Multifactorial Traits of SARS-CoV-2 Cell Entry Related to Diverse Host Proteases and Proteins

  • The most effective way to control newly emerging infectious disease, such as the severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) pandemic, is to strengthen preventative or therapeutic public health strategies before the infection spreads worldwide. However, global health systems remain at the early stages in anticipating effective therapeutics or vaccines to combat the SARS-CoV-2 pandemic. While maintaining social distance is the most crucial metric to avoid spreading the virus, symptomatic therapy given to patients on the clinical manifestations helps save lives. The molecular properties of SARS-CoV-2 infection have been quickly elucidated, paving the way to therapeutics, vaccine development, and other medical interventions. Despite this progress, the detailed biomolecular mechanism of SARS-CoV-2 infection remains elusive. Given virus invasion of cells is a determining factor for virulence, understanding the viral entry process can be a mainstay in controlling newly emerged viruses. Since viral entry is mediated by selective cellular proteases or proteins associated with receptors, identification and functional analysis of these proteins could provide a way to disrupt virus propagation. This review comprehensively discusses cellular machinery necessary for SARS-CoV-2 infection. Understanding multifactorial traits of the virus entry will provide a substantial guide to facilitate antiviral drug development.

간호대학생을 위한 코로나19 간호교육 프로그램 개발 및 평가 (The development and effects of a COVID-19 nursing education program for nursing students)

  • Purpose: This study was conducted to develop and test the effects of a coronavirus disease 2019 (COVID-19) education program for nursing students. Methods: This study used a non-equivalent control group pretest-posttest design. A total of 71 undergraduate nursing students who had completed a fundamental nursing course and were in their third or fourth year of university participated in this study (experimental group n=38, control group n=33). The COVID-19 education program was developed according to the stages of analysis, design, development, implementation, and evaluation. The program consisted of three weekly sessions that each lasted about two hours. The data were collected from February 26 to March 26, 2022 and were analyzed using the chi-squared, Fisher's exact, and independent t-tests. Results: Knowledge about COVID-19, knowledge about personal protective equipment and attitudes toward COVID-19 were not significantly different between the experimental and control groups. However, significant differences were found between the experimental and control groups regarding their confidence in COVID-19 patient nursing care (t=3.02, p=.004). Conclusion: It was confirmed that the developed program was effective in enhancing the confidence of nursing students in performing nursing for COVID-19. Therefore, it is proposed that the education program developed in this study be used as part of an infection control education program to improve nursing students' abilities to respond to emerging infectious diseases, including COVID-19.

시군구 수준의 사회자본 추이와 사회자본과 COVID-19 관련 행위와 인식 간의 관계 (Social Capital Trends and the Relationship between Social Capital and COVID-19-Related Behaviors & Perceptions)

  • Background: The influence of social capital on the spread of coronavirus disease 2019 (COVID-19) and related behaviors and perceptions has been recognized during the COVID-19 pandemic. This study aims to analyze the trends in social capital using primary data from the Korean Community Health Survey, which is the only available source in Korea for local-level social capital analysis. It also investigates the relationship between various variables, including social capital, as factors influencing COVID-19-related behaviors and perceptions. Methods: The study analyzed the temporal trends of social capital using raw data from four community health surveys conducted in 2017, 2019, 2020, and 2021. A multilevel analysis was conducted to examine the relationship between social capital and COVID-19-related behaviors and perceptions following the onset of the COVID-19 pandemic in 2020. Results: Social capital consists of trust, bonding social capital, and bridging social capital. Within the trust sub-factor, trust in neighbors (Trust-1) declined after the COVID-19 pandemic, whereas trust in safety and general environment (Trust-2) and trust in medical services and public transportation (Trust-3) increased. Additionally, the gap between municipalities narrowed. COVID-19-related behaviors and perceptions, such as adherence to COVID-19 prevention measures, return to normal activities, and fear of COVID-19, showed improvement in 2021 compared to the previous year. Individual-level trust in neighbors was associated with reduced fear of COVID-19, while community-level trust in neighbors was associated with increased fear of COVID-19. Conclusion: Social capital plays a role in mitigating public health crises, and it is necessary to implement active policies that address the gap in social capital between metropolitan and rural areas. Strengthening risk communication regarding emerging infectious diseases such as COVID-19 is crucial.

코로나19 이후 장애학생 교육 격차 지원을 위한 특수교육 현황과 요구 (Current status and needs for special education to support educational gaps for students with disabilities after COVID-19)

  • 2023년 코로나19가 4급 감염병으로 전환되며 안정추세에 접어들었지만 특수교육 현장에서는 코로나19 기간 동안 원격 수업 등 비대면 학습상황으로 인해 발생한 장애학생의 학업, 사회적 발달 격차를 지원하는 것의 중요성이 대두되고 있다. 이에 본 연구에서는 코로나19이후 장애학생의 교육 현황과 학업 결손을 파악하고 이를 지원하기 위해 17개시도 특수교사 2,214명을 대상으로 설문조사를 실시하고 결과를 분석하였다. 연구 결과, 코로나19로 인해 장애학생들은 학업, 정서, 행동 측면에서 발달지연과 교육격차가 심화되었고 이를 지원하기 위한 방안으로 인력지원, 심리상담, 정서행동에 대한 의료적 지원 요구가 높게 나타났다. 본 연구결과를 토대로 후속 결과를 제안하였다.

