HISTOPATHOLOGIC STUDY ON TEETH AND TIBIAL BONE OF THE EXPERIMENTAL RICHATIC RATS (실험적(實驗的) 구루병(病) 백서(白鼠)의 치아(齒牙) 급(及) 경골(脛骨)에 관(關)한 병리(病理) 조직학적(組織學的) 연구(硏究))

  • The purpose of this study was to observe the effect on teeth and tibia bone histopathologically in rats by the richitogenic diet. For this purpose, 48 sprague-dewley rats, weighing 80g or more, divided into 6 groups, and sacrificed on the 1st, 2nd, 3rd, 4th, 5th, 6th weeks after experiments respectively. The tissues contain tooth, and tibial bone were fixed in 10% normal formalin solution, decalcified in Plank-Rychlo solution, embedded in paraffin, sectioned in $6-8{\mu}$ as usual manner, the tissues were stained in hematoxylin eosin, and examined hisopathologically. Follow results were attained 1. Pyknotic appearance of odontoblastic layer was noticed on the 1st week, and increased on the 2nd weeks, and it is appeared that degeneration and dearrangements of odontoblasts on the 4 weeks. But a little recovered on the 6th weeks. 2. The thickness of predentin layer was increased on the 2nd weeks, and increased remarkedly on the 3rd weeks and reached the maximum on the 5th weeks. 3. The interglobular dentin was appeared in spotty shape on the 2nd weeks, and increased on the 4th, 5th, weeks, and large amount of inter-globular dentin was appearanced on the 6th weeks. 4. It is appeared that enamel hypoplasia on the 4th weeks. 5. Epiphyseal and metaphyseal plate of tibia was increased from/on the 2nd weeks, increased maximumly on the 5th weeks. And pyknotic, dearrangements, and hyperchromatic appearances of chondrocytes on the plate were increased on the 1st week.

  • The aim of this study was to investigate the level of radiopacity of esthetic dental restorative materials and determine the optimum level of radiopacity in pediatric dentistry. Disks of 8 dental restorative material groups as the experimental group, 7mm in diameter and 2mm thick, were radiographed with intact human deciduous teeth and aluminum stepwedge standard. Radiopacity was evaluated with an image analysis program following the digitization of the radiographs using a flatbed scanner with transparency unit. All materials and tooth structure also the significant difference except FP, VB, VM. For the radiopacity of esthetic restorative dental materials to exceed that of enamel, it should be greater than 1.7mm of equivalent thickness of aluminum.

  • Purpose: Heterotopic calcification is the abnormal deposition of calcium salts in tissues other than bone and enamel, and it occurs in the form of dystrophic calcification or metastatic calcification. This deposition can occur under many conditions, but in some rare cases, it may develop in burns and nonhealed scars. It is difficult to treat the combination of heterotopic calcification and ulceration in scar tissues by using conservative therapy and to determine the margin of excision in such cases. Our study proposes the use of intraoperative C-arm-guided mapping of lesions with heterotopic calcification, and adequate excision of ulcers in chronic scars where heterotopic calcification is also observed. Methods: This study included 2 patients and was conducted from January 2010 to July 2010. The first patient was a 63-year-old woman who presented with atypical calcium deposits and chronic ulceration in the lower one-third region of the right leg. The second patient was a 38-year-old man who presented with a nonhealing ulcer that had developed on the right leg 3 months earlier he had a history of 40% scalding burns on the entire body. Surgery is the most reliable method for treating heterotopic calcification therefore, both patients were treated using intraoperative C-arm-guided marginal mapping of heterotopic calcification, followed by release of contracture, and eventually split-thickness skin grafting. Results: Plain radiographs of the leg showed spotty radiopaque areas in the hard part of the scar well superficial to the underlying bones. Histopathological analysis revealed multiple foci of calcified deposits, increased fibrosis, and inflammation in the scar tissue. Surgery-related complications were not observed. Conclusion: C-arm guided excision of calcified scars and the release of contracture can cure nonhealing ulcers and may therefore prevent recalcification.

