A Study on the Electromyographic Activity of Masticatory Muscles, Muscle Tenderness and Occlusal Contacts in Patients with Temporomandibular Disorders (측두하악장애환자에서 근활성, 근압통 및 교합접촉에 관한 연구)

  • The purpose of this study was to investigate the peak electromyographic activity(EMG) and time intervals in firing sequence of Anterior temporalis(TA) and Masseter(MM) on clenching, the number of tender points in the head and neck muscles, the occlusal contact state on clenching, and their relationship. 78 patients with Temporomandibular Disorders(TMDs) participated and were classified into articular or muscular group by clinical signs and symptoms. BioEMG$\textregistered$ of integrated masticatory function analyzer, Biopak$\textregistered$ system(Bioresearch Inc., USA), was used to measure EMG and related items, and T- Scan$\textregistered$(Tekscan, USA), computerized occlusal analyser, was used to record occlusal contact state on maximum voluntary clenching. EMG and occlusal contact were synchronously recorded and analysed with SAS Statistical program. The results of this study were as follows : 1. In total subjects, EMG of TA was lower than that of MM in articular group but in muscular group, vice versa, As a result, the ratio TA to MM was significantly different between the two groups. no significant differece. 3. The number and force of occlusal contact were more in articular group, but there was no significant difference in Total Left-Right statistics(TLR) between the two groups. 4. In unilaterally affected subjects for muscle function, no significant difference was observed between the affected side and the contralateral side in articular group, but in muscular group, the item of the number of tender points showed significant difference. However, for occlusal contact items, contact force in articular group shows significant difference between the two sides. 5. Rate of coincidence of the first firing side of TA with affected side or preferred chewing side was higher and not different between the two groups, but no significant correlation was showed between the first firing side and the first occlusal contact side.

  • Purpose: Finger injury by green onion cutting machine is one of the common hand injuries in the kitchen. It has a unique feature: there are multiple parellel laceration 3 - 5 mm wide. There are two directions of injuries(vertical, oblique). It may involve bone, tendon, nerve, and vessel injuries. We discuss its management and the long - term progress. Methods: We have treated six patients from 2003 to 2007. We carried out low tension approximation with thin suture materials to avoid ischemia and performed the additional operation as nail bed repair, tenorrhaphy, open reduction, vessel anastomosis, and composite graft. We reviewed the record of initial injury and collected the follow - up record. Results: They were all middle aged - women who had worked in the kitchen. Right hand was dominent over left hand. The ratio of the directions was 3 : 3 (vertical : oblique). They were all competely healed although there were three atrophy, four hyperesthesia, and one nail deformity. Conclusion: Finger injury by green onion cutting machine is a unique pattern of laceration with various accompanied injuries. It may look like a severe form of injury, but in most cases have relatively favorable progress. We have to perform careful examination of accompanied injuries and carry out the proper management. First and foremost, the user especially in the middle aged women should be warned to be careful in handling this risky machine.

  • The purpose of this study was to investigate the effects of immediate non-functional loading by analyzing histomorphology around the implant tissues in dogs. Five eight- to nine-month-old full-grown dogs weighing around 12 kg were used in the study. Group I (control group) comprised those in which delayed loading was applied to the right side of the mandible, and Group II (experimental group) consisted of dogs in which immediate loading was performed on the left side of the mandible. Resorbable blast media (RBM)-treated double-threaded US III implants measuring 3.5 mm in diameter and 11 mm long were used in the study. Each animal received four implants in each group, for a total of 40 implants. Cemented type abutments were used after implantation. An 8-week period was allowed for bone healing and an abutment was placed after exposing the periosteum for loading. An implant sample was obtained from bone blocks taken when the dogs were killed at 16 weeks after loading. A Mann-Whitney U-test was performed to evaluate statistical significance. Student's t-test was used for the histological evaluation. The bone formation ratio in Groups 1 and 2 was 88.23 and 86.41%, respectively. No significant difference in new bone formation was observed in the two groups. As no significant difference was seen in new bone formation between the delayed and immediate loading groups, early loading might be possible after implant placement.

