저온, 고압력용 강재 구조물의 용접부균열 발생과 그 대책에 관한 연구 (A study on the cracking mechanism of the welded parts in steel structures for the use of low temperature and high pressure)

  • When the low temperature service steels are used as materials for welded structure, some problems-brittleness and weld cracking, etc.-occur in welded part due to the change of mechanical and metallurgical characteristics resulted from the thermal cycle during the welding procedure. In this study, the experiments were conducted to investigate the change of mechanical and metallurgical characteristics of the welded part for the low temperature and high pressure service steels. Moreover, the Static and Dynamic Implant Test Method was introduced to this study in order to find out the mechnism of weld cracking. In addition, the fracture toughnesses of welded bond were inspected under the various low temperature environments. Main results obtained are as follows; 1) The effect of the hydrogen on the fatigue characteristics of the weld bond can be estimated by the new self-contrived Dynamic Implant Test equipment. 2) The fine micro-structure and low hardness in the heat affected zone can be obtained by the small heat input multi-pass welding. 3) The susceptibility of the delayed cracking is largely affected by the condition of used electrode. 4) The transition temperature of the fracture surface in weld bond appears to be higher 20 .deg. C than that in base metal.

Late reconstruction of extensive orbital floor fracture with a patient-specific implant in a bombing victim

  • Fractures of the orbital floor and walls are among the most frequent maxillofacial fractures. Virtual three-dimensional (3D) planning and use of patient-specific implants (PSIs) could improve anatomic and functional outcomes in orbital reconstruction surgery. The presented case was a victim of a terrorist attack involving improvised explosive devices. This 58-year-old female suffered severe wounds caused by a single piece of metal from a bomb, shattering the left orbital floor and lateral orbital wall. Due to remaining hypotropia of the left eye compared to the right eye, late orbital floor reconstruction was carried out with a personalised 3D printed titanium implant. We concluded that this technique with PSI appears to be a viable method to correct complex orbital floor defects. Our research group noted good aesthetic and functional results one year after surgery. Due to the complexity of the surgery for a major bony defect of the orbital floor, it is important that the surgery be executed by experienced surgeons in the field of maxillofacial traumatology.

Interdisciplinary rehabilitation of a root-fractured maxillary central incisor: A 12-year follow-up case report

  • Single-tooth implantation has become a common treatment solution for replacement of a root-fractured maxillary incisor in adults, but the long-term esthetic results can be unfavorable due to progressive marginal bone loss, resulting in gingival recession. In this case report, a maxillary central incisor with a root fracture in its apical one-third was orthodontically extruded and extracted in a 21-year-old female. Implant surgery was performed after a 3-month healing period, and the final crown was placed about 12 months after extraction. After 12 years, favorable osseous and gingival architectures were visible with adequate bone height and thickness at the buccal cortical plate, and no gingival recession was seen around the implant-supported crown. Although modern dentistry has been shifting toward simplified, clinical procedures and shorter treatment times, both general dentists and orthodontists should be aware of the possible long-term esthetic advantages of orthodontic extrusion of hopelessly fractured teeth for highly esthetically demanding areas and should educate and motivate patients regarding the choice of this treatment solution, if necessary.

상악 완전 무치악 환자에서 CM LOC® Pekkton® attachment를 이용한 임플란트 유지 피개의치 수복 증례 (Implant-retained overdenture with CM LOC® Pekkton® in maxillary edentulous patient)

  • 무치악은 총의치나 임플란트 보철물로 수복이 가능하며 임플란트를 이용한 피개의치는 총의치보다 유지와 안정이 우수하고, 임플란트 고정성 보철물에 비해 저렴하고 심미적이다. CM $LOC^{(R)}$ $Pekkton^{(R)}$ 어태치먼트는 여자부가 poly-ether-ketone-ketone으로 마모저항성이 뛰어나다. 한편, SR Ivocap system은 가압주사성형법으로 의치의 수직고경 변화가 적고 강도가 우수하다. 본 증례에서 상악은 4개의 임플란트 식립 후 CM $LOC^{(R)}$ $Pekkton^{(R)}$ 어태치먼트와 SR Ivocap system을 이용한 임플란트 유지 피개의치를, 하악은 가철성 국소의치를 제작하였고 심미적 및 기능적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

