안면 백반증 치료 평가를 위한 딥러닝 기반 자동화 분석 시스템 개발 (Development of a Deep Learning-Based Automated Analysis System for Facial Vitiligo Treatment Evaluation)

  • Vitiligo is a condition characterized by the destruction or dysfunction of melanin-producing cells in the skin, resulting in a loss of skin pigmentation. Facial vitiligo, specifically affecting the face, significantly impacts patients' appearance, thereby diminishing their quality of life. Evaluating the efficacy of facial vitiligo treatment typically relies on subjective assessments, such as the Facial Vitiligo Area Scoring Index (F-VASI), which can be time-consuming and subjective due to its reliance on clinical observations like lesion shape and distribution. Various machine learning and deep learning methods have been proposed for segmenting vitiligo areas in facial images, showing promising results. However, these methods often struggle to accurately segment vitiligo lesions irregularly distributed across the face. Therefore, our study introduces a framework aimed at improving the segmentation of vitiligo lesions on the face and providing an evaluation of vitiligo lesions. Our framework for facial vitiligo segmentation and lesion evaluation consists of three main steps. Firstly, we perform face detection to minimize background areas and identify the face area of interest using high-quality ultraviolet photographs. Secondly, we extract facial area masks and vitiligo lesion masks using a semantic segmentation network-based approach with the generated dataset. Thirdly, we automatically calculate the vitiligo area relative to the facial area. We evaluated the performance of facial and vitiligo lesion segmentation using an independent test dataset that was not included in the training and validation, showing excellent results. The framework proposed in this study can serve as a useful tool for evaluating the diagnosis and treatment efficacy of vitiligo.

하안면 고경 변화의 인지도에 관한 연구 (Level of perception related to changes in lower facial height)

  • 교정치료 결과 나타나는 안모의 개선에 대하여 환자들이 느끼는 인지도에 관한 정보를 교정의사들이 이해하여야 하며, 진단과 치료계획 수립 시 이에 대한 기준이 중요하다. 이에 본 연구는 하악의 수직적 변화를 통해서 야기되는 측모 및 정모의 변화를 인지할 수 있는 최소한의 수직적 변화량을 알아보고자 보기 좋은 안모를 갖는 가상의 안모를 컴퓨터 영상으로 정모와 측모를 만들고 하안면부인 stomion에서 chin까지 1, 2. 3, 4mm 변화시킨 디지털 영상을 이용하여 40명의 미술학도를 관찰자로 인지도 조사를 하였다. 그 결과는 환자가 수직고경의 변화를 인지하게 하기 위해서는 측모에서 최소한 2mm 이상, 정모에서 3mm 이상의 변화가 필요하였으며, 하악의 수직적 변화에 대한 인지도는 정모의 변화보다는 상대적으로 측모의 변화에서 높았다. 또한 치료 전이나 치료과정에서 환자가 접하게 될 수 있는 안모 변화에 대한 사전 정보는 변화에 대한 인지율을 현저하게 높여 주었다.

Facial Soft Tissue Augmentation using Autologous Fat Mixed with Stromal Vascular Fraction

  • Background Autologous fat grafting evolved over the twentieth century to become a quick, safe, and reliable method for restoring volume. However, autologous fat grafts have some problems including uncertain viability of the grafted fat and a low rate of graft survival. To overcome the problems associated with autologous fat grafts, we used uncultured adipose tissue-derived stromal cell (stromal vascular fraction, SVF) assisted autologous fat grafting. Thus, the purpose of this study was to evaluate the effect of SVF in a clinical trial. Methods SVF cells were freshly isolated from half of the aspirated fat and were used in combination with the other half of the aspirated fat during the procedure. Between March 2007 and February 2008, a total of 9 SVF-assisted fat grafts were performed in 9 patients. The patients were followed for 12 weeks after treatment. Data collected at each follow-up visit included clinical examination of the graft site(s), photographs for historical comparison, and information from a patient questionnaire that measured the outcomes from the patient perspective. The photographs were evaluated by medical professionals. Results Scores of the left facial area grafted with adipose tissue mixed with SVF cells were significantly higher compared with those of the right facial area grafted with adipose tissue without SVF cells. There was no significant adverse effect. Conclusions The subjective patient satisfaction survey and surgeon survey showed that SVF-assisted fat grafting was a surgical procedure with superior results.