Comparison between dental and basal arch forms in normal occlusion and Class III malocclusions utilizing cone-beam computed tomography

  • Objective: The purpose of this study was to investigate the relationship between the mandibular dental and basal arch forms in subjects with normal occlusion and compare them with those of Class III malocclusion using cone-beam computed tomography (CBCT). Methods: CBCT images of 32 normal occlusion (19 males, 13 females; 24.3 years) and 33 Class III malocclusion subjects (20 males, 13 females, 22.2 years) were selected. Facial axis and root center points were identified from the left to right mandibular first molars. Distances between the facial axis and root center points for each tooth were calculated, and 4 linear and 2 ratio variables were measured and calculated for each arch form. The variables were compared between groups by independent t-test. Pearson correlation coefficient was applied to assess the relationships between dental and basal variables within each group. Results: The mandibular dental and basal intercanine widths were significantly greater in the Class III group than in normal occlusion subjects (p < 0.05). The dental and basal intercanine widths as well as the dental and basal intermolar widths were strongly correlated in normal occlusion and moderately correlated in Class III malocclusion. Conclusions: The dental arch form demon strated a strong positive correlation with the basal arch form in the normal occlusion group and moderate correlation in the Class III malocclusion group. These results might be helpful for clinicians to have a better understanding of the importance of basal arch form in the alveolar bone.

The Effect of Aqua Exercise on Female Patients with Knee Degenerative Arthritis (수중운동이 무릎퇴행성관절염 여성 환자에 미치는 영향)

  • The purpose of this study was to investigate the effect of aqua exercise on women 50 years of age or older with knee degenerative arthritis, and then examining the therapeutic value of aqua exercise in degenerative arthritis of the knee. The subjects of the study were 115 residents 50 years of age or older who had been diagnosed as degenerative arthritis of the knee by doctors from medical institutions and applied for the aqua exercise program in the Public Health Care Center in Suseong-gu in 2006 and 2007. Experiments were conducted to all the subjects with the application of the 8-week aqua exercise program (2 times a week, 50 minutes a time), designed in the study, and the differences before and after the aqua exercise were compared through questionnaires for measuring the state of the body such as the knee extension angles, weight, body fat, and abdominal obesity. The data was analyzed by t-test and the Wilcoxon method through SPSS 14.0 and the statistical significance level ${\alpha}$ was set at 0.05 and 0.01. The results of the study are as the following. First, In comparison of before and after the aqua exercise, there was improvement in the extension angle of the right knee(p<0.01), while there was some improvement in the extension angle of the left knee, it was not significant. Second, In comparison of before and after the aqua exercise there were statistically significant differences in weight(p<0.05), body fat ratio, and abdominal obesity(p<0.01). The results of this study showed that the aqua exercise program to female patients with knee degenerative arthritis significantly increased the extension angle of the knee. Therefore it is thought that aqua exercise is an effective meditating method to improve their symptoms of female patients 50 years of age or older

Clinical Investigation of Surgical Spontaneous Pneumothorax (외과적 자연기흉의 임상적 고찰)