THE INFLUENCE OF pH ON REMINERALIZATION OF ARTIFICIAL DENTAL CARIES (법랑질 인공우식의 재광화에 미치는 pH의 영향에 관한 연구)

  • Much evidence now exists from both in vivo and in vitro studies to support the claim that small caries lesions can 'heal'. But, there are still different views on the mechanism of remineralization. So in order to find the best condition for the remineralization of incipient dental caries in maximum efficient way, the author conducted the experiment which reveals the effect of pH on remineralization. 40 specimens of sound permanent teeth without demineralization or crack, $100{\mu}m$ in thickness, were immersed in lactic acid buffered demineralization solution for 4 days. Dental caries with surface zone and subsurface lesion were artificially produced. All specimens were immersed in lactic acid buffered remineralization solution of pH 4.0, 4.5, 5.0, 5.5 containg fluoride ion for 10 days. The results were obtaind by observing the specimens for every 10 days under polarized microscopy at x25. 1. Remineralization did not occur in entire depth of body of lesion at given degree of saturation and concentration of fluoride ion. 2. The pattern of remineralization has increased according to increase of pH. So it can be concluded that supersaturated solution with fluoride ion can be affected by pH in remineralization of enamel, and pH 5.5 seems to be very effective in remineralization of deep and surface zone of dental caries. However, more complex factors exist in achieving complete remineralization and further continuous researches are needed to clarify the factors.

  • The patients' demand for treatment of unesthetic anterior teeth is steadily growing. Accordingly, several treatment options such as porcelain laminate veneers(PLV) have been proposed to restore the esthetic appearance of the dentition. Porcelain laminate veneers are considered minimally invasive, but they also require removal of sound enamel. One critical step in the porcelain laminate veneer technique is the achievement of sufficient ceramic thickness, and several different strategies for tooth preparation can be found in the literature. This clinical report describes a step-by-step protocols for preparation of these restorations used with the silicone index from diagnostic wax-up and the acrylic resin mock-up. Conservative use of porcelain laminate veneers provided satisfactory esthetic outcomes and preserved sound tooth structure.

Preparation and antimicrobial assay of ceramic brackets coated with TiO2 thin films

  • Objective: Different methods have been utilized to prevent enamel demineralization and other complications during orthodontic treatment. However, none of these methods can offer long-lasting and effective prevention of orthodontic complications or interventions after complications occur. Considering the photocatalytic effect of $TiO_2$ on organic compounds, we hoped to synthesize a novel bracket with a $TiO_2$ thin film to develop a photocatalytic antimicrobial effect. Methods: The sol-gel dip coating method was used to prepare $TiO_2$ thin films on ceramic bracket surfaces. Twenty groups of samples were composed according to the experimental parameters. Crystalline structure and surface morphology were characterized by X-ray diffraction and scanning electron microscopy, respectively; film thickness was examined with a surface ellipsometer. The photocatalytic properties under ultraviolet (UV) light irradiation were analyzed by evaluating the degradation ratio of methylene blue (MB) at a certain time. Antibacterial activities of selected thin films were also tested against Lactobacillus acidophilus and Candida albicans. Results: Films with 5 coating layers annealed at $700^{\circ}C$ showed the greatest photocatalytic activity in terms of MB decomposition under UV light irradiation. $TiO_2$ thin films with 5 coating layers annealed at $700^{\circ}C$ exhibited the greatest antimicrobial activity under UV-A light irradiation. Conclusions: These results provide promising guidance in prevention of demineralization by increasing antimicrobial activities of film coated brackets.

