A study of pre-hospital emergency medical system at correctional institutions (교정시설의 병원 전 응급의료체계에 대한 고찰)

  • The purpose of EMS is to provide emergency patients with prompt and suitable emergency treatments therefore it saves their lives and makes them recover fast to return to normal. The purpose of correctional administration is to return inmates to society as sound citizens after their service by providing various correctional education, reformational programs, vocational training and etc. In this way, the aim of correctional administration and EMS is like mutual agreement by their returning to society safely. Therefore this study makes some propositions like the followings for the safety between many inmates who can have physical injury and exposure to diseases caused by particularity of subculture within correctional institutions and the staffs who work for them. It is said about efficient pre-hospital EMS through various causes of emergency situation in the correctional institution, system, manpower, facility, equipments and problems and so on. 1. Recruit the first-rate EMT(emergency medical technician) as central role of pre-hospital EMS according to each correctional institution and working terms. 2. Equip specialized transference system with symptom of patients and purpose of transference. 3. Emergency medical equipments and first-aid medicines should be equipped for field and ambulance. 4. Educate correctional officers as first responders and inmates within correctional institutions about systematic emergency treatment.

Projection of Demand and Supply for Emergency Medical Technician by Using Produce Constants (배출상수를 이용한 응급구조사 수급전망)

  • Purpose : It was to find out demand and supply of EMT from 2007 to 2045 and to expand scope of practice of paramedic in Korea. Methods : This study was conducted by applying a projection formula to the data from admission quota for EMT of the Ministry of Education & Human Resources Development, rate of successful candidates of annual report of the National Health Personnel Licensing Examination Board etc. Results : The number of EMTs were 6,043 paramedics, 5,378 EMT-Bs until 2006 and two produce constants derived from simple estimation were paramedic 0.81, EMT-B 0.86. On the American basis of 5.6 EMTs per 10,000, the number of paramedics under the present act will reach the basis around 2015(5.02), the number of paramedics under the amended act will reach the basis around 2030(5.50). Conclusion : There was relationship between scope of practice of paramedic in the act and demand-supply of paramedic, this necessitates surveys, studies, amendment of the act, legalization for expanded scope of practice of paramedic.

The effects of emotional labor and job stress on the fatigue of 119 emergency medical technicians (119구급대원의 감정노동과 직무스트레스가 피로에 미치는 영향)

  • Purpose: This is a descriptive research study that aims to identify factors affecting fatigue by examining the degree of emotional labor, job stress, and fatigue of 119 emergency medical technicians (119 EMTs). The correlation between emotional labor, job stress, and fatigue was also confirmed. Methods: A total of 139 EMTs working in the Daejeon area participated in this study. Data were collected from July 6-15, 2021. Results: When the number of dispatches was high and the job was dissatisfying, the degree of emotional labor was high. In addition, the shorter the work experience and the lower the job satisfaction are, the higher the job stress is. The degree of fatigue was significantly higher in female than male 119 EMTs, and when they had short work experience and were dissatisfied with their job, the degree of fatigue was higher. Moreover, there was a significant correlation between emotional labor, job stress, and fatigue of the 119 EMTs. Conclusion: To reduce emotional labor, job stress, and fatigue of 119 EMTs, appropriate interventions for each factor and institutional improvements are required.

Prehospital care and improvement of 119 emergency medical technician for the insect bite patients (119 구급대원의 곤충교상환자에 대한 응급처치 현황 및 개선방안)

  • Purpose : This study aimed to improve quality of prehospital emergency care for the insect bite patients by figuring out its current situations and problems. Methods : This study was conducted to 219 insect bite patients who were transferred to the hospital by 119 ambulance in D-City from July 1, 2009 to June 30, 2011. This study was a descriptive study by 119 run sheets. A total of 171 emergency medical technicians(EMTs) working in D-city and C-province completed the questionnaire. All of data were analyzed by SPSS 18.0. Results : Among the 219 patients, bee sting accounted for 62.6% and wasp sting accounted for 20.1%. Some patients complained of dizziness, headache, dyspnea, and disturbance of consciousness. The 119 EMTs initially treated the patients with oxygen supply to 57 patients (31.1%). Knowledge toward insect bite was statistically significant (p=.000) between advanced EMTs and basic EMTs. The obstacle to the prehospital care, many EMTs answered that the obstacle was due to the lack of 119 EMTs. Conclusion : It is necessary to differentiate the patients' symptoms and provide the better prehospital emergency care to verify the exact symptoms and signs.

