Distribution of HPV Genotypes in Cervical Cancer in Multiethnic Malaysia

  • Background: Cervical cancer is the third commonest type of cancer among women in Malaysia. Our aim was to determine the distribution of human papilloma virus (HPV) genotypes in cervical cancer in our multi-ethnic population. Materials and Methods: This was a multicentre study with a total of 280 cases of cervical cancer from 4 referral centres in Malaysia, studied using real-time polymerase chain reaction (qPCR) detection of 12 high risk-HPV genotypes. Results: Overall HPV was detected in 92.5% of cases, in 95.9% of squamous cell carcinomas and 84.3%of adenocarcinomas. The five most prevalent high-risk HPV genotypes were HPV 16 (68.2%), 18 (40%), 58 (10.7%), 33 (10.4%) and 52 (10.4%). Multiple HPV infections were more prevalent (55.7%) than single HPV infections (36.8%). The percentage of HPV positive cases in Chinese, Malays and Indians were 95.5%, 91.9% and 80.0%, respectively. HPV 16 and 18 genotypes were the commonest in all ethnic groups. We found that the percentage of HPV 16 infection was significantly higher in Chinese (75.9%) compared to Malays (63.7%) and Indians (52.0%) (p<0.05), while HPV 18 was significantly higher in Malays (52.6%) compared to Chinese (25.0%) and Indians (28%) (p<0.05). Meanwhile, HPV 33 (17.9%) and 52 (15.2%) were also more commonly detected in the Chinese (p<0.05). Conclusions: This study showed that the distribution of HPV genotype in Malaysia is similar to other Asian countries. Importantly, we found that different ethnic groups in Malaysia have different HPV genotype infection rates, which is a point to consider during the implementation of HPV vaccination.

  • The investigation of the curriculum in other countries provides meaningful implications to reflect our own curriculum. Since Korea is now under the curriculum revision, international comparative research was conducted with the curricula of Singapore and India to elicit some implications. These two countries were especially chosen because their curricula have not been actively investigated yet. Singapore mathematics curriculum starts the tracking based on students' mathematical ability from the 4th grade, and provides different curricula for the three tracks. This differentiated curriculum provides rich implications to next Korean curriculum which aims to classify the contents based on students' mathematical achievements. Indians, who have contributed significantly in the history of mathematics, have unique mathematics curriculum, remote from so called 'canonical curriculum'. After the U.S. announced the Curriculum and Evaluation Standard for School Mathematics in 1989 and the Principles and Standards for School Mathematics in 2000, many countries benchmarked these NCTM documents, and Korea was no exception. Since each country has their own school system, educational environment, and national mentality, it is not desirable to just adopt the curriculum of other countries. In this regard, Indians who have preserved their own mathematics curriculum can be a model. In sum, when we revise the curriculum, it is required to keep the balance between the open-mindedness to accept the strengths of other curricula, and the conservative attitude to preserve our own characteristics of the curriculum.

Lack of any Association of the CTLA-4 +49 G/A Polymorphism with Breast Cancer Risk in a North Indian Population

  • Cytotoxic T-lymphocyte antigen 4 (CTLA-4) is an important protein involved in the regulation of the immune system. The +49 G/A polymorphism is the only genetic variation in the CTLA-4 gene that causes an amino acid change in the resulting protein. It is therefore the most extensively studied polymorphism among all CTLA-4 genetic variants and contributions to increasing the likelihood of developing cancer are well known in various populations, especially Asians. However, there have hiterto been no data with respect to the effect of this polymorphism on breast cancer susceptibility in our North Indian population. We therefore assayed genomic DNA of 250 breast cancer subjects and an equal number of age-, sex- and ethnicity-matched healthy controls for the CTLA-4 +49 G/A polymorphism but no significant differences in either the gene or allele frequency were found. Thus the CTLA-4 +49 G/A polymorphism may be associated with breast cancer in other Asians, but it appears to have no such effect in North Indians. The study also highlights the importance of conducting genetic association studies in different ethnic populations.

