A Systematic Review on the Causative Medicines for Stevens-Johnson Syndrome (스티븐스-존슨증후군을 유발하는 주요 의약품별 위험도에 대한 체계적 문헌고찰)

  • Background: Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are immune-complex-mediated hypersensitivity reactions that predominantly involve skin and mucous membranes. Despite the low incidence, both are considered medical emergencies as the mortality rate has been estimated at 30-50%. Although as many as half of cases are idiopathic, several drugs have been implicated as main cause of SJS/TEN. This review therefore aimed to identify drugs that were potentially associated with SJS/TEN and compare the relative risk of the medications. Method: A comprehensive search was performed using MEDLINE, EMBASE and 5 Korean databases. We defined study drugs as non-steroidal anti-inflammatory drugs (NSAIDs), antibiotics, antiepileptics, and allopurinol. Only epidemiologic studies investigating associations between the above drugs and drug-induced SJS/TEN were included. Two reviewers independently selected and evaluated candidate papers and extracted odds ratios or incidence rates. Meta-analysis was performed only for drugs that were reported from 4 or more studies. Results: We found 8 case-control studies, 3 cohort studies and 1 RCT. The ranges of adjusted ORs were 0.6-34.0 for NSAIDs, 1.6-302.0 for antiepileptics, 0.3-10.0 for antibiotics and 1.0-187.0 for allopurinol. The drug with the highest incidence of SJS/TEN was carbamazepine (40 persons/1,000 DDD). Conclusion: Finally, the risk was highest in first 8 weeks after onset of treatment in all drugs.

Review for Clinical Studies of Oriental Medicine on the Prevention of Ovarian Hyperstimulation Syndrome (난소과자극증후군의 예방에 관한 한의 임상 연구 고찰)

  • Objectives: This review is aimed at assessing the efficacy and effectiveness of oriental medicine for the prevention of Ovarian Hyperstimulation Syndrome (OHSS) through literature research and overview. Methods: Database searching was conducted to identify relevant randomized controlled trials (RCTs) on oriental medicine for the prevention of Ovarian Hyperstimulation Syndrome. Studies were searched from Journal of Korean Obstetrics and Gynecology, Korean studies Information Service System, Korean Medical Database, China National Knowledge Infrastructure, Cochrane library, PubMed and EmBase up to 7th November, 2019. Results: Ten RCTs were finally selected. Eight studies intervened with oral Chinese herb medicine, one is intervened with Chinese medicine enema and the other with acupuncture. Eight studies concluded that intervention with oriental medicine significantly decreased OHSS incidence. Five studies showed significantly higher pregnancy rate in the intervention groups. Two studies reported higher ovulation rate and other two studies showed more maturated eggs than the control groups. Four studies showed opposite results in serum Estradiol level. Vascular Endothelial Growth Factor level was significantly lower in the intervention groups in two studies. Conclusions: From ten studies, oriental medicine reduced OHSS incidence rate and showed preventable effectiveness. Further strictly designed studies and acupuncture intervened studies are needed to establish evidences.

Studies on the Incidence of Reproductive Disorder in Hanwoo (한우 암소의 번식장애에 관한 조사 연구)

  • A survey was carried out to investigate the symptoms and occurance of reproductive disorder in Hanwoo(Korean native cattle). Data of the reproductive disorder of 561 heads from 28 farm households have been collected from Dec. '95 to Nov. '96 and analyzed calving no, nutritional body condition and housing forms. The results obtained are summarized as follows: 1. The incidence of reproductive disorder was 20.1% and the major common symptoms were repeat breeding(39.8%), anestrous(31.0%), nymphomania(10.6%) and subestrous(8.9%). 2. The incidence of reproductive disorder according to the calving no., that of heifer, calved one to two calving no. and over three calving no. were 20.6%, 13.9% and 34.5%, respectively. 3. Reproductive disorder incidence according to the nutritional body condition was 18.3%, 14.6% and 48.7% at body condition score(BCS) less than 2.0, 2.5 to 3.0 and over than 3.5, respectively. At BCS over 3.0, the symptoms of common reproductive disorder were repeat breeding(17.6%), anestrous(12.2%), nymphomania(10.8%) and subestrous(2.7%). 4. The incidence of reproductive disorder according to the housing form 15.8% and 34.6% for group feeding in open house and individual stanchion feeding in stall, respectively. In group feeding, reproductive disorder incidence of cows raised in space of more than 9.9$m^2$ per head was 14.1%, while that of cows raised less than 9.9$m^2$ per head was 18.2%. And incidence of repeat breeding, aneestrous and subestrous was more frequent in individual stanchion feeding than group feeding.

