A Case of Infantile Fibromatosis of the Infratemporal Fossa (측두하부와에 발생한 영아섬유종증 1례)

  • The fibromatoses are defined as a group of non encapsulated, non metastasizing, fibrous tumors that have a tendency for infiltrative growth and local recurrence following surgical excision and can be divided into fascial and musculoaponeurotic fibromatoses. Infantile fibromatosis is childhood counterpart of musculoaponeurotic fibromatosis and is more common in the head and neck, the shoulder and upper arm and the thigh. Treatment includes excision, radiation, steroid and chemotheraphy, etc. Treatment of choice is complete excision but postoperative deformity and sequelae should be considered. Recently, we have experienced a case of infantile fibromatosis originating from infratemporal fossa and report this case with a review of literatures.

Arytenoidopexy with Bilateral Vocal Cord Excision (laryngofissure) for the Relief of Laryngeal Paralysis (후두마비 개에 대한 피열연골고정술)

  • This study was conducted to assess the application of spectrograms in the diagnosis of laryngeal paralysis and the arytenoidopexy with bilateral vocal cord excision (laryngofissure) for the relief of laryngeal paralysis. Laryngeal paralysis represented two types of stridor A and B (AN, PN). There were significant differences in Dominant Frequency ( DF, p<0.01), which were $3590{\pm} 209.81 Hz, 7445\{pm}418.54 Hz$, respectively. After arytenoidopexy with bilateral vocal cord excision (laryngofissure), $PaO_{2} levels (100.57{\pm}7.59 mmHg) were more increased than that (86.5 {\pm} 3.34 mmHg)$ of pre-operative dogs.

Studies on the Wound Healing by the PVA-soft hydrogel (화상 및 창상 치료용 외용제제 개발에 대한 기초 연구)

  • A PVA-soft hydrogel, which is a semi-solid form in container, whereas after applying on the skin, it formed a thin layer within a few minutes. In this study, we prepared a novel type PVA-soft hydrogel containing 6-methyluracil as active drug, and the therapeutic value was characterized. To evaluate the therapeutic value of the PVA-soft hydrogel containing drug, various animal models, thermal burn model, incision & excision wound rat model were used. We also measured the wound size and breaking strength to calculate the wound healing extent after single or multiple administration. The wound size of soft hydrogel treated group decreased rapidly than that of control group after multiple dosing in excision wound model. And, the breaking strength of the soft hydrogel treated group was greater than that of control group in incision wound model.

  • A case of a peroneal nerve palsy caused by repeatedly recurred intraneural ganglion cyst is presented. A 19 year old male suffered from tingling sensation on the foot dorsum more than one year and underwent two times of mass excision and nerve palsy was recorvered. But it was recurred once more after 10 months after the second excision. The mass was located in the fibro-osseous tunnel against the fibular neck and the origin of the peroneus longus. The third complete excision was done and full recovery was obtained in 6 months.

Clear Cell Hidradenoma in a Young Male Nipple: A Case Report (남성 유두에 발생한 투명세포 한선종의 치험례)

  • Purpose: Clear cell hidradenoma, now regarded as an eccrine sweat gland tumor on the basis of its enzyme histochemical and electron microscopic features, occurs as a solitary tumor in most instances. Methods: A 17 year old male presented with asymptomatic nodule, which had developed on nipple with a four years of history and total excision and purse-string suture was then performed. Results: No recurrence was observed 2 months after excision. Histologically, it showed a well circumscribed tumor composed of characteristic clear epithelial cells which are focally arranged in glandular patterns. Conclusion: This case is unique in that the tumor developed on the young male nipple, unusual site and this report emphasizes the benefit of local excision to prevent recurrence of these tumor.

Thirteen cases of Perianal Adenoma in Dogs (개의 항문주위 선종 13예)

  • This study comprised 13 cases having canine perianal adenoma at Veterinary Medical T~aching Hospital, Seoul National University from January 2000 to October 2003. All referred dogs were intact male and mean age of dogs was 11.5 years (ranged from 7 to 15 years). The most prevalent breed was Shih Tzu. The clinical signs were presence of 1 to 5 cm in diameter firm mass, perianal swelling, hemorrhage, pain and dyschezia. The masses were located either in the perineum (12 cases) or tail (5 cases). Five dogs had masses with ulceration. Concurrent prostatomegaly and seminoma was found in 5 and 1, respectively. Castration was performed in 5 cases. In 8 cases, surgical excision of the tumor was performed with castration. There was no recurrence in all cases after removal. In all cases with castration, perianal adenomas were regressed within 1-2 months. In one case among them, excision of the tumor was also performed because of persistent ulceration. According to these clinical results, perianal adenoma excellently responded to castration with or without surgical excision of the tumor.

