Bone Homeostasis and Gut Microbial-Dependent Signaling Pathways

  • Although research on the osteal signaling pathway has progressed, understanding of gut microbial-dependent signaling pathways for metabolic and immune bone homeostasis remains elusive. In recent years, the study of gut microbiota has shed light on our understanding of bone homeostasis. Here, we review microbiota-mediated gut-bone crosstalk via bone morphogenetic protein/SMADs, Wnt and OPG/receptor activator of nuclear factor-kappa B ligand signaling pathways in direct (translocation) and indirect (metabolite) manners. The mechanisms underlying gut microbiota involvement in these signaling pathways are relevant in immune responses, secretion of hormones, fate of osteoblasts and osteoclasts and absorption of calcium. Collectively, we propose a signaling network for maintaining a dynamic homeostasis between the skeletal system and the gut ecosystem. Additionally, the role of gut microbial improvement by dietary intervention in osteal signaling pathways has also been elucidated. This review provides unique resources from the gut microbial perspective for the discovery of new strategies for further improving treatment of bone diseases by increasing the abundance of targeted gut microbiota.

Geostationary Satellite Station Keeping Robustness to Loss of Ground Control

  • For the vast majority of geostationary satellites currently in orbit, station keeping activities including orbit determination and maneuver planning and execution are ground-directed and dependent on the availability of ground-based satellite control personnel and facilities. However, a requirement linked to satellite autonomy and survivability in cases of interrupted ground support is often one of the stipulated provisions on the satellite platform design. It is especially important for a geostationary military-purposed satellite to remain within its designated orbital window, in order to provide reliable uninterrupted telecommunications services, in the absence of ground-based resources due to warfare or other disasters. In this paper we investigate factors affecting the robustness of a geostationary satellite's orbit in terms of the maximum duration the satellite's station keeping window can be maintained without ground intervention. By comparing simulations of orbit evolution, given different initial conditions and operations strategies, a variation of parameters study has been performed and we have analyzed which factors the duration is most sensitive to. This also provides valuable insights into which factors may be worth controlling by a military or civilian geostationary satellite operator. Our simulations show that the most beneficial factor for maximizing the time a satellite will remain in the station keeping window is the operational practice of pre-emptively loading East-West station keeping maneuvers for automatic execution on board the satellite should ground control capability be lost. The second most beneficial factor is using short station keeping maneuver cycle durations.

Community Case Managers' Experiences of Overcoming Suicide Crises among Late Adolescents in South Korea

  • Purpose: Various intervention strategies are needed to reduce the suicide rate among late adolescents. Therefore, we aimed to investigate the factors that can help overcome suicidal tendencies in late adolescents from the perspective of case managers. Methods: This qualitative study explored the reinforcing factors that can help overcome suicidal behaviors in late adolescents, by conducting focus group interviews with case managers from the local community. Interviews with 10 case managers were recorded and transcribed, and the data were analyzed using the content analysis method. Results: The main theme of this study was "a step in a new direction". Three categories were derived: "inner changes", "another possibility opened up by the interest of significant others", and "connecting with community resources to help overcome". Each category had subcategories that influenced the intention and course of action of behaviors to overcome the suicide crises. Conclusion: Our findings, from the exploration of the experiences of case managers, provide an in-depth understanding of the reinforcing factors for overcoming suicide among late adolescents. They provide useful preliminary data for the development of effective suicide prevention programs for the adolescents.

Factors Affecting HPV(Human Papilloma Virus) Vaccination Intentions of Female Adolescents and their Mothers (여성 청소년과 그 어머니의 사람유두종바이러스 예방접종의도 영향요인)

  • Purpose: The purpose of this study was to identify factors that affect the intention of HPV vaccination in 12-year-old female adolescents and their mothers. Method: A descriptive study was conducted with 64 HPV-unvaccinated female adolescents and their mothers in W city, Republic of Korea. Data collection was conducted from October to November in 2019. The data were analyzed using SPSS WIN 24.0 Program. Descriptive statistics, paired t-test, independent t-test, χ2-test and logistic regression analysis were conducted. Result: According to the results, 57.8% and 62.5% of female adolescents and mothers had the intentions of HPV vaccination, respectively. In the logistic regression analysis for the factors affecting female adolescents' and mothers' HPV vaccination intentions, there was no significant factor in female adolescents. In the case of mothers, the significant factors were cervical cancer knowledge and perceived disability. Conclusion: In order to increase the HPV vaccination rate, educational programs are needed to provide knowledge of cervical cancer. A variety of intervention strategies should be developed to decrease the perceived disability of HPV vaccination health beliefs.

A Comprehensive Framework and Approaches for Enhancing Mental Health in Medical Students (의과대학생의 정신건강 증진을 위한 지원의 틀과 방안)

  • Research suggests that medical students frequently experience mental health problems such as stress, burnout, and depression, which may, in turn, affect suicidal ideation and behaviors. Since mental health problems profoundly impact academic achievement and professionalism, it is vital to understand factors influencing students' mental health and identify strategies to provide the necessary support. Some relevant influencing factors range from the personal level, including gender, personality traits, perfectionism, and social support, to the environmental level, including the grading system, educational phases, exposure to patients' death, mistreatment, and culture of medicine. In this regard, a comprehensive mental health support system that encompasses environmental interventions, as well as personal-level support, is needed. Simultaneously, proactive approaches that address the improvement of self-care and alleviation of systemic burdens are essential, together with the predominant reactive approaches focusing on problems and deficits. Altogether, we proposed a framework for enhancing mental health constructed by four categories (personal-reactive, environmental-reactive, personal-proactive, environmental-proactive) based on the intervention level and goal of support. All four categories have important implications, and one cannot replace the other, but expanding environmental-proactive support will allow more students to learn how to pursue health independently. We expect that this comprehensive framework for enhancing mental health could expand support systems for medical students' personal and professional development.

