Purpose: The study was performed to compare the contents of jobs and job satisfaction of resident nurses at free elderly homes with nursing homes in Korea. Method: The subjects consisted of 138 resident nurses at free elderly homes and nursing homes. Data were collected from the 1st of January to the 22nd of February in 2005 using a structured questionnaire containing questions on general characteristics. contents of the nurses' daily tasks, health program. job satisfaction and pay satisfaction. Collected data were analyzed using SPSS (version 11.5) through descriptive statistics, $X^2-test$, t-test and ANOVA. Result: For nurses at free elderly homes and nursing homes. their daily tasks consisted of health assessment. medication care and eating care. The general job satisfaction of the nurses was similar between those at free elderly homes and those at nursing homes, but working conditions and pay were significantly different according to the sub content of job satisfaction. Job satisfaction was significantly different according to marital status in resident nurses at the free elderly home, and according to the experience of living with the elderly and the experience of receiving special training or lectures in elderly nursing in resident nurses at free nursing homes. Conclusion: Resident nurses at free elderly homes and nursing homes had various types of tasks, and it is necessary to differentiate tasks and payment systems for them according to their job.
The aim of this research was to analyze the differences for the oral health status of the elderly people living in the long-term care facilities and the non-resident elderly. The study was conducted on 469 elderly people of the same age, gender and basic living conditions. Elderly residing in long-term care facilities had higher dental caries prevalence(p=0.019), DT(p=0.001), and MT(p=0.047) than non-resident elderly, and had lower FT(p<0.001) than non-resident elderly. The elderly living in facilities were 1.93 times more likely to be caries than non-residents, and 0.73 times more likely to have a filled tooth. The probability of denture use was 0.15 times for the maxillary and 0.13 times for the mandibular. The probability of denture needs was 5.61 times for the maxillary and 5.65 times for the mandibular. All of these results showed significant differences. As a result of this, it can be used as basic data for establishing oral health policy for oral health promotion of the elderly living in Long-term care facilities.
Purpose: This study was to classify elderly in long-term care hospitals for using Resource Utilization Group(RUG-III) and to consider feasibility of payment method based on RUG-III classification system in Korea. Method: This study designed by measuring resident characteristics using the Resident Assessment Instrument-Minimum Data Set(RAI-MDS) and staff time. The data were collected from 382 elderly over sixty-year old, inpatient in the five long-term care hospitals. Staff time was converted into standard time based on the average wage of nurse and aids. Result: The subjects were classified into 4 groups. The group of Clinically Complex was the largest(46.3%), Reduced Physical Function(27.2%), Behavior Problem(17.0%), and Impaired Cognition(9.4%). The average resource use for one resident in terms of care time(nurses, aids) was 183.7 minutes a day. Relative resource use was expressed as a case mix index(CMI) calculated as a proportion of mean resource use. The CMI of Clinically Complex group was the largest(1.10), and then Reduced Physical Function(0.93), Behavior Problem(0.93), and Impaired Cognition(0.83) followed. The difference of the resource use showed statistical significance between major groups(p<0.0001). Conclusion: The results of this study showed that the RUG-III classification system differentiates resources provided to elderly in long-term care hospitals in Korea.
The objective of this research study was twofold; 1) to explore and identify health supportive design factors in Swedish elderly care homes and 2) to understand their usefulness and suggest implication in Korean elderly care settings. A descriptive and explorative method was applied using a combination of field studies and semi-structured interviews. Three study trips were carried out during Sept. $14^{th}$ and Oct. $12^{th}$ 2005; two facilities situated in Stockholm suburbs and one in the south of Sweden. According to this research, the valuable factors to support health and well-being for the elderly are as follows; 1) Community integration: These elderly care homes are generally places close to a residential area center or a city center. Services are often shared between residents and community members at large, consequently there is a flow of "visitors" of all ages connecting with the facility on a daily basis. 2) Homelike environment: A noteworthy aspect of Swedish elderly care homes is keeping the facility appearance as homelike as possible. The associations with home may be explored through the appearance and configuration of both the exterior and interior of the building. These homes seemed to be designed with a conscious aim to create a homelike setting. 3) Small scale approach: Clustering of resident rooms is one method through which the small scale approach can be achieved in larger facilities. With unit clusters, the facility can foster opportunities for social interactions among resident. 4) Accessibility to garden and nature: The courtyard is a well developed concept in planning elderly care homes in Sweden. They are generally safe and easily accessible to the residents. Studying Swedish models may provide practical knowledge of how the physical setting may improve resident's health in Korean elderly care homes.

