치과 임플란트 실패와 합병증의 후향적 연구 (Dental implant failures and complications - Retrospective study)

  • The dental implants in modern dentistry brought out a new era in everyday dental practice. Increasing of dental implants usage, various failures and complications has occurred. Failures and complications of dental implant treatments that can happen in implant surgery, in mechanical or prosthetic problem and in biological aspect. The aim of this study was to assess implant failures for 6 years as well as find out how to overcome implant failure. In Jeju National University hospital, 26 dental implants in 17 patients were removed by implant failure through 2013.1.1.-2018.12.31. Each implant failure case was analyzed in report form with various informations about failure retrospectively. The present study showed 26 failed dental implants of 17 patients were removed. 13 implants(50%) of 8 patients(47.06%) were failed before functional loading and 13 implants(50%) of 9 patients(52.94%) were failed after functional loading. 11 implants(42.31%) of 7 patients(41.18%) with medical disease were extracted. 6 implants(23.08%) of 5 patients(29.41%) with additive bone graft were failed. We discuss with 26 failed dental implants of 17 patients about their causes, solutions and prevention retrospectively for 6 past years. Precise diagnosis and treatment planning are needed. Medically compromised patients and patients with implants and additive bone grafts should be cautiously treated with high failing possibilities.

임플란트 식립 전 연조직 증대를 위한 교정적 정출술 (Forced orthodontic eruption for augmentation of soft tissue prior to implant placement)

  • 교정적 정출술(Forced orthodontic eruption, FOE)은 각화 치은과 치조골량을 증가시키는 비침습적인 치료 방법이다. 정출력에 의해 치아가 정출되면 치은 섬유와 치주 인대가 신장되고 조골 세포의 유도가 촉진되어 새로운 골형성이 일어나게 된다. 또한 치은 변연이 치관 방향으로 이동되면서 부착 치은의 폭경이 증가된다. 반면, 점막 치은 경계는 일정하게 유지가 된다. 이러한 치료 효과들로 인해 교정적 정출술은 implant site development를 위해 사용될 수 있다. 본 증례 보고에서는 외상으로 인한 치관 파절로 수복이 불가능한 상악 전치부를 교정적 정출술로 연조직 증대를 이룬 후 치조골 이식술(block bone grafting)과 임플란트 식립을 통해 심미적으로 수복한 증례를 통해 교정적 정출술의 효과를 기술하고자 한다.

  • Various imaging modalities have been used for dental implant assessment in the different stages of implant treatment. Basic imaging, such as panoramic and periapical radiographs, are generally useful and cost-effective but do not provide the cross-sectional visualization or interactive image analysis that can be obtained with more sophisticated imaging techniques, such as Cone-Beam Computed Tomography(CBCT) imaging. This article includes the applications of CBCT imaging and their diagnostic contribution to presurgical evaluation, treatment planning, and postoperative assessment of dental implants.

나사풀림과 나사파절이 동반된 실패한 임플란트 고정성 보철물의 회복증례 (Reconstruction of failed fixed implant prosthesis accompanied by abutment screw loosening and fracture : A case report)

  • In the case of failed fixed implant prosthesis accompanied by abutment screw fracture, fractured screw fragment must be removed to use the existing implant fixtures. A 61-year-old male patient, who had a failed maxillary fixed implant prosthesis accompanied by three abutment screw fracture, hoped to reconstruct the maxillary implant prosthesis, while maintaining the existing implant fixtures. To use the existing implant fixtures, fractured screw fragments were removed. A maxillary implant overdenture using available existing implants was planned. Bar-attachment with Locator was used for implant splinting, denture stability, and retention. Final impression was taken after treatment of peri-implantitis. Jaw relation registration was taken to evaluate available interarch space for bar-attachment. After fabricating bar-attachment, centric relation was taken. Implant overdenture using bar-attachment with Locator was delivered after wax-denture evaluation. This case report showed that a satisfactory clinical result was achieved by implant overdenture using existing implant fixtures in a maxillary edentulous patient.

