Phonosurgery after Laser Cordectomy (레이저 성문절제술 후의 음성수술)

  • Endoscopic laser cordectomy is known as an oncologically sound procedure for T1 and selected T2 glottic carcinoma ; it has comparable local control rate and better long-term laryngeal preservation rate when compared with those of radiotherapy. Even if results of the reported voice outcome studies after surgery or radiotherapy are diverse and controversial, resection deeper than the body layer of the vocal fold (type III, IV, V cordectomy) usually leads to aerodynamic insufficiency during phonation and results in poor voice quality. A keyhole defect or development of synechiae at the anterior commissure after type VI cordecomy may also result in unsatisfactory vocal outcome. However, many advances in phonosurgical techniques are reported to be successfully applied in the reconstruction of glottal defect that is subsequent to endoscopic laser cordectomy. In case of glottal insufficiency, voice restoration can be achieved by means of augmentation of the paraglottic space or medialization of the excavated vocal fold. Injection laryngoplasty with synthetic materials or autologous fat is gaining its popularity for restoring minor glottal volume defect because of its convenience. Laryngeal framework surgery, especially type I thyroplasty with premade implant systems or Gore-Tex, is most frequently used to correct larger glottic volume defect. In case of anterior commissural keyhole defect, additional procedure including laryngofissure may be required. For anterior commissural synechiae, laryngeal keel may be inserted for several weeks or mitomycin-C may be repeatedly applied after the division of adhesive scar to prevent restenosis. In this paper, current concepts and the authors' experiences of phonosurgical reconstruction of vocal function after endoscopic cordectomy will be introduced.

The Effects of Hatching Time on Body Weights and Body Measurements in Female Lines of Meat Type Breeders (육용종계 모계통에 있어서 부화시간에 따른 체중과 체척치에 관한 연구)

  • This study was conducted to investigate the effects of hatching time on body weights and body measurements in White Plymouth Rock selected for female lines of broiler parents stock, Thirty cockerels were mated to 300 hens and the hatching eggs produced by each hen were pedigreed for sire and dam. The total of 975 chickens were classified into 14 groups by hatching time and their body weights and body measurements were recorded every 2 weeks. The results obtained were as follows: 1. The body weight at 4,6 and 8 weeks of age, and the length of keel and shank were decreased as hatching times were delayed. Correlation coefficient between hatching tine and body weights or body measurements was negative. 2. Chickens from strain D were hatched 7.4 hours later in male and 7.2 hours in female than chickens from strain C and the growth rate of strain C was superior to that of. strain D.

  • Most inflammatory disorders are usually treated using anti-inflammatory drugs including non-steroidal anti-inflammatory drugs (NSAID) and steroidal anti-inflammatory drugs (SAID). In a prolonged use, however, they may frequently produce adverse side-effects. Thus, it is necessarily required to develop a new anti-inflammatory drug with little side-effects. Nephrite has been widely used by traditional oriental medicine to cure the various chronic diseases. In order to verify the anti-inflammatory activity of nephrite, the TPA (12-O-tetradecanoylphorbol-acetate) or the croton oil-induced edema was developed in the mouse ears and the nephrite powder suspension or the nephrite water was directly applied to the ear edema. It was found that nephrite could significantly reduce the ear swelling implying its strong potential as an active anti-inflammatory agent when comparing to indomethacin, a non-steroidal anti-inflammatory drug.

Effect of Bee Venom Acupuncture on the Recovery of Chondrocyte Phenotype in Rabbit Cartilage (봉독약침이 연골세포 활성에 미치는 영향)

  • Articular cartilage is an important factor for studying the arthritic diseases. The chondrocyte isolated from cartilage has the characieristic of differentiation and rf-differentiation when cultured in monolayer The bee venom (BV) acupuncture was investigated to examine their abilities on chondrocyte re-differentiation via rabbit chondrocyte primary culture. And the expression profiles of type II collagen (COL2) was analyzed using RT-PCR and western hybridization at third passage chondrocyte. In general, the mRNA expression of type II collagen was reduced step by step with the subculture of the chondrocyte. However, after the BV treatment for 48hr at third passage, the expressions of type II collagen were found to be significantly up-regulated, the same result was confirmed by western blotting. These results suggested that the diluted solution of BV for herb-acupuncture was very effective on the recovery of articular chondrocyte phenotype.

