Care Workers' Perceptions of Professionalism and Job Satisfaction (요양보호사의 전문성 인식과 직무만족에 관한 연구 -노인요양시설을 중심으로-)

  • This study aims to examine care workers' role perceptions of their occupation and job satisfaction with their work. They are core human resources in elderly care work practices and their perceptions of their occupation are likely to affect their work performance and job satisfaction. A mail survey is conducted targeting to 400 care workers who are at work in long-term care facilities located in Seoul, Gyeonggi and Pusan. Research findings are as follows: firstly, the perception level of care workers' professionalism is 3.38, and job satisfaction is 3.41. Secondly, the correlation analysis between perception level of professionalism and job satisfaction, coefficient is .553, which shows positive relations between two variables. Thirdly, the study shows that care workers' perceptions of professionalism significantly affects their job satisfaction. This study concludes that it is necessary to take a variety of measures in contribution to improve the quality of service through reinforcement of perception of professionalism higher than present level of care workers who supply direct service in long-term care facilities.

Effects of Multisensory Stimulation Using Familiarity: Persons with Dementia in Long-term Care Facility in Korea (익숙함을 이용한 다감각적 자극 프로그램의 효과: 시설 입소 치매 노인)

  • Purpose: The purpose of the study was to examine the effects of multisensory stimulation (MSS) using familiarity on persons with dementia (PWDs) residing in nursing homes in Korea. Methods: A nonequivalent control group with pre and posttests was used. Fifty one PWDs were included if they: 1) were over 65 yr old, 2) were diagnosed with dementia, 3) had no visual or speech impairments, 4) were able to communicate, and 5) had spent more than one month in a nursing home. The experimental group (n=25) received a 55 min MSS program twice a week for 10 weeks. The outcome variables included were cognition, activities of daily living, grip strength, depression, wandering, and aggressive behaviors. Repeated ANOVA was used for data analysis. Results: There were no significant differences in demographics or the main variables at pretest. Cognition, depression, wandering, and aggressive behaviors were significant over time between the two groups. Grip strength was only significant when accounting for interaction between group and time. Conclusion: An intervention of MSS using familiarity was marginally effective in improving cognition, depression, wandering, and aggression. Future study is suggested with a larger sample and longer treatment to retest the effects of MSS.

Care of the Elderly with Dementia and the Need for Group Homes from Middle and Upper Class Families in Korea

  • Concerns about paid-facilities for the elderly with dementia from middle and upper class families have been discussed lately in Korea. Small-scale, home-like group homes for the elderly with dementia were developed in Sweden in the 1980, but they have not been effectively introduced in Korea where they remain a comparatively new concept. The group home, however, would provide a good alternative to Koreans who feel guilty when they have to leave their frail parents in large-scale facilities or hospitals instead of caring for them at home. The aim of this paper is, first, to define the care of the demented elderly who come from upper and middle class families in Korea and, secondly, to discuss the need for group homes to help care for them. A survey was done by questionnaire and was answered by 577 respondents who belonged to middle and upper class families living in Korea in December 2000. Methods of analysis were frequency, mean, and Chi-square. The results of this study were as follows: 1) the traditional notion that the eldest son should be burdened with the primary responsibility for a demented parent has weakened remarkably; 2) a small-scale long-term care facility was considered the most desirable living environment for the demented elderly; 3) the facilities families needed for professional hospitals for victims of dementia, senior citizen centers, large and small-scale long-term care facilities, day-care centers, and short-stay and service housing. The study also revealed that there was low awareness of the group home. The most preferred type of structure for group homes was a single-family detached house, and the most preferred management system was small-scale that could create home-like atmosphere. Additionally the respondents wanted group homes to consist of 6-8 residents with 2-4 persons per room. The results of this study strongly suggest that policy makers should encourage the development of smallscale group homes as an alternative form of housing for the elderly with dementia.

A Study on Residence Experiences of Elderly Living in Long-Term Care Facilities (노인요양시설 노인의 시설 거주경험에 관한 연구)

  • This study was conducted to explore the experience and its meaning of the elderly residents in long-term care facilities(LTCFs). It is aimed to find the role of family and society for the qualitative care of elderly living in LTCF and to seek social welfare measures. For this purpose, qualitative research data was collected by conducting in-depth interviews and participation observations on 8 elderly residents at 4 LTCFs in Seoul, Incheon, and Gyeonggi Province. The collected data were analyzed using Giorgi's phenomenological research method. The analysis results show that 33 semantic units, 14 subcomponents, and 4 upper components. The 4 upper components showed up as 「Crumbling my existence」, 「The life thrown into an unfamiliar environment」, 「New relationship where discomfort and gratitude coexist」 and 「My life I want to find」. Among them, 「My life I want to find」 was the essence of the living experiences of elderly residents in LTCF. Through this study, it was suggested creating an environment to provide facilities access preparation program for the elderly, developing and activating various family participation programs, providing conditions of Aging in place for the elderly, and developing and activating community resource linkage programs.

The Effect of Functional Disability on Ego Integrity and Depression among the Elderly in the Residential Care Facility (장기요양시설 노인의 기능장애와 자아통합감, 우울과의 관계)

  • The purpose of this study is to investigate the association of functional disability, ego integrity and depressive symptoms, and analyze the mediating effect of ego integrity among the elderly, residing in the long-term care facilities. The data were collected by using non-random purposive and convenient sampling methods and 247 elderly who reside in the nursing home care, located in Daegu city and Kyungpook provinces participated in this study. Findings of this study indicates that there are statistically significant relationships among functional disability, ego integrity and depressive symptoms of participants. In addition, the mediating effect of ego integrity on the relationship between functional disability and depressive symptoms is statistically significant and shows partial mediating effect. Based on the findings of this study, possible practice implications for improving functional and mental health of the elderly in residential care home and recommendation for further study were provided.