Preference and intake frequency of high sodium foods and dishes and their correlations with anthropometric measurements among Malaysian subjects

  • This study investigated the preference and intake frequency of a list of 15 commonly available high sodium Malaysian foods/dishes, dis cretionary salt use, and their possible association with demographics, blood pressures and anthropometric measurements among 300 Malaysian university students (114 males, 186 females; 259 ethnic Chinese, 41 Indians; 220 lean, 80 overweight). French fries and instant soup noodle were found to be the most preferred and most frequently consumed salty food, respectively, while salted fish was least preferred and least frequently consumed. Males had a significantly higher intake frequency of at least 6 of the salty foods, but the preference of most salty foods was not significantly different between genders. Ethnic Chinese significantly preferred more and took more frequently traditional and conventional Malaysian foods like asam laksa (a Malaysian salty-sour-spicy noodle in fish stock), salted biscuits and salted vegetable, while Indians have more affinity and frequency towards eating salty Western foods. Body Mass Index was significantly negatively correlated with the intake frequency of canned/packet soup and salted fish while waist circumference was significantly positively correlated with the preference of instant noodle. Also, an increased preference of potato chips and intake frequency of salted biscuits seemed to lead to a decreased WHR. Other than these, all the other overweight/obesity indicators did not seem to fully correlate with the salty food preference and intake frequency. Nevertheless, the preference and intake frequency of asam laksa seemed to be significant negative predictors for blood pressures. Finally, increased preference and intake frequency of high sodium shrimp paste (belacan)-based foods like asam laksa and belacan fried rice seemed to discourage discretionary salt use. In conclusion, the preference and intake frequency of the high sodium belacan-based dish asam laksa seems to be a good predictor for ethnic difference, discretionary salt use and blood pressures.

Cyclooxygenase 2 gene polymorphisms and chronic periodontitis in a North Indian population: a pilot study

  • Purpose: Cyclooxygenase (COX) enzyme catalyzes the production of prostaglandins, which are important mediators of tissue destruction in periodontitis. Single nucleotide polymorphisms of $COX_2$ enzyme have been associated with increasing susceptibility to inflammatory diseases. The present study evaluates the association of two single nucleotide polymorphisms in $COX_2$ gene (-1195G>A and $8_{473}$C>T) with chronic periodontitis in North Indians. Methods: Both SNPs and their haplotypes were used to explore the associations between $COX_2$ polymorphisms and chronic periodontitis in 56 patients and 60 controls. Genotyping was done by polymerase chain reaction followed by restriction fragment length polymorphism. Chi-square test and logistic regression analysis were performed for association analysis. Results: By the individual genotype analysis, mutant genotypes (GA and AA) of $COX_2$-1195 showed more than a two fold risk (odds ratio [OR]>2) and $COX_2$ $8_{473}$ (TC and CC) showed a reduced risk for the disease, but the findings were not statistically significant. Haplotype analysis showed that the frequency of the haplotype AT was higher in the case group and a significant association was found for haplotype AT (OR, 1.79; 95% confidence interval, 1.03 to 3.11; P=0.0370) indicating an association between the AT haplotype of $COX_2$ gene SNPs and chronic periodontitis. Conclusions: Individual genotypes of both the SNPs were not associated while haplotype AT was found to be associated with chronic periodontitis in North Indians.

Mixture of Expert 모형에 기반한 당뇨병 진단 분류 (Classification of the Diagnosis of Diabetes based on Mixture of Expert Model)

  • 당뇨병은 급성합병증을 예방하고 장기간의 합병증의 위험도를 감소하기 위하여 지속적인 치료와 환자 자가 관리 교육이 필요한 만성질환이다. 또한 전 세계적으로 당뇨병에 대한 유병률과 사망률이 대부분의 인구집단에서 역학적 비율에 도달하였다. 많은 연구에서 당뇨병에 대한 조기진단은 적절한 치료와 생활습관을 지키는 관리를 통하여 발병을 예방하는데 도움을 줄 수 있으며, 이를 통하여 당뇨병의 합병증을 감소시키고 생존률을 향상시킬 수 있다고 보고하고 있다. 본 연구는 PIMA Indians 당뇨 데이터에 대하여 mixture of expert 모형을 적용하여 당뇨유병환자의 여부를 분류하고, 이를 로지스틱 회귀분석, 신경망분석의 성능과 비교함으로서 그 유용성을 주장하고자 하였다. 연구결과 정확도 및 ROC 곡선, c-통계량에서 ME 모형이 다른 분류도구들에 비해서 높게 나타남을 확인할 수 있었다.

