Tandem High-dose Chemotherapy and Autologous Stem Cell Transplantation in Children with Brain Tumors : Review of Single Center Experience

  • The prognosis of brain tumors in children has improved for last a few decades. However, the prognosis remains dismal in patients with recurrent brain tumors. The outcome for infants and young children in whom the use of radiotherapy (RT) is very limited because of unacceptable long-term adverse effect of RT remains poor. The prognosis is also not satisfactory when a large residual tumor remains after surgery or when leptomeningeal seeding is present at diagnosis. In this context, a strategy using high-dose chemotherapy and autologous stem cell transplantation (HDCT/auto-SCT) has been explored to improve the prognosis of recurrent or high-risk brain tumors. This strategy is based on the hypothesis that chemotherapy dose escalation might result in improvement in survival rates. Recently, the efficacy of tandem HDCT/auto-SCT has been evaluated in further improving the outcome. This strategy is based on the hypothesis that further dose escalation might result in further improvement in survival rates. At present, the number of studies employing tandem HDCT/auto-SCT for brain tumors is limited. However, results of these pilot studies suggest that tandem HDCT/auto-SCT may further improve the outcome. In this review, we will summarize our single center experience with tandem HDCT/auto-SCT for recurrent or high-risk brain tumors.

Diuretic Action of Angiotensin II in Dog (Angiotensin Ⅱ의 이뇨작용(利尿作用))

  • Angiotensin II, adminstered (infused or injected) intravenously, elicited the antidiuretic action with the decreased parameters of renal function at a small dose ($0.01\;{\mu}g/kg/min$), whereas, at a large dose (0.03, $0.1\;{\mu}g/kg/min$ and $5.0\;{\mu}g/kg$), it produced the diuretic action accompanied the increased amounts of sodium and potassium excreted in urine ($E_{Na}\;and\;R_K$). At this time, glomerular filtration rates (GFR) were weakened slightly and renal plasma flows (RPF) were reduced markedly, and then filtration fractions (FF) were increased. Angiotensin II, infused into a renal artery, exhibited antidiuretic action at a small dose ($0.003\;{\mu}g/kg/min$), and diuretic action at a large dose ($0.01\;{\mu}g/kg/min$), only in infused (experimental) kidney. The mechanism of the action was similar to the cases of the intravenous angiotensin II. The above results suggest that angiotensin II of a large dose produced diuretic action due to mechanism inhibiting reabsorption of electrolytes in renal tubules, mainly in proximal tubule in dog.

  • An Area Radiation Monitoring System (ARMS) ionization chamber, which had an 11.8 L active volume, was fabricated and performance-tested at KAERI. Low leakage currents, linearities at low and high dose rates were achieved from performance tests. The correlation coefficients between the ionization currents and the dose rates are 1 at high dose rate and 0.99 at low dose rate. In this study, an integration-type ARMS ionization chamber was tested over a year for an evaluation of its long-term stability at a radioisotope (RI) repository of the Young-gwang nuclear power plant. The standard deviation of dose rate of 1 day data and over a 100-days mean value were 6.2 $\mu$R/h and 2.9 $\mu$R/h, respectively. The fabricated ARMS ionization chamber showed stable performance from the results of the long-term tests. Design and performance characteristics of the fabricated ionization chamber for the ARMS from performance-tests are also addressed.

  • We fabricated a 15 m long position-sensitive plastic scintillation optical fiber (PSOF) detector consisting of a PSOF, two photomultiplier tubes, four fast amplifiers, and a digitizer. A single PSOF was used as a sensing part to estimate the gamma-ray source position, and 137Cs, an uncollimated solid-disk-type radioactive isotope, was used as a gamma-ray emitter. To improve the sensitivity, accuracy, and measurement time of a PSOF detector compared to those of previous studies, the performance of the amplifier was optimized, and the digital signal processing (DSP) was newly designed in this study. Moreover, we could measure very low dose rates of gamma-rays with high sensitivity and accuracy in a very short time using our proposed PSOF detector. The results of this study indicate that it is possible to accurately and quickly locate the position of a very low dose rate gamma-ray source in a wide range of contaminated areas using the proposed position-sensitive PSOF detector.