BONE RESPONSE OF THREE DIFFERENT SURFACE IMPLANTS : HISTOMORPHOMETRIC, PERIO TEST VALUE AND RESONANCE FREQUENCY ANALYSIS IN BEAGLE DOGS

  • Statement of problem. The intial stability for osseointegration of implant has been an interesting factor. Especially, in the case of poor bone quality or immediately loaded implant, various strategies have been developed focusing on the surface of materials to improve implant fixation to bone. The microscopic properties of implant surfaces play a major role in the osseous healing of dental implants. Purpose. The aims of this study are to perform a histologic and histomorphometric comparison of the healing characteristics of three different surfaces and the comparison of resonance frequency analysis (RFA) values measured by $Osstell^{TM}$ and perio-test values (PTV) measured by Periotest. Material and methods. A total of 24 screw titanium implants (Dentium Co., Seoul, Korea) with 6mm in length and 3.4mm in diameter, were placed in the mandible of 4 beagle dogs. Implants were divided into three groups following the surface treatment methods: Group I is machined(control group). Group II is anodically oxidized. Group III is coated 500nm in thickness with hydroxyapatite(HA) by ion beam assisted deposition(IBAD) on the anodized oxidization. Bone blocks from 2 dogs were caught after 3 weeks of covered healing and another blocks from 2 dogs after 6 weeks. RFA values and PTV were measured right after insertion and at 3 and 6weeks. Histomorphometric analysis was made with Kappa Image Base System to calculate bone-to-implant contact (BIC) and bone area inside the threads. Pearson's correlation analyses were performed to evaluate the correlation between RFA and PTV, BIC and bone area ratio of three different surfaces at 3 and 6 weeks. Results. 1) In all surface treatment methods, the RFA values decreased and the PTV values increased until 6 weeks in comparison to initial values. 2) At 3 weeks, no significant difference was found from bone-to-implant contact ratio and bone area ratio of three different surface treatment methods(P>0.05). However, at 6 weeks, different surface treatment methods showed significantly different bone-toimplant contact ratio and bone area ratio(P<0.05). 3) In the implants with the IBAD on the anodic oxidization, significant difference was found between the 3 weeks and the 6 weeks bone area ratio(P<0.05). 4) Correlation was found between the RFA values and the bone area ratio at 3 and 6 weeks with significant difference(P<0.05). Conclusions. These results indicate that the implants with the IBAD on the anodic oxidization may have a high influence on the initial stability of implant.

하악 부분 무치악 환자에서 Milled Bar와 부착장치를 이용한 임플란트 피개의치 수복 증례 (Implant overdenture using milled bar and attachment in partially edentulous mandible: a case report)

  • 부분 무치악 환자에 있어 심한 골소실으로 인해 치관부 수직높이가 과도해질 우려가 있는 경우에는 임플란트 식립 후 고정성 보철물로 수복시 임플란트 주변 골의 응력 증가 및 나사 풀림이 발생할 가능성이 증가할 수 있다. 이러한 경우에 milled bar 및 가철성 보철물로 수복할 수 있는데, milled bar는 단일 임플란트 간의 일차적인 스플린팅 효과와 안정성을 부여할 수 있다. 또한 milled bar 사용 시 Advanced Dental Device-Treatment Of Choice(ADD-TOC), 자석 부착장치 같은 부가적인 부착장치 사용을 통해 임플란트 피개의치의 추가 유지력을 부여할 수 있다. 본 증례의 환자는 하악 좌측 구치부 평활근육종으로 인한 절제술 후 심한 골소실 및 다수 치아를 상실한 부분 무치악 환자로 장기간의 하악 구치부 상실로 인한 대합치는 정출된 상태였다. 상악 좌측 대구치는 교정적 압하를 통해 교합평면을 회복하였고, 하악 좌측 대구치 부위는 임플란트 식립 후 milled bar에 부가적인 부착장치인 ADD-TOC, 자석 부가장치를 이용하여 임플란트 피개의치로 수복하였다. 이를 통해 장기적으로 기능적, 심미적인 만족할 만한 결과를 얻었기에 보고하는 바이다.

