보육교사 대상 영유아 건강관리 교육의 효과 (The Korea Academia-Industrial cooperation Society)

  • 본 연구는 보육교사들의 영유아 건강관리 능력을 향상시킬 수 있는 교육프로그램을 개발하여 적용하고 그 효과를 측정하기 위함이다. 연구대상은 3개 도시, 400명의 보육교사들이며 영유아 건강관리 교육 전 후의 교육효과를 검정하기 위해 단일군 전후설계 유사실험연구로 실시되었다. 자료수집 기간은 2013년 9월초부터 10월말까지 이루어졌으며, 영유아 건강관리 교육의 내용은 영유아 건강관찰, 영유아 응급처치, 영유아 건강문제관리 세 영역으로 구성되었다. 교육은 주1회, 3시간씩 3주간 총 9시간으로 이루어졌다. 영유아 건강관리 교육을 실시한 후 보육교사들의 '영유아 건강관찰', '영유아 응급처치', '영유아 질병관리'에 있어서의 지식점수는 교육전보다 교육후에 유의미하게 높은 것으로 나타났으며, 이러한 결과는 보육교사를 위한 영유아 건강관리 교육 프로그램이 효과가 있었다고 해석된다. 그러므로 보육교사의 영유아 돌봄의 질을 향상시키기 위해 이와 같은 교육이 보수교육에 적용되기를 기대한다.

응급실 간호사의 방사선 방어행위 영향요인 (Factors Affecting Radiation Protection Behaviors among Emergency Room Nurses)

  • Purpose : This study aimed to examine factors affecting radiation protective behaviors among emergency room nurses by assessing knowledge, attitude, and environment for radiation protection. Methods : The study employed a cross-sectional design. Subjects were a convenience sample of 129 nurses working in emergency rooms of three general hospitals. Data were collected using self-report questionnaires and analyzed using t-test, ANOVA, Pearson correlation coefficients, and multiple regression. Results : The more the nurses received radiation safety education (t=2.26, p=.026), used protective gear (t=4.40, p<.001), and took health screenings (t=2.65, p=.009), the higher their levels of radiation protection behavior. There were significant relations between radiation protective behaviors and attitude (r=.27, p=.002), and radiation protective behaviors and environment for radiation protection (r=.55, p<.001). The factors affecting radiation protective behaviors were protective environment (β=.53, p<.001), protective attitude (β=.32, p<.001), and the use of protective gear (β=.24, p=.002). Conclusions : The government, hospital administrators, and radiation protection-related organization should adopt the following measures to protect emergency room nurses from radiation: research and development of shield instrument, medical examination for emergency room nurses, protocol development of radiation protection behaviors, extension of education chances of radiation protection, and encouraging the use of protective equipment.

응급실 간호사의 아동학대 신고의도 (Intention to Report Child Abuse of Emergency Room Nurse)

  • Purpose: The purposes of this study were to examine a attitude, subjective norm, perceived behavior control, intention to report child abuse and to identify predictors of emergency room (ER) nurses' intention to report child abuse cases. Methods: A total of 364 nurses in ER participated in this study. Data were collected by self-administered questionnaire and analyzed with descriptive statistics, t-test or ANOVA, Pearson correlation, and stepwise multiple regression analysis. Results: 84.1 % of emergency room nurses reported feeling of obligation to report child abuse. Attitude, subjective norm, and perceived behavior control were positively correlated with intention to report child abuse. Intention to report child abuse was predicted by emergency room nurse's attitude, perceived behavior control, subjective norm, ER nurse career, and recognition as a reporting obligator. Conclusion: Child abuse education program should be developed to help ER nurses to report child abuse case and positive attitude toward reporting the cases.

응급환자의 수술 후 관리를 위한 시뮬레이션기반 교육프로그램의 효과 (Effect of a simulation-based program for post-operative care of emergency patients)

  • Purpose: This study aimed to confirm the effects of a simulation-based program on knowledge and clinical performance in the post-operative management of emergency patients. Methods: This was a pre- and post-research design with a nonequivalent control group and randomly sampled 29 experimental and control groups, respectively from nursing department juniors for 4 weeks from September of 2014. The experimental group received lectures, team study, team simulation, and debriefing in post-operative management of simulation-based emergency patients and control group conducted in the traditional lecture-type setting. Educational learning effects were measured by using the knowledge and clinical performance measurement tools of 15 and 20 items, respectively. Data were analyzed by using the SPSS program, including frequency, ratio, and results from the Chi-square test, Fisher's exact test, Kolmogorov-Smirnov test, t-test. Results: Our research results indicate that, the experimental group showed significantly higher knowledge and clinical performance score compared with the control group. Conclusion: We confirmed that education on post-operative management of simulation-based emergency patients was an effective educational method to improve the knowledge and clinical performance of nursing students.