Management of severe hidradenitis suppurativa with biologic therapy and wide excision

  • Hidradenitis suppurativa (HS) is a chronic inflammatory follicular occlusive disease that involves the intertriginous areas. Treatment methods include conventional topical and systemic medication, radiotherapy, biologic agents, and surgical excision. Of late, there has been an increased focus on the use of biologic agents in patients with moderate to severe HS. Here, we present the case of a 46-year-old man with Hurley stage III HS for whom wide excision was ultimately curative, after aggressive medical therapy with the use of infliximab and adalimumab had succeeded in limiting the body surface area affected by the disease. This case demonstrates the effective treatment of severe HS with a combination of biologic therapy and surgery.

  • Congenital epulis (CE) is an extremely rare benign tumor of the gingiva that is found on the alveolar ridge of newborns, and the main treatment option is simple excision. Postoperative prognosis is very good, and spontaneous regression may occur despite incomplete excision. This report presented a rare case of CE and its healing process after surgery under local anesthesia. The treatment plan was decided upon through consultation between a medical team and the patient's family, with surgical excision for the main lesion, which benefitted from surgery, and follow-up for a very small-sized lesion, which was thought to be appropriate for a newborn. No recurrence was found after its removal, and favorable healing was observed.

Recurrence and Extraneural Metastasis in 31 Meningeal Hemangiopericytomas (31예 수막 혈관외피세포종에 있어서의 재발 및 신경계외 전이)

  • Purpose : Meningeal hemangiopericytoma(M-HPC), characterized by a high local recurrency and metastatic potential, is a rare neoplasm arising from perivascular pericytes. A retrospective study was performed to identify the recurrence and extraneural metastasis in M-HPC. Materials and Methods : We reviewed the records of 31 M-HPC patients treated from 1982 through 1999 at our institution. The time to recurrence and the various parameters affecting recurrence were determined. Extreneural metastasis was also analyzed. Results : The rate of local recurrency was 38.7%(12/31). The overall average recurrence-free period(RFP) before the first recurrence was 104 months, with overall recurrence-free rates(RFRs) at 5 and 10 years after first surgery of 59.2% and 33.6%, respectively. Of the 12 patients who experienced local recurrence, 4 had recurrences 5 years later after the first surgery. Complete excision at the first operation significantly extended the average time before first recurrence from 43 to 111 months. The 5-year RFRs for the groups of complete excision and incomplete excision were 72.7% and 20.8%, respectively(p=0.0060). Although there was no statistical significance, complete excision followed by adjuvant radiotherapy of more than 50Gy extended the RFP. The 5-year RFRs for the groups of complete excision and complete excision with adjuvant radiotherapy were 70.3% and 100%, respectively(p=0.3359). Four patients(12.9%) presented one or more extraneural metastases that were developed at an average of 107 months after the first operation with the 5- and 10-year metastasis rates of 4.4% and 24.9%, respectively. Conclusions : M-HPC has a propensity to recur either locally or at distant sites after surgical resection. Complete excision is the most important factor to reduce recurrence. However, even with complete excision, adjuvant radiotherapy of more than 50Gy significantly reduces the risk of recurrence. Local and distant recurrences may occur after a prolonged disease-free interval, emphasizing the need for long-term follow-up.

  • DNA damage accumulates in cells as a result of exposure to exogenous agents such as benzopyrene, cigarette smoke, ultraviolet light, X-ray, and endogenous chemicals including reactive oxygen species produced from normal metabolic byproducts. DNA damage can also occur during aberrant DNA processing reactions such as DNA replication, recombination, and repair. The major of DNA damage affects the primary structure of the double helix; that is, the bases are chemically modified. These modification can disrupt the molecules'regular helical structure by introducing non-native chemical bonds or bulky adducts that do not fit in the standard double helix. DNA repair genes and proteins scan the global genome to detect and remove DNA damage and damage to single nucleotides. Direct reversal of DNA damage, base excision repair, double strand break. DNA repair are known relevant DNA repair mechanisms. Four different mechanisms are distinguished within excision repair: direct reversal, base excision repair, nucleotide excision repair, and mismatch repair. Genetic variation in DNA repair genes can modulate DNA repair capacity and alter cancer risk. The instability of a cell to properly regulate its proliferation in the presence of DNA damage increase risk of gene mutation and carcinogenesis. This article aimed to review mechanism of excision repair and to understand the relationship between genetic variation of excision repair genes and head and neck cancer.
