A Study on the Effectiveness of Strengths-based Case Management for the Elderly (재가노인 대상 강점관점 사례관리의 효과성 연구)

  • This study attempted to: (1) operate a Strengths-based case management program; (2) determine the effectiveness of strengths-based case management for the elderly; (3) evaluate the effect of the coordinating program on the capabilities of ADL, IADL, and life-satisfaction of the elderly. The subject of this study was 8 home-bound elderly in lowest income class in Gwang-ju City. The program was practiced from November 2008 to April 2009 for 6 months period. The outcome of study has shown that strengths-based case management has determined a fair of amount of effectiveness in carrying out their short-term tasks. The program tried to assess the strengths of the elderly, and evaluate the outcomes of the program using ADL, IADl items and life-satisfaction developed for the study. The data collected by pre-test and post-test of the program showed that the scores of life-satisfaction were improved statistically significantly. But capabilities of ADL, IADL items were slightly improved, but not statistically significant. Successive effort to operate strengths-based case management must be required for the improvement of capabilities of ADL, IADL, and quality of life of the elderly.

Factors Influencing Health Care Needs of Solitude Elderly (일 지역 독거 노인의 간호요구에 영향을 미치는 요인)

  • Purpose: This study was to identify factors that influence the health care needs of that over 65 years of age in Mokpo, Korea. Method: The data was collected from June 2002 to September 2002. The subjects were 120 homebound solitude elderly(age=76.8). Subjects were interviewed with structured questionnaire in order to identify the health care needs, health variables (perceived health status, risk of malnutrition, K-IADL), psychological variables(self-esteem, depression) and demographic variables. physiological health variables (height, weight, blood pressure, pulse, blood sugar)were assessed after the interview. Result: In general perceived health status was poor, risk of malnutrition was high, number of disease was 3 disease, self-esteem was low but depression was high and health care needs were relatively high. Among the elderly education & counseling needs topped the list. In regression analysis, health care needs were significantly influenced by IADL(23%), duration of solitudes(4%), sex(3%), and education(l%). These variables explained 31% of the variances in health care needs. Conclusion: The result identified that health care needs should be a considered in IADL, female, duration of solitudes and education for the solitude elderly.

Psychosocial Factors and Its Related Factors Among the Elderly People in a City (도시지역 노인들의 사회심리적 요인과 그의 관련요인)

  • This study was performed to determine the relationships among self-esteem, state anxiety, interpersonal behavior trait and locus of control, and to reveal its related factors. The interviews were performed, during the period from April 1st, to June 30th, 2011, to 396 elderlies in Daejeon city. As a results, the self-esteem was negatively correlated with state anxiety and interpersonal behavior trait, while it was positively correlated with locus of control. While state anxiety was positively correlated with interpersonal behavior trait, it was negatively correlated with locus of control, and interpersonal behavior trait was positively correlated with locus of control. Multiple stepwise analysis revealed that the factors of influence on self-esteem included IADL, spouse, mastication of food and amnesia. The factors of influence on State anxiety included mastication of food, IADL, spouse, subjective health status, disability of body, subjective sleep evaluation and educational level. The factors of influence on Interpersonal dependent behavior trait included spouse, IADL, monthly income and subjective health status. The factors of influence on locus of control included spouse and visual acuity. Above results suggested that the self-esteem, state anxiety, interpersonal behavior trait and locus of control of subjects were significantly related with the sociodemographic characteristics, health related factors and health status.

The Influences of Visual and Hearing Impairment on Activities of Daily Living for the Community Dwelling Elderly (재가노인의 시청각기능장애가 일상생활수행능력에 미치는 영향)

  • Purpose: The aim of this study is to identify the influence of visual and hearing impairment on the activities of daily living of community dwelling elderly. Methods: Data were collected by home visiting interviewers from 452 older people aged 65 years or older living in community. Resident Assessment Instrument MDS-HC(2.0version) was used for data collection. Data analysis for descriptive statistics, Chi-square test and multiple regression was made by SAS 6.2 Results: 34.7% of the subject had hearing impairment and 64.3% had visual impairment Among IADL. one half of them were dependent in ordinary house work and meal preparation. In the case of ADL. 13.9% of subjects were dependent in bathing and 8.9% in personal hygiene. There was significant difference in IADL performance by visual and hearing impairment On the other hand, ADL performance showed the significant difference. only in the case of hearing impairment. As the result of input of visual and hearing impairment in the process of regression. variances were increased from 3% to 11%. Conclusions: Large proportions of older people living in the community have visual and hearing impairment. It could be confirmed that hearing and vision were significant factors influencing on IADL performance of older people. Intervention and support policy for elderly needs to focus on improvement of visual and hearing impairment.

Influence of Clinical and Demographical Variables on Depression, Instrumental Activities of Daily Living, and Cognitive Function in Patients with Parkinson's Disease (파킨슨병 환자의 인구학적.임상적 특성에 따른 우울, 일상생활 수행능력 및 인지기능)

  • Purpose: To investigate the influence of clinical and demographical variables on depression, instrumental activities of daily living (IADL), and cognitive function in patients with Parkinson's disease. Method: Using a structured questionnaire data were collected from 100 participants registered in the neurology department of C university hospital. Duration and stage of disease, fall history, vision impairment, duration and quality of sleep, orthostatic hypotension, ambulation impairment, and use of walking aid were included in clinical variables. Depression, IADL, and cognitive function were assessed using Kee's GDSSF-K, Cho's scale, and K-MMSE. Collected data were analyzed using the SAS program. Results: The depression score for the participants was 7.78, higher than cut-off score(5). Participants who were male, living with spouse, above high school education, high economic status, with no vision impairment, and no ambulation impairment revealed high cognitive scores. Average IADL score were significantly higher for participants who were male, who had high economic status, low stage of disease, and no ambulation impairment. Depression IADL & cognitive function scores were significantly different according to ambulation impairment. Scores for ADL and cognitive function were positively correlated. Conclusion: It is recommended that make programs for patients with Parkinson's disease, clinical and demographic variables should be considered according to their individual needs.