Effect of Bee Venom Herb-acupuncture on the Repair of Articular Full-thickness Defect in Rat (흰쥐 퇴행성 관절염모델을 이용한 봉독약침의 치료효과)

  • Articular cartilage is an important target for studying the arthritic diseases. To verify the therapeutic effects of bee venom herb-acupuncture in vivo, 3${\mu}$l of diluted solution of bee venom for herb-acupuncture were injected into articular cavity once a day during 3 months after making full-thickness defects in rat articular cartilage. Histological examination and immunohistochemistry indicated that the chondrocyte-like tissue was formed during the repair process of cartilage injury, and the expression of a cartilage-specific protein, collagen type II, were significantly activated. It means that the expression of the gene encoding type I collagen was down-regulated, whereas those of collagen type II were up-regulated. Histological examination by hematoxylin-eosin staining indicated that the cells regained their original round morphology. In addition, a homogeneous distribution of articular cartilage extracellular matrices was detected around the cells. These results suggested that bee venom herb-acupuncture was very effective on the recovery of articular chondrocyte phenotype.

  • To investigate cellular regulation of phosphate metabolism between catabolically repressed and derepressed states in Saccharomyces uvarum, the activities of polyphosphatases, the analysis of polyphosphate and cytochemical observation of volutin granules were examined according to the culture phase and under various phosphate concentrations. As the results, tripolyphosphatase activity was increased more than six-fold during catabolic repression as compared with those of catabolic derepression and the polyphosphatase activity increased at the time of maximal accumulation of acid insoluble polyphosphate 'B'. Of the low molecular weight polyphosphates, tripolyphosphate was mainly detected by thin layer chromatography. When the synthesis of volutin granules in derepressed cells was observed cytochemically, acid insoluble polyphosphate localizing at the cell wall was primarily synthesized and then transferred into the cytoplasm, nucleus and/or vacuole.

THE ANALYSIS OF INITIAL APICAL FILE SIZE BEFORE AND AFTER CORONAL FLARING (Coronal flaring 전, 후 초기근관장 파일크기의 분석)

  • The purpose of this study was to compare the initial apical file(IAF) first Ole that fits to the apex in each canal before and after early flaring to analyze if the size of file to fit to the apex would increase after flaring. Eighty anterior teeth with complete apical formation and patent foramens were selected. The samples were randomly divided into 4 groups(GG, OS, GT, PT Group) of 20 teeth each. A file was fit to the apex in each canal and that size recorded. Radicular flaring were completed using different types of instruments. After flaring a file was again fit to the apex in the same manner as before and its size recorded. The results of this study were as follows : 1. The mean diameter of IAF before flaring(file diameters in $mm{\times}10^{-2}$) was $19.81{\pm}8.32$ before and $25.94{\pm}9.21$ after(p<0.05). 2. The increase in diameter of IAF was approximately one file size for all groups. 3. Ranking of increasing diameter of IAF were GG>CT>OS>PT group. There was a statistically significant difference between before and after flaring(p<0.05). 4 Ranking of the time for flaring were GG>GT>OS>PT group. There was a statistically significant difference between GG group and other groups(p<0.05). 5. In the case without change of IAF diameter, they showed decrease in force after flaring when IAF was pulled out from root canal(p<0.05). This study suggested that early radicular flaring increases the file size that is snug at the apex, and awareness of that difference gives the clinician a better sense of canal size. Early flaring of the canal provides better apical size information and with this awareness, a better decision can be made concerning the appropriate final diameter needed for complete apical shaping.