Measurement of Uptake Rates of Internal Organs Including Thyroid Gland and Daily Urinary Excretion Rates for Adult Korean Males (한국남자 성인을 대상으로 한 방사성옥소($^{131}I$)의 갑상선 및 각 장기별 잔류율과 소변 일일배설률 측정)

  • In this study, uptake rates of internal organs and daily urinary excretion rates were measured to get more reliable estimation results for Korean. Radioactive iodine($^{131}I$) of $100{\mu}Ci$ was administered by ingestion to 28 adult males for the experiment and then the radioactivity in thyroid gland, liver, stomach, small intestine, kidneys, and urine was measured after time intervals of 2, 4, 6 and 24 hours. Uptake rates of each organ and daily urinary excretion rates were calculated on the basis of these experimental results. As a result, uptake rates of 19.70% for thyroid and daily urinary excretion rates of 71.12%, on the average, were indicated. The maximum of uptake rates and daily urinary excretion rates were recorded after 2 hours of administration of $^{131}I$, but those rates were decreased gradually later. It was also found that uptake rates were the highest in stomach, followed by the left kidney, liver, small intestine and right kidney except for thyroid gland. In this experiment, the calculated uptake change rate in thyroid gland after 24 hours of administration of $^{131}I$ was different from that of ICRP-54/67(30%) and ICRP-78(25%). Thus, it is necessary to apply more reliable approach, reflecting the characteristic of Korean physiology and to obtain the basic data of results using this approach for calculation of the internal adsorbed dose. In the future, this approach can be helpful for the internal dose assessment of radiation workers in a nuclear power plant or in a hospital.

Characterization of Radiation Field in the Steam Generator Water Chambers and Effective Doses to the Workers (증기발생기 수실의 방사선장 특성 및 작업자 유효선량의 평가)

  • Characteristics of radiation field in the steam generator(S/G) water chamber of a PWR were investigated and the anticipated effective dose rates to the worker in the S/G chamber were evaluated by Monte Carlo simulation. The results of crud analysis in the S/G of the Kori nuclear power plant unit 1 were adopted for the source term. The MCNP4A code was used with the MIRD type anthropomorphic sex-specific mathematical phantoms for the calculation of effective doses. The radiation field intensity is dominated by downward rays, from the U-tube region, having approximate cosine distribution with respect to the polar angle. The effective dose rates to adults of nominal body size and of small body size(The phantom for a 15 year-old person was applied for this purpose) appeared to be 36.22 and 37.06 $mSvh^{-1}$) respectively, which implies that the body size effect is negligible. Meanwhile, the equivalent dose rates at three representative positions corresponding to head, chest and lower abdomen of the phantom, calculated using the estimated exposure rates, the energy spectrum and the conversion coefficients given in ICRU47, were 118, 71 and 57 $mSvh^{-1}$, respectively. This implies that the deep dose equivalent or the effective dose obtained from the personal dosimeter reading would be the over-estimate the effective dose by about two times. This justifies, with possible under- or over- response of the dosimeters to radiation of slant incidence, necessity of very careful planning and interpretation for the dosimetry of workers exposed to a non-regular radiation field of high intensity.

Effects of Guanabenz on Renal Function in Dog (개의 신장기능에 미치는 Guanabenz의 영향)