Supraeruption as a consideration for implant restoration

  • Purpose: The aim of this study was to compare the prevalence, amount, and rate of supraeruption of the maxillary second molar according to sex, age, and history of periodontitis. Methods: Data were collected retrospectively from the charts and panoramic radiographs of 65 patients who were scheduled to undergo implant placement at the site of the mandibular second molar. The amount of supraeruption of the maxillary second molar and the alveolar bone level of the neighboring teeth were measured on digital panoramic radiographs. The prevalence was evaluated in each group, and univariate and multivariate logistic regression analyses were used to identify factors influencing the prevalence of supraeruption. The amount and the rate of supraeruption were compared between pairs of groups using the Mann-Whitney U test. For all tests, P values <0.05 were considered to indicate statistical significance. Results: Supraeruption occurred in 78% of the patients. The prevalence of supraeruption was affected by sex, age, and history of periodontitis. The mean amount of supraeruption was 0.91 mm and the mean rate of supraeruption was 0.14 mm/month. The amount and the rate of supraeruption showed no significant differences according to sex, age, or the distance from the cementoenamel junction to the alveolar bone crest (P>0.05). Conclusions: These results show that the amount of supraeruption on the maxillary second molar was similar to the thickness of the enamel on the occlusal surface. When a single implant is scheduled to be placed on the mandibular second molar, supraeruption of the antagonist should be considered.

A Scanning Electron Microscopic Study on the Effect of Nd:YAG Laser Irradiation on the Cervical Cementum (Nd:YAG 레이저조사가 치경부 백악질에 미치는 영향에 관한 주사전자현미경적 연구)

  • In order to obtain the basic data concerning the optimal lasing conditions in using Nd:YAG laser as an adjunctive modality of periodontal therapy of root planing without irreversible structural deterioration of cervical cementum, the author selected 36 human teeth having no cervical abrasion and caries (; 12 anteriors, 12 premolars. 12 molars) extracted due to periodontal diseases, and divided them into 4 groups as Group I, II, III and IV (; each group of 3 anteriors, 3 premolars, 3 molars), and prepared a cementum specimen with thickness of $1.0mm{\pm}0.2mm$ sectioned longitudinally at the middle of mesio-distal thickness (; Group I) or horizontally at 1mm-2mm below the cemento-enamel junction (; Group II, III, IV) from each tooth of each group by low speed diamond wheel saw, and treated them with 0.5 M ethylene diamine tetraacetic acid (; EDTA, pH=7.4) for 2 minutes for elimination of remnants during tooth-sectioning. And the author applied the laser energy from a fiberoptic delivered, free running, pulsed Nd:YAG laser (; wavelength 1064nm. pulse duration $120{\mu}sec$, fiber diameter $320{{\mu}m}$) to cementum surfaces in triplicates of one experiment under the following lasing conditions: 1. stationary mode of fiber in contact to cementum surfaces without air-spray (; Group I )/with air-spray (; Group II), 2. unidirectional moving mode of fiber in contact to cementum surfaces under speed of 3mm-4mm/sec without air-spray (; Group III)/with air-spray (; Group IV), 3. energy per pulse (mJ/pulse) [; energy density ($J/cm^{2}$)] in order of 1.0W/10Hz (100J/pulse); $124J/cm^{2}$, 0.5W/10Hz (50mJ/pulse); $62J/cm^{2}$, 0.4W/10Hz (40mJ/pulse); $50J/cm^{2}$, 0.3W/10Hz (30mJ/pulse); $37J/cm^{2}$, 4. exposure time of 1 second. And the author applied the platinum coating on surfaces of cementum specimens, and evaluated the characteristics of ultrastructural change on surfaces of cementum using the scanning electron microscopy. In general the ultrastructural loss of cervical cementum irradiated under the same lasing condition of laser energy density occurred least in specimens of Group IV. And especially, the ultrastructural loss of cervical cementum irradiated under the laser energy density of $37J/cm^{2}$ almost did not occur in specimens of Group IV. Therefore, it is considered that the pulsed Nd:YAG laser should be applied with the lasing conditions of unidirectional moving mode of fiber in contact to cementum surfaces under speed of 3mm-4mm/sec with air-spray and of laser energy density within $37J/cm^{2}$ as an adjunctive modality of periodontal therapy of root planing without irreversible structural deterioration of cervical cementum.

