국산과 중국산 현삼의 성분함량 (Quantitative Analysis Comparison of Korea and China Scrophulariae Radix)

  • Scrophulariae Radix(Scrophulariaceae) is a perennial growing in all parts of korea, China and Asia. It has been used as a folk medicine in treating guttural diseases acting as an antiphlogistics, antipyretics and analgesics. In this report, to compare with Korea and China product, quantitative analysis of harpagoside and p-methoxy cinnamic acid(4-MCA) was performed by HPLC method. Using a Luna $C_{18}(2)$ column, we carried out quantitative analysis with 0.1% phosphoric acid in water and acetonitrile as gradient condition. Harpagoside and 4-MCA were detected at retention time of about 11.5 and 15.0 min, respectively. Contents of harpagoside and 4-MCA proved that Korean products were higher than China more than twice.

  • Objective The analysis method based on the herbal formula's effects is a general tool, in traditional medicine. In effective applications of herbal formula, Korean herbal medicines traditionally used the classification methods based on the curative effects through the various compositions of herbal formulas. However' the effects of herbal formulas were not filed systemically in ancient literatures, and the standards to confirm their effects are not clear. Thus, it is not easy to classify herbal formulas according to the curative effects. Furthermore, there are no standards to estimate the effects of prescriptions frequently directed in clinic. In this study, we aimed to provide the methodology of classifying the curative effects of herbal formulas by calculating the combination of the effect of each compositional herb through the DB systems. Results : By comparing effects of herbal formula with those of compositional herbs, we found that about 25-50% of the herbal effects were included in herbal formula's effects. These results showed that the prospective estimation of herbal formula's effects may be possible through the DB systems filing herbal effects. To enhance the accuracy in explaining the herbal formula's effect, more studies are needed by giving prominence to major effects and by subtracting minor effects.

한약 치료를 시행한 재발성 아프타성 구내염 환자의 임상소견에 관한 추적 관찰 연구 (A Follow-up Study of Clinical Characteristics of Recurrent Aphthous Stomatitis Patients Treated with Herbal Medicine)

  • Objectives: The purpose of this study was to report the long-term efficacy of herbal medicine for treatment of patients with recurrent aphthous stomatitis (RAS). Methods: We reviewed the clinical records of 17 patients with RAS who visited the Oral Diseases Clinic of Kyung Hee University Korean Medicine Hospital from September 1st, 2014 to August 29th, 2017. The patients were treated with herbal medicine for more than 3 consecutive weeks, followed by a telephone survey. We evaluated the clinical characteristics of RAS, and compared the short-term and long-term efficacy of herbal medicine treatment. Results: After treatment with herbal medicine, the clinical characteristics improved in the 17 patients with RAS, especially in terms of the duration and interval of recurrence. Prolonged effects were reported upon the follow-up telephone survey. Conclusion: Herbal medicine treatment may have a long-term effect in patients with RAS.

마우스에서 방풍통성산의 급성 독성 연구 (A Study on the Acute Toxicity of Bangpungtongsung-san(Fangfengtongsheng-San) in Mice)

  • Objectives Bangpungtongsung-san(Fangfengtongsheng-san) has been traditionally prescribed a a restorative medicine. Methods In this study, we investigated the acute toxicity of water-extracted Bangpungtongsung-san(Fangfengtongsheng-san). 20 male and 20 female mice were orally treated of Bangpungtongsung-san(Fangfengtongsheng-san) at the respective doses of 0 (control group), 1250, 2500 and 5000mg/kg for 14days. Results We observed survival rates, general toxicity, change of body weight and examined microscopic changes of some organs. Conclusions Compared with the control group, we could no find any toxic alteration in all treated groups (1250, 2500 and 500mg/kg). $LD_{50}$ of Bangpungtongsung-san(Fangfengtongsheng-san) might be over 5000mg/kg and it was very safe to ICR mice.

KHP 기준에 의한 보험용 단미엑스산제의 지표성분함량 확인연구 (A quantitative analysis of marker compounds in single herb extracts by the standard of KHP)

  • Objectives : Since single extract powders was released at 1987, the insurance fee has not been changed, but the price of raw material has been increasing. According to this, Pharmaceutical company couldn't invest on quality of the product, so, the quality of single extracts went down and lost the consumer's trust. We checked the contents of marker ingredients in single extract thereby to recover the reliability of insurance-covered herbal preparations. Methods : we bought total twelve products of eight different single extracts of two main pharmaceutical companies among total 65 kinds of single extract powder stipulated in KHP (The Korean Herbal Pharmacopoeia) monograph III at Jan. 2013. Assays of selected single extract powders are performed by KHP regulation. And we surveyed price fluctuation of relevant herb raw materials from 2005 to 2012. Results : Among twelve products, eight single extract powders were suitable by the KHP regulation. But four products didn't reach the content amount of KHP. Marker contents in the single extracts product of Pueraria Root, Licorice, Peony root and Scutellaria Root of A company were 70%, 1%, 23.7% and 75.1%, respectively. Conclusions : We can acertain whether there's a quality problem in the insurance-covered single extract powders. But, A company is no longer producing these improper single extract powders. As a medicine, single extract powders needs to be strictly quality controlled by the company, and regularly monitored by the KFDA.