응급외상 환자 시뮬레이션 적용 효과 (Implementation Effects of Emergency Trauma Patient Simulation)

  • Purpose: The purpose of this study was to explore EMT-paramedic students' experience of simulation education and analyze the confidence before and after education, learning attitude and course evaluation. Method: Research survey was conducted on 38 EMT-paramedic students during November, 2011 and EMT-paramedic students' experience of simulation education was analyzed after applying head, spinal, and chest injury scenario. The confidence before and after education, learning attitude and course evaluation in gender were analyzed by Mann-Whitny U test and the difference of confidence before and after education was analyzed by Wilcoxon signed rank test and learning attitude & course evaluation were analyzed by evaluating frequency, percentage, mean, standard deviation by using SPSS WIN 17.0 program. Results: 1. Students experienced various advantages such as increasing interest and self-reflection on learning, critical thinking ability, and EMT-paramedic-role experience and recognition of importance of teamwork. Students also pointed out disadvantages such as gap between real situation and simulation, limit of time and equipments, and burden of demonstration. 2. The confidence between before and after education, learning attitude and course evaluation in gender were not significant different statistically. 3. Confidence mean score elevated from 5.53(before education) to 5.87(after education), but the difference in their confidence did not show significant difference statistically. 4. Total mean score in learning attitude after simulation education was 3.70 out of 5.00, which is considerably very high. 5. Total mean score in course evaluation was 3.89 with score of 3.83 in evaluation in learning environment and 3.99 in evaluation of debriefing. Conclusion: The finding of this study demonstrate that the simulation education can provide a safe and repetitive practice environment, improve problem-solving ability and critical thinking, and increase the confidence in prehospital emergency care; therefore, simulation may be the new effective EMT-paramedic education strategy.

보육현장 아동학대에 대한 근거기반 맞춤형 개입 모델 개발 및 적용: 다학제간 협력적 연계 시스템을 중심으로 (The Application of Customized Evidence-based Counseling for Abused Preschool Children in Day Care Centers: Focusing on the Multidisciplinary Cooperative Linking System)

  • Objective: The purpose of this study was to explore the application of the multidisciplinary model, which is customized evidence-based counseling for abused preschool children done by a day care center teacher. Methods: The participants were 16 preschool children which were abused by a day care center teacher and their parents. The data were analyzed by using the Wilcoxon Signed-rank test. Results: The multidisciplinary cooperative service model based on customized evidence-based counseling for abused preschool children, their parents, and day care center teachers was developed. Based on this model, customized evidence-based Post-Traumatic Stress Disorder (PTSD) emergency intervention was provided to preschool children abused by a day care teacher in Incheon. The multidisciplinary cooperative emergency intervention service proved to be effective in reducing emotional and behavioral problems of abused preschool children and enhancing the mental health of parents. Conclusion/Implications: The results of this study indicated that the customized evidence-based counseling for abused preschool children done by a day care center teacher using the multidisciplinary cooperative linkage system would be effective, and that child abuse prevention by day care center teachers and intervention services should be provided systematically at the national level.

응급실 간호사와 응급구조사의 외상성 사건의 경험, 외상후 스트레스장애에 대한 지식, 태도 및 회복탄력성 (Experiences of traumatic events, knowledge and attitudes concerning post-traumatic stress disorder, and resilience among nurses and paramedics working in emergency department)

  • Purpose: This study aimed to assess exposure to traumatic events, knowledge and attitudes concerning post-traumatic stress disorder (PTSD) and the level of resilience among nurses and paramedics working in emergency departments. Methods: Data were collected from May 22 to June 12, 2022, using a self-administered survey questionnaire. The participants comprised 135 nurses and 80 paramedics working in emergency departments. The collected data were analyzed with descriptive statistics, a t-test, and an analysis of variance with Scheffé's test. Results: Compared with emergency room nurses, paramedics were more positive about the government's spending on job opportunities for people with PTSD. There were no significant differences in attitude regarding government strategies and people with PTSD between nurses and paramedics. Paramedics had higher scores on the effective treatment for PTSD, while emergency room nurses showed higher scores on effective psychotherapy. General knowledge of PTSD differed according to sex (t=-2.33, p=.021) and education level (F=3.21, p=.042). Resilience scores differed significantly according to sex (t=2.02, p=.045), education level (F=4.10, p=.018), self-reported economic state (F=10.34, p<.001), and self-reported health (F=11.57, p<.001). Conclusion: The findings support that emergency department nurses and paramedics are in need of self-care programs to support their mental health and indicate that intervention programs should be developed to enhance resilience in emergency department professionals.

