부분비만 개선을 위한 매선과 약침의 병행 치료 임상 효과 증례 보고 (A Case Report for the Effects of Pharmacopuncture Combined Thread Implantation Therapy to Improve Localized Obesity)

  • 총 5예의 부분 비만 환자 중, 복부비만 3예, 허벅지 및 종아리 부분 비만 각 1예의 환자를 대상으로 2종의 약침요법 및 매선요법을 병행하여 2주 간격으로 3회 시술하였다. 그 결과, 치료부의 둘레 감소를 확인할 수 있었다. 본 연구의 결과를 바탕으로, 향후 더 많은 환자를 대상으로 하여 약침 및 매선요법의 부분 비만 효과에 대한 보다 구체적인 연구가 필요할 것으로 생각된다.

약용 및 야생식물로부터 트롬빈 저해물질의 탐색 (Screening of Thrombin Inhibitors from Medicinal and Wild Plants)

  • Inhibitory activities of 264 methanol extracts, which were prepared from different parts of 210 kinds of wild and medicinal plants, against human thrombin were evaluated. Based on the anti-coagulation activity determined by thrombin time and activated partial thromboplastin time, the 14 extracts were screened. The fibrinolytic activity, heat stability and inhibition of other proteolytic digestive enzymes, such as pepsin, papain, trypsin and chymotrypsin, of the 14 extracts were further determined, and Ginko biloba (herba), Ephedra sinica (radix), Reynoutria elliptica (herba), Amomum tsao-ko Crevost (fructus), and Magnolia officinalis Rehd. et Wils (bark) were finally selected as possible plant sources for anti-thrombosis agent. These results suggested that medicinal and wild plants could be the potential source of thrombin inhibitor.

비만 치료 연구에서 마황의 안전성 분석 (Analysis of Safety of Mahuang in Studies for Treatment of Obesity)

  • Objectives: The purpose of this study is to analyze the adverse events and safety of Mahuang used as anti-obesity treatment in Korean medicine. Methods: PubMed, EMBASE, Cochrane, KoreaMed, Oriental Medicine Advanced Searching Integrated System (OASIS) and SCIENCE-ON were used as searching for randomized controlled trials investigating the safety of Mahuang for treating obesity from January, 2000 to July, 2021. Results: Total 6 studies were included for analyzing. In most studies, heart rate was higher in experimental group than in placebo group with statistically significant difference. Both systolic and diastolic blood pressure had no significant differences between both groups, but systolic blood pressure got lower in experimental group. Most adverse events reported in experimental group were dry mouth, constipation, diarrhea, palpitation and insomnia, but serious adverse events were not reported. Conclusions: This study provides the evidence about the safety of Mahuang for treating obesity. However, increase of heart rate and minor adverse events could be occurred. Further study is needed to confirm the safety of Mahuang.

문헌 고찰을 통한 비만 치료 한약의 장내미생물 변화 연구 (A Study on Changes in the Gut Microbiome of Herbal Medicines for Treating Obesity Using Literature Review)

  • Objective This study identified useful herbal medicines and prescriptions for obesity treatment by analyzing herbal medicines and prescriptions that showed meaningful results in weight loss by changing the gut microbiota. Methods Using PubMed, we selected and studied 23 papers showing meaningful results on weight loss through changes in the gut microbiota using herbal medicines. Result Of the 23 selected papers, 17 were of studies using herbal medicines, and 6 of studies using prescriptions. Pomegranate peel, Flos Lonicera, Rehmannia glutinosa, Rhein, Coix seed, Platycodon grandiflorus, mulberry leaves, Poria cocos, asperuloside, Bupleuri radix, Astragalus polysaccharides, Ephedra sinica, Ganoderma lucidum, Hirsutella sinensis, Caulis spatholobi, aconite, and Bletilla striata were used as herbal medicine. Linggui zhugan-tang, Bofutsushosan, Shenling baizhu powder, Chowiseungcheng-tang, Daesiho-tang, and Yijin-tang were used as prescription. Conclusion Seventeen herbal medicines and six prescriptions associated with meaningful results in weight loss through changes in the gut microbiota, suggest the possibility of treatment and prevention of obesity through herbal medicine.