When rate of stated and public owned demesne in residential environment improvement is low under the limited resources of revenues, the new infrastructure for the maintenance of the existing estate has increased land reduction rate thus the place where lot of small estate find it difficult to accept as the feature of residential environment improvement. Although 'Diffusion model of residential environment improvement' which designed to overcome existing residential environment improvement, is middle of progress, there is not any suggestion made to solve problem caused by short of estate in order to maintain small estate and total Infrastructure. In feature of resident, we also notice ageing not only facilities and building. 'Elderly housing' can be presented as an alternative living style to solve housing and elderly matter which are caused by accumulation of individual estate and aging at once. Therefore, this research identifies resident's perception about elderly housing and recognizes task and potentialities that has to be solved in order to apply virtually to residential environment improvement.
This study is researched to provide design guidelines of service facilities and programs that apartment residents want along with the increased quality of life. This study is signifiant because it sought effective methods to activate the com-munity culture through service facilities in apartment complexes and the design standard and planning direction for residents. The study results are as follows. The purpose of this study was to provide basic information about efficient space use in the dining and bathing area through the analysis of service flowing. Four researchers observed the service flowing and the using behavior at those areas. The results of the study were as follows: Dining service was proceeded as resident moving, waiting, meal serving, dining, moving and arranging in order. The waiting stage was one of the problematic processes since the staffs made the residents wait to) long at a fixed position. The program right before the meal serving will be helpful for reducing tediousness of the elderly residents. Another problem was that the area was not big enough for the meal sowing. The legal regulation Is needed to prescnbe for the size of dining area per resident. The flowing of bathing service was proceeded as staff preparation, moving, waiting, undressing, bathing, drying, dressing, moving and arranging in order. There were more problems in the dressing area than in the bathing area. The elderly with stretcher or wheelchairs had difficulty in entering the narrow doorway. The dressing area was so crowded with the staffs, undressed elderly, dressed elderly, and other laundries. The division of dressing and undressing area is required to avoid the confusion of the users in the area.

The group of Clinically Complex was the largest (46.3%), and then Reduced Physical Function(27.2%), Behavior Problems (17.0%), and Impaired Cognition (9.4%) followed. Homogeneity of the RUG-III groups was examined by total coefficient of variation of resource use. The results showed homogeneity of resource use within RUG-III groups. Also, the difference in resource use among RUG major groups was statistically significant (p<0.001), and it also showed a hierarchy pattern as resource use increases in the same RUG group with an increase of severity levels(ADL). Conclusion: The results of this study showed that the RUG-Ill classification system differentiates resources provided to elderly in long-term care facilities in Korea.
Purpose: This study was conducted to examine the effect of multidimensional program on cognition, physical function and depression among institutionalized elderly and the relationship between study variables and resident's characteristics and health related variables. Method: This study involved a one group pre and post test, comparison of variables over a 12 month period. To investigate this research question, data of 114 residents of a nursing home were analyzed. Results: There was statistically significant difference in MMSE-K (t=-2.63, p=.010), ADL (t=-2.85, p=.005), and depression (t=4.66, p=.000) before and after program participation. Conclusion: These results indicate that, for a year their regular involvement in a broad spectrum of multidimensional program activities can improve in cognitive, physical and emotional perspectives, but the level of IADL decreased significantly (t=-6.72, p=.000). Further testing is required with the control group, to compare with community resident elders in order to explore the effects on social skill of elderly.
Elderly population has increased rapidly and adjusting the environment is in demand to support their health. To improve the living environment of elderly, an appropriate plan for customized home modification is needed upon the personal contexts. The purpose of this study is to explain the sequence and to build an insight on customized home modification for vulnerable single elderly. It is a case study based on single target that includes 6 months of execution period deeply seeking the appropriate plan for implementation. The target is a male elderly aged 74 living in a single story detached house. The various research methods are applied in this study including exploratory, participatory, action and trade-off preference researches. The result shows that plan for home modification is complex outcome of interlocking uniqueness between residence environment and resident situation. If customized modification is targeted for residentially vulnerable social group, more sensitive approach aiming for the highest result is required due to financial constraint. To conclude, comprehensive understanding of overall circumstances of a dwelling, a resident and various stakeholder is essential throughout the process. Since the aging-in-place of elderly cannot be achieved without self-sustaining living environment, customized home modification should be a tool to meet housing suitability. It is important to strengthen the experts and future workforce with networks of local residents to share opinions and appropriate plans.