Identification of shear transfer mechanisms in RC beams by using machine-learning technique

  • Machine learning technique is recently opening new opportunities to identify the complex shear transfer mechanisms of reinforced concrete (RC) beam members. This study employed 1224 shear test specimens to train decision tree-based machine learning (ML) programs, by which strong correlations between shear capacity of RC beams and key input parameters were affirmed. In addition, shear contributions of concrete and shear reinforcement (the so-called Vc and Vs) were identified by establishing three independent ML models trained under different strategies with various combinations of datasets. Detailed parametric studies were then conducted by utilizing the well-trained ML models. It appeared that the presence of shear reinforcement can make the predicted shear contribution from concrete in RC beams larger than the pure shear contribution of concrete due to the intervention effect between shear reinforcement and concrete. On the other hand, the size effect also brought a significant impact on the shear contribution of concrete (Vc), whereas, the addition of shear reinforcements can effectively mitigate the size effect. It was also found that concrete tends to be the primary source of shear resistance when shear span-depth ratio a/d<1.0 while shear reinforcements become the primary source of shear resistance when a/d>2.0.

Burnout and compassion competency to explain job satisfaction among nurses working in intensive care units or emergency rooms (중환자실과 응급실 간호사의 소진과 공감역량이 직무만족도에 미치는 영향)

  • Purpose : This study aimed to examine the relationships of burnout and compassion competency with job satisfaction among nurses working in the intensive care units and emergency rooms. Methods : A correlational survey was conducted from August 1 to August 14, 2018 and involved in 102 nurses working at four university hospitals for more than 1 year. A structured questionnaire was used to measure burnout, compassion competency, and job satisfaction, and data were analyzed using hierarchical multiple regression and mediation effect analysis. Results : The mean scores of burnout, compassion competency, and job satisfaction were 2.58, 4.35, and 2.95, respectively. Burnout and compassion competency along with working night shifts and a wish to transfer explained 35.3% of variance in job satisfaction. Compassion competency showed a small but significant mediating effect in the relationship between burnout and job satisfaction (indirect effect -.0446, 95% CI -.1133, -.0075). Conclusion : Burnout and compassion competency were the significant influencing factors of job satisfaction among nurses working in intensive care units and emergency rooms. Since compassion competency showed a significant mediating effect, further studies should focus on intervention strategies for compassion competency to improve job satisfaction in this population.

Factors Influencing on Quality of Life in Korean Women with Breast Cancer and Cervical Cancer Using The 7th KNHANES (유방암과 자궁경부암 진단 성인 여성의 삶의 질 영향요인)

  • This study was conducted to identify the level of quality of life and factors influencing on quality of life in Korean women with breast cancer and cervical cancer. This study was secondary analysis using The seventh Korea National Health and Nutrition Examination Survey (KNHANES). In the result of this study, women with the lower age, higher household income, and higher self-rated health status, had the higher level of quality of life. Following the result, intervention strategies are needed to enhance self-rated health and to improve quality of life in women with lower age and lower household income.

The Relationship Between Foreign Student Academic Stress and Depression Tendency: The Mediating Effect of Self-Esteem (외국인 유학생의 학업스트레스와 우울과의 관계에서 자아존중감의 매개효과)

  • Objectives: The purpose of this study was to examine the mediating effect of self-esteem on the relationship between academic stress and depression in foreign students in Korea. Methods: The subjects of this study were 343 foreign students enrolled in universities and graduate schools in Daegu and Gyeongbuk area, and data were collected by conducting a questionnaire survey using the internet. The collected data was analyzed using SPSS 23.0 and PROCESS macro Model No 4. Results: The results were as follows. First, academic stress of foreign students increased the level of depression statistically significantly. Second, academic stress significantly reduced the self-esteem of foreign students. Third, the self-esteem of foreign students partially mediated the effect of academic stress on depression (p<0.05). In other words, it suggests that academic stress lowered self-esteem, and that the lowered self-esteem had an effect on the increase in depression. Conclusions: These results suggest that an intervention program to reduce academic stress is needed as a countermeasure for the alleviation and prevention of depression in foreign students, and that strategies for improving self-esteem should be considered as important.

Effects of Simulation-based Learning on Stress, Problem Solving Ability, Self-Efficacy, and Resilience of College Nursing Students

  • Objectives: The objective of this study was to explore the effects of the simulation-based learning program on stress, problem-solving ability, self-efficacy, and resilience of final-year nursing students in a college in South Korea. Methods: The design of the study was a one-group pretest-posttest. The participants of this study were final-year nursing students in 2018. A total of 105 students completed it. The intervention was an 8-week simulation-based practice course. The primary and secondary outcome measures were baseline and follow-up questionnaires regarding demographic factors, stress, problem-solving ability, self-efficacy, and resilience. Results: Problem-solving ability (t=6.567, p<.001), self-efficacy in four situations (p<.001) and resilience (t=2.352, p=.021) increased after simulation-based learning than before learning. Stress also increased after simulation-based learning compared to before learning (t=5.960, p<.001). The level of stress, self-efficacy, and resilience were mainly related to participants' satisfaction with their clinical placement, and interpersonal relationships (p<.05). Conclusions: Simulation-based learning is expected to improve nursing students' problem-solving ability, self-efficacy, and resilience. This can lead to induce learning motivation of nursing students, improve their coping strategies for solving problems, and ultimately provide high-quality care.