노인 장기요양기관에 종사하는 사회복지사의 임종 케어 경험과 역할에 관한 질적 연구 (Qualitative Study on Social Workers' Experiences and Roles during End-of-Life Care in Elderly Long-Term Care Facilities)

  • 본 연구는 노인 장기요양 서비스를 제공하는 기관에서 종사하는 사회복지사들이 임종 케어 과정에서 겪는 심리·사회적 경험 및 역할에 대해 알아보았다. 심층 면접을 통한 자료 분석 결과 사회복지사들은 임종 케어 과정에서 빈번한 죽음 경험으로 인해 기가 빠지고 소진됨을 경험하였으며 돌아가신 노인분들에게 더 잘해드리지 못한 것에 대해 아쉬움과 정든 분에 대한 그리움을 나타냈다. 그리고 갑작스러운 죽음이 발생하면 가족들의 비난과 불평을 겪기도 하였다. 임종 케어시 사회복지사들의 주된 역할로 죽음 징후가 나타나면 가족들에게 수시로 연락하여 소통하였고 임종 후 주 부양자 가족들을 위로하고 지지하였다. 응급상황 대처를 위한 사전준비와 응급조치에 대한 법적 안전망, 사회복지사들의 소진을 막기 위한 재충전 프로그램 및 지지 모임, 임종 케어를 위한 표준화된 매뉴얼과 교육의 필요성이 제시되었다. 또한 노인분의 존엄한 죽음을 위하여 죽음 준비교육, 호스피스 케어, 사전 의료의향서 등의 필요성도 강조되었다.

원주기독병원 응급실로 내원한 치과 응급환자에 관한 임상적 연구 (THE CHARACTERISTICS ON THE DENTAL EMERGENCY PATIENTS OF WONJU CHRISTIAN HOSPITAL FOR LAST 10 YEARS)

  • The appropriate care to the dental emergency patients is much important in the aspect of community dental service. To attain such a purpose, the sacred duty of the training of oral and maxillofacial surgeons is required. So, a retrospective study on the characteristics of dental injuries and diseases in emergency care unit will be very meaningful. This study was carried by reviewing the charts and radiographic films of 3,394 patients, treated for dental emergency at Wonju Christian Hospital, Republic of Korea, from January 1, 1993 to December 31, 2002. All patients were classified to 6 groups including trauma, toothache, infection, hemorrhage, TMJ disorder and the others. The clinical characteristics of diseases and treatment modalities according to each group were analyzed. The trauma (73.9%) was the most frequent cause in dental emergency patients, and acute toothache, odontogenic infection, oral hemorrhage, and TMJ disorder were next in order. Gender prediction was male (68%), there were many patients on May and December in the monthly frequency, and the most frequent age group was from 0 to 9 years. In the trauma group, male (68.6%) was predominant, and soft tissue injuries and primary closures were the most frequent type of injury and treatment. In jaw fractures, traffic accidents were the most cause and the weakest site was mandibular symphysis area, and mandibular angle, condyle, and body area were next in order. In the acute toothache group, the cause was dental pulpitis mostly and treatment for that was drug administration mainly. Buccal space abscess in infection group had the largest incidence (24.5%), and common treatments were incision and drainage and medications. In the hemorrhage group, a major cause was postoperative bleeding (60.3%) and hemostasis was obtained by pressure dressing, curettage and suture. For the TMJ disorder group, the peak incidence (63.8%) was shown in the post-traumatic myofascial pain dysfunction syndrome and its primary care was medication such as analgesics and sedatives. In the other group, the various specific symptoms were complained due to acute sialadenitis, trigeminal neuralgia, acute stomatitis, chemical burn, terminal stage neuritis of head and neck cancer, and foreign body aspiration. In conclusion, for the rapid and proper care of the emergency dental diseases, well-trained education should be presented to the intern and resident course of oral and maxillofacial surgery. And it is demanded that oral and maxillofacial surgeons must be prepared in knowledge and skill for such emergency care.

응급구조과 학생의 죽음에 관한 의식 (The Death Orientation of Paramedic Student)

  • Objective : The purpose of this study was to investigate the perception of death in paramedic student. The results of this study will help develop education program for death orientation, paramedic students education and practice. Methods : A total of 201 paramedic students filled out the questionnaire. The perception of death was examined using questionnaires designed for examining Death Orientation. The data were analyzed with SPSS 18.0 statistics program for frequency, percentage, average, standard deviation, t-test, ANOVA, cronbach alpha coefficient, factor analysis. cronbach alpha coefficient was .866. Results : The paramedic students' got a score of $2.35({\pm}.48)$ on the Orientation on death as average. The reason is that death is not yet pressing them at all and ahead of their lives they have many days to live. With respect to the Death Orientation, significant differences was found in experience of death(t=2.318, p=.021). Group of death experience was higher than group of no death experience. In view of the attitude on afterlife, students responded no afterlife(24.9%), unknown after death(22.4%). Conclusion : The results of this study suggest that paramedic curriculum should include education program on death and improve quality of prehospital care. Additional studies are needed to establish death education for paramedic.