Osteoplastic Reconstruction of Post-enucleatic Microorbitalism

  • Background Patients who have undergone enucleation during infancy due to retinoblastoma can develop microorbitalism due to the decreased growth stimulation from the eyeball and the surrounding soft tissues. Anatomically, the orbit consist of parts of the frontal bone superiorly, the maxilla inferiorly, the ethmoid bone medially, and the zygoma laterally. Considering the possibility of surgically expanding the orbit using tripod osteotomy, in this study we conducted tripod osteotomy on adult patients with microorbitalism of retinoblastoma. Methods Tripod osteotomy was conducted to expand the orbital volume in adult patients with microorbitalism due to enucleation in infancy for retinoblastoma. The orbital volume was measured using the Aquarius Workstation ver. 4.3.6 and the orbit width was measured with preoperative and postoperative 3-dimensional facial bone computed tomography (CT) imaging. Preoperative and postoperative photographs were used to visualize the difference produced by the surgery. Results The orbital volume of the affected side was 10.3 $cm^3$ before and 12.5 $cm^3$ after the surgery, showing an average increase in volume of 2.2 $cm^3$ (21.4%). The increase in the obital width was confirmed by the preoperative and postoperative 3-dimensional facial CT images and aesthetic improvement was observed by the preoperative and postoperative photographs. Conclusions Tripod osteotomy, which realigns the orbital bone, zygoma, and maxilla, is used to correct posttraumatic malunion as well as non-traumatic congenital abnormalities such as that seen in facial cleft. We applied this procedure in microorbitalism secondary to enucleation for retinoblastoma to allow orbital expansion and correct asymmetry.

A morphometric analysis of maxillary central incisor on the basis of facial appearance in Korea

  • PURPOSE. We aimed to identify a more esthetic width-to-length ratio by analyzing maxillary central incisor of Korean adult population. Information regarding tooth ratio in the Korean population may be useful to clinicians when restoring anterior teeth. MATERIALS AND METHODS. A total of 40 men and 40 women were included in this study. The NON-MEDIA group included 40 non-celebrities, who were healthy and 25 to 39 years old, with normal dentofacial appearance. The MEDIA group consisted of 40 celebrities selected only on the basis of their soft-tissue facial appearance. The facial photographs of 40 celebrities were collected from the Internet websites. The width and length of the maxillary central incisor were measured using Adobe $Photoshop^{(R)}$ 7.0, a software for analysis, measurement and edition of photographs. Data were analyzed statistically using the Independent t-test at 5% statistical significance level. RESULTS. The mean ratio for the MEDIA group was 0.77, whereas that of the NON-MEDIA group was 0.88. The difference between the two groups was statistically significant. No significant gender differences were found in the width-to-length ratio in MEDIA group. In NON-MEDIA group, however, there were significant differences between female and male. CONCLUSION. After analyzing maxillary anterior teeth of Korean adults, our results were in accordance with the general Western standards of esthetics. A dentist restoring the natural look and beauty of teeth will have to consider those standards.

남성 의복착용자의 의복유행과 얼굴의 매력성이 인상에 미치는 영향(I) -직업추론 및 의복인상을 중심으로- (The Effect of Clothing Type and Facial Attractiveness of Men Clothed on Impressions ( I ))

  • The purpose of this study was 1) to investigate the effect of male clothing type and facial attractiveness on the subjects' inferences of occupation for the wearers, 2) to investigate the effect of male clothing type on the inferences of occupational characteristics. The stimuli were composed of 24 color photographs which were made of 6 types of clothing and 18 wearers who were made of 3 levels of facial attractiveness in combination with 6 types of clothing. The subjects were 288 male and 288 female university students. The subjects chose an occupation among 19 occupations presented for 6 types of clothing and the wearers. Also the subjects' inferences of occupational characteristics for the clothing were measured by semantic\ulcornerdiffenential scales. The results were as follows: 1) As a whole, the infered occupation for each of the clothing types was consistent with the expected occupations in this study. "Therefore there were stereotypes about male clothing type. At the same time, the halo effect of facial attractiveness appeared when subjects infered wearer's occupation. 2) The subjects' responses to the semantic-differential scales about 6 types of clothing were factor analyzed to identify the underlying constructs of occupational characteristics. At the result, 4 factors were emerged. The first factor was interpreted to Apperance/ Ability, the second, Indi~iduality, the third, Naivety, and the fourth, Dominancy. 3) The male clothing type influenced on the 3 factors except the Dominancy factor.nancy factor.

