  • Objectives : This study is to report the effect of herbal medicine and acupuncture on a clinical pregnancy. Methods : From february 2014 to july 2014, a prospective analysis study was performed in 20 patients after taking her medicine and acupuncture treatment. Results : After treatment, two patients naturally became pregnance and one patient became pregnance after in vitro fertilization-embryo transfer (IVF-ET). Conclusions : This study suggests that herbal medicine and acupuncture treatment are useful and shows possibility to increasing pregnancy rates.

The Effect of Assisted Hatching (AHA) on Pregnancy Rates in Human IVF-ET (인간의 체외수정배아이식술에서 보조부화술이 임신률에 미치는 영향에 관한 연구)

  • In human IVF-ET, the development and morphology of the embryo have been known to affect implantation and pregnancy rates(PRs). Recently, pregnancy has been reported to related to the embryos with thick zona-pellucida, high levels of fragmentation, poor blastomere development and zona hardening. Although the mechanism of implantation is unclear, it is thought that the hatching process precedes implantation and that the hatching is related to implantation and PRs. This study was carried out to investigate the effect of assisted hatching(AHA) on the improvement of PRs in human IVF-ET. The results were as follows; 1. The PRs of the AHA group (40.8%) was significantly higher than that of control group(27.2%)(P<0.01). 2. According to the age of patients, the PRs of control and AHA groups were 33.9%(20/59), 44,4%(12/27) in <30 yrs, 26.1%(30/115), 38.3%(18/47) in 31-35 yrs, 22.4%(13/58), 41.4%(12/29) in >36 yrs, respectively. 3. According to the factors of infertility in AHA group, unexplained(immunologic factor) (40.0%) and male factors(41.9%) were higher than female(tubal obstruction, endometriosis, adhesion) factor (28.9%). As a result, it is suggested that AHA technique improve the PRs in poor prognosis patients. It is concluded that AHA method can be used to improve the PRs in human lVF-ET.

Variable localization of Toll-like receptors in human fallopian tube epithelial cells

  • Objective: To determine the localization, expression, and function of Toll-like receptors (TLRs) in fallopian tube epithelial cells. Methods: The localization of TLRs in fallopian tube epithelial cells was investigated by immunostaining. Surprisingly, the intensity of staining was not equal in the secretory and ciliated cells. After primary cell culture of fallopian tube epithelial cells, ring cloning was used to isolate colonies of ciliated epithelial cells, distinct from non-ciliated epithelial cells. The expression of TLRs 1-10 was examined by quantitative real-time polymerase chain reaction, and protein localization was confirmed by immunostaining. The function of the TLRs was determined by interleukin (IL)-6 and IL-8 production in response to TLR2, TLR3, TLR5, TLR7, and TLR9 ligands. Results: Fallopian tube epithelial cells expressed TLRs 1-10 in a cell-type-specific manner. Exposing fallopian tube epithelial cells to TLR2, TLR3, TLR5, TLR7, and TLR9 agonists induced the secretion of proinflammatory cytokines such as IL-6 and IL-8. Conclusion: Our findings suggest that TLR expression in the fallopian tubes is cell-type-specific. According to our results, ciliated cells may play more effective role than non-ciliated cells in the innate immune defense of the fallopian tubes, and in interactions with gametes and embryos.

Infertility of Transgenic Mice Experssing Human Growth Hormone Gene (사람 성장호르몬 유전자를 발현하는 형질전환생쥐의 불임성)

  • Many transgenic mice expressing human growth hormone gene were infertile. To investigate the infertility of these transfenic mice, it was looked into the estrus cycle and sexual behaviour and also tested through in vitro fertilization whether the germ cells of these mice normal or not. The infertile female transgenic mice were mated to the fertile males of ICR strain, but in almost all of them the vaginal plugs were not detected and their estrus cycles by vaginal smear were almost irregular which kept up estrus or diestrus stage. Many male transgenic mice did not have the ability of sexual behaviour. Therefore the viability of germ cells in infertile male transgenic mice was investigated by in vitro fertilization, but the sperm were normally fertilized with the eggs and the transgene of parent was passed on to the progeny. These results consequently suggest that the infertility of transgenic mice experssing human growth hormone gene may be due to the physiological activity of human growth hormone, not germ cells.