  • A clinical investigation was reported on 17 cases of spontaneous pneumothorax requiring surgical mana-gement. Males outnumbered females 15:2. Determination of the etiology in this series showed that the majority were pulmonary tuberculosis and paragonimiasis. Several others had pneumonia, lung abscess, cyst and blebs. It is of particular interest that the acute inflammation of respiratory system was younger age group, pulmonary tuberculosis & paragonimiasis were between 2 nd and 3 rd decades, and lung abscess, cyst, blebs were above 4 th decade. Pulmonary tuberculosis was far advanced bilateral and active. The ratio of right to left side was 13:6 and both side involved in 2 cases. In about half cases of patients, above 50%-collapsed lung associated with mediastinal shifting developed. The complications were pleural effusion and bronchopleural fistula. The former was 13 cases [76.4%] in which 3 cases combined with mixed infection, and latter was 5 cases. As the management, 11 cases were subjected to intercostal or rib resection drainage with continuous suc-tion. Among 11 drainage cases, 8 cases were successful in acute stage and 3 cases failed in chronic stage. This faiure was due to interference with re-expansion of collapsed lung for peel formation and broncho-pleural fistula. The open thoractomy was applied in 9 cases, among which primary operation were 5 cases and drainage failure were 4 cases. Among 11 cases subjected to the open thoracotomy, wedged resection was performed in 3 cases including paragonimiatic cyst, and pneumonectomy in 1 case-tuberculosis, and decortication only was performed in 2 cases in paragonimiasis. Decortication & lung resection was carried out in 2 patients among which ruptured lung abscess 1 case and ruptured multiple blebs 1 case. There was no case of death but prognosis of the tuberculosis may be poor because of far advanced bilateral and active pulmonary tuberculosis.

A Qualitative Analysis on Paraspinal Muscles in Patients with Acute Low Back Pain and Chronic Low Back Pain (급성 요통환자와 만성 요통환자에서 척추주변근육의 정량적 분석)

  • This study was conducted to measure and analyze the changes in paraspinal muscles of acute and chronic low back pain patients using MRI, and to provide clinical basic data for diagnosis and treatment for low back pain. For this purpose, 20 patients with acute low back pain frome August 2012 to January 2013 which occurred within 12 weeks, and 20 patients with chronic low back pain that progressed over 12 weeks, were chosen as subjects, and their MRI measurements were compared with one another. As a result, in relation to in the fatty degeneration ratio of the left spine and right spine, there were significant differences in erector spinae and multifidus(p<.001), and in relation to the Fat Infiltration ratio between all the groups, there were significant differences in psoas major, erector spinae and multifidus between the acute low back pain patient group and the chronic low back pain patient group(p<.001). In the post-hoc test, multifidus and erector spinae in the acute low back pain group and chronic low back pain group showed the highest Fat Infiltration ratio. The serious Fat Infiltration of multifidus and erector spinae in the chronic low back pain group led to weakened strength of muscles that stabilize the spine. In conclusion, it is considered that this study would present important data and basis in making acute and chronic low back pain patients pay more attention to multifidus and psoas major during rehabilitation exercise, and selecting a rehabilitation exercise program.

The Design and Evaluation of a Diagonally Splitted Column to Improve Text Readability on a Small Screen (소형 스크린 상에서의 텍스트 가독성 향상을 위한 대각분할 칼럼 디자인과 평가)

  • Nowadays, reading text from screens is prevailing in everyday life. The advent of mobile information devices such as a cellular phone, PDA, and e-book reader facilitates us to enjoy various text-based contents any time and anywhere. Most studies comparing screen and paper readability show that screens are less readable than paper. Furthermore, the decrease of line length and number of lines that can be displayed on the screen of mobile information devices deteriorate text readability. This study investigated parameters affecting text readability on small screens and designed a new text layout to improve readability. We suggested a diagonally splitted layout of rectangular column, which is supposed to facilitate eye movement to trace text flow with ease. The experiment comparing readability between a traditional rectangular column and a diagonally splitted column was conducted. The result of experiment revealed that there is no significant difference between the two text layouts in terms of subjective satisfaction of reading task and a level of comprehension. However, in the screen size of $4000mm^2\;and\;8000mm^2$, reading speed was increased 18.9% and 34.0% respectively from a traditional rectangular column to a diagonally splitted column. We conducted a consecutive experiment to scrutinize the cause that improved the performance in readability task remarkably. The readability of text in a traditional rectangular column was compared with a left triangular column and a right triangular column in the condition of $4000mm^2/3:1$ ratio screen. The performance measurements revealed that participants read 21.1% and 67.6% faster respectively with the left triangular column and right triangular column than with the rectangular column. In consequence, the improvement of readability in the diagonally splitted column was attributed mainly to the increase of reading speed in the right triangular column. This research verified that the diagonally splitted column improve text readability on a small screen and this result is expected to make a contribution to designing an efficient text layout for mobile information devices