Effects of Bariatric Surgery on Facial Features

  • Background Bariatric surgeries performed in the USA has increased twelve-fold in the past two decades. The effects of rapid weight loss on facial features has not been previously studied. We hypothesized that bariatric surgery will mimic the effects of aging thus giving the patient an older and less attractive appearance. Methods Consecutive patients were enrolled from the bariatric surgical clinic at our institution. Pre and post weight loss photographs were taken and used to generate two surveys. The surveys were distributed through social media to assess the difference between the preoperative and postoperative facial photos, in terms of patients' perceived age and overall attractiveness. 102 respondents completed the first survey and 95 respondents completed the second survey. Results Of the 14 patients, five showed statistically significant change in perceived age (three more likely to be perceived older and two less likely to be perceived older). The patients were assessed to be more attractive postoperatively, which showed statistical significance. Conclusions Weight loss does affect facial aesthetics. Mild weight loss is perceived by survey respondents to give the appearance of a younger but less attractive patient, while substantial weight loss is perceived to give the appearance of an older but more attractive patient.

안와 경사의 분석을 위한 정모 두부규격방사선사진, 3D-CT의 유용성 평가 (VALIDITY OF POSTERIOR ANTERIOR CEPHALOMETRIC AND 3D-CT FOR ORBITAL CANTING ANALYSIS)

  • Purpose: The purpose of this study was to estimate validity of posterior anterior cephalometric and 3D-CT for orbital canting analysis. Materials and methods: Three trained observers classified two patients group using standardized frontal photographs of facial asymmetry patients. Group A consisted of patients with facial asymmetry and orbital canting(n=19), and group B consisted of patients with only facial asymmetry(n=43). Orbital canting was measured with line of bilateral inferior orbitale. Orbital canting measurement was done with posterior anterior cephalometric and 3D-CT. Each horizontal reference line was established by bilateral GWSO(cephalometric), FZS(3D-CT). Maxillary canting and mandibular deviation angle were also measured and analyzed with orbital canting. Results: The mean orbital canting was $3.03{\pm}1.00^{\circ}$ in Group A and $1.11{\pm}0.76^{\circ}$ in Group B in frontal photograph. The mean orbital canting was $1.20{\pm}0.74^{\circ}$ in group A and $1.22{\pm}0.65^{\circ}$ in group B by cephalometric analysis(p>0.05). In 3D-CT, orbital canting was almost paralleled with horizontal reference line. The orbital canting, maxillay canting and mandibular deviation between two groups showed no significant differences except madibular deviation in 3D-CT. Conclusion: Common analysis of posterior anterior cephalometric and 3D-CT is not valide method to evaluate orbital canting for facial asymmetry patients with orbital canting.

The subjects were 216 male and 216 female university students. The results were as follows: First, the responses to the semantic-differential scales were factor analyzed to identify the underlying constructs of occupational characteristics. As the result, 4 factors were emerged. The first factor was interpreted to Appearance·Ability, the second, Individuality, the third, Dominance, and the fourth, Naivety. Second, the type of clothing influenced on all the 4 factors, and was more influencial than the facial attractiveness which influenced on the 3 factors except the Naivety factor. Third, There were differences perceptions for the wearers beteen subjects 'sex type; The male subjects perceived for the wearers with categorization more exactly than the female's. Fourth, there were accordances between free-association and inference in semantic-differential scales for the wearers.

Three-dimensional Face Recognition based on Feature Points Compression and Expansion

  • Many researchers have attempted to recognize three-dimensional faces using feature points extracted from two-dimensional facial photographs. However, due to the limit of flat photographs, it is very difficult to recognize faces rotated more than 15 degrees from original feature points extracted from the photographs. As such, it is difficult to create an algorithm to recognize faces in multiple angles. In this paper, it is proposed a new algorithm to recognize three-dimensional face recognition based on feature points extracted from a flat photograph. This method divides into six feature point vector zones on the face. Then, the vector value is compressed and expanded according to the rotation angle of the face to recognize the feature points of the face in a three-dimensional form. For this purpose, the average of the compressibility and the expansion rate of the face data of 100 persons by angle and face zone were obtained, and the face angle was estimated by calculating the distance between the middle of the forehead and the tail of the eye. As a result, very improved recognition performance was obtained at 30 degrees of rotated face angle.