Trend Analysis of Lung Cancer Incidence Rates in Ninawa Province, Iraq, from 2000 to 2010 - Decrease and Recent Stability

  • Background: Lung cancer is the most frequent malignancy of men worldwide. In Ninawa in Iraq, lung cancer ranks first among cancers diagnosed in men. Since no prior studies have been conducted on incidence trends in our population the present investigation of rates during 2000-2010 was therefore performed. Materials and Methods: Registy data for lung cancer cases were collected from the Directorate of Health in Ninawa-Mosul Continuing Medical Education Center. We restricted our analyses to men categorized according to the age groups of 0- 39, 40-49, 50-59, 60-69 and 70+ years. The significance of incidence rate trends during 2000-2010 was tested using Poisson regression. Age-standardized rates (ASR), and age-specific rates per 100,000 population were calculated. Results: A total of 1,206 incident lung cancer were registered among males, accounting for 15.5% of all male cancers registered during 2000-2010. It ranked first throughout the period. Median age at diagnosis was 69 (mean $66.8{\pm}11.0$) years. The incidence rate of all male lung cancers in Ninawa (all ages) decreased from 26.4 per 100,000 in 2000 to 12.7 in 2010 (APC=-6.55%, p<.0001). The incidences in age groups 40-49, 50-59, 60-69 and 70+ decreased in earlier years and recently appeared (2007-2010) stable. The incidence in age group (0-39) remained stable between 2000-2010. Squamous cell carcinoma (SCC)was the most common type of lung cancer, while adenocarcinoma was relatively rare. Conclusions: With the data from Directorate of Health in Ninawa during the period 2000-2010, lung cancer is the most common cancer but generally declining. Among all age groups, the recent incidence of lung cancer remained stable. The SCC predominance suggests change in tobacco habits as an important factor in the trends observed.

The Incidence of Aneurysmal Subarachnoid Hemorrhage in Youngdong District, Korea

  • Objective : The purpose of this study is to investigate the incidence of aneurysmal subarachnoid hemorrhage (SAH) in Youngdong district for 10 years. Methods : From Jan. 1997 to Dec. 2006, 732 patients (327 males, 405 females, mean age: $54.8{\pm}13.1$ years) with spontaneous SAH were admitted to our hospital. We reviewed the medical records and radiological findings regarding to the ictus of SAH, location and size of the ruptured aneurysms, Hunt-Hess grade and Fisher grade on admission, personal details such as address, age, and sex, and previous history of medical diseases. Results : In these 732 patients, 672 cases were confirmed as aneurysmal SAH. Among them, 611 patients (262 males, 349 females, mean age: $54.9{\pm}13.2$ years) came from Youngdong district. The average crude annual incidence of aneurysmal SAH for men, women, and both sexes combined in Youngdong district was $7.8{\pm}1.7$, $10.5{\pm}2.7$, and $9.1{\pm}2.1$ per 100,000 population, respectively. Because of the problems related to the observation period and geographical confinement, it was suspected that the representative incidence of aneurysmal SAH in Youngdong district should be made during the later eight years in six coastal regions. Therefore, the average age-adjusted annual incidence for men, women, and both sexes combined was $8.8{\pm}1.4$, $11.2{\pm}1.3$ and $10.0{\pm}1.0$, respectively in the coastal regions of Youngdong district from 1999 to 2006. Conclusion : In overall, our results on the incidence of aneurysmal SAH was not very different from previous observations from other studies.

The Incidence of Stroke by Socioeconomic Status, Age, Sex, and Stroke Subtype: A Nationwide Study in Korea

  • Objectives: To date, studies have not comprehensively demonstrated the relationship between stroke incidence and socioeconomic status. This study investigated stroke incidence by household income level in conjunction with age, sex, and stroke subtype in Korea. Methods: Contributions by the head of household were used as the basis for income levels. Household income levels for 21 766 036 people were classified into 6 groups. The stroke incidences were calculated by household income level, both overall within income categories and further by age group, sex, and stroke subtype. To present the inequalities among the six ranked groups in a single value, the slope index of inequality and relative index of inequality were calculated. Results: In 2005, 57 690 people were first-time stroke patients. The incidences of total stroke for males and females increased as the income level decreased. The incidences of stroke increased as the income level decreased in those 74 years old and under, whereas there was no difference by income levels in those 75 and over. Intracerebral hemorrhage for the males represented the highest inequality among stroke subtypes. Incidences of subarachnoid hemorrhage did not differ by income levels. Conclusions: The incidence of stroke increases as the income level decreases, but it differs according to sex, age, and stroke subtype. The difference in the relative incidence is large for male intracerebral hemorrhage, whereas the difference in the absolute incidence is large for male ischemic stroke.