문헌 고찰을 통한 비만 치료 한약의 장내미생물 변화 연구 (A Study on Changes in the Gut Microbiome of Herbal Medicines for Treating Obesity Using Literature Review)

  • Objective This study identified useful herbal medicines and prescriptions for obesity treatment by analyzing herbal medicines and prescriptions that showed meaningful results in weight loss by changing the gut microbiota. Methods Using PubMed, we selected and studied 23 papers showing meaningful results on weight loss through changes in the gut microbiota using herbal medicines. Result Of the 23 selected papers, 17 were of studies using herbal medicines, and 6 of studies using prescriptions. Pomegranate peel, Flos Lonicera, Rehmannia glutinosa, Rhein, Coix seed, Platycodon grandiflorus, mulberry leaves, Poria cocos, asperuloside, Bupleuri radix, Astragalus polysaccharides, Ephedra sinica, Ganoderma lucidum, Hirsutella sinensis, Caulis spatholobi, aconite, and Bletilla striata were used as herbal medicine. Linggui zhugan-tang, Bofutsushosan, Shenling baizhu powder, Chowiseungcheng-tang, Daesiho-tang, and Yijin-tang were used as prescription. Conclusion Seventeen herbal medicines and six prescriptions associated with meaningful results in weight loss through changes in the gut microbiota, suggest the possibility of treatment and prevention of obesity through herbal medicine.

훈증요법을 병행한 말초성 안면신경마비 환자에 대한 임상적 고찰 (The Clinical Study for Peripheral Facial Paralysis Treated with Herbal Stream Therapy)

  • Objective : This study was performed to investigate the effect of herbal steam therapy on peripheral facial paralysis. Method : 33 patients with peripheral facial paralysis were treated with Korean Medicine therapy including herbal steam therapy. We evaluated the effect of herbal steam therapy by Yanagihara grading system and satisfaction measurement. Results : 1. 33 patients were treated with korean medicine therapy and $27.85{\pm}16.44$ times of herbal steam therapy. 2. After treatment, Y-system score was significantly increased(p-value<0.001) from $18.45{\pm}6.5196$ to $35.42{\pm}5.2501$. 3. 87.88% of patients was satisfied with herbal steam therapy. Conclusions : Herbal stream therapy seem to be effective to improve symptoms of peripheral facial paralysis. Further studies will be needed to identify the beneficial of herbal steam therapy on peripheral facial paralysis.

A Survey on the Status of Employees of Traditional Korean Medicine Hospitals

  • Objectives: To undertake manpower-related improvements based on a comparison between specialists in the traditional Korean medicine hospitals(TKMH) and their counterparts in Western medicine Methods: A survey of the TKMH based on questionnaire sheets dispatched to them by mail(57 of 142 responded) in the June December, 2008 period, and on almanac statistics provided by the Ministry for Health, Welfare and Family Affairs of Korean Government. Results: Overall, the workforce engaged in the traditional Korean medicine hospitals comprises traditional Korean medical doctors(28%), nurses(23%), administrative staffs(19%), assistant nurses(9%), medical record keepers(2%), nutritionists(2%), herbal pharmacists(1%), and others(16%). Each hospital has 16.5 traditional Korean medical doctors on average, which can be broken down into 6.2 specialists, 1.3 generalists, and 9.3 residents/interns. Only 10.7% of whole of traditional Korean medical doctors work in the hospitals, compared to 54.5% of Western medicine doctors. The ratio of traditional Korean medical doctors to the entirety of employees in the TKMH is 2.5 times higher than their Western medicine counterparts, while the ratio of medical technicians to the entire employees in the TKMH is 20 times lower than in the Western medicine counterparts. Conclusions: To provide more qualified medical service in the TKMH, they will be required to increase the proportion of non medical doctor employees, like Western medicine counterparts.

Sweet BV 약침을 이용한 견봉하 점액낭염을 동반한 어깨충돌증후군 환자 치험 1례 (Case Study of shoulder Impingement Syndrome with Subacromial bursitis Improved as Sweet BV herbal acupuncture)

  • Objectives : The purpose of this study is to report the improvement after Sweet BV herbal acupuncture therapy of shoulder impingement syndrome with Subacromial bursitis. Methods : We treated a patient having shoulder pain due to shoulder impingement syndrome with Sweet BV herbal acupuncture therapy. We checked visual analogue scale (VAS) score and range of movement (ROM). Results : We treated shoulder pain. Thereafter ROM improved and VAS score dropped to the level 3 points from 10 points at onset. Conclusions : This report shows Sweet BV herbal acupuncture therapy has effectiveness on shoulder impingement syndrome with Subacromial bursitis.

소아청소년 환자의 한약 치료 동향 - 녹용을 중심으로 - (A Study on the Trends of Korean Medicine in Children and Adolescents - Focused on Deer Antler -)

  • Objectives This study aimed to provide a basis for the research on the trend and safety of Korean herbal medicines among children and adolescents. Methods Patients aged <19 years who received outpatient treatment at the Korean pediatrics department of Kyung Hee University Hospital from January 1, 2017 to October 31, 2022 were included. The current state of annual visits and herbal medicine prescriptions was analyzed. A retrospective chart review was conducted by examining the medical records of subjects who received herbal medicines, including deer antler. Adverse drug reactions in patients who received herbal medicine were also investigated. Results A total of 57,457 people visited the Korean pediatrics department, and 51,768 people (90.1%) were prescribed herbal medicine. Among these, 21,826 patients (42.2%) were prescribed with deer antler. A total of 117 adverse drug reactions were reported among the patients who received herbal medicines. Conclusions This study aimed to determine the trends and safety of herbal medicine treatment, particularly deer antlers, in children and adolescents. Well-designed, long-term, multi-institutional follow-up studies are required to expand the field of Korean pediatrics.
