Prevalence and associates of obesity and overweight among school-age children in a rural community of Thailand

  • Purpose: Information about overweight and obesity among students in rural areas of Thailand is limited. Therefore, we aimed to determine overweight and obesity prevalences and associated factors among school-aged children in a rural community of Thailand. Methods: We selected 9 public schools through cluster sampling in 2 provinces located in central Thailand in 2016. Anthropometric measurements were measured using standard techniques, classified as overweight (>1 standard deviation [SD]) and obese (>2 SD) with respect to their age and sex using 2007 World Health Organization reference charts. Standardized questionnaires on risk factors were sent to parents to be completed together with their child. Results: Among 1,749 students, 8.98% had overweight and 7.26% had obesity. Mean age (range) was 11.5 years (5-18 years). Independent factors associated with overweight and obesity included primary school student (reference as secondary school) (adjusted odds ratio [aOR], 2.25; 95% confidence interval [CI], 1.24-4.08; P=0.07), mother's body mass index (aOR, 1.07; 95% CI, 1.02-1.12; P=0.001), self-employed father (aOR, 1.99; 95% CI, 1.12-3.55; P=0.018), number of siblings (aOR, 0.61; 95% CI, 0.47-0.81; P=0.001), having sibling(s) with obesity (aOR, 1.82; 95% CI, 1.20-2.77; P=0.005), more than one (aOR, 7.16; 95% CI, 2.40-21.32; P<0.001), consuming 2-3 ladles of rice/meal (aOR, 2.14; 95% CI, 1.38-3.32; P=0.001), consuming >3 ladles of rice/meal (aOR, 2.69; 95% CI, 1.11-6.46; P=0.27), watching <2 hours of television/day (aOR, 2.18; 95% CI, 1.19-4.01; P=0.012), and watching >2 hours of television/day (aOR, 2.60; 95% CI, 1.36-4.96; P=0.004). Conclusion: Many sociodemographic, dietary, and behavioral factors were related to overweight and obesity among school-aged children not only in urban but also rural communities of Thailand.

  • Purpose: This study was conducted to differentiate the factors associated with the BMI group of adolescents between rural and urban areas, and provide baseline data in the development of health education programs considering their personal and environmental characteristics. Methods: Secondary data analysis was performed using the data from an on-line survey of the 9th adolescence behavioral health in 2013. The instruments included general, behavioral, and psychosocial characteristics. This study was a cross-sectional survey conducted with 26,513 adolescents. The data were analyzed using a t-test, two-way ANOVA, and multinomial logistic regression analysis. Results: Significant differences in the prevalence of obesity between urban and rural areas were nated and rural areas exhibited a higher prevalence of obese adolescents (p<0.05). The factors associated with the BMI group were the mothers' education level (low) and lower age of adolescents (middle school) in rural areas, wherase those for urban areas were gender (boys) and the fathers' education level (low) (p<0.05). Conclusion: Because there was a significant difference in the prevalence of obesity between rural and urban areas, it is necessary to develop a tailored education program considering the regional and environmental characteristics that the adolescents belonged to in order to prevent and control adolescent obesity.

초등보건교사의 학교기반 아동비만 예방관리사업에 대한 인식과 경험 (School-based Obesity Prevention and Management Programs as Perceived and Experienced by Elementary School Health Teachers)

  • Purpose: Elementary schools are regarded as the best community setting for implementing obesity prevention programs and health teachers (HTs) are the main health professionals who can provide obesity prevention services. However, there is insufficient information on the actual conditions of school-based obesity prevention/ management programs in Korean elementary schools. The purpose of the study was to investigate the actual conditions of school-based obesity prevention/management programs, perceived and experienced by elementary school HTs, which may provide basic data for developing effective and sustainable programs for early childhood. We investigated (1) HTs' ratings on the program's importance, the confidence in the program's implementation, and the factors associated with the program's success (2), the correlates of the importance and confidence levels (3), the program's effectiveness and the factors associated with its sustainability, which were evaluated by HTs, and (4) the correlates of the effectiveness levels. Methods: The participants, 147 HTs working for elementary schools in Seoul were asked to fill out self-administrated questionnaires through mail. Results: The participants' rate on the program's importance was 7.1 on average (0~10 scale) and that on the confidence in the program's implementation was 6.2 (0~10 scale). Moreover, > 25% of the participants reported student and parent participation as a factor associated with the program's success. Of the total participants, 45.6% reported they had actually experienced the program, giving 4.8 points (0~10 scale), on average, in the evaluation of the program's effectiveness and > 22.0% of the participants regarded student and parent participation as a factor associated with the program's sustainability. The HTs' confidence in the program's implementation was a strong correlate of the effectiveness evaluated by HTs. Conclusion: School-based obesity prevention/management programs tend to be highly prioritized by HTs in elementary schools. To implement an effective obesity program in elementary schools, educational support for HTs is needed in order to enhance their confidence levels on the program's implementation.