A new method to predict the critical incidence angle for buildings under near-fault motions

  • It is well known that the incidence angle of seismic excitation has an influence on the structural response of buildings, and this effect can be more significant in the case of near-fault signals. However, current seismic codes do not include detailed requirements regarding the direction of application of the seismic action and they have only recently introduced specific provisions about near-fault earthquakes. Thus, engineers have the task of evaluating all the relevant directions or the most critical conditions case by case, in order to avoid underestimating structural demand. To facilitate the identification of the most critical incidence angle, this paper presents a procedure which makes use of a two-degree of freedom model for representing a building. The proposed procedure makes it possible to avoid the extensive computational effort of multiple dynamic analyses with varying angles of incidence of ground motion excitation, which is required if a spatial multi-degree of freedom model is used for representing a building. The procedure is validated through the analysis of two case studies consisting of an eight- and a six-storey reinforced concrete frame building, selected as representative of existing structures located in Italy. A set of 124 near-fault ground motion records oriented along 8 incidence angles, varying from 0 to 180 degrees, with increments of 22.5 degrees, is used to excite the structures. Comparisons between the results obtained with detailed models of the two structures and the proposed procedure are used to show the accuracy of the latter in the prediction of the most critical angle of seismic incidence.

Incidence Analysis of Recurrent Milk Fever in Korean Domestic Dairy Cattle (국내 사육중인 젖소에서 발생하는 재발성 유열의 특징 분석)

  • Milk fever is a metabolic disease with manifestation of clinical signs due to hypocalcemia, which usually occurs within 48-72 h after delivery. However, even after a successful treatment of milk fever, recurrence of milk fever may occur, and studies on recurrent milk fever are still lacking. Accordingly, the present study was conducted for the purpose of identifying the characteristics of recurrent milk fever according to farm, season, parity, and dystocia that can cause physiological changes in the mother during peri- and postpartum periods. The analysis results showed that the incidence rate of initial and recurrent milk fever according to breeding farm was 5.7%-14.1% and 3.1%-7.2%, respectively, demonstrating a positive correlation between the initial and recurrent milk fever (r = 0.613, p < 0.01). With respect to season, the incidence rate of initial and recurrent milk fever during summer was 12.3% and 7.5%, respectively, which were significantly higher than that of other seasons (p < 0.05). In addition, the recurrence rate, the ratio of recurrence relative to initial milk fever, was highest during summer with 62.7%. Regarding parity, the incidence rate of initial and recurrent milk fever in 3rd parity was 11.1% and 5.8%, respectively, which was significantly higher than in 1st and 2nd parity (p < 0.05). Furthermore, the recurrence rate in 4th parity was 64.1%, showing a pattern of increase in incidence rate with increase in parity. Finally, there were no differences in the incidence rate of initial and recurrent milk fever according to eutocia and dystocia. The findings indicated that the incidence rate of initial milk fever should be reduced to effectively prevent the recurrent milk fever, while animals with 3rd parity or higher should be expected to occur high rate of recurrent milk fever, especially during summer, and the necessary preparations should be made for intensive treatment of such individuals.

Incidence of congenital hypothyroidism by gestational age: a retrospective observational study

  • Background: Congenital hypothyroidism (CH) is the leading cause of preventable physical and intellectual disabilities. This study aimed to assess the incidence and clinical characteristics of CH in newborns. Methods: We retrospectively reviewed the medical records of all newborns delivered at the Pusan National University Hospital between January 2011 and March 2021. The incidence of CH was compared according to gestational age, birth weight, and small for gestational age (SGA). The patients aged ≥3 years who could not maintain normal thyroid function and required levothyroxine treatment were diagnosed with permanent CH. Logistic regression analysis was performed to compare CH risks. Results: Of 3,722 newborns, 40 were diagnosed with CH (1.07%). Gestational age and birth weight were significantly associated with CH incidence. The odds ratios (ORs) of CH in infants delivered at 32-37, 28-31, and <28 weeks were 2.568 (95% confidence interval [CI], 1.141-5.778), 5.917 (95% CI, 2.264-15.464), and 7.441 (95% CI, 2.617-21.159) times higher, respectively, than those delivered at term. The ORs of CH in infants weighing 1,500-2,499 g, 1,000-1,499 g, and <1,000 g were 4.664 (95% CI, 1.928-11.279), 11.076 (95% CI, 4.089-29.999), and 12.544 (95% CI, 4.350-36.176) times greater, respectively, than those in infants weighing ≥2,500 g. The OR of CH was 6.795 (95% CI, 3.553-13.692) times greater in SGA than in non-SGA infants. Conclusion: The CH incidence in South Korea has increased significantly compared with that in the past. Gestational age, birth weight, and SGA were significantly associated with CH incidence.