슬관절 전치환술에서 후방 안정 임플란트의 오정렬이 경골 기둥에 미치는 영향 (Influence of Malalignment on Tibial Post in Total Knee Replacement Using Posterior Stabilized Implant)

  • Recently, it has been reported that the posterior stabilized implant, which is clinically used for the total knee replacement (TKR), may have failure risk such as wear or fracture by the contact pressure and stress on the tibial post. The purpose of this study is to investigate the influence of the mal alignment of the posterior stabilized implant on the tibial post by estimating the distributions of contact pressure and von-Mises stress on a tibial post and to analyze the failure risk of the tibial post. Finite element models of a knee joint and an implant were developed from 1mm slices of CT images and 3D CAD software, respectively. The contact pressure and the von-Mises stress applying on the implant were analyzed by the finite element analysis in the neutral alignment as well as the 8 malalignment cases (3 and 5 degrees of valgus and varus angulations, and 2 and 4 degrees of anterior and posterior tilts). Loading condition at the 40% of one whole gait cycle such as 2000N of compressive load, 25N of anterior-posterior load, and 6.5Nm of torque was applied to the TKR models. Both the maximum contact pressure and the maximum von-Mises stress were concentrated on the anterior-medial region of the tibial post regardless of the malalignment, and their magnitudes increased as the degree of the malalignment increased. From present result, it is shown that the malalignment of the implant can influence on the failure risk of the tibial post.

A 5-year retrospective clinical study of the Dentium implants

  • PURPOSE. The aim of this retrospective study was to evaluate cumulative survival rate (CSR) of Implantium implants followed for 5 years and association between risk factors and the CSR. MATERIALS AND METHODS. A total of two hundred forty-nine Implantium Implants System (Dentium, Seoul, Korea) placed in ninety-five patients from 2004 to 2009 were investigated with several identified risk factors (sex, systemic disease, smoking, alchohol, reason of tooth loss, length, arch (maxilla or mandible), replace tooth type (incisor, canine, premolar or molar) Kennedy classification, prosthodontic type, prosthodontic design, opposite dentition, abutment type, occlusal material, occlusal unit, splint to tooth, cantilever, other surgery). Clinical examination (mobility, percussion, screw loosening, discomfort, etc.) and radiographic examination data were collected from patient records including all problems during follow-up period according to protocols described earlier. Life table analysis was undertaken to examine the CSR. Cox regression method was conducted to assess the association between potential risk factors and overall CSR. RESULTS. Five of 249 implants were failed. Four of these were lost before loading. The 5-year implant cumulative survival rate was 97.37%. Cox regression analysis demonstrated a significant predictive association between overall CSR and systemic disease, smoking, reason of tooth loss, arch, Kennedy classification and prosthodontic design (P<.05). The screw related complication was rare. Two abutment screw fractures were found. Another complications of prosthetic components were porcelain fracture, resin facing fracture and denture fracture (n=19). CONCLUSION. The 5-year CSR of Implantium implants was 97.37 %. Implant survival may be dependent upon systemic disease, smoking reason of tooth loss, arch, Kennedy classification and prosthodontic design (P<.05). The presence of systemic diseases and combination of other surgical procedures may be associated with increased implant failure.