Bacterial communities in the feces of insectivorous bats in South Korea

  • Bats serve as vectors and natural reservoir hosts for various infectious viruses, bacteria, and fungi. These pathogens have also been detected in bat feces and can cause severe illnesses in hosts, other animals, and humans. Because pathogens can easily spread into the environment through bat feces, determining the bacterial communities in bat guano is crucial to mitigate potential disease transmission and outbreaks. This study primarily aimed to examine bacterial communities in the feces of insectivorous bats living in South Korea. Fecal samples were collected after capturing 84 individuals of four different bat species in two regions of South Korea, and the bacterial microbiota was assessed through next generation sequencing of the 16S rRNA gene. The results revealed that, with respect to the relative abundance at the phylum level, Myotis bombinus was dominated by Firmicutes (47.24%) and Proteobacteria (42.66%) whereas Miniopterus fuliginosus (82.78%), Rhinolophus ferrumequinum (63.46%), and Myotis macrodactylus (78.04%) were dominated by Proteobacteria. Alpha diversity analysis showed no difference in abundance between species and a significant difference (p < 0.05) between M. bombinus and M. fuliginosus. Beta-diversity analysis revealed that Clostridium, Asaia, and Enterobacteriaceae_g were clustered as major factors at the genus level using principal component analysis. Additionally, linear discriminant analysis effect size was conducted based on relative expression information to select bacterial markers for each bat species. Clostridium was relatively abundant in M. bombinus, whereas Mycoplasma_g10 was relatively abundant in R. ferrumequinum. Our results provide an overview of bat guano microbiota diversity and the significance of pathogenic taxa for humans and the environment, highlighting a better understanding of preventing emerging diseases. We anticipate that this research will yield bioinformatic data to advance our knowledge of overall microbial genetic diversity and clustering characteristics in insectivorous bat feces in South Korea.

Characterization of a Small Plaque Variant Derived from Genotype V Japanese Encephalitis Virus Clinical Isolate K15P38

  • Genotype V (GV) Japanese encephalitis virus (JEV) has been predominantly reported in the Republic of Korea (ROK) since 2010. GV JEV exhibits higher virulence and distinct antigenicity compared to other genotypes, which results in reduced efficacy of existing vaccines. Research on GV JEV is essential to minimize its clinical impact, but the only available clinical strain in the ROK is K15P38, isolated from the cerebrospinal fluid of a patient in 2015. We obtained this virus from National Culture Collection for Pathogens (NCCP) and isolated a variant forming small plaques during our research. We identified that this variant has one amino acid substitution each in the PrM and NS5 proteins compared to the reported K15P38. Additionally, we confirmed that this virus exhibits delayed propagation in vitro and an attenuated phenotype in mice. The isolation of this variant is a critical reference for researchers intending to study K15P38 obtained from NCCP, and the mutations in the small plaque-forming virus are expected to be useful for studying the pathology of GV JEV.

Selection of Vaccinia Virus-Neutralizing Antibody from a Phage-Display Human-Antibody Library

  • Although smallpox was eradicated in 1980, it is still considered a potential agent of biowarfare and bioterrorism. Smallpox has the potential for high mortality rates along with a major public health impact, eventually causing public panic and social disruption. Passive administration of neutralizing monoclonal antibodies (mAbs) is an effective intervention for various adverse reactions caused by vaccination and the unpredictable nature of emerging and bioterrorist-related infections. Currently, vaccinia immune globulin (VIG) is manufactured from vaccinia vaccine-boosted plasma; however, this production method is not ideal because of its limited availability, low specific activity, and risk of contamination with blood-borne infectious agents. To overcome the limitations of VIG production from human plasma, we isolated two human single-chain variable fragments (scFvs), (SC34 and SC212), bound to vaccinia virus (VACV), from a scFv phage library constructed from the B cells of VACV vaccine-boosted volunteers. The scFvs were converted to human IgG1 (VC34 and VC212). These two anti-VACV mAbs were produced in Chinese Hamster Ovary (CHO) DG44 cells. The binding affinities of VC34 and VC212 were estimated by competition ELISA to $IC_{50}$ values of $2{\mu}g/ml$ (13.33 nM) and $22{\mu}g/ml$ (146.67 nM), respectively. Only the VC212 mAb was proven to neutralize the VACV, as evidenced by the plaque reduction neutralization test (PRNT) result with a $PRNT_{50}$ of ~0.16 mg/ml (${\sim}1.07{\mu}M$). This VC212 could serve as a valuable starting material for further development of VACV-neutralizing human immunoglobulin for a prophylactic measure against post-vaccination complications and for post-exposure treatment against smallpox.