Enterohepatic Recycling of Estrogen and its Relevance with Female Fertility

  • Enterohepatic recycling of estrogen after oral administration of 1 mg non-radioactive estriol was studied in fourteen women selected as the control subjects and ten infertile women in whom the infertility was appearing to be of endocrine origin. The extent of enterohepatic recycling of estriol ($E_3$) during the early follicular phase of menstrual cycle was assessed by monitoring during 48 h the urinary excretion of its two major metabolites i.e; estriol 16 $\alpha$-glucuronide ($E_3-16$$\alpha-G$) and estriol-3 glucuronide ($E_3$-3-G). The change in urinary level of $E_3$-3-G with respect to ($E_3-16$$\alpha-G$G was considered to reflect the extent of enterohepatic recycling of estriol. Lower values of urinary output of both metabolites in the infertile women as compared with the control subjects and the urinary excretion profile of both metabolites during 48 h after estriol ingestion reveal that the reduced extent of enterohepatic recycling could possibly be one of the factors which contribute towards the incidence of infertility in women.

Live birth in a woman with recurrent implantation failure and adenomyosis following transfer of refrozen-warmed embryos

  • The aim was to report a healthy live birth using re-vitrified-warmed cleavage-stage embryos derived from supernumerary warmed embryos after frozen embryo transfer (ET) in a patient with recurrent implantation failure (RIF). The case was a 39-year-old female with a history of polycystic ovarian syndrome and adenomyosis, along with RIF. After ovarian hyperstimulation, 33 cumulus-oocyte complexes were retrieved and fertilized with conventional in vitro fertilization and intracytoplasmic sperm injection. Because of the risk of ovarian hyperstimulation syndrome, 16 grade B and C embryos were vitrified. After 3 and 6 months, 3 and 4 B-C warmed embryos were transferred to the uterus, respectively. However, implantation did not take place. Ten months later, four embryos were warmed, two grade B 8-cell embryos were transferred, and two embryos were re-vitrified. One year later, the two re-vitrified cleavage-stage embryos were warmed, which resulted in a successful live birth. This finding showed that following first warming, it is feasible to refreeze supernumerary warmed embryos for subsequent ET in patients with a history of RIF.

A Case Report of One Patient with Infertility Accompanied by Adenomyosis and Uterine Myoma (자궁근종 및 자궁선근증을 동반한 불임환자 증례보고)

  • Objectives: Adenomyosis is the disease caused by ingrowth of lining tissue into the uterine muscle. Final treatment of the disease is hysterectomy. But if a patient want to get pregnant, there is a tendency to avoid surgical treatment. This paper introduces the potential of oriental medical treatment for infertility caused by adenomyosis through a case study. Methods: A 34-year-old female patient, who wants to get pregnant, was diagnosed with adenomyosis. She wants to take oriental medical treatment in the ${\bigcirc}{\bigcirc}$ Oriental Hospital. We applied herb-medication, acupuncture and moxibustion. Results: Overall condition including the symptoms was relieved and succeeded in pregnancy after oriental medical treatment. Conclusions: Oriental medical treatment has an effect infertility caused by adenomyosis.

Study on Status Survey in Female Infertility Patients Admitted to Korean Medicine Hospital (한방병원에 내원한 구사(求嗣) 환자의 실태 분석 및 현황 조사)

  • Purpose: This study was to investigate status survey and cost of infertility patients in ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital. Methods: From January 2012 to June 2012, 171 new patients were admitted to ${\bigcirc}{\bigcirc}$ Korean Medicine Hospital for treatment of infertility. We reviewed their medical records retrospectively and had telephone consultations to find out whether they were pregnant or not. Results: The mean age of outpatients was $32.72{\pm}4.08$ years and mean BMI of outpatients was $20.26{\pm}2.68$. 55.0% of patients who did not have childbirth or miscarriage. 82.5% of patients had normal menstrual period. 93.5% of patients visited OB/GYN, 33.3% were treated with ovulation induction, 18.7% underwent intrauterine insemination, and 18.1% underwent in vitro fertilization and embryo transfer. 99.7% were treated with acupuncture and moxibustion, 100% had taken Herbal Medicine. The mean treatment duration of outpatients was $8.84{\pm}8.17$ weeks, and the mean medical expense was $761,994{\pm}586,502$ won. It was found that 32.4% of patients were pregnant after treatment. Conclusions: We investigated status survey and cost of infertility patients. Further study about Korean medical treatments on infertility is required.