Comparison of the torque stability of Implant Torque Controllers

  • Tightening of the screws in implant restorations should be accurate and precise. If applied torque is too low, screw loosening would be occurred. With too high torque, the screw fracture might take place. Various torque generating devices are developed and employed to apply a proper torque. The purpose of this investigation was to determine and compare the accuracy of the torque controllers. In this study, 4 types of torque controllers were used; electronic torque controller, torque limiting device, torque indicating device and contra angle torque driver. Digital torque gauge was employed to measure the de-torque value. Thirty cycles of tightening and loosening were done with each torque controller. All implant torque controllers have shown slight errors and deviations. The torque liming device exhibited the most accurate data. No significant difference was found among the mean de-torque values of the electronic torque controller, torque indicating device and contra angle torque driver. In the limitation of this study, it would be recommended that the implant torque controllers should be checked whether uniformed and precise torque can be generated and a measuring error should be corrected.

임플란트용 실링 어버트먼트의 개발 및 구조해석을 통한 성능분석 (Development of a Sealing-Type Abutment for Implant and the Performance Evaluation via Structural Analysis)

  • Currently, dental implants are widely used as artificial teeth due to their good chewing performance and long life cycle. Generally, a dental implant consists of an abutment as the upper part and a fixture as the lower part. When chewing forces are repeatedly applied to a dental implant, a gap is often generated at the interfacial surface between the abutment and the fixture, and it results in some deterioration such as loosening of the fastening screw, dental retraction and fixture fracture. To enhance the sealing performance for coping with such problems, this study proposes a new sealing-type abutment having a number of grooves along the conical surface circumference, and it carries out finite element analysis in consideration of the external chewing force and pretension between the abutment and the fixture. The result shows that the proposed sealing-type abutment shows an enhanced sealing performance in terms of the contact pressure in comparison with a conventional abutment.

Multiple implant therapy with multiple inductions of general anesthesia in non-compliant patients with schizophrenia: A case report

  • The effectiveness of dental implants in patients with disability, who are non-compliant during treatment, is controversial because of their poor oral health. Thus, oral health-care and management in such patients is concerning. Moreover, limited information is available on prognosis after implant placement. Herein, we describe a patient with schizophrenia who underwent dental implantation under multiple inductions of general anesthesia (5 times) and required conservative treatment and tooth extraction for multiple dental caries and retained roots because of inadequate oral health-care. Postoperatively, fracture of the prosthodontics and progression of dental caries were observed, and with 3 additional inductions of general anesthesia, conservative treatment, implant surgery, and prosthesis implantation were conducted. Postoperative 12-month follow-up since the last prosthesis implantation showed successful results. For patients with schizophrenia, multiple implantation can reduce horizontal bone loss and achieve aesthetic results compared to treatment with removable prosthodontics and could serve as an alternative treatment modality.

Reconstruction of extended orbital floor fracture using an implantation method of gamma-shaped porous polyethylene

  • Background: The conventional surgical method for reconstructing orbital floor fractures involves restoration of orbital continuity by covering an onlay with a thin material under the periorbital region. However, in large orbital floor fractures, the implant after inserting is often dislocated, leading to malposition. This study aimed to propose a novel implanting method and compare it with existing methods. Methods: Among patients who underwent surgery for large orbital floor fractures, 24 who underwent the conventional onlay implanting method were compared with 21 who underwent the novel ${\gamma}$ implanting method that two implant sheets were stacked and bent to resemble the shape of the Greek alphabet ${\gamma}$. When inserting a ${\gamma}$-shaped implant, the posterior ledge of the orbital floor was placed between the two sheets and the bottom sheet was impacted onto the posterior wall of the maxilla to play a fixative role while the top sheet was placed above the residual orbital floor to support orbital contents. Wilcoxon signed-rank test and Mann-Whitney U test were used for data analyses. Results: Compared to the conventional onlay method, the gamma method resulted in better restoration of orbital contents, better improvement of enophthalmos, and fewer revision surgeries. Conclusion: Achieving good surgical outcomes for extended orbital floor fractures is known to be difficult. However, better surgical outcomes could be obtained by using the novel implantation method of impacting a ${\gamma}$-shaped porous polyethylene posteriorly.