  • Recently, the task in which the number of people of the emergency medical technician whom it boards the ambulance is unreasonably insufficient and in which the fire fighter one person gets in the ambulance and which transfers the patient comes into question often. When the emergency patient is generated, it has to transfer to the special hospital in which above anything else, the measure which is quick and exact is needed and where there is the medical device which is suitable for particularly, the patient. This paper implementations the emergency medical system by the smart phone. The implemented system monitors the heart beat of the patient the monitoring among the patient transport with the real-time type. It can grasp the medical history information of the patient, and etc. in the past. And the system provides the emergency hospital which the patient requires and the pre-hospital phase provides the environment in which the disposition which is quick and efficient is possible to the emergency patient.

  • Purpose: The first responder's role during a cardiac arrest scene is to initiate CPR. The AHA has recognized and included the first responder's role for improving the survival rate of cardiac arrest patients. Health personnel working in nursing, emergency care, dental hygiene, radiology, and ocular optics frequently confront sudden cardiac arrest while working. This study was to identify the relationship between the educational experience and recognition with the level of knowledge about CPR for college students. Method: Five hundred forty college students enrolled in the department of nursing science, radiological technology, ocular optics, emergency medical technician, or dental hygiene in Daejeon city were surveyed. The tool used was CPR knowledge developed by the authors based on a literature review including 2005 AHA's CPR guideline. Result: The higher educational experience of CPR was, the higher the level of knowledge. The knowledge of the students in nursing or emergency medical technician was higher than students in dental hygiene, radiology, and ocular optics. Conclusion: CPR class should be included in the curriculum for college students in order to improve their accuracy as a first responder to cardiac arresting patients.

The experience of violence and turnover intention of female 119 emergency medical technicians (여성 119구급대원의 폭력 경험과 이직 의사)

  • Purpose : The purpose of this study was to provide an appropriated direction for fire service organizational management by analyzing the violence experiences and turnover intention of female 119 emergency medical technicians (119 EMT) in Korea. Methods : The questionaries were obtained from 415 female 119 EMT in Korea from April 27 to March 8 in 2009. The collected data were analyzed with SPSS WIN 19.0 program. Results : Female 119 EMTs experienced 94.9%-verbal insults, 57.1%-violence aggression, 62.89%- potential violence, 32.5%-physical assault. About 76.4% of the subjects experienced to feel shame about the verbal insults. They had a stress level (mean 5.47 out of 10) and about 41.2% was more than seven points. About 39.3% of the subjects responded turnover intention. Conclusion : We proposed the preventive program against violence as well as critical incident stress debriefing. These results are expected to be useful in making human resource management plans to lower turnover intention.

Assessing the forensic knowledge of 119 emergency medical technicians (119 구급대원의 법의학 지식에 대한 연구)

  • Purpose: It is crucial that 119 emergency medical technician (EMT) have adequate knowledge regarding forensic science. This study aimed to assess the forensic knowledge of EMTs in the Republic of Korea. Methods: This study was conducted on EMTs of five fire stations in D metropolitan city between August 6 and August 13, 2018. A questionnaire consisting of items on crime-related incident recognition (i.e., mechanical asphyxiation, trauma, sex crimes, and child abuse), forensic evidence management (i.e., evidence collection, preservation, and recording) was administered to the participants. Finally, 119 questionnaires were analyzed. Results: The ratio of correct answers for crime-related incidents recognition was 55.82%, which was lower than that for forensic evidence management(84.5%). In particular, the scores for the type of neck compression(16.5%) and wound assessment(44.0%) were low. The percentage of correct answers for forensic evidence management was 78.4% for evidence collection, 84.4% for evidence preservation, and 90.6% for evidence recording. Previous forensic education experience was not a significant variable. Conclusion: Specific and practical forensic science education on perception of crime-related incident, including asphyxia and wound identification, and forensic evidence collection is needed.

Comparison of endotracheal intubation speed and ease by using the supraglottic airway laryngopharyngeal tube: A manikin study (성문위기도기 인후두튜브(SALT)를 이용한 기관내삽관 신속성과 용이성 비교: 마네킨을 이용한 연구)

  • Purpose: The purpose of this study was to improve airway management ability by comparing the speed, angle of the neck, and confidence and ease of supraglottic airway laryngopharyngeal tube (SALT) and endotracheal intubation via direct laryngoscopy. Methods: The subjects of this experimental research study with a randomized crossover design were 44 emergency medicine technician - paramedics working in the fire department of 'J' - do. SPSS version 19.0 was used in the statistical analysis. Results: Speed and angle of the neck (p<.001), as well as confidence and ease (p<.001), showed significant differences between endotracheal intubation with a SALT and endotracheal intubation via direct laryngoscopy. Conclusion: If endotracheal intubation via direct laryngoscopy is difficult to use or in trauma patients, using a SALT is safe and enables fast intubation. Moreover, in order to improve the efficiency of advanced airway management, the application of SALT should be introduced in the domestic scene.