The Relative Influence of Diet and Physical Activity on Obesity in China

  • The purpose of this study is to investigate the relative influence of diet and physical activity on obesity. The subjects were 155 adults aged 35-52 years from 24 neighborhood committees in 4 urban districts of Beijing (male : 78, female : 77). They were divided into normal weight, overweight and obese groups according to their BMI. The general information of the subjects was collected by interview-administered questionnaire. Dietary intake was obtained by three-day(two weekdays and one weekend day) food weighted method, physical activity was assessed by a validated combination of data obtained from activity monitors, bicycling information and activity records. There were no significant differences of age, gender, height, educational, family economic level, smoking and drinking between different groups. The proportion of flour intake was higher in obese group compared to normal weight and overweight groups, and that of vegetables is lower in obese group. The physical activity (PAL) was not significantly different between two groups of the normal, overweight and obese groups. After the adjustment for confounding factors using logistic regression model, we found that the proportion of flour intake was positively associated with obesity, while the proportion of vegetable intake was inversely associated with obesity. It is concluded that dietary patterns were associated with obesity and diets composed of more vegetables and less staple combined with physical activities could contribute to obesity prevention.

Treatment Costs and Factors Associated with Glycemic Control among Patients with Diabetes in the United Arab Emirates

  • Background: We aimed to estimate the proportion of patients with diabetes who achieved target glycemic control, to estimate diabetes-related costs attributable to poor control, and to identify factors associated with them in the United Arab Emirates. Methods: This retrospective cohort study used administrative claims data handled by Abu Dhabi Health Authority (January 2010 to June 2012) to determine glycemic control and diabetes-related treatment costs. A total of 4,058 patients were matched using propensity scores to eliminate selection bias between patients with glycosylated hemoglobin (HbA1c) <7% and HbA1c ${\geq}7%$. Diabetes-related costs attributable to poor control were estimated using a recycled prediction method. Factors associated with glycemic control were investigated using logistic regression and factors associated with these costs were identified using a generalized linear model. Results: During the 1-year follow-up period, 46.6% of the patients achieved HbA1c <7%. Older age, female sex, better insurance coverage, non-use of insulin in the index diagnosis month, and non-use of antidiabetic medications during the follow-up period were significantly associated with improved glycemic control. The mean diabetes-related annual costs were $2,282 and $2,667 for patients with and without glycemic control, respectively, and the cost attributable to poor glycemic control was $172 (95% confidence interval [CI], $164-180). The diabetes-related costs were lower with mean HbA1c levels <7% (cost ratio, 0.94; 95% CI, 0.88-0.99). The costs were significantly higher in patients aged ${\geq}65$ years than those aged ${\leq}44$ years (cost ratio, 1.45; 95% CI, 1.25-1.70). Conclusion: More than 50% of patients with diabetes had poorly controlled HbA1c. Poor glycemic control may increase diabetes-related costs.

Sarcopenic Obesity Frequency and Associated Risk Factors in Young Korean Women: A Comprehensive Cross-Sectional Analysis

  • PURPOSE: Sarcopenic obesity (SO) is a clinical condition that combines sarcopenia and obesity. This study examined the frequency of SO in young Korean females between 20 and 29 years of age. METHODS: The study involved 1,000 participants. The height, weight, body mass index (BMI), waist circumference, skeletal muscle mass index, fasting glucose, triglyceride, total cholesterol levels, systolic and diastolic blood pressure, alcohol consumption, and smoking status were the research variables. The skeletal muscle mass index was calculated as appendicular skeletal muscle mass (ASM) divided by the BMI. The ASM was assessed using dual X-ray absorptiometry. Complex sampling analysis and multiple logistic regression were used for analysis. RESULTS: A .74(.30-1.80) frequency of SO was observed. The statistically significant risk factors in females were height, weight, BMI, waist circumference, skeletal muscle mass index, total cholesterol, systolic blood pressure, and diastolic blood pressure (p < .05). CONCLUSION: Young Korean adults with SO have a .74(.30-1.80) frequency of occurrence that is linked to specific risk factors. Hence, primary care clinicians and health care professionals should consider these factors when patients require a referral for early detection and treatment. Healthcare professionals and clinicians can identify potential SO patients by acknowledging these risk factors.

