Objectives: Recent studies have reported that delayed initiation of clozapine can affect clinical response in patients with treatment-resistant schizophrenia (TRS). This study aimed to explore the relationship between delayed initiation of clozapine and acute treatment response. Methods: Sixty-five inpatients with TRS who started clozapine for the first time were included through a retrospective chart review. Acute treatment response was defined as a 30% reduction in the Positive and Negative Syndrome Scale score or a Clinical Global Impression of Improvement score of 1 (very much improved) or 2 (much improved) at 4 weeks after initiating clozapine. Results: After meeting the TRS criteria, the mean delay for initiating clozapine was approximately 13.8 months. The delay was shorter in patients who showed a better response to clozapine in logistic regression analysis (p=0.037). Conclusion: Our findings suggest that reducing the delay in initiating clozapine increases the effectiveness of clozapine in patients with TRS.

Hazardous work zones (i.e., hot, warm, and cold) are typically established by emergency response teams during hazardous materials (HAZMAT) calls but less consistently for fire responses to segment personnel and response activities in the immediate geographic area around the fire. Despite national guidelines, studies have documented the inconsistent use of respiratory protective equipment by firefighters at the fire scene. In this case-series report, we describe warm zone gas levels using multigas detectors across five independent fire incident responses all occurring in a large South Florida fire department. Multigas detector data collected at each fire response indicate the presence of sustained levels of volatile organic compounds in the "warm zone" of each fire event. These cases suggest that firefighters should not only implement strategies for multigas detector use within the warm zone but also include respiratory protection to provide adequate safety from toxic exposures in the warm zone.

Purpose: This study was conducted to describe the experience of school health teachers in regard to the COVID-19 pandemic in South Korea. Methods: We conducted a qualitative study using content analysis. Ten school health teachers were recruited from 6 elementary schools, 2 middle schools and 2 high schools in Seoul, using purposive sampling. They participated in semi-structured in-depth interviews in person or using an online communication system from January to February, 2021. Interviews were transcribed verbatim and analysed using qualitative content analysis. Results: Three main categories and nine generic categories emerged from the analysis. Firstly, it was discovered that school health teachers had psychological and physical stress to cope with COVID-19 due to the 1) fear of the unexpected infectious disease, 2) burden of having to deal with it alone, 3) breakdown of personal life and physical exhaustion and 4) heavy duty as a health teacher caused by the lack of an organic cooperation system with institutions related to school infectious diseases. Secondly, school health teachers had an increased sense of empowerment in regard to infectious disease management as a result of 1) feeling rewarded and appreciated and 2) gaining confidence and trust in infectious disease management. Finally, school health teachers experienced the urgent need for an effective response strategy for infection control because of the 1) confusion over infectious disease response due to lack of practical manuals applicable to the field as well as training, 2) disappointing response system without an expert response team dedicated to managing infectious diseases in schools, and 3) growing awareness of the need for change. Conclusion: It is expected that school health teachers' experience of COVID-19 will be used as important data for building effective and advanced school infectious disease response systems.