지의류 추출염액에 의한 견섬유 염색 (Dyeing of Silk with Lichen Extract)

  • Lichen had been used as dyestuffs before the discovery of synthetic dyes in Europe and by American Indians. To investigate the dyeability of Korean native lichens, 10 kinds of lichen were collected and colorants were extracted from them by boiling in water and fermenting in ammonia water. Silk fabrics were dyed with lichen extract, then the colour differences of dyed silk fabrics were measured before and after color fastness test. The most part of the lichen extracts(Fc, Hh, Me, Pl, Xm, Pr, Cj, Ld, Ry) dyed silk yellowish or reddish brown. The range of colors were 2.4Y 6.5∼10YR 6/7. Pa extract dyed silk purple(1.25RP 5/6). Color differences of laundered specimen with the silk before laundering were very small, but color of the dyed silks after exposing to light showed big differences with the specimen before exposing to light. The colors of the silks dyed with lichens changed from yellowish or reddish brown to greenish brown by laundering and by exposing to light. But the specimen dyed Pa extract changed from purple to blueish purple by laundering and reddish purple by exposing to light.

Association of Hepatocyte Nuclear Factor-$4{\alpha}$ (HNF-$4{\alpha}$) Polymorphisms (rs1884614) with Type 2 Diabetes in Korean Population

  • The hepatocyte nuclear factor-$4{\alpha}$ (HNF-$4{\alpha}$), transcription factor involved in the regulation of serum lipid and glucose levels, has recently been reported to be associated with type 2 diabetes. Therefore, we investigated the genotype for the rs1884614 of HNF-$4{\alpha}$ gene in Korean population and compared genotype of patients with control group. 100 patients (Male 63, Female 37), who previously underwent type 2 diabetes (T2DM) and 100 controls (Male 36, Female 64) participated in this study. According to our present study there was no association between rs1884614 polymorphism in HNF-$4{\alpha}$ gene and T2DM in Koreans although other reports showed that HNF-$4{\alpha}$ polymorphisms might be associated with the pathogenesis of T2DM in Pima Indians et al. We assume that this finding should contribute to understanding of type 2 diabetes in Korean population in detail at genetic level.

브라질의 '탄생': 16세기 유럽과 브라질 보고문학에 나타난 이상향 (Birth of Brazil: Utopianism in Europe and Brazilian Informative Literature of Sixteenth Century)

  • This paper deals with the study of how the 'birth' of Brazil happened and how the European utopianism was represented in Brazilian Informative Literature of the sixteenth century, that is, the first literary manifestations of Brazil. There were at that time the Renaissance humanism and the scientific development that encouraged dozens of illustrious navigators for new discoveries ultra-seas, like Christopher Columbus who discovered America. Writings such as Letter to King Manuel of Pero Vaz de Caminha, had mostly an intention to inform Europeans about climates, indians and possibility to discover gold or silver. Main narrative characteristics were uncertainty and exageration, which ironically helped to attract more discoveries and explorations in the New World. Americo Vespucio's Mundus Novus inspired Thomas More to write Utopia, in which the author described through a Portuguese sailor the ideal but unrealizable society. Utopianism regarding the imaginary island of 'Brazi', well known among Europeans since long ago, may have influenced the current name of the country: 'Brazil'. On the other hand, utopianism shown in Brazilian Informative Literature worked as a justification for Europeans to explore and colonize the New World.

Anatomy acts concerning body and organ donations across the globe: past, present and future with a special emphasis on the indian scenario

  • From the era of pre-historic times, the ancient Indians and the Greeks highlighted the importance of body and organ donations thereby emphasizing the need for anatomical sciences in medicine through the use of effective dissections for the same. However, after the Renaissance, there was a surge in dissections throughout the world, particularly in Europe, as a result of which various laws were enacted by governments concerning the procurement of bodies for the purpose of scientific dissections, which were later promulgated throughout the world through various anatomical acts. The situation in India was quite similar to that of Britain until its independence in 1947, after which different Indian states formulated their own anatomy acts that had their own merits and pitfalls. Hence, this literature review serves to highlight the various acts throughout history and would serve as a guide to emphasize the future perspectives of formulating a centralized unified anatomy act for the Indian nation that could possibly be the need of the hour.
