Method :This study analyse and review existing data and documents related to traditional Korean medicine and health policy using contents analysis method. To collect the information on outcome measures evaluating the programmes, this study reviewed annual reports for health promotion programmes using traditional Korean medicine(HP-TKM) of Hub public health centers, as pilot public health centers, which have implemented the health promotion programmes collectively. Additionally, the review included research articles, government documents and book chapters on the topics related assessments in health promotion. Results :HP-TKM are stroke prevention education, smoke free program, health promotion according to Four Constitutional Medicine, home visiting treatment, etc. However, there are only a few studies of traditional medicine focused health promotion evaluation strategies. The benefits of health promotion programs using TKM can be categorized as non-health benefit, physiological, psychological and physical effects. To manage and monitor the intervention programmes efficiently, attention should be given to developing relevant and valid outcome measures for evaluating the programmes by government and public health center. Conclusion :Up to now, considering number of researchers, research projects undertaken or published articles and reports, within traditional Korean medicine there is a lack of capacity in research. Thus, government should pay more attention to developing relevant and valid outcome measures for evaluating the programs.

  • This study examines the relative efficiency of 34 local public medical centers in Korea. In order to concretely grasp the causes of inefficiency and suggest improvement solutions, the desirable set point was established based on the actual value which needs to be improved. As a result of analysis, we determined that output must be increased while input is held constant in order to improve effectiveness through the actual value and set point. Efficiency cannot be achieved if input is augmented solely to increase output. Also, to increase output, the medical centers need to improve their quality without increasing any input for customer visits and hospitalizations. Further, in order to strategically promote effectiveness and efficiency of local public medical centers, these medical centers must focus on input resource and patient interchange through the mutual cooperation with local private hospitals and university medical centers.

  • This study was conducted to assess the referral patterns to specialist from general practitioners in health center and perceived needs of referred patients for specialist care in health center. The study subjects were 249 patients who visited to health centers and were referred to other medical facilities. The data were obtained from questionnaire survey which was conducted in Kyongju-City Health Center, Seongju-Gun Health Center and Koryong-Gun Health Center in Kyongsangbuk-Do, from June 10 to October 17, 1995. The total referral rate was 2.7%. The proportion of patients who wished to be referred to medical specialists was 85.9%, and the proportion of patients referred by general practitioners in health centers was 14.1%. Among the patients who wished to be referred to medical specialists, 45.9% visited directly to health centers, 34.6% visited health centers via local clinics and 19.5% visited health centers to get referral permission only. The reasons for getting referral permission in health centers were easy geographical accessibility(34.6%), easy to get referral permission in health centers(27.1%), and very difficult to get referral permission in local clinics(16.4%). Among the diseases of referred patients, diseases of the musculoskeletal system and connective tissue were most prevalent on a whole, but diseases of nervous system and sensory organs were comparatively high among the patients who wished to be referred to medical specialists and infectious and parasitic diseases were comparatively high among the patients referred by general practitioners in health centers. The most favorable medical facility was general hospital including university hospital in both groups of patients who wished to be referred to medical specialist and the patient referred by general practitioners in health centers. Regarding the needs for specialist care in health center, 75.2% of patients who wished to be referred to medical specialists and 74.3% of patients referred by general practitioners in health center wanted the specialist care. The most frequently requested specialty is internal medicine(47.1%), and then orthopedics and general surgery. Based on above results, this study revealed that the majority of patients referred from health center wished to be referred to medical specialists at their own will, so, referral system at health center level should be changed. And if specialist care in health center be provided, the medical care by internist could be provided first, and then that of orthopedics and general surgery could be provided. These kinds of medical cares could be covered by local clinicians as a part-time job on a voluntary basis.

The study subjects were 146 health workers and 197 infants mothers. The results were as follows : At present, the only major nutrition services for infants were vaccination and dental care. Proper nutrition management services were available to infants. Nutrition knowledge scores were 16.8 for health workers and 15.3 for mothers out of 20 possible points. Health workers strongly demanded a well-organized nutrition education program, government support, audio-visual materials and the employment of a community nutritionist. The public health workers, in particular, demanded the development of education programs for breastfeeding and weaning. The infants mothers demanded services of nutrition information and teaching of cooking and menu planning. Based on this, the results suggest that the employment of a community nutritionist and the development of practical nutrition service programs for infants are needed very urgently for public health centers.

  • This study aimed at figuring out the current state of 26 family-health support centers in Gyeonggi-do, in order to prepare an activation plan for the centers. These centers are the main mediators of the family health operation. This study also aimed to understand the center users' satisfaction and demands. The results were as follows. First, a total of 65,794 participants from 9,960 businesses were processed. The majority had attended various integrated family services, followed by family counseling and/or family-friendly culture-establishing businesses. Second, the main users' profiles were as follows: average age, 41; average number of persons in the family, 3.78; education above foundation-level degree; diverse jobs that included special occupations; average monthly incomes of 3,390,000 won; and a middle-class living standard (as self-described by participants). Third, the users' main concern for their families was "financial difficulties," and they had an interest in education and counseling. Although they thought education and counseling were suitable solutions, they hesitated because they did not think their problems were serious enough to merit them. Fourth, the satisfaction level of their participation in their center's program was generally high, at 3.9-4.2 points. The majority of them said they would attend again in the future.

With the use of nursing models of health. professional nurses in nursing centers diagnose and treat human responses to potential and actual health problems and offer holistic, client-centered health service. In nursing centers professional nursing services include health education, health promotion, and health-related research. A nursing center is comprised of the advisory and exacutive commitee; the advisory commitee serves consultants and links community needs to the nursing center, while the director of the exacutive commitee identifies the potential resources to generate funds, support, and facilitate the activities of staffs in a nursing center. Nursing centers mobilize various financal resources for reimbursement of services from college and insurance companies, collect minimum service fees from the client, and further collect fees for providing programs to community groups, this also includes membership fees, and donations. The services provided by nursing centers focus on services related to primary prevention, health maintenance & health promotion, direct nursing care for acute & chronic diseases, and holistic care for actual and potential health problems. The client satisfaction for the services was high. Students also showed positive reponses for their clinical experiences and independent working conditions. 2. The degree of knowledge, attitudes, and practices for breast cancer. and an individual's intention to participate in the BSEE program. The subjects of this study were 308 females in K-city in the Republic of Korea. Data were collected using a self-administered questionnaire. The mean age of the respondents was 35.0 years old. Those who already participated in the BSEE were 64.9%, and those who had support and encouragement to practice BSE from significant others were 25.1 %. Clients intent to participate in the BSEE were 37.0%. The mean score of knowledge(2.4 out of 5 points) and practices(1.8 out of 5 points) for breast cancer were quite low, but the mean score of attitudes was relatively positive04.5 out of 20 point) for breast cancer. Those who already had BSEE showed significantly high scores in knowledge(t=6.48, p<0.01), attitudes (t=10.54, p<0.01). and practices(t=57.07, p<0.001) for breast cancer than those who had not participated in the BSEE. In all age groups no intention to participate in the BSEE was higher than who the intention to participate. These findings suggest some strategies should be developed to increase the awareness of breast cancer's early detection.