  • The purpose of this study is to investigate the relationship between "Mental Status and Dementia Tests" and "predictive diagnosis of severity of dementia". This is a descriptive research, analyze the data collected from Alzheimer's patients and identify which "Mental Status and Dementia Tests" has the most impact to determine the severity of dementia. For this study, Alzheimer's patient's CDR, MMSE-K, SGDS, NPI-Q, BADL and IADL were collected and analyzed. This study will provide a predictive factor to determine the severity of dementia when "Mental Status and Dementia Testss" are being used and also to provide the right treatment. For this study, a total of 617 cases of data from Alzheimer's patients were collected and analyzed with SPSS Statistics. In addition, effective "Mental Status and Dementia Tests" for evaluating the severity of dementia were CDR, ADL, MMSE-K, and SGDS whereas NPI-Q and IADL. Based on the findings of the study, it was recommended to implement more efficient diagnostic method by utilizing a standardized "Mental Status and Dementia Tests".

  • Purpose: This study was conducted to examine the effect of visiting nursing of the visiting nursing services based on the district management system on the subjects of stroke patients. Method: A nonequivalent control group pretest-posttest design was applied to 50 stroke patients (31 from the experimental group, 19 from the control group). To evaluate the effects of visiting nursing services. health status(SF-36) and activity daily living(ADL/IADL) were measured from June 2003 to November 2004. Result: Health condition and the activities of daily living (ADL/IADL) of the subjects who received visiting nursing service were shown to be improved. Conclusion: Visiting health service model based on the district management system in public health center is considered to be an effective measure of visiting nursing services in large cities.

The Relationship of Perceived Health Status, Activities of Daily Living and Nutrition Status in the Community-Dwelling Korean Elderly (재가 노인의 주관적 건강, 일상생활 수행 능력, 영양 상태 간의 관계)

  • Purpose: This study aimed to identify the associated factors of the perceived health status of the elderly in Korea and to provide basic data for developing nursing interventions for the elderly's health management. Methods: This study used a descriptive correlational research design. The subjects of this study were 335 elderly people over 60 yr living in an urban city. Data were collected through personal interviews using questionnaires from September 2006 to March 2007. Empirical indicators of perceived health status were measured by SF-36, nutritional screening initiative (NSI), activities of daily living (ADL) Index, and instrumental activities of daily living (IADL) scale. The collected data were analyzed by descriptive statistics, Pearson's correlation, and hierarchical regression. Results: The mean age of the subjects was 72.8 and 57.0% of subjects were female. 41.5% of variance in physical health was explained by nutrition, ADL, IADL, and the number of years attending school. Among them the most important factor was ADL. 13.2% of variance in mental health was explained the number of family, nutrition, and IADL. Among them the most important factor was nutrition. Conclusion: It is necessary to develop supportive interventions for improving the perceived health status of elderly people by considering the most important factors shown in this study.

The Association among Activity of Daily Living, Instrumental Activities of Daily Living, Depression and Isolation in Disabled People (장애인의 일상생활동작, 도구적 일상생활동작, 우울 및 고립감의 관계)

  • Purpose: The purpose of this study was to evaluate the association among of Activity of Daily Living (ADL), Instrumental Activities of Daily Living (IADL), depression and isolation in disabled people. Method: The subjects consisted of 761 disabled people in Korea. Data were analyzed using descriptive statistics, t-test, ANOVA, and Pearson's correlational statistics. Results: There were significant differences in age, cause of disability, type of disability, degree of disability, education, job, perceived health condition, and economic status between disabled people with high and low levels of ADL and IADL. Depression showed significant differences in type of disability, degree of disability, education, job, perceived health condition, and economic state; isolation showed significant differences in age, cause of disability, type of disability, education, job, residence, perceived health condition, and economic status. A significant association was identified between depression and isolation (r=.69, p<.001). Conclusion: There was a high incidence of depression in disabled men with high levels of isolation and ADL and IADL dependence. An effort to increase disabled people's ADL, IADL, and independence levels is needed, and nursing interventions should be designed and tested to increase their community involvement and decrease the sense of isolation and depression.

Factors related to Quality of Life in the Elderly People in Long-term Care Center (장기요양시설 노인의 삶의 질에 관련된 요인)

  • This study examined the relationship between the sociodemographic characteristics, health related factors, physical functions (ADL, IADL), mental functions (CES-D, MMSE-K), oral health impact profile (OHIP-14), and the quality of life (WHOQOL-BREF) for the elderly in long-term care facilities. The research was conducted in 602 facility allowance beneficiaries authorized to be eligible for long-term care in long-term care facilities through personal interviews using a structured questionnaire from May 1 to June 30, 2016. As a result, the quality of life was lower among females than males in the group receiving government subsidies than the group whose livelihood was maintained by themselves or their children, in the group with a longer period of care, in the drinking group than the non-drinking group, in the group with irregular exercise than the regular exercise, in the group with irregular meals than regular meals, in the group with poor subjective health conditions than good subjective health condition, in the group with a smaller number of daily toothbrushing, in the group with xerostomia than no xerostomia, in the group with a lower OHIP-14, in the group with a lower ADL and IADL, and in the group with a lower CES-D and MMSE-K. In particular, the quality of life was affected more by health-related factors and CES-D and MMSE-K than by other factors. Therefore, it is necessary to make efforts to prevent depression and cognitive impairment, including health-related behavior, with the objective of improving the quality of life for the elderly in long-term care facilities.