항생제 사용과 세균성 어류질병의 치료 (The utilization of antibiotics and the treatment of bacterial diseases in fish)

  • 항생제란 어떤 미생물의 생성물질로서 다른 미생물의 증식을 억제하거나 살균작용을 나타내는 것을 의미한다. 현재까지 알려진 4000여종의 항생제중, 수산용으로는 약 20여종이 사용되고 있다. 이러한 항생제의 분류는 화학적 구조와 부수되는 기능에 의하는 것이 일반적이며 그 효과는 적용되는 병원성균의 종류와 어류가 처해있는 환경에 따라 대단히 영향을 받는다. 그러므로 항생제의 종류와 적용방법은 정확한 질병에 대한 원인 규명이 있은 후 결정되어져야 할 것이다. 무절제하게 사용된 항생제는 자연적으로 발생하여 존재하고 있는 내성균을 선택적으로 증식 가능케 한다. 이미 일본의 수많은 양어장에서 내성균 출현의 보고가 있었고, 많은 종류의 내성균은 R plasmid를 가지고 있다는 것도 밝혀졌다. 우리나라의 양어장에서도 내성균 출현과 R plasmid에 의한 내성균 확산은 곧 문제가 될 수 있는 가능성이 크므로 전반적인 역학조사가 있어야 할 것이다. 상용항생제의 어류에 대한 약동력학, 잔존량, 임상적용 실험등에 대한 연구는 상당히 되어 있어 적절한 이들 항생제의 사용이 가능하지만, 현재까지 대부분의 수산용 항생제에 대해서는 연구해야 될 부분이 아직 많이 남아 있다. 그러므로 어류질병 치료와 항생제의 정확한 사용을 위해서는 양식사업을 하고 있는 현장과 진단을 할 수 있는 실험실의 긴밀한 협조관계가 무엇보다 중요하다 할 것이다.

폐렴환자에서 진동 공명 영상 검사(VRI)의 유용성 (Usefulness of Vibration Response Imaging (VRI) for Pneumonia Patients)

  • Background: Pneumonia is commonly seen in outpatient clinics. it is widely known as the most common cause of death from infectious disease. Pneumonia has been diagnosed by its typical symptoms, chest X-ray and blood tests. However, both chest X-rays and blood tests have limitations in diagnosis. Thus primary care clinicians usually have been constrained due to a lack of adequate diagnostic tools. Vibration response imaging (VRI) is a newly emerging diagnostic modality, and its procedure is non-invasive, radiation-free, and easy to handle. This study was designed to evaluate the diagnostic usefulness of the VRI test among pneumonia patients and to consider its correlation with other conventional tests such as Chest X-ray, laboratory tests and clinical symptoms. Methods: VRI was performed in 46 patients diagnosed with pneumonia in Konkuk University Medical Center. VRI was assessed in a private and quiet room twice: before and after the treatment. Sensors for VRI were placed on a patient's back at regular intervals; they detected pulmonary vibration energy produced when respiration occurred and presented as specific images. Any modifications either in chest X-ray, C-reactive protein (CRP), white blood cell count (WBC) or body temperature were compared with changes in VRI image during a given time course. Results: VRI, chest X-ray and CRP scores were significantly improved after treatment. Correlation between VRI and other tests was not clearly indicated among all patients. But relatively severe pneumonia patients showed correlations between VRI and chest X-ray, as well as between VRI and CRP. Conclusion: This study demonstrates that VRI can be safely applied to patients with pneumonia.

Apodemus agrarius as a new definitive host for Neodiplostomum seoulense

  • A total of 1,496 rodents and insectivores were live-trapped at Yeoncheon-gun (n = 351), Paju-shi (804), and Pocheon-gun (343), Gyeonggi-do (Province), and examined for intestinal helminths, including Neodiplostomum seoulense, seasonally from December 2004 to September 2005. Six species of rodents, including Apodemus agrarius (1,366), Mus musculus (32), Micronytus fortis (28), Eothenomys regulus (9), Micronys minutus (6), and Cricetulus triton (3), and 1 species of insectivores Crocidura lasiura (54) were collected. A total of 321 adult N. seoulense were collected from 19 (1.4%) A. agrarius. The worm burden ranged from 1 to 101 per A. agrarius (mean; 16.9). No N. seoulense was observed in other rodent or insectivore species examined. The infection rate during autumn (4.5%) was higher than those during spring (0.8%), summer (0.8%), and winter (0.5%). The average number of N. seoulense in infected A. agrarius was the highest in spring (66.0 specimens), followed by autumn (15.2), winter (4.5), and summer (3.3). This study first confirms that A. agrarius is a natural definitive host for N. seoulense, and demonstrates that the infection rates and intensities vary seasonally and geographically.