An implant-supported removable partial denture for a patient with post-inflammatory scar contracture caused by burn complications: a clinical report

  • The scars and contracture around the oral-facial region may cause difficulty in prosthodontic treatment to restore esthetics and function for the patients, who suffered severe burns. This article presents a technique that uses a fixed partial denture prepared with a conventional milling technique and an attachment to support anterior cantilever removable partial denture, thereby providing a more esthetically acceptable and functional result.

컴퓨터 보조 기반 치아 지지 서지컬 템프레이트를 이용한 상악구치부 임플란트 식립 (Implant Fixture Installation in the Posterior Maxilla Using a Tooth-supported Surgical Template Based on Computer Assisted Treatment Planning)

  • Two patients with partial edentulous maxilla were scheduled to undergo installation of implant fixtures using a tooth-supported surgical template based on computer assisted treatment planning. After 3-dimensional (3D) computed tomographic scanning was transferred to the OnDemand3D (Cybermed Co., Seoul, Korea) software program for virtual planning, fixtures of MK III Groovy RP implant of the Br${\aa}$nemark System (Nobel Biocare AB Co., G$\ddot{o}$teborg, Sweden) was installed using the In2Guide (CyberMed Co., Seoul, Korea) tooth-supported surgical template with a Quick Guide Kit (Osstem Implant Co., Seoul, Korea) system in the posterior maxilla of each patient. Sinus floor elevation with a xenogenic bone graft procedure was also performed simultaneously in one patient. Fixture installations were completed successfully without complications, such as sinus mucosa perforation, bony bleedings, fenestrations, or others. During the last two-year follow-up period after prosthetics delivery, each implant was found to be fine with no other minor complications. The entire procedures are reported and the literatures on use of tooth-supported surgical template was reviewed.

장기간 사용한 이중관 의치의 실패원인 및 자연치와 임플란트를 이용한 이중관 의치 수복증례 (Causes of failures of long-term used double crown denture and new rehabilitation with dental implant and tooth combined denture using remaining teeth and implants)

  • 치주적으로 지지가 불리한 소수의 잔존치가 남은 환자에게 있어 하이브리드 텔레스코픽형 이중관의치는 장기적으로 안정적인 예후를 보이는 치료가 된다. 편측 악궁에 소수 잔존치가 남고 반대측 악궁에 다수의 자연치가 존재 하는 경우 임플란트를 식립하여 자연치와 혼재된 형태의 이중관 의치를 제작 할 시 의치의 지지 및 안정요소의 향상과 더불어 저작력의 균형을 맞추어 줄 수 있게 된다. 본 증례는 55세 여자 환자로 만성 치주염으로 인해 치주 지지가 감소한 소수 잔존치 상태에서 자연치만으로 하이브리드 텔레스코픽형 이중관 의치 치료를 통해 9.5년간 의치를 사용하였다. 9.5년 후 지대치 파절로 내원 하여 임플란트의 식립을 통해 자연치와 임플란트가 혼재된 하이브리드 텔레스코픽형 이중관 의치로 재수 복 하였으며, 이를 통해 재 치료에서도 장기적 예후의 안정성을 기대 할 수 있다.

임플란트 즉시 식립에 의한 연속된 상악 전치부의 심미적 수복 증례 (Esthetic restoration in continuous maxillary anterior area using immediate implant placement: A case report)

  • 상악 전치부를 발치 해야 하는 경우, 즉시 식립을 통한 임플란트 고정성 보철물의 수복을 고려할 수 있다. 이 경우, 수술 횟수 및 치료기간의 감소, 흡수가 일어나지 않은 잔존 골상에 식립이 가능하다는 장점이 있으나, 연속된 상악 전치부인 경우 임플란트와 임플란트간 거리에 대한 고려 및 예지성 있는 치료를 위한 정확한 진단 및 치료계획의 수립이 추가적으로 고려되어야 한다. 본 증례에서는 63세 남환의 잔존치근을 발거 후 연장된 상악 전치부에 골이식을 동반한 2개의 임플란트 즉시 식립 및 임시 보철물을 장착 후, 임플란트 고정성 보철물로 수복하여 심미적, 기능적으로 만족할만한 임상 결과를 보였기에 이를 보고하는 바이다.