지역사회 정신건강복지센터를 이용하는 만성정신질환자의 비만 관련요인 (Obesity and Related-factors in Patients with Chronic Mental Illness Registered to Community Mental Health Welfare Centers)

  • Purpose: The purpose of study was to examine the relationship between obesity and its associated factors (psychiatric symptom, duration of illness, type of medication, physical activity, dietary habits, depressive symptom, and stress) in patients with chronic mental illness registered to community mental health welfare centers. Methods: This was a cross-sectional correlation study using a convenience sampling. A total of 392 participants were recruited from community mental health welfare centers. The obtained data were analyzed using binary and multinomial logistic regression. Results: Atypical antipsychotic medication, duration of illness, dietary habits (overeating, and drinking instant coffee) were significantly contributed variables into body mass index (BMI) obesity. Atypical antipsychotic medication and instant coffee were significantly related to abdominal obesity. Conclusion: These results emphasized the needs of tailored obesity-preventive management for the community-dwelling patients with chronic mental illness, topically focusing on the administration of atypical antipsychotic medication, duration of illness, and dietary habits.

Chitinase 3-Like 1 (CHI3L1) Polymorphism Contributes to Visceral Obesity and Obesity-related Inflammation Induces Chi3l1 in Adipocytes

  • Abdominal obesity is considered as one of the most risky factors governing the development of metabolic diseases. Here we identify that human chitinase 3-like 1 (CHI3L1, also called YKL-40 in human) single nucleotide polymorphism (SNP), rs883125, is associated with abdominal obesity in Korean women. Korean women subjects with the rs883125 G/G or C/G genotype present higher waist-hip ratio than subjects with C/C genotype suggesting that human subjects who G nucleotide substitution at the rs883125 tended to more accumulate intra-abdominal fat at the abdominal cavity. In addition, Chi3l1 gene expression is increased in adipose tissue from obese mice and pro-inflammatory cytokine enhances Chi3l1 expression in adipocytes, indicating that Chi3l1 is greatly related with obesity and obesity-induced pro-inflammatory responses. Taken together, the minor allele of rs883125 is associated with a higher prevalence of abdominal obesity in Korean women. These findings suggest that genotype of rs883125 can be a biomarker of incident abdominal obesity and abdominal obesity-related metabolic diseases.

사무직 여성의 비만요인에 따른 NK세포 활성도 관계 연구 (A Study on the Relationship between Obesity Factors and NK Cell Activity in White-Collar Females)

  • 효과적인 비만의 중재는 대상 인구에서 가장 관련성이 있는 인구통계학적 건강 관련 및 직무 관련 요인을 고려해야 한다. 가장 강력한 비만 관련 요인은 직업군 또는 연령 범주에서 다룰 수 있다. 이 연구의 목적은 심리적인 직무 스트레스와 면역관련 인자의 혈액분석의 상관성에 대한 최신지견을 제공하는 것이다. 우리는 여성 근로자의 비만 관련 매개 변수 뿐만 아니라 자연살해세포활성(NKCA)을 평가하였다. 비만과 관련된 혈당(p<0.05)은 연령에서 유의한 차이를 보였다. 또한, 비만군과 non-비만군에서는 NK 세포의 활성에 큰 차이를 보였다(p<0.05). 단계적 다중 회귀 분석에서 NKCA는 비만 관련 요인과 밀접한 상관성을 보였다. 향후에는 면역인자들의 활성도가 증가하는 생물학적 변화를 연구해야 할 것이다.

비만처방에서의 안전한 마황사용 지침 (The Safety Guidelines for use of Ma-huang in Obesity Treatment)

  • Objectives : Ma-huang (Ephedra sinica) is frequently prescribed for obesity management in oriental medicine. The main component is ephedrine alkaloids which can have serious adverse side effects such as heart attack, stroke, sudden death. There are no scientific guidelines for Ma-huang usage in the safe treatment of obesity in oriental medicine. We reviewed published studies on its safety to make evidence based guidelines. Methods : We searched electronic databases up to May 2006. We limited evidence to controlled trials for efficacy or safety, case reports for safety, and studies for Ma-huang contents analysis. Results and Conclusions : In clinical trials for weight loss, Ma-huang and ephedrine promote modest short-term weight loss but have no serious adverse effects, have only a few adverse effects associated with increased risk of psychiatric, autonomic, gastrointestinal symptoms and heart palpitations. In case reports, there have been serious adverse effects including stroke, heart attack, and death using typical doses of ephedrine or no associated illness. There are factors related to serious adverse effects, such as overuse, lack of standardization, individual sensitivity, and interactions with other drugs. Studies relating to these factors should be analyzed for safe use of Ma-huang and ephedrine. After analyzing related studies, we suggest guidelines for Ma-huang usage. We propose that the dosage should be within 4.5-7.5g per day for up to 6 months for generally healthy individual. It's use is contraindicated in individuals with heart disease, thyroid disease, diabetes mellitus, hypertension, psychiatric disorders, glaucoma, urination disorders, enlarged prostate, persons using MAOIs, methyldopa and sympathomimetic agents.