The Comparative Study of Alveolar Bone Level and Root Form of the Mandibular Molar on Radiographic Image and Clinical Examination (방사선사진과 임상검사에서 하악 대구치 치근의 형태학적 구조 및 치조골 수준에 관한 비교연구)

  • Periodontal defects of the furcation are characterized by several inherent anatomic factors that can make successful periodontal therapy difficult and results unpredictable. The severity and rate of occurrence of periodontal disease are directly related to the location of the furcation relative to the cementa-enamel junction and anatomical form of the root by limiting the accessibility and effectiveness of the periodontal instrumentation. This study investigated the reliability and accuracy of panoramic radiograph diagnoses of the periodontal state of mandibular molars, particularly regarding the diagnosis of furcation area periodontal defects, treatment planning, and prognosis prediction. This study examined a total of 110 teeth belonging to 33 subjects (19 male, 14 female) presenting with incipient to moderate periodontitis 4-7mmpocket depth. The alveolar bone level, length and width of the root trunk, and root separation angle were measured using the panoramic radiograph and compared to the results taken directly by retracting a full-thickness flap. The results of the study are as follows: 1. Data regarding the alveolar bone level of the mandibular first molar showed that the directly taken surgical measurements resulted in $5.1{\pm}0.9mm$ that was slightly deeper than the corresponding panoramic measurement resulted in $4.8{\pm}0.8mm$, but these differences were statistically insignificant (p>0.05). 2. The data of the directly taken surgical measurement of the mandibular second molar $(5.1{\pm}1.1mm)$ was slightly deeper than the corresponding panoramic measurement $(4.7{\pm}1.2mm)$, but these differences were statistically insignificant (p>0.05). 3. The measured values of the length and width of the mandibular first molar root trunks were determined to be $4.1{\pm}0.6mm$ and $7.3{\pm}0.9mm$, respectively, while the values of the mandibular second molar root trunks were determined to be $4.6{\pm}1.3mm$ and $7.6{\pm}0.9mm$ respectively. The differences between these values were found to be statistically significant (p<0.01). 4. The measured values of the root separation angle showed that the mandibular first molars averaged $34.5{\pm}4.4^{\circ}$, while the mandibular second molars averaged $23.0{\pm}10.0^{\circ}$. The differences between these values were found to be statistically significant (p<0.01).

Radiopacity of contemporary luting cements using conventional and digital radiography

  • Purpose: This study evaluated the radiopacity of contemporary luting cements using conventional and digital radiography. Materials and Methods: Disc specimens (N=24, n=6 per group, ø$7mm{\times}1mm$) were prepared using 4 resin-based luting cements (Duolink, Multilink N, Panavia F 2.0, and U-cem). The specimens were radiographed using films, a complementary metal oxide semiconductor (CMOS) sensor, and a photostimulable phosphor plate (PSP) with a 10-step aluminum step wedge (1 mm incremental steps) and a 1-mm-thick tooth cut. The settings were 70 kVp, 4 mA, and 30 cm, with an exposure time of 0.2 s for the films and 0.1 s for the CMOS sensor and PSP. The films were scanned using a scanner. The radiopacity of the luting cements and tooth was measured using a densitometer for the film and NIH ImageJ software for the images obtained from the CMOS sensor, PSP, and scanned films. The data were analyzed using the Kruskal-Wallis and Mann-Whitney U tests. Results: Multilink (3.44-4.33) showed the highest radiopacity, followed by U-cem (1.81-2.88), Panavia F 2.0 (1.51-2.69), and Duolink (1.48-2.59). The $R^2$ values of the optical density of the aluminum step wedge were 0.9923 for the films, 0.9989 for the PSP, 0.9986 for the scanned films, and 0.9266 for the CMOS sensor in the linear regression models. Conclusion: The radiopacities of the luting materials were greater than those of aluminum or dentin at the same thickness. PSP is recommended as a detector for radiopacity measurements because of its accuracy and convenience.