  • In this study attempts were made to observe the effects of guanabenz on renal function in dog, which manifests the antihypertensive action by inhibition of sympathetic tone through stimulating the presynaptic adrenoceptor (${\alpha}_2-adrenoceptor$). Guanabenz, when injected at a dose of $30.0{\mu}g/kg$, or infused at a dose of $3.0{\mu}g/kg/min$ intravenously, produced diuretic action with increased amounts of $Na^+\;and\;K^+$ in urine, and with decreased reabsorption rates of $Na^+\;and\;K^+$ in renal tubules. It was also observed that the rates of osmolar and free water clearances were increased, but the glomerular filtration rate and renal plasma flow were not changed. Guanabenz injected at a dose of $3.0{\mu}g/kg$ into a carotid artery or infused intravenously at a dose of $3.0{\mu}g/kg/min$ in a state of water diuresis elicited the diuretic action of the similar aspect as a case of guanabenz given intravenously. The diuretic action produced by guanabenz was completly blocked by pretreatment of i.v. prazosin, ${\alpha}_1-adrenoblocking$ agent, or of i.v. yohimbine, ${\alpha}_2-adrenergic$ blocking agent. Prazosin, when given into a renal artery, inhibited the diuretic action by i.v. guanabenz in only injected kidney, whereas in case of yohimbine the action was inhibited in both kidney. Guanabenz infused at a dose of $1.0{\mu}g/kg/min$ into a renal artery exhibited no significant changes of renal function in both kidney. In denervation experiments, guanabenz given intravenously produced typical diuretic action in innervated kidney, whereas in denervated kidney, it did not affect the action at initial period but exhibited the action with increase of only free water clearance at later period. These results suggest that guanabenz produced diuretic action in dog by inhibition of electrolyte reabsorption rates in renal tabules, mainly proximal tubule and of ADH release, which is mediated by stimulating of central sympathetic ${\alpha}_2-receptor$.

Comparing the performance of two hybrid deterministic/Monte Carlo transport codes in shielding calculations of a spent fuel storage cask

  • This study systematically compared two hybrid deterministic/Monte Carlo transport codes, ADVANTG/MCNP and MAVRIC, in solving a difficult shielding problem for a real-world spent fuel storage cask. Both hybrid codes were developed based on the consistent adjoint driven importance sampling (CADIS) methodology but with different implementations. The dose rate distributions on the cask surface were of primary interest and their predicted results were compared with each other and with a straightforward MCNP calculation as a baseline case. Forward-Weighted CADIS was applied for optimization toward uniform statistical uncertainties for all tallies on the cask surface. Both ADVANTG/MCNP and MAVRIC achieved substantial improvements in overall computational efficiencies, especially for gamma-ray transport. Compared with the continuous-energy ADVANTG/MCNP calculations, the coarse-group MAVRIC calculations underestimated the neutron dose rates on the cask's side surface by an approximate factor of two and slightly overestimated the dose rates on the cask's top and side surfaces for fuel gamma and hardware gamma sources because of the impact of multigroup approximation. The fine-group MAVRIC calculations improved to a certain extent and the addition of continuous-energy treatment to the Monte Carlo code in the latest MAVRIC sequence greatly reduced these discrepancies. For the two continuous-energy calculations of ADVANTG/MCNP and MAVRIC, a remaining difference of approximately 30% between the neutron dose rates on the cask's side surface resulted from inconsistent use of thermal scattering treatment of hydrogen in concrete.

Radiation Shielding Calculation on Shield System of CANDU 6 Plant Using the Coupled DOT4.2 and QAD-CG Codes (DOT4.2-QAD-CG 접속법을 이용한 CANDU 6 발전소 차폐 계통에 대한 방사선 차폐 계산)

  • DOT4.2-QAD-CG coupling method was used to analyze the dose rates outside the side and the bottom shield system of CANDU 6 plant. The average dose rates at the main airlock and the new fuel loading area are approximately 6 $\mu$Sv/h as it is required. The calculated dose rates have a good agreement with the measurements at the operating CANDU 6 plant. The method used in this paper can be applied to the radiation shielding analysis of Wolsong 2, 3, and 4 CANDU 6 type plants which will be constructed in the near future.

Radiation-Induced Graft Copolymerization of Acrylic Acid onto Polyester

  • The radiation-induced graft polymerization of acrylic acid onto polyester fabric was investigated with accelerated electron beams as ratiation source at high dose rates. Homopolymerization was suppressed by addition of cations which is known as homopolymerization inhibitor, but this practical advantage was obtained at the expense of grafting efficiency. The rate of grafting (%/sec) was proportional to the 0.82th power of dose rates over the range from 1.6$\times$10$^{6}$ to 10$\times$10$^{6}$ rad/sec. The grafted polyester fabric showed considerable improvement in moisture regain and antistatic properties.
