tangerine Pulp, Sea Tangle or Prickly Pear Cactus on Lipid Level, Intestinal Cholesterol Absorption, Platelet Aggregation and Liver Tissue in Hypercholesterolemic Rats)

  • This study done to investigate the hypocholesterolemic effect of ow dietary fiber in a high cholesterol diet in aspect of cholesterol absorption and excretion using Sprague Dawley rats. After feeding diets containing 0.5% cholesterol and 5% tangerine pulp, sea tangle or prickly pear cactus for four weeks, we measured the levels of plasma and liver cholesterol and triglyceride, fecal neutral sterols and fecal radioactivity after ingestion 14(sup)C-cholesterol. We also examined platelet aggregation and histological change in liver tissues in association of hypercholesterolemia. The liver to body weight ratio was significantly(p<0.01) lower in rats fed prickly pear cactus than in other groups. The levels of plasma total cholesterol, LDL-cholesterol and triglyceride were decreased significantly(p<0.01) in the prickly pear cactus group compared with the control, while there was no difference in the liver levels of total cholesterol and triglyceride among groups. Fecal corprostanol and cholesterol were significantly(p<0.01) higher groups of control and prickly pear cactus compared to the other two groups. Radioisotope excretion after ingestion of 14(sup)C-cholesterol was higher in the control group than in tangerine pulp group or sea tangle group, with the highest in prickly pear group. Radioisotope excretion was the highest during the 2nd and 3rd days in all groups. Hematocrit and platelet aggregation were decreased in all fiber groups compared to the control, but not statistically different. Microscopic examination showed that cholesterol diet cause a fat accumulation in the liver and prickly pear cactus decreased the fat accumulation. Result indicates that prickly pear cactus has hypocholesterolemic effect by decreasing absorption and increasing excretion of cholesterol, thereby protective effect on fatty liver. Control group fed diet containing high cholesterol and low fiber seems to have a self control system in cholesterol absorption and excretion preventing hypercholesterolemia. 잣 성분의 혈중 콜레스테롤 저하효과 (Cholesterol-Lowering Effect of Pine Nut in Plasma of Rats)

  • 잣의 혈중 콜레스테롤 저하효과를 동물실험을 통하여 확인하기 위하여 기본식이(정상군), 기본식이에 콜레스테롤을 첨가한 식이(대조군), 그리고 기본식이에 콜레스테롤과 여러 농도의 잣을 함유한 실험식이(실험군)를 각 군별로 섭취하게 한 결과 잣의 농도에 따른 식이섭취량과 체중변화량의 유의적인 차이는 크게 나타나지 않았다. 식이효율은 저농도로 잣을 섭취한 경우에는 대조군과 차이를 나타내지 않았지만 10% 이상의 고농도의 잣을 섭취한 경우에는 식이효율이 높았다. 본 연구에 사용한 농노범위(1-20%)에서는 잣 섭취에 의해 혈중 콜레스테롤 함량과 동맥경화지수가 감소하는 효과가 나타나지 않았으며 이는 잣에 함유된 다량의 지방이 섭취됨으로써 잣이 지니는 콜레스테롤 저하효과가 감쇄하였기 때문으로 사료된다. 또한 잣 성분이 HMG-CoA reductase와 cholesterol esterase의 저해활성을 지니는지 확인하기 위하여 hexane과 물을 사용하여 잣 성분을 추출한 후 각 추출물에 대한 HMG-CoA reductase와 cholesterol esterase의 저해활성을 측정한 결과 잣의 hexane 추출물은 66%의 HMG-CoA reductase 저해활성을 지니고 있었으며 물 추출물은 저해활성을 나타내지 않았다. 한편 잣의 hexane 추출물과 물 추출물은 cholesterol esterase에 대한 저해활성은 지니고 있지 않았다. 결론적으로 혈중 콜레스테롤의 함량 저하효과는 가공하지 않은 잣의 섭취에 의해서는 기대하기 어려우므로, 향후 잣의 hexane 추출물 중의 유용성분이 혈중 콜레스테롤 함량 저하효과를 지니는지에 대한 동물실험을 수행할 가치가 있다고 판단된다.

Hypocholesterolemic effects of curcumin via up-regulation of cholesterol 7a-hydroxylase in rats fed a high fat diet

  • There is an increasing interest in curcumin (Curcuma longa L.) as a cardiovascular disease (CVD) protective agent via decreased blood total cholesterol and low-density lipoprotein-cholesterol (LDL-cholesterol) level. The aim of this study was to investigate further the potential mechanism in the hypocholesterolemic effect of curcumin by measuring cholesterol 7a-hydroxylase (CYP7A1), a rate limiting enzyme in the biosynthesis of bile acid from cholesterol, at the mRNA level. Male Sprague-Dawley rats were fed a 45% high fat diet or same diet supplemented with curcumin (0.1% wt/wt) for 8 weeks. The curcumin diet significantly decreased serum triglyceride (TG) by 27%, total cholesterol (TC) by 33.8%, and LDL-cholesterol by 56%, respectively as compared to control group. The curcumin-supplemented diet also significantly lowered the atherogenic index (AI) by 48% as compared to control group. Hepatic TG level was significantly reduced by 41% in rats fed with curcumin-supplemented diet in comparison with control group (P < 0.05). Conversely, the curcumin diet significantly increased fecal TG and TC. The curcumin diet up-regulated hepatic CYP7A1 mRNA level by 2.16-fold, compared to control group p (P < 0.05). These findings suggested that the increases in the CYP7A1 gene expression may partially account for the hypocholesterolemic effect of curcumin.

호박찜 추출물의 보충섭취가 여대생의 혈중 지질과 혈당에 미치는 영향 (Effects of Pumpkin Water Extract Supplement on Serum Lipid and Fasting Glucose Levels in Female Collegians)

  • To elucidate the effect of a 4 week dietary supplementation with pumpkin water extract on serum lipid and fasting glucose levels, 41 female collegians residing in the Gyeonggi area were recruited. The subjects were divided into three groups: placebo-control (PC, n=11), treatment A (TA, 100 ml/day, n=13), and treatment B (TB, 300 ml/day, n=17). Total serum cholesterol, high-density lipoprotein (HDL)-cholesterol, low-density lipoprotein (LDL)-cholesterol, triglyceride (TG), and fasting glucose were analyzed. The average age, height, weight, and body mass index of the subjects were 21.3 years, 161.1 cm, 53.0 kg, and 20.4 $kg/m^2$, respectively. The average total cholesterol, HDL-cholesterol, LDL-cholesterol, TG, and fasting glucose level (mg/dl) were 171.4${\pm}$26.1, 63.5${\pm}$11.4, 92.6${\pm}$22.9, 76.8${\pm}$32.0, and 89.1${\pm}$8.2, respectively. There were no significant differences among the three groups in any measured parameters, and between the pre- and post-study values of the all parameters. The results indicated that the 4 week pumpkin water extract supplementation (100 ml, 300 ml) did not influence the serum total cholesterol, HDL-cholesterol, LDL-cholesterol, TG and fasting glucose level in female collegians.