태음인(太陰人) 마황정천탕(麻黃定喘湯)의 기원, 변천과정 및 구성 원리 (Reconsideration on the Origins, Changes, and Constructive Principle of Mahwangjeongchun-tang)

  • Objectives This paper was aimed to review the preceding research on the origins, changes and constructive principle of Mahwangjeongchun-tang. Method Based on Jema Lee's works such as Dongyisusebowon and Dongmuyoogo, we analyzed contents related to Mahwangjeongchun-tang and its modification. Moreover, we reviewed physiological and pathological pecularities of Taeeumin symptomatology and applied the theory to induce the principle of the modified Mahwangjeongchun-tang in Sasang medicine. Results and Conclusion 1) The origins of Mahwangjeongchun-tang might be Mahwang-tang, Jungchun-tang and Saengmaek-san. 2) Mahwangjeongchun-tang had been changed in composition when it moved from Manbyonghoichun to Dongyisusebowon. 3) Mahwangjeongchun-tang consists of a) Platycodi Radix (桔梗), Liriopis Tuber(麥門冬) and Scutellariae Radix(黃芩) which directly or indirectly enhance lung function, b) Ephedra Sinica(麻黃) which affects circulation of lung companion, c) Prunus Armeniaca(杏仁) and Ginkgo Biloba(白果) which affect wiwan's upward circulatory force, d) Radish Seed (蘿葍子) which affects small intestine's digesting effect, e) Mori Cortex Radicis(桑白皮) and Farfarae Flos(款冬花) which affect cough and asthma.

마황 단기복용이 사상체질인의 불안에 미치는 영향 : 이중맹검 임상시험 (Short-Term Effects of Mahuang on State-Trait Anxiety According to Sasang Constitution Classification : A Double-Blind Randomized Controlled Trial)

  • Background : Mahuang (Ephedrae Herba, Ephedra sinica $S_{TAPE}$) has been widely used to treat respiratory disease in Asian over the past thousand years. The main ingredient of Mahuang is ephedrine, whose affects on the autonomic nervous system induce some adverse effects like vasoconstriction, hypertension, tachycardia, miosis, insomnia, dizziness, headache, etc. Although there were lots of reports about adverse effects of Mahuang, there were no clinical studies which evaluated the adverse effects of Mahuang on the autonomic nervous system by objective numerical value in the past decade. Objectives : The purpose of this report was to provide an objective assessment of state-trait anxiety that is caused by Mahuang, and to identify anxiety of Mahuang according to different Sasang constitution classifications. Methods : The study design was a double-blind randomized placebo-controlled trial. The subjects of this study were 79 adults aged between 20 to 40 who agreed to participate. Because 8 adults dropped out, a total of 71 subjects entered the study. They were allocated through randomization to a Mahuang group (N=50) and placebo group (N=21). Each group took three opaque capsules (every opaque capsule containing 2g of Mahuang or none) twice a day. To compare the state and trait anxiety before and after taking Mahuang, we checked the anxiety by using STAI-KYZ. Results : The following results were obtained. Short-term administration of Mahuang significantly increased state-anxiety, but in the placebo group, there were no significant changes in state-anxiety. In the Mahuang group except females, there was more significant increase in state-anxiety of Soeumin than Soyangin and Taeumin in the 2nd measurement. Conclusion : It is suggested that the ingestion of Mahuang can increase sympathetic activity and induce anxiety. There was a significant difference among Sasang constitution classification. Especially, the response is stronger in Soeumin than other constitutions. If we use Mahuang according to the Sasang constitution classification in clinic, we could not only minimize the anxiety but maximize the potential curative value.

마황 복용이 정산인의 심박변이도에 미치는 영향에 대한 무작위배정 이중맹검 임상연구 (Effect of Mahuang on Heart Rate Variability in Adults : a Double-Blind, Placebo-Controlled, Randomized Trial)