A Pilot Study for the Practical Usage of Traditional Korean Medicine Diagnostic Methods in Women Infertility (여성 불임환자에 대한 한의 진단도구 활용을 위한 기초연구)

  • Objectives: This study was performed to obtain reference data of traditional Korean medicine diagnostic methods for the practical usage in women infertility. Methods: The study involved 38 female patients of age 20-38 years that had diagnosed of infertility. All the subjects answered to pattern identification questionnaire and Questionnaire for the Sasang Constitution Classification (QSCC II). Pattern identification scores and constitution types have been analyzed with variables of infertility factors and age. Statistical analysis was performed by adopting descriptive and inferential tests. Results: In pattern identification questionnaire analysis, the patterns were observed in order of frequency: liver stasis; blood deficiency; kidney-yang deficiency; kidney-yin deficiency; damp-heat; blood stasis; and phlegm. The group, less than 35 years old, had the higher average score of liver stasis (p<0.05). Also, the group with tubal and peritoneal factors, had the higher average score of blood deficiency (p<0.05). In QSCC II analysis, So-yang type had the higher average score of liver stasis, kidney-yang deficiency, and kidney-yin deficiency. So-eum type had the higher average score of liver stasis, kidney-yang deficiency, kidney-yin deficiency, and blood deficiency. While, Tae-um type had the higher average score of liver stasis and phlegm. Conclusions: We would accumulate the clinical data for the practical usage of traditional Korean medicine diagnostic methods in women infertility.

Combined Hysterosalpingography and Laparoscopy in Infertility (복강경하(腹腔鏡下)에서의 Hysterosalpingogram)

  • Hysterosalpingograms (H.S.G.) have been done for several decades to determine causative factors in female infertility. However, the H.S.G. only reverals uterine cavity and tubal patency or inpatency. The author prefers to find more details in regard to the status and condition of the female reproductive organs and their surrounding tissue as they pertain to infertility. H.S.G. in combination with laparoscopic examination reveals the following results. Preparation and method of performance of H.S.G. during laparoscopy in a healthy reproductive age women are as follows. When laparoscopy is not contraindicated, NPO is ordered with routine bowel preparation. Analgesics administered by injection prior to procedure are valium 10mgs and pethidine 50-100mgs. The radiographic procedure is the same as for any HSG technique. During laparoscopy a solution of 3 to 10 ccs. of 60% hypaque sodium is used. Fluroscopic scout films are obtained A-P and oblique views as well as a delayed check film. 1. Age distribution of primary and secondary infertility in this studies involving tubal factors was as follows: 20-29 age group showed 46% incidence and in the 30-39 age group, 50% incidence. Duration of infertility in this study group was the following: 1-2 years showed 26.7%, 3-5 years 53.8%, and 6-9 years 13.3%. 2. Indications of laparoscopic examination were as follows: Secondary infertility in 35% of the cases, obscure tubal occlusion on previous H.S.G. in 25%, unknown origin in 11.7%, and the remaining cases included pelvic pain, small masses, dysmenorrhea, and uterine anomaly. The laparoscopic examination showed clearly the reproductive organs and the surrounding tissues in the pelvic cavity. The abnormal tubal findings there revealed were tuberculous salpingitis and hydrosalpinx in 10% each, endometriosis and peritubabl adhesions in 6.7% each, biconuate uterus in 3.3%. The remaining 58.3% of the cases showed normal findings. Laparoscopic observation for possible myoma nodules, streak ovary, and peritubal adhesions was also done at this time. 3. Comparative tubal findings in combined H.S.G. and laparoscopic examination revealed the following. Bilateral tubal occlusion was present in 14% (7cases) on laparoscopic examination but on H.S.G. 38% (19 cases) were noted. However, tubal occlusion and peritubal adhesions were found in 26% (13 cases) upon laparoscopy and only 8% (4 cases) on H.S.G. examination alone. Normal pelvic findings were present in 60% (27 cases).