THE IRRIGATING EFFECT BEFORE AND AFTER CORONAL FLARING (Coronal flaring 전, 후의 근관세척효과)

  • The objective of this in vitro study was to evaluate the efficacy of a Ca(OH)$_2$removal before and after early coronal flaring using different types of instruments. 100 plastic blocks with 30$^{\circ}$artificial curved canals were used in this study and randomly divided into a control group and 4 experimental groups(GG, OS, GT, PT Group) 20 teeth each. The canals were instrumented, and Ca(OH)$_2$was temporary filled into the each canal. Irrigation was peformed with Max-i-Probe 25- 30-gauge probes before and after recapitulation. The results of this study were as follows 1. There were no significant difference among the groups in size of irrigating needle(p<0.05). 2. There was a significant difference between before and after recapitulation regardless size of irrigating needle(p<0.05). 3. Before recapitulation, there was a significant difference between 25- and 30-gauge needle in all groups(P<0.05). 4. After recapitulation, there was a significant difference between 25- and 30-gauge needle in the control group(p<0.05). But there were no significant difference among the experimental groups. It is concluded that the effectiveness of canal irrigation was decided to the depth of irrigating needle into the canal. The effect of canal irrigation tend to facilitate by the early coronal flaring. The recapitulation was the most effective during canal irrigation regardless the size of irrigating needle. Therefore, the recapitulation is a mandatory way to facilitate the effectiveness of canal irrigation during canal enlargement.

Analysis of C-shaped root canal configuration in maxillary molars in a Korean population using cone-beam computed tomography

  • Objectives: The purpose of this study was to investigate the incidence of root fusion and C-shaped root canals in maxillary molars, and to classify the types of C-shaped canal by analyzing cone-beam computed tomography (CBCT) in a Korean population. Materials and Methods: Digitized CBCT images from 911 subjects were obtained in Chosun University Dental Hospital between February 2010 and July 2012 for orthodontic treatment. Among them, a total of selected 3,553 data of maxillary molars were analyzed retrospectively. Tomography sections in the axial, coronal, and sagittal planes were displayed by PiViewstar and Rapidia MPR software (Infinitt Co.). The incidence and types of root fusion and C-shaped root canals were evaluated and the incidence between the first and the second molar was compared using Chi-square test. Results: Root fusion was present in 3.2% of the first molars and 19.5% of the second molars, and fusion of mesiobuccal and palatal root was dominant. C-shaped root canals were present in 0.8% of the first molars and 2.7% of the second molars. The frequency of root fusion and C-shaped canal was significantly higher in the second molar than the first molar (p < 0.001). Conclusions: In a Korean population, maxillary molars showed total 11.3% of root fusion and 1.8% of C-shaped root canals. Furthermore, root fusion and C-shaped root canals were seen more frequently in the maxillary second molars.

The antinociceptive and anti-inflammatory effect of water-soluble fraction of bee venom on rheumatoid arthritis in rats

  • We recently demonstrated that bee venom (BV) injection into acupoint (i.e. Zusanli) produced more potent anti-inflammatory and antinociciptive effect in Freunds adjuvant induced rheumatoid arthritis (RA) model as compared with that of non-acupoint injection(i.e back). However, the precise components underlying BV-induced antinociceptive and/or anti-inflammatory effects have not been fully understood. Therefore, we further investigated the anti-arthritic effect of BV after extracting the whole BV according to solubility (water soluble: BVA, ethylacetate soluble: BVE). Subcutaneous BVA treatment (0.9 mg/kg/day) into Zusanli acupoint was found to dramatically inhibit paw edema and radiological change (i.e. new bone proliferation and soft tissue swelling) caused by Freunds adjuvant injection. In addition, the increase of serum interleukin-6 by RA induction was normalized by the BVA treatment as similar with that of non-arthritic animals. On the other hand, BVA therapy significantly reduced arthritis induced nociceptive behaviors (i.e., nociceptive score for mechanical hyperalgesia and thermal hyperalgesia). Furthermore, BVA treatment significantly suppressed adjuvant induced Fos expression in the lumbar spinal cord at 3 weeks post-adjuvant injection. However, BVE treatment (0.05 mg/kg/day) has not any anti-inflammatory and anti-nociceptive effect on RA. Based on the present results, we demonstrated that BVA might be a effective fraction in whole BV for long-term treatment of RA-induced pain and inflammation. However, it is clear necessary that further fraction study about BVA was required for elucidating an effective component of BVA.