  • Objectives : Mahuang (Ephedra sinica), well known as an herbal medicine in the East and West, contains a relatively high percentage of ephedrine known as sympathomimetic alkaloid. We investigated the effects of Mahuang on sympathetic nervous system with heart rate variety (HRV). Time and frequency domain analysis of HRV is a noninvasive technique capable of providing information on autonomic modulation of the sinus node. Methods : We investigated 57 healthy volunteers consisting of 37 subjects in the Mahuang group and 20 subjects in the placebo group. Study form was a randomized, placebo-controlled, double-blind clinical trial. The 37 subjects in the Mahuang group took 3 Mahuang capsules (1 capsule equivalent to 2g herb Mahuang) twice a day at 10 a.m. and 2 p.,., while the 20 subjects in the placebo group took 3 placebo-capsules filled with glutinous rice powder at the same times. Mahuang medicine and placebo medicine were made into opaque capsules. We measured HRV at 3 p.m. 1 or 2 days before medication and at 3 p.m. after medication. Results : Mean-RR and SDNN of the Mahuang group significantly decreased compared with that of the placebo group, but the heart rate of the Mahuang group significantly increased compared with the placebo. HRV-Index, RMSSD and SDSD of the Mahuang group significantly decreased compared with that of the placebo group, but PNN50 of the Mahuang group significantly increased compared with placebo. Ln(TP), Ln(VLF), Ln(LF) and Ln(HF) of the Mahuang group significantly decreased compared with those of the placebo group. There were no significant differences in normalized LF, normalized HF and LF/HF ratio between the Mahuang and placebo groups. Conclusion : The results suggest that Mahuang in healthy adults tends to reduce the autonomic nervous system within the normal range.

감비환과 가르시니아 캄보지아 병용 투여의 체중감량 효과와 안전성: 후향적 관찰연구 (Effect and Safety of Combined Treatment of Gambihwan and Garcinia Cambogia on Weight Loss: A Retrospective Observational Study)

  • Objectives: Obesity is becoming more popular disease worldwide. Because of the side effects of conventional obesity treatment modality, herbal medicine treatment is becoming more preferred. Gambihwan which including Ephedra sinica Staph is widely used in traditional Korean Medicine practice for obesity treatment. Garcinia cambogia is a kind of health functional food that has body fat reducing effect. Nowadays, ephedra and Garcinia cambogia are often used simultaneously in clinical practice of Korean Medicine. However, the effectiveness and safety of combination therapy in obesity treatment is not well established. Methods: We conducted retrospective observational study to explore effectiveness and safety of combination therapy. We evaluated effect of combined treatment of Gambihwan and Garcinia cambogia on body weight, body mass index, body fat mass, and waist hip ratio reduction. We also assessed safety via liver function test and adverse event. Results: Finally, 23 patients were included. In paired t-test, body weight significantly decreased from 64.50±14.50 kg to 62.94±13.85 kg (P<0.001) and body mass index were also significantly decreased from 24.43±3.79 kg/㎡ to 23.83±3.59 kg/㎡ (P<0.001). Body fat mass was also reduced. Aspartate transaminase and alanine aminotransferase were not significantly increased. There were no drug-induced liver injury and no severe adverse event. Conclusions: In our retrospective review, we found combination therapy of Gambihwan and Garcinia cambogia reduce body weight, body mass index and body fat mass. There were no severe adverse event and drug induced liver injury which indicated safety of combination therapy in obesity treatment.

Screening for Anti-diabetic Effects of Prescribed Korean Traditional Medicines

  • Herb extracts commercially used in Korea were screened for PPAR-${\gamma}$ agonist test and ${\alpha}$-glucosidase inhibition assay. Total 16 herb plants had a PPAR-${\gamma}$ agonist activity. Specially, Alisma orientale Juz (108.41%), Ephedra sinica (98.22%), Sasa japonica Makino var. purpurascens Nakai (140.68%), Astragalus membranaceus Bunge (106.79%) and Cnidium officinale Makino (113.00%) showed high PPAR-${\gamma}$ agonist activity rate compared with rosiglitazone's (167.46%). And Cornus officinalis S. et Z. (90.3%), Cinnamomum cassia Blume (89.2%), Psoralea corylifolia L. (89.8%), Paeonia japonica (Makino) Miyabe (92.4%) and Paeonia suffruticosa Andr (93.2%), showed high ${\alpha}$-glucosidase inhibition rates. These results support previous reports of the efficacy of Oriental medicinal plants used for diabetes mellitus.

  • 동의보감의 소갈 처방에서 사용하고 있는 한약재 중에서 인슐린성 물질을 탐색하기 위해 3T3-L1 지방세포 모델을 이용하여 3T3-L1 섬유아세포의 증식 및 3T3-L1 지방세포의 분화에 미치는 영향을 살펴보았다. 3T3-L1 섬유아세포의 증식율은 반하, 후박, 대황, 오가피, 창출과 보두의 열수추출물을 첨가하였을 때 세포의 분화작용에는 영향을 미치지 않았다. 마황, 과루인, 현삼과 고삼의 추출물은 3T3-L1 섬유아세포의 분화를 촉진시켰으며, 이는 인슐린성 물질이 함유되어 있을 가능성이 높음을 시사하고 있다.