Objective: To assess the effect of vitamin D administration on the skin blood flow response to occlusion and heat. Design: Cross-sectional study. Methods: Twenty age matched subjects; 10 who had diabetes and 10 who were controls were administered 4,000 IU of vitamin D3 for 3 weeks at breakfast. The function of the endothelial cells was evaluated in 2 ways; first, the response to 4 minutes of vascular occlusion of the skin was measured with a laser Doppler flow meter. Second, the skin blood flow response to local heat at 42 degrees C for 6 minutes was examined. Results: The results of the experiments showed that the blood flow response to heat was reduced after 3 weeks administration of vitamin D in the subjects with diabetes and in the control subjects (p<0.05). The response to occlusion was not significantly different within each group before and after vitamin D administration, but the group with diabetes had a significantly lower blood flow response to occlusion than did the controls (p<0.05). Conclusions: Acute doses of vitamin D may impair nitric oxide production and reduce blood flow to tissue during stressors in people with diabetes.
The purpose of this study is to determine the issues of supplementation and improvement to prepare for the outbreak of new infectious diseases such as new variants of coronavirus disease 2019 (COVID-19) to guide work for a strategic new response to infectious disease. Public officials of Jeju Special Self-governing Province and health personnel responding to COVID-19, working at six public health centers in Jeju-do region were administered a survey about additional preparations to be made in the future, based on the period when COVID-19 was treated legally as a first-class infectious disease. Frequency analysis was conducted on the collected data. The Likert 5-point scale and Kruskal-Wallis test were used to compare the scores for effective response to emerging infectious diseases according to demographics. Among the important factors identified for effective response to new infectious diseases, 'facilitation of cooperation with public institutions' and 'facilitation of cooperation with private institutions' had the highest scores. In the future, when a patient presents with a new infectious disease, the step that needs to be supplemented in each phase of the public health center's response is 'immediate response team operation'. Further, public health centers responded that 'expansion of dedicated personnel related to infectious diseases' needs to be improved to respond to new infectious diseases. Along with the results of this study, considering the difficulties experienced by health personnel responding to new infectious diseases in preparation for future outbreaks of new infectious diseases, and to respond effectively, detailed and clear guidelines for responding to quarantine of patients of new infectious diseases will be needed.
Climate-related events unevenly affect society, worsening mental health disparities among vulnerable populations. This paper highlights that lesbians, gays, bisexuals, transgender, queers, and other individuals identifying as sexual and gender minorities (LGBTQ+) (LGBTQ+) could be considered a climate-vulnerable population in the Philippines, one of the most climate-vulnerable countries. As such, this paper elucidated that LGBTQ+ Filipinos can be marginalized in climate response efforts due to their sexual orientation and gender minority identities. According to the minority stress theory, discrimination against LGBTQ+ individuals may predispose them to mental health problems. Thus, there is a need to institute an LGBTQ+ inclusive mental health response for climate-related events to address discrimination against LGBTQ+ individuals and uphold their mental health.
The purpose of this study was to identify the response patterns of Health Locus of Control (HLOC) and to analysis of the health promoting behaviors depending on the response patterns of HLOC in Korean adults. The sample was composed of 300 healthy adults ranged from twenties to seventies. In data analysis, SPSS PC/sup +/ program was utilized for descriptive statistics, pearson correlation, ANOVA and cluster analysis. The results of the study were as follows : 1. The average scores of HLOC in Korean were HLOC-I : 24.12(range 6-30), HLOC-P : .21.72, HLOC-C : 17.46. 2. The total mean score of the health promoting behavior scale was 2.50 (range 1-4), and the mean scores on the subscales were ; self actualization 2.60, health responsibility 2.14, exercise 2.28, nutrition 3.16, interpersonal support 2.74, stress management 2.15. 3. The HLOC-I and health promoting behavior were correlated positively(r =0.184, p=0.001), and the HLO-C and the health promoting behavior were correlated negatively(r=-0.102, p=0.039). The HLOC-P and the health promoting behavior weren't correlated in the level of statistical significance. 4. The response patterns of HLOC in Korean adults were identified 6 types, such as pure internal, pure chance, believers in control, yea sayer, nay sayer, and complex control. The type of believers in control and the type of yea sayer were subdivided in two different types. The believers in control was the largest group(33.67%), and yea sayer was the next large group(26.33%). The pure chance and the complex control type was the smallest groups. 5. There was a significant difference in health promoting behavior depending on the response patterns of HLOC(F=2.67, p=0.010). There were also significant differences in 4 subscales of health promoting behavior : self actualization(F=3.12, P=0.003), health responsibility(F=2.15, P=0.038), nutrition(F=5.08, p=0.000), and interpersonal support(F=2.26,p=0.029). These results suggest that the response patterns of HLOC is the important factor to predict the level of health promoting behavior in Korean adults.
Purpose: This study aimed to determine the mediating effects of aggressive stress response on the association between clinical practice stress and clinical practice satisfaction in ophthalmic optics majors and provide basic data that could help form opticians' professionalism and make clinical practice more effective. Methods: This study was conducted in 236 sophomores and juniors in Daegu and North Gyeongsang Province who had finished clinical practice. Hierarchical regression analysis and mediating effect testing were performed to determine the associations among clinical practice stress, clinical practice satisfaction, and aggressive stress response and determine the mediating effects of aggressive stress response on the association between clinical practice stress and clinical practice satisfaction. Results: Clinical practice stress was significantly negatively correlated with clinical practice satisfaction and aggressive stress response (p<0.01), and aggressive stress response was significantly positively correlated with clinical practice satisfaction (p<0.01). The association between clinical practice stress and clinical practice satisfaction was more likely to be explained by the addition of clinical practice stress than by being aggressive stress response controlled, which statistically significantly met the criteria for mediating effects (p<0.001). Conclusions: It is necessary to pay more attention to program development and teaching methods within the department of ophthalmic optics so that ophthalmic optics majors can utilize aggressive stress response to overcome clinical practice stress effectively and improve clinical practice satisfaction.