A Qualitative Analysis on Familial Caregivers' Burden in Utilizing a Nursing Home for the Elderly (유료 노인전문요양원 이용 경험에 관한 질적 연구)

  • The principal objective of this study was to analyze and conceptualize the socio-psychological burden in utilizing a nursing home for elderly. The subjects were five elderly from a private nursing home located in Seoul and their familial caregivers. An old male and three females were currently staying at the facility, and a female had been discharged already from there. Data were collected through depth interviews, observations and review of records at the facility For analysis, the data were classified by similar contents among significant expressions and factors in common. The subjects' motives to consider admission to the nursing home might be attributed to familial caregivers' burden, a shortage of support, environmental improvement and feeling of helplessness for the case elderly. The concept of burden is including family members' being badly off in living, their weariness, complications among family members, feeling psychological uneasiness, and hospital expenses. The identified image of nursing homes for the elderly in Korea was generally negative at the point of high cost, unreasonable requisites and limitations for admission to the facilities, inferior situations, and especially in that there were few long-term care facilities within the community boundary. From their experience of nursing homes, the interviewees have felt the sentiments of sorry for their old parents, with the thought of being an undutiful, bitterness, and empathy. Additionally, they expressed a sense of anxiety of relative deprivation against the fact that there were no long-term care facilities available for the middle class. On the basis of these, multi-dimensional needs could be identified for the elderly with chronic illnesses.

A Study on Designation and Management of Seoul-Type Elderly Care Facilities (서울형 노인요양시설 지정 및 운영을 위한 정책 연구)

  • Purpose: The purpose of this study to assess long-term care facilities in Seoul and certificate Seoul-type elderly care facilities. After certification, Seoul city can provide certificated facilities with budget and support to improve service quality. Methods: Questionnaires to elderly care facilities experts and field surveys to facilities in Seoul have been conducted for the data collection. Results: The result of this study can be summarized into three points. The first one is that a direction of operation for Seoul-type elderly care facilities is to improve manpower and provide special program for enforcing quality assurance. The second one is that this study provide index of evaluation for Seoul-type elderly care facilities. The indexes include management, facility environment and safety, service delivery process, and service results. The third one is that management plan for this policy should be carried out. The management plan include demonstration project, opinion research. Implications: Seoul-type elderly care facilities can have positive effects on quality of service. For the introduction of this policy, Seoul city and autonomous Gu have to make careful preparations for this policy.

Effect of Professional Oral Healthcare Program on the Oral Status of Elderly Residents in Long-Term Care Facilities (전문가 구강위생관리가 장기요양시설 재원노인의 구강상태에 미치는 효과)

  • This study was conducted to investigate the effects of professional oral healthcare program in eldery residents long-term care facilities. Ninety-four elderly residents from 5 different facilities in Asan participated in this study. The subjects were divided into 3 groups, with 32 in experimental group I, 30 in experimental group II, and 32 in the control group. Subjects in experimental group I were treated by a dental hygienist with professional oral healthcare and received daily oral care from caregivers who had completed elderly oral healthcare course. Subjects in experimental group II received daily oral care from caregivers who had completed elderly oral healthcare course. Control group received daily oral care from caregivers. These 3 groups were tested for dental plaque, halitosis, tongue coating, and salivary flow at baseline, and after 4 and 12 weeks of treatment. There were significant differences in the dental plaque index between the groups, mediate times, and mediate methods, in halitosis between the mediate methods, and in tongue coating between the mediate times, as well as the mediate methods. Finally, salivary flow was significantly different with regard to mediate methods between experimental group II and the control group. Therefore, to improve the oral health condition of elderly residents, involvement of a part-time dentist and scheduled professional oral healthcare are necessary. Furthermore, elderly oral healthcare education for nursing staff, including managers, should be provided.

  • Recently, South Korea is reaching aged society through ageing society. Accordingly, facility related to aged is increased consistently because of effort to improve the welfare of the elderly through elderly long term care insurance in 2008. however, the number of fires in the elderly facilities is increasing rapidly too. It is becoming bigger and higher and the characteristics of person who is unable to afford are mostly having difficult evacuation. so fire safety guildelines are urgently needed but korea research is insufficient. In this study, we propose a fire scenario through investigation of fire causes for fire safety guideline development of elderly facility, and describe to the fire safety countermeasure.

Architectural Improvement plans in a quarantine system for Senior facilities (노인요양시설 방역을 위한 시설 개선 방안)

  • Purpose: Senior long-term care facilities are vulnerable to a cluster infection because of frequent physical contact, large group dining, communal living, and room sharing. This study aims to provide architectural improvement plans for a quarantine system in the facilities. Methods: Actual quarantine action data, guidelines from governments and institutes, in-depth interviews with facility staff are analyzed. Results: To prevent a cluster infection in the senior facilities, it is necessary to provide an architectural plan focusing on increasing the number of single rooms and isolation rooms, providing isolation rooms for staff, separation of a soiled room from a clean room, planing an entry vestibule and a visitor's room. Implications: It is important to analyze the existing condition of facilities that had been going through cohort isolation and provide the architectural solution to strengthen infection control.