Biomechanical Comparative Analysis of Two Goal-kick Motion in Soccer (두 가지 축구 골킥 동작의 운동역학적 비교 분석)

  • The purpose of this study is to reveal the effects of two different kicks, the drop kick and the punt kick, into the kicking motion, through the kinetic comparative analysis of the kicking motion, which is conducted when one kicks a soccer goal. To grasp kinetic changing factors, which is performed by individual's each body segment, I connected kicking motions, which were analyzed by a two dimension co-ordination, into the personal computer to concrete the digits of it and smoothed by 10Hz. Using the smoothed data, I found a needed kinematical data by inputting an analytical program into the computer. The result of comparative analysis of two kicking motions can be summarized as below. 1. There was not a big difference between the time of the loading phase and the time of the swing phase, which can affect the exact impact and the angle of balls aviation direction. 2. The two kicks were not affected the timing and the velocity of the kicking leg's segment. 3. In the goal kick motion, the maximum velocity timing of the kicking leg's lower segment showed the following orders: the thigh(-0.06sec), the lower leg(-0.05sec), the foot(-0.018sec) in the drop kick, and the thigh(-0.06sec), the lower leg(-0.05sec), the foot(-0.015sec) in the punt kick. It showed that whipping motion increases the velocity of the foot at the time of impact. 4. At the time of impact, there was not a significant difference in the supporting leg's knee and ankle. When one does the punt kick, the subject spreads out his hip joint more at the time of impact. 5. When the impact performed, kicking leg's every segment was similar. Because the height of the ball is higher in the punt kick than in the drop kick, the subject has to stretch the knees more when he kicks a ball, so there is a significant affect on the angle and the distance of the ball's flying. 6. When one performs the drop kick, the stride is 0.02m shorter than the punt kick, and the ratio of height of the drop kick is 0.05 smaller than the punt kick. This difference greatly affects the center of the ball, the supporting leg's location, and the location of the center of gravity with the center of the ball at the time of impact. 7. Right before the moment of the impact, the center of gravity was located from the center of the ball, the height of the drop kick was 0.67m ratio of height was 0.37, and the height of the punt kick was 0.65m ratio of height was 0.36. The drop kick was located more to the back 0.21m ratio of height was 0.12, the punt kick was located more to the back 0.28m ratio of height was 0.16. 8. There was not a significant difference in the absolute angle of incidence and the maximum distance, but the absolute velocity of incidence showed a significant difference. This difference is caused from that whether players have the time to perform of not; the drop kick is used when the players have time to perform, and punt kick is used when the players launch a shifting attack. 9. The surface reaction force of the supporting leg had some relation with the approaching angle. Vertical reaction force (Fz) showed some differences in the two movements(p<0.05). The maximum force of the right and left surface reaction force (Fx) didn't have much differences (p<0.05), but it showed the tendency that the maximum force occurs before the peak force of the front and back surface (Fy) occurs.

The Study on the Accident Injury Severity Using Ordered Probit Model (순서형 프로빗 모형을 이용한 사고심각도 분석)

  • In recent years, the rapid growth of vehicles have increased traffic crashes. Since they can cause the economic losses and have put the life qualify in danger, there should be numerous efforts to reduce traffic crashes. To reduce traffic crashes, this research seeks to improve the safety of intersections by analysing causations of injury severity with Ordered Probability Model. This research applied the Ordered Probit Model, which assumes that ${\epsilon}_i$(random error) is normally distributed, for model calibration and used $p^2$ (likelihood ratio) and $x^2$ (Chi-square) for model selection. The results show that minor road traffic, heavy vehicle rates, major and minor right-turn rates, presence of lightings, speed limits, instructive line for left-turn traffic are significant factors affecting crash severities at signalized intersections.
